    Police Use Unconscious Driver and Passenger as Anti-Drug Advertisement

    On Thursday, September 8th, 2016 Ohio police officers posted photographs of a couple and their son on Facebook. The pictures display a woman and man, unconscious in the front seats of their vehicle, and a four-year-old child awake in the back seat. The city police took these pictures after conducting a traffic stop on September 7th; they had noticed the couple’s car weaving between traffic lanes, and upon stopping the car, noticed “the man’s head ‘bobbing back and forth his speech was almost unintelligible.’” James Acord, the driver of the vehicle, informed officers that he was taking the passed out woman, Rhonda Pasek, to a hospital, after which he lost consciousness. An ambulance was called and paramedics administered Narcan, a medication used to block the effects of opioids, particularly during overdose.     Acute military psychiatric casualties from the war in Iraq

    Background: The view that most military personnel evacuated from war zones are suffering from combat stress reactions, or are otherwise traumatised by the horrors of war, has an impact on all aspects of military psychiatry. Aims: To delineate the reasons for psychiatric aeromedical evacuation from Iraq from the start of build-up of UK forces in January 2003 until the end of October that year, 6 months after the end of formal hostilities. Method: A retrospective study was conducted of field and in-patient psychiatric assessments of 116 military personnel evacuated to the UK military psychiatric in-patient facility in Catterick Garrison. Results: Evacuees were mainly non-combatants (69%). A significant proportion were in reserve service (21%) and had a history of contact with mental health services (37%). Only 3% had a combat stress reaction. In over 85% of cases evacuation was for low mood attributed to separation from friends or family, or difficulties adjusting to the environment. Conclusions: These findings have implications especially for screening for suitability for deployment, and for understanding any longer-term mental health problems arising in veterans from Iraq

    Sensitization of ovarian cancer cells to cisplatin by genistein: the role of NF-kappaB

    <p>Abstract</p> <p>Background</p> <p>Platinum-resistance (PR) continues to be a major problem in the management of epithelial ovarian cancer (EOC). Response to various chemotherapeutic agents is poor in patients deemed PR. Genistein, a soy isoflavone has been shown to enhance the effect of chemotherapy in prostate and pancreatic cancer cells <it>in vitro </it>and <it>in vivo </it>by reversing chemo-resistance phenotype. The goal of this study was to investigate the effects of combination therapy with genistein and cisplatin as well as other cytotoxic conventional chemotherapeutic agents in platinum-sensitive (PS) and resistant EOC cells.</p> <p>Methods</p> <p>The PS human ovarian cancer cell line A2780 and its PR clone C200 cells were pretreated with genistein, followed by the combination of genistein and either cisplatin, taxotere or gemcitabine. Cell survival and apoptosis was assessed by MTT and histone-DNA ELISA. Electrophoretic mobility shift assay (EMSA) was used to evaluate NF-ÎşB DNA binding activity. Western blot analysis was performed with antibodies to Bcl-2, Bcl-xL, survivin, c-IAP and PARP.</p> <p>Results</p> <p>Reduction in cell viability, and corresponding induction of apoptosis was observed with genistein pretreatment followed by combination treatment with each of the drugs in both cell lines. The PS cell line was pretreated for 24 hours; in contrast, the PR cell line required 48 hours pretreatment to achieve a response. The anti-apoptotic genes c-IAP1, Bcl-2, Bcl-xL, survivin and NF-ÎşB DNA binding activity were all found to be down-regulated in the combination groups.</p> <p>Conclusion</p> <p>This study convincingly demonstrated that the current strategy can be translated in a pre-clinical animal model, and thus it should stimulate future clinical trial for the treatment of drug-resistant ovarian cancer.</p

    Intra-operative cell salvage in South Africa: Feasible, beneficial and economical

    Background. Healthcare services in KwaZulu-Natal (KZN) frequently experience shortages of blood products. Alternatives to traditional blood sources are needed. One possibility is cell salvage and autologous blood transfusion. Few data exist relating to their use in the South African context.Objectives. To ascertain whether cell salvage (CS) is beneficial, feasible and economical in our setting, with the anticipation that results would excite further studies and potentially change current practice and improve existing protocols. Methods. This study reviewed current intraoperative Cell Saver use at Grey’s and Edendale hospitals in Pietermaritzburg, KZN. Current practices were observed and compared with the costs of the equivalent amount of red cell concentrate (RCC) purchased from the South African National Blood Service (SANBS). All cases of Cell Saver usage over a 1-year period from July 2012 to June 2013 were analysed. Results. The total volume of blood transfused from CS was 55 735 ml, approximately equivalent to 186 RCC units. The comparative cost of equivalent units of RCC from SANBS was R258 445. The total cost of Cell Saver disposables during the study period was R206 047, or R263 478 when Cell Saver machine depreciation costs were included. More than one CS blood unit was available for transfusion in 66% of cases. No additional staff were required to operate the Cell Saver, which was successfully used by medical officers. Conclusions. This study showed that intraoperative CS use is feasible, has potential patient benefit by reducing blood bank blood transfusion, and is financially comparable to purchasing the equivalent number of SANBS RCC.

    Evaluation of δD and δ18O as Natural Markers of Invertebrate Source Environment and Dispersal in the Middle Mississippi River-Floodplain Ecosystem

    Movement of invertebrates among large rivers, tributaries, and floodplain lakes or dispersal of adult aquatic insects from riverine or floodplain habitats may provide important subsidies to food webs in receiving habitats. Intensive sampling at habitat interfaces and artificial labeling are two approaches to assess freshwater invertebrate dispersal, but these are difficult to implement at a landscape scale. Natural chemical tracers have been used to track dispersal of fishes and marine invertebrates, but the potential applicability of stable isotope ratios as natural tracers of invertebrate dispersal in freshwater environments has not been assessed. We evaluated stable hydrogen and oxygen isotopes (δD and δ18O) as natural markers of source environment and dispersal of macroinvertebrates in the middle Mississippi River, tributaries, and floodplain wetlands. Water and invertebrates were collected from 12 sites during 2007-2008. Water δD and δ18O differed among the river, its tributaries, and floodplain wetlands and were strongly correlated with invertebrate δD and δ18O. Variability in invertebrate δ18O rendered it ineffective as an indicator of invertebrate source environment. Mean δD of Mississippi River invertebrates differed from δD of invertebrates from floodplain wetlands; δD distinguished invertebrates from these two environments with \u3e 80% accuracy. Neither δD nor δ18O of aquatic insects changed following emergence from their natal site. Preservation method (ethanol or freezing) did not affect invertebrate δD or δ18O. Invertebrate δD may be a useful natural tracer of natal environment and dispersal in the Mississippi River-floodplain ecosystem and other freshwater systems where spatial variation in water δD is present

    Phylogeography of Japanese encephalitis virus:genotype is associated with climate

    The circulation of vector-borne zoonotic viruses is largely determined by the overlap in the geographical distributions of virus-competent vectors and reservoir hosts. What is less clear are the factors influencing the distribution of virus-specific lineages. Japanese encephalitis virus (JEV) is the most important etiologic agent of epidemic encephalitis worldwide, and is primarily maintained between vertebrate reservoir hosts (avian and swine) and culicine mosquitoes. There are five genotypes of JEV: GI-V. In recent years, GI has displaced GIII as the dominant JEV genotype and GV has re-emerged after almost 60 years of undetected virus circulation. JEV is found throughout most of Asia, extending from maritime Siberia in the north to Australia in the south, and as far as Pakistan to the west and Saipan to the east. Transmission of JEV in temperate zones is epidemic with the majority of cases occurring in summer months, while transmission in tropical zones is endemic and occurs year-round at lower rates. To test the hypothesis that viruses circulating in these two geographical zones are genetically distinct, we applied Bayesian phylogeographic, categorical data analysis and phylogeny-trait association test techniques to the largest JEV dataset compiled to date, representing the envelope (E) gene of 487 isolates collected from 12 countries over 75 years. We demonstrated that GIII and the recently emerged GI-b are temperate genotypes likely maintained year-round in northern latitudes, while GI-a and GII are tropical genotypes likely maintained primarily through mosquito-avian and mosquito-swine transmission cycles.
