    Decrease in pulmonary function and oxygenation after lung resection

    Respiratory deficits are common following curative intent lung cancer surgery and may reduce the patient's ability to be physically active. We evaluated the influence of surgery on pulmonary function, respiratory muscle strength and physical performance after lung resection. Pulmonary function, respiratory muscle strength (maximal inspiratory/expiratory pressure) and 6-min walk test (6MWT) were assessed pre-operatively, 2 weeks post-operatively and 6 months post-operatively in 80 patients (age 68±9 years). Video-assisted thoracoscopic surgery was performed in 58% of cases. Two weeks post-operatively, we found a significant decline in pulmonary function (forced vital capacity −0.6±0.6 L and forced expiratory volume in 1 s −0.43±0.4 L; both p<0.0001), 6MWT (−37.6±74.8 m; p<0.0001) and oxygenation (−2.9±4.7 units; p<0.001), while maximal inspiratory and maximal expiratory pressure were unaffected. At 6 months post-operatively, pulmonary function and oxygenation remained significantly decreased (p<0.001), whereas 6MWT was recovered. We conclude that lung resection has a significant short- and long-term impact on pulmonary function and oxygenation, but not on respiratory muscle strength. Future research should focus on mechanisms negatively influencing post-operative pulmonary function other than impaired respiratory muscle strength

    Preparo da veia safena na cirurgia de revascularização miocárdica: uma nova técnica -no touch- que mantém a parede da veia íntegra e proporciona uma alta perviabilidade imediata

    BACKGROUND: The technique of harvesting the saphenous vein (SV) for coronary artery bypass grafting (CABG) influences the fate of vein grafts. A new no touch (NT) technique of SV preparation was developed where the vein is harvested with a pedicle of surrounding tissue, which protects the vein from spasms therefore obviating the need for distension. METHOD: A prospective randomized study in 156 patients who underwent CABG was performed comparing three SV harvesting techniques. The techniques were conventional (c) (adventitial stripping of the vein, manual distention and storing in saline solution); Intermediate (I) (after adventitial stripping, the vein was left in situ, covered with a papaverine-soaked compress, and stored in heparinized blood); and no touch (SV dissected with its surrounding tissue was left in situ, covered with a saline-soaked compress and stored in heparinized blood). A morphological study of the endothelium was preformed using scanning electronic microscopy and an angiographic assessment of the vein graft patency was performed at 18 months mean follow-up time. Also an immunohistochemistry assessment was performed to identify the enzyme, nitric oxide synthase (NOS) in the vein wall. RESULTS: The preservation of the endothelial cell integrity was greater with the no touch technique than with the other procedures. At angiographic follow up, the patency for NT was 95.4%, 88.9% for grafts in group C and 86.2% for grafts in group I. The immunohistochemistry assessment revealed NOS in all three layers of the vein wall that was prepared by the no touch technique. However, a great reduction of this enzyme in veins treated by the conventional technique was observed. CONCLUSION: The endothelial integrity and NOS activity were better preserved when using the no touch technique for vein graft harvesting. The vasorelaxation and thrombo-resistant activities of nitric oxide (NO) may be responsible for the reduced of vasospasms and improved patency rate. Furthermore, the mechanical properties provided by the cushion of surrounding tissue in graft harvested by NT technique may contribute to the observed high patency rate.OBJETIVO: O objetivo deste trabalho é apresentar uma nova técnica no touch de preparo da veia safena e sua importância clínica. Esta nova técnica consiste na retirada da veia safena do seu leito, juntamente com um pedículo de tecido adiposo, protegendo a veia contra espasmos e, conseqüentemente, da necessidade de distendê-la durante a operação. MÉTODO: O estudo foi realizado em dois tempos: 1) Inicialmente, foi realizado um estudo prospectivo e randomizado, envolvendo 156 pacientes que foram submetidos à operação de revascularização miocárdica. Neste estudo nós comparamos a técnica no touch (NT) com duas outras: a convencional (C) e a intermediária (I). Foi feita uma avaliação detalhada da morfologia endotelial, utilizando a microscopia eletrônica, enquanto que a perviabilidade das pontes foi determinada com um exame angiográfico executado num período médio de 18 meses após a operação. 2) Finalmente, a enzima óxido nítrico síntase (NOS) foi identificada com ajuda do estudo imunohistoquímico e também pela localização auto-radiográfica de (3H)-L- Nitro-Arginina (NOARG). RESULTADOS: A avaliação morfológica mostrou integridade endotelial de 97% nas veias que foram tratadas pela técnica NT; enquanto quase metade da superfície endotelial das veias tratadas pelas outras duas técnicas exibiu ausência de células endoteliais. O exame angiográfico revelou perviabilidade de 95,4% para as pontes do grupo NT, 88,9% para as pontes do grupo (C) e 86,2% para as pontes do grupo (I). O estudo imunohistoquímico revelou a presença de NOS nas três camadas que compõem a parede da veia quando, as mesmas, foram tratadas pela técnica NT. Por outro lado, foi observada uma elevada redução desta enzima nas veias que foram tratadas pela técnica (C). A análise auto-radiográfica confirmou os resultados imunohistoquímicos. CONCLUSÃO: A integridade endotelial e a atividade da enzima óxido nítrico síntase foram melhor mantidas com o uso da técnica NT no preparo da veia safena. A atividade vasodilatadora e bloqueadora da agregação plaquetária causada pelo óxido nítrico (NO) pode ser a responsável pela proteção da veia contra o espasmo, como também pela alta perviabilidade imediata da mesma.     Efeito do óxido de grafeno no crescimento da microalga Pseudokirchneriella subcapitata sob influência de substâncias húmicas.

    O grafeno é um nanomaterial promissor na remoção de pesticidas de águas, transporte de fármacos em medicamentos e transporte de agroquimicos em formulações. A qualidade do lodo agrícola usado como biofertilizante pode ser comprometida já que é esperado que materiais a base de grafeno estejam presentes nas estações de tratamento de efluentes em quantidades consideráveis A microalga Pseudokirchneriella subcapitata é uma clorofícea bioindicadora que tem sido amplamente utilizada em estudos de ecotoxicidade de poluentes e recomendada no registro de agentes químicos e biopesticidas. No presente trabalho avaliou-se o efeito do óxido de grafeno (OG) na taxa de crescimento de culturas desse organismo-teste, sob diferentes concentrações do nanomaterial e de influência do ácido húmico (AH) neste efeito. Pois a última substância pode afetar a biodisponibilidade do nanomaterial. Como resultado, observou-se que a concentração que inibe o crescimento algáceo em 50% durante um período de 72 h de exposição (CE50-72h) foi de 66,60 e 242,78 mg L-1, respectivamente, na ausência e na presença de AH. Não tendo se constatado diferença significativa (p>0,05) entre estes valores, o primeiro categoriza o OG como ?levemente tóxico? para P. subcapitata. Os valores de concentração de efeito não observado (CENO) foram respectivamente de 4,93 e 0,26 mg L-1 para a alga exposta na ausência ou na presença do material húmico. Estas concentrações, juntamente com outros valores de parâmetros de ecotoxicidade determinados em outros bioindicadores da cadeia trófica aquática, são de utilidade para orientar políticas públicas quanto ao estabelecimento de níveis de segurança do OG. Graphene is a promising nanomaterial in the removal of pesticides from water, transport of drugs in medicines and transport of agrochemicals in formulations. The quality of the agricultural sludge used as a biofertilizer can be compromised since graphene-based materials are expected to be present in wastewater treatment plants in considerable quantities. The microalgae Pseudokirchneriella subcapitata is a bioindicator chloroficea that has been widely used in ecotoxicity studies of pollutants and it was recommended in the registration of chemical agents and biopesticides. In the present work it was evaluated the effect of graphene oxide (OG) on the growth rate of this test-organism under different concentrations of the nanomaterial and under the influence of humic acid (HA). This last substance may affect the bioavailability of the nanomaterial. As a result, the concentration that inhibits the growth rate by 50% over a 72 h exposure period (EC50-72 h) was found to be 66.60 and 242.78 mg L-1, respectively, in the absence and in the presence of AH. No significant difference (p>0.05) was found between both values and the former categorizes OG as \\\"slightly toxic\\\" for P. subcapitata. The non-observed effect concentration (CENO) values were respectively 4.93 and 0.26 mg L-1 for algae exposed in the absence or in the presence of the humic material. These concentrations, together with other values of ecotoxicity parameters determined in other bioindicators of the aquatic trophic chain, are useful for guiding public policies regarding the establishment of safety levels of OG

    Glucose-6-phosphate dehydrogenase (G6PD) deficiency is an enzymopathy in which reduced NADPH concentrations are not maintained, resulting in oxidative damage. We evaluated G6PD activity, oxidative stress levels and Trolox equivalent antioxidant capacity in individuals with the A-(202G>A) mutation for G6PD deficiency. Five hundred and forty-four peripheral blood samples were screened for G6PD deficiency; we also analyzed lipid peroxidation products measured as thiobarbituric acid reactive species and Trolox equivalent antioxidant capacity. Men with the A-(202G>A) mutation had lower G6PD activity than women with the same mutation. Individuals with the A-(202G>A) mutation also differed in mean Trolox equivalent antioxidant capacity values but not for thiobarbituric acid reactive species values. We concluded that A-(202G>A) mutation is associated with reduced G6PD activity and increased Trolox equivalent antioxidant capacity. ©FUNPEC-RP

    Geographic Tongue and Fissured Tongue in 348 Patients with Psoriasis: Correlation with Disease Severity

    Geographic tongue (GT) and fissured tongue (FT) are the more frequent oral lesions in patients with psoriasis. The aims of this study were to compare the prevalence of GT/FT between psoriasis group (PG) and healthy controls (HC) and investigate the correlation between GT/FT and psoriasis severity using the PASI and age of psoriasis onset. Three hundred and forty-eight PG and 348 HC were selected. According to the age of psoriasis onset, the individuals were classified as having early psoriasis and late psoriasis. The severity of vulgaris psoriasis was determined according to PASI. A follow-up was conducted in patients with psoriasis vulgaris (PV) with GT to evaluate the progression of oral and cutaneous lesions. The FT and GT were more frequent in PG than in HC. The incidence of GT was higher in patients with early psoriasis and that of FT in late-psoriasis. There is association between psoriasis intensity and GT; and a higher monthly decrease of PASI score in patients without GT. The presence of GT and FT is higher in PG than in the HC. GT is associated with disease severity and may be a marker of the psoriasis severity

    Evidence of contamination by oil and oil products in the Santos-Sao Vicente estuary, Sao Paulo, Brazil

    Different components of the mixed function oxidase (MFO) system and the levels of fluorescent aromatic compounds in bile (FACs) were measured in Cathorops spixii in order to assess the impact of polycyclic aromatic hydrocarbons (PAHs). Fish were sampled in an estuary (Santos/Sao Vicente) with a history of contamination by PAHs, mainly due to the presence of the industrial complex of Cubatao city and of another of low anthropogenic influence (Cananeia) on the Brazilian coast. FACs were higher in fish from the polluted site, and the PAH 5 and 6-ring metabolites were the most frequent - with 14% and 15%, respectively. Levels of the different components of the MFO system showed the same variation profile as the FACs for both estuaries.
