    Importance of structural history in the summit area of Stromboli during the 2002–2003 eruptive crisis inferred from temperature, soil CO2, self-potential, and electrical resistivity tomography

    International audienceThe 2002-2003 eruptive crisis of Stromboli volcano in the Aeolian Islands raised the question of how to assess the stability of the flanks of this volcanic edifice during such a crisis. To provide a response to this question, we analyzed a detailed fluid flow mapping plus the reiteration of a profile located in the vicinity of the active vents using the self-potential method, temperature data, soil-gas (CO2) measurements, and electric resistivity tomography. Coupling the interpretation of these methods that are sensitive to the flow of gas and water in the ground indicates the position of areas of mechanical weakness. In addition, they can be used to monitor the change in the discharge of fluids associated with these features before and during the 2002-2003 eruptive crisis. Our results emphasize the importance of old structural boundaries, such as the Large Fossa crater, in the development of the new set of fractures observed during the 2002-2003 eruptive crisis. Between October 2002 and January 2003, the use of CO2 soil-gas technique evidenced an increase in the discharge of CO2 outside the Large Fossa crater boundaries, along the failure boundary of the southern Sciara del Fuoco area. Self-potential and temperature measurements made before the 2002-2003 eruptive crisis reveal significant changes along the main structural boundaries of the Fossa area. The development of these anomalies is interpreted as an increase of the permeability of the structure from May 2000 to May 2002. Between January 2003 and March 2003 the reiteration of self-potential, temperature, and CO2 measurements shows an increase of fluid discharge along weakness planes located inside the Large Fossa crater boundary. They evidence no change outside this structural boundary. The importance of the Large Fossa crater boundary in controlling the deformation and fluid flow from January to March 2003 has been attested by the development of the fractures inside the Large Fossa crater boundary, and also with a network of electrooptical distance measurement stations located inside and outside this ancient crater. This multidisciplinary approach to fluid flow assessment before and during an eruptive crisis is complementary to geodetic measurements of the deformation of the edifice. It demonstrates for the first time the powerful potential of combining electrical resistivity tomography, self-potential, temperature, and soil CO2 measurements in assessing the position of the planes of mechanical weakness in a volcanic edifice

    Abstract Purpose of Review To provide the radiologist with basic knowledge about normal and abnormal findings in the prostatic mp-MRI, taking a look at the possible diagnostic pitfalls commonly seen in daily clinical practice, allowing him to recognize and consequently avoid them. Recent Findings Prostate mp-MRI has now become commonly used in most diagnostic imaging centers, as a precise, accurate and above all non-invasive tool, useful in the diagnosis, staging and follow-up of prostate diseases, first of all prostatic carcinoma. For this reason, it is important to take into account the existence of numerous possible anatomic and pathologic processes which can mimick or masquerade as prostate cancer. Summary Through the combination of anatomical (T2WI) and functional sequences (DWI/ADC and DCE), the mp-MRI of the prostate provides all the information necessary for a correct classification of patients with prostate disease, cancer in particular. It is not uncommon, however, for the radiologist to make errors in the interpretation of imaging due to conditions, pathological or otherwise, that mimic prostate cancer and that, consequently, affect the diagnostic/therapeutic process of patients.     Geochemical survey of hot springs around Ubinas, the most historical active volcano in southern Peru

    Ubinas (5675 m a.s.l.) is known as the most active volcano in southern Peru during historical times, with 23 minor eruptive or intense fumarolic crises since 1550. Due to its intense explosive recurrence and to the high proximity of the homonymous village (4000 inhabitants), only 6 km far away from the summit crater, Ubinas appears as one of the highest priority in volcano monitoring in southern Peru. The purpose of this survey has been (1) in a first step to evidence the geochemical characteristics of the hot springs located around the volcano in order to define (2) in a second step an appropriate fluid geochemical monitoring program. (1) The analysis of the waters collected around the volcano displays evident geochemical trends that can be interpreted as mixing processes between 3 end-members: fresh meteoric waters, magmatic fluids, sometimes interacting with a deep chloride reservoir. (2) Since 1999, we began a geochemical water monitoring program of two hot springs located in the lower flank of the volcano. This hydrothermal water have been characterized by a constancy in sulfate and chlorine concentrations before the big earthquake (Mw 8.4) occurred in Peru on June 23rd, 2001, causing hundreds of fatalities and serious damages along all the southern Peruvian coast. After this event, one new hot spring appears near to our sampling area and sulfate and chlorine display an increase in concentration. This phenomenon could be attributed to a consequence of the earthquake of June 23rd, 2001, increasing the permeability of the fault system crossing the volcano. In this way, an higher permeability in the soil along preferential regional faults could allow a better interaction between the ascending magmatic gases (in particular HCl and SO2/H2S gas species responsible respectively of chlorine and sulfate concentration in hydrothermal waters) and the shallower fresh meteoric waters

    The comparison of outcomes from tyrosine kinase inhibitor monotherapy in second- or third-line for advanced non-small-cell lung cancer patients with wild-type or unknown EGFR status

    Background: Second-line treatment for advanced non-small-cell lung cancer (NSCLC) patients includes monotherapy with a third-generation cytotoxic drug (CT) or a tyrosine kinase inhibitor (TKI). These options are the actual standard for EGFR wild-type (WT) status, as patients with EGFR mutations achieve greater benefit by the use of TKI in first-line treatment. Some clinical trials and meta-analyses investigated the comparison between CT and TKI in second-line, but data are conflicting. Methods: We designed a retrospective trial to gather information about TKI sensitivity in comparison with CT. We selected from clinical records patients treated with at least 1 line of CT and at least 1 line of TKI. We collected data about age, sex, performance status, comorbidity, smoking status, histotype, metastatic sites, EGFR status, treatment schedule, better response and time-to-progression (TTP) for each line of treatment and overall survival (OS). Results: 93 patients met selection criteria. Mean age 66,7 (range: 46-84). M/F ratio is 3:1. 39 EGFR-WT and 54 EGFR-UK. All patients received erlotinib or gefitinib as second-line treatment or erlotinib as third-line treatment. No TTP differences were observed for both second-line (HR:0,91; p = 0,6333) and third-line (HR:1.1; p = 0,6951) treatment (TKI vs CT). A trend of a benefit in OS in favor of 3rd-line TKI (HR:0,68; p = 0,11). Conclusions: This study explores the role of TKIs in EGFR non-mutated NSCLC patients. OS analysis highlights a trend to a benefit in patients who received TKI in third-line, even if this result is statistically non-significant. Further analysis are needed to find an explanation for this observation

    The seismic sequence of 30 May - 9 June 2016 in the geothermal site of Torre Alfina (central Italy) and related variations in soil gas emissions

    In the framework of a medium-enthalpy geothermal exploitation project, seismicity and soil gas emissions have been monitored in the area of Castel Giorgio-Torre Alfina since 2014. A dedicated local seismic network deepened the knowledge of the natural local seismicity in terms of source mechanisms, high-quality event localization and magnitude estimation. From November 2014 to May 2016, 846 seismic events were recorded, with a magnitude range of Md 0.1-2.8 and hypocentres 4-8 km depth. On 30th May 2016 a Mw 4.3 earthquake occurred near Castel Giorgio, followed by almost 1700 aftershocks; the moment tensor solution depicts a WNW-ESE oriented normal fault. An overview of the epicentral distributions since 2014, highlights that the active tectonic structures are NE-SW and WNW-ESE orientated. The diffuse soil CO2 flux is monitored since 2013 in six target areas located around the future production and reinjection wells, in order to assess the level of background natural degassing. In all target areas the maximum value of soil CO2 flux has been recorded during the 2016 seismic sequence. However, the values of δ13C of the emitted CO2 indicated a shallow biological origin of the gas. At Torre Alfina, the Solfanare natural gas emission, with a CO2 dominated gas, has same composition of the gas hosted in the geothermal reservoir. Here, high values of diffuse soil CO2 flux were recorded. During the 2016 seismic sequence, the Solfanare gas was continuously analysed by an automatic gas- chromatographic station. Results show that apart from small perturbations, no significant compositional variations were recorded. The significant contribution of CLVD and isotropic components suggest a possible opening of fluid cracks below the geothermal reservoir hosted in fractured Mesozoic limestones. The seismo-tectonic scenario indicates that the Solfanare fault was not activated.     Immunotherapy: is a minor god yet in the pantheon of treatments for lung cancer?

    Immunotherapy has been studied for many years in lung cancer without significant results, making the majority of oncologists quite skeptical about its possible application for non-small cell lung cancer treatment. However, the recent knowledge about immune escape and subsequent 'cancer immunoediting' has yielded the development of new strategies of cancer immunotherapy, heralding a new era of lung cancer treatment. Cancer vaccines, including both whole-cell and peptide vaccines have been tested both in early and advanced stages of non-small cell lung cancer. New immunomodulatory agents, including anti-CTLA4, anti-PD1/PDL1 monoclonal antibodies, have been investigated as monotherapy in metastatic lung cancer. To date, these treatments have shown impressive results of efficacy and tolerability in early clinical trials, leading to testing in several large, randomized Phase III trials. As these results will be confirmed, these drugs will be available in the near future, offering new exciting therapeutic options for lung cancer treatment

    Strategies used to inhibit postoperative swelling following removal of impacted lower third molar

    Postoperative swelling following different surgical strategies is an area of great interest. The main part of literature on the topic deals with swelling after extraction of low impacted third molar. In this review, we have analyzed publications of the last 20 years with a pubmed search using the following key words: impacted third molar, swelling third molar, wisdom tooth, edema jaw, corticosteroids and extraction third molar, antibiotic prophylaxis and tooth extraction. Attention has often been focused on corticosteroid therapy administered by diverse routes (orally, IV, IM, topically) and at different time schedules (before or after surgery or both). This investigation revealed how the use of different molecules and dosages makes the obtained results hardly comparable. Similar conclusions can be drawn from studies aimed at evaluating the efficacy of antibiotic therapy administered either before or after surgery. A complete review has also to take into account different surgical strategies used including various flaps, no traumatic osteothomy, and primary or secondary closure. The use of pharmacological therapy and application of an ice pack is critical in the postoperative period and has always provided positive results. However, even if it is difficult to come to definite conclusions, due to the variability of the design of studies analyzed, the postoperative discomfort identified with edema, pain and trismus following wisdom tooth removal is influenced by various factors such as the difficulty of the surgical procedure involved, age and gender of the patient, and experience of the surgeon. The pharmacological therapy when performed with corticosteroids seems to improve control of the postoperative swelling related with this kind of surgeries

    A Robust Initialization of Residual Blocks for Effective ResNet Training Without Batch Normalization

    Batch Normalization is an essential component of all state-of-the-art neural networks architectures. However, since it introduces many practical issues, much recent research has been devoted to designing normalization-free architectures. In this paper, we show that weights initialization is key to train ResNet-like normalization-free networks. In particular, we propose a slight modification to the summation operation of a block output to the skip-connection branch, so that the whole network is correctly initialized. We show that this modified architecture achieves competitive results on CIFAR-10, CIFAR-100 and ImageNet without further regularization nor algorithmic modifications.Comment: 16 pages (4 pages of supplementary material), 9 figures, 2 tabl

    International audienceStromboli volcano (Italy) is characterized by a permanent mild explosive activity disrupted by major and paroxysmal eruptions. These strong eruptions could be triggered by phreato-magmatic processes. With the aim of obtaining a better understanding of ground water flow in the vicinity of the active vents, we carried out a set of geophysical measurements along two profiles crossing the Fossa area (through the Pizzo, the Large and the Small Fossa craters). These measurements include electrical resistivity, induced polarization, self-potential, temper-ature and CO2 ground concentration. These methods are used in order to delineate the crater boundaries, which act as preferential fluid flow pathways for the upflow of hydrothermal fluids. The absence of fumarolic activity in the Fossa area and the ground temperature close to 100 ◦C at a depth of 30 cm indicate that the hydrothermal fluids condense close to the ground surface.
