It has been difficult to place the program as the duality of the revolutionary promises justified through buzzwords give one impression, while the lack of providing clear methods or plans for the revolutionary changes it sets out to implement gives another. Thereby, the program can be analyzed differently based on which element and aspect is focused on.B-D

    Development and Use of Mark Sense Record Cards for Recording Medical Data on Pilots Subjected to Acceleration Stress

    A TIME-HONORED system of recording medical histories and the data obtained on physical and laboratory examination has been that of writing the information on record sheets that go into a folder for each patient. In order to have information which would be more readily retrieved, 'a program was initiated in 1952 by the U. S. Naval School of Aviation Medicine in connection with their "Care of the Flyer" study to place this information on machine record cards. In 1958, a machine record card method was developed for recording medical data in connection with the astronaut selection program. Machine record cards were also developed by the Aero Medical Laboratory, Wright-Patterson AFB, Ohio, and the Aviation Medical Acceleration Laboratory, Naval Air Development Center, Johnsville, Pennsylvania, for use in connection with a variety of tests including acceleration stress.1 Therefore, a variety of systems resulted in which data of a medical nature could easily be recalled. During the NASA, Ames Research Center centrifuge studies/'S the pilot subjects were interviewed after each centrifuge run, or series of runs, and subjective information was recorded in a log book by the usual history taking methods referred to above. After the methods Were reviewed, it' was recognized that a card system would be very useful in recording data from our pilots after they had been exposed to acceleration stress. Since the acceleration stress cards already developed did not meet our requirements, it was decided a different card was needed

    Centrifuge Study of Pilot Tolerance to Acceleration and the Effects of Acceleration on Pilot Performance

    A research program the general objective of which was to measure the effects of various sustained accelerations on the control performance of pilots, was carried out on the Aviation Medical Acceleration Laboratory centrifuge, U.S. Naval Air Development Center, Johnsville, PA. The experimental setup consisted of a flight simulator with the centrifuge in the control loop. The pilot performed his control tasks while being subjected to acceleration fields such as might be encountered by a forward-facing pilot flying an atmosphere entry vehicle. The study was divided into three phases. In one phase of the program, the pilots were subjected to a variety of sustained linear acceleration forces while controlling vehicles with several different sets of longitudinal dynamics. Here, a randomly moving target was displayed to the pilot on a cathode-ray tube. For each combination of acceleration field and vehicle dynamics, pilot tracking accuracy was measured and pilot opinion of the stability and control characteristics was recorded. Thus, information was obtained on the combined effects of complexity of control task and magnitude and direction of acceleration forces on pilot performance. These tests showed that the pilot's tracking performance deteriorated markedly at accelerations greater than about 4g when controlling a lightly damped vehicle. The tentative conclusion was also reached that regardless of the airframe dynamics involved, the pilot feels that in order to have the same level of control over the vehicle, an increase in the vehicle dynamic stability was required with increases in the magnitudes of the acceleration impressed upon the pilot. In another phase, boundaries of human tolerance of acceleration were established for acceleration fields such as might be encountered by a pilot flying an orbital vehicle. A special pilot restraint system was developed to increase human tolerance to longitudinal decelerations. The results of the tests showed that human tolerance of longitudinal deceleration forces was considerably improved through use of the special restraint system

    A Flight Evaluation of an Airborne Physiological Instrumentation System, Including Preliminary Results Under Conditions of Varying Accelerations

    A physiological instrumentation system capable of recording the electrocardiogram, pulse rate, respiration rate, and systolic and diastolic blood pressures during flight has been developed. This instrumentation system was designed for use during control studies at varied levels of acceleration in order to monitor the well-being of the pilot and at the same time to obtain data for study of the relationships between his various physiological functions and his performance capability. Flights, made in a T-33 aircraft, demonstrated the ability of the system to obtain the desired physiological data in flight. The data obtained in these flights, although limited in nature, indicate a slowing of the pulse rate under the subgravity conditions of brief duration. There appeared to be a proportional nearly in-phase relationship between pulse rate and acceleration. A decrease in diastolic blood pressure together with an increase in pulse pressure was noted during subgravity conditions and an elevation of the diastolic pressure together with a decrease in pulse pressure du-ring increased accelerations. No change worthy of note was seen in the records of the systolic blood pressure, the respiration rate, or the electrocardiogram over the range of acceleration studied (0 to 3 g)

    Physiological Effects of Acceleration Observed During a Centrifuge Study of Pilot Performance

    An investigation was conducted by the National Aeronautics and Space Administration, Ames Research Center, and the Naval Air Development Center, Aviation Medical Acceleration Laboratory, to study the effects of acceleration on pilot performance and to obtain some meaningful data for use in establishing tolerance to acceleration levels. The flight simulator used in the study was the Johnsville centrifuge operated as a closed loop system. The pilot was required to perform a control task in various sustained acceleration fields typical of those that Might be encountered by a pilot flying an entry vehicle in which he is seated in a forward-facing position. A special restraint system was developed and designed to increase the pilot's tolerance to these accelerations. The results of this study demonstrated that a well-trained subject, such as a test pilot, can adequately carry out a control task during moderately high accelerations for prolonged periods of time. The maximum levels of acceleration tolerated were approximately 6 times that of gravity for approximately 6 minutes, and varied slightly with the acceleration direction. The tolerance runs were in each case terminated by the subject. In all but two instances, the cause was extreme fatigue. On two occasions the subject terminated the run when he "grayed out." Although there were subjective and objective findings involving the visual and cardiovascular systems, the respiratory system yielded the more critical limiting factors. It would appear that these limiting factors were less severe during the "eyeballs-out" accelerations when compared with the "eyeballs-in" accelerations. These findings are explained on the basis of the influence that the inertial forces of acceleration have on the mechanics of respiration. A condensed version of this report was presented at the Annual Meeting of the Aerospace Medical Association, Miami Beach, May 5-11, 1960, in a paper entitled "Ability of Pilots to Perform a Control Task in Various Sustained Acceleration Fields.

    Real-World Goal Setting and Use of Outcome Measures According to the International Classification of Functioning, Disability and Health: A European Survey of Physical Therapy Practice in Multiple Sclerosis

    Goal setting is a core component of physical therapy in multiple sclerosis (MS). It is unknown whether and to what extent goals are set at different levels of the International Classification of Functioning, Disability and Health (ICF), and whether, and to which, standardized outcome measures are used in real life for evaluation at the different ICF levels. Our aim was to describe the real-world use of goal setting and outcome measures in Europe. An online cross-sectional survey, completed by 212 physical therapists (PTs) specialized in MS from 26 European countries, was conducted. Differences between European regions and relationships between goals and assessments were analyzed. PTs regularly set goals, but did not always apply the Specific, Measurable, Achievable, Realistic, Timed (SMART) criteria. Regions did not differ in the range of activities assessed, but in goals set (e.g., Western and Northern regions set significantly more goals regarding leisure and work) and outcome measures used (e.g., the Berg Balance Scale was more frequently used in Northern regions). Quality of life was not routinely assessed, despite being viewed as an important therapy goal. Discrepancies existed both in goal setting and assessment across European regions. ICF assists in understanding these discrepancies and in guiding improved health-care for the future.</jats:p

    Gene expression differences in relation to age and social environment in queen and worker bumble bees

    Eusocial insects provide special insights into the genetic pathways influencing aging because of their long-lived queens and flexible aging schedules. Using qRT-PCR in the primitively eusocial bumble bee Bombus terrestris (Linnaeus), we investigated expression levels of four candidate genes associated with taxonomically widespread age-related pathways (coenzyme Q biosynthesis protein 7, COQ7; DNA methyltransferase 3, Dnmt3; foraging, for; and vitellogenin, vg). In Experiment 1, we tested how expression changes with queen relative age and productivity. We found a significant age-related increase in COQ7 expression in queen ovary. In brain, all four genes showed higher expression with increasing female (queen plus worker) production, with this relationship strengthening as queen age increased, suggesting a link with the positive association of fecundity and longevity found in eusocial insect queens. In Experiment 2, we tested effects of relative age and social environment (worker removal) in foundress queens and effects of age and reproductive status in workers. In this experiment, workerless queens showed significantly higher for expression in brain, as predicted if downregulation of for is associated with the cessation of foraging by foundress queens following worker emergence. Workers showed a significant age-related increase in Dnmt3 expression in fat body, suggesting a novel association between aging and methylation in B. terrestris. Ovary activation was associated with significantly higher vg expression in fat body and, in younger workers, in brain, consistent with vitellogenin's ancestral role in regulating egg production. Overall, our findings reveal a mixture of novel and conserved features in age-related genetic pathways under primitive eusociality
