    An unusual foreign body of esophagus

    We report a rare case of an unusually long foreign body (Datun) impacted in the esophagus of a 56 year-old gentleman. He was literate, without any psychiatric illness and had been using ā€œNeemā€ (Azadirachta indica) stick for cleaning his teeth for the past twenty years. Neem sticks are used for brushing teeth, perhaps one of the earliest and very effective dental care. On closer questioning he revealed his habit of passing the Neem stick into his throat with the aim of cleaning it too while cleaning his teeth. He presented to our emergency early in the morning with this strange long foreign body impacted in his esophagus which was removed successfully using a Jacksonā€™s adult rigid oesophagoscope. We believe this to be the first case of such an unusually long foreign body to be reported in the literature

    Influence of Bulk Fluid Velocity on the Efficiency of Electrohydrodynamic Pumping

    The efficiency of conversion of electrical power into fluidic power in an electrohydrodynamic (EHD) pump depends on the bulk fluid velocity. An analytical formulation is developed for calculation of the efficiency of an EHD pump, with and without the presence of a superimposed flow due to an externally imposed pressure gradient. This formulation is implemented into a numerical model, which is used to investigate the effect of bulk fluid velocity on the efficiency of the EHD action. In particular, the net flow due to the combined action of EHD and a positive or negative external pressure gradient is computed. Both ion-drag pumps and induction EHD pumps are considered. Pumps based on the ion-drag principle that are studied include a one-dimensional pump, a twodimensional pump driven by a stationary potential gradient, and another driven by a traveling potential wave. Two-dimensional repulsion-type and attraction-type induction pumping caused by a gradual variation in the electrical conductivity of the fluid is also investigated. The efficiency of EHD pumps exhibited a strong dependence on bulk fluid velocity: for the two-dimensional steady ion-drag pump, for example, the efficiency increased from less than 2% to 22% under the influence of an external pressure gradient. The corresponding increase in efficiency for a two-dimensional repulsion-type EHD pump was from 0.26% to 24.5%

    Development of representative conceptual groundwater flow model is an important step before translating it into a numerical model. In this paper, a methodology for development of conceptual groundwater flow model has been presented in which spatially distributed values for groundwater recharge has been utilized instead of lump sum average values of recharge normally obtained by water budgeting method. The study also extensively uses GIS for preprocessing of hydrological, hydrogeological and geological data. In our view, the methodology presented here provides better tools for building a conceptual model for tackling groundwater modeling problems.     Analysis and Prediction of Constriction Resistance for Contact Between Rough Engineering Surfaces

    The constriction resistance of a semi-infinite cylinder terminating in the frustum of a cone in a gaseous environment is analyzed numerically. The variation of constriction resistance with the ratio of contact radius to cylinder radius, the cone angle, and the gas-to-substrate thermal conductivity ratio is investigated. Nonlinear curve fitting is used to develop a comprehensive predictive correlation for the constriction resistance as a function of these parameters. The parameters are investigated over a wide range of values covering the range expected in practical applications. The correlation could be used in conjunction with an appropriate surface deformation model to predict the thermal contact resistance between rough metallic surfaces

    Source of the Bursty Bulk Flow Diffuse Aurora: Electrostatic Cyclotron Harmonic and Whistler Waves in the Coupling of Bursty Bulk Flows to Auroral Precipitation

    Electron cyclotron harmonic (ECH) and whistler chorus waves are recognized as the two mechanisms responsible for the resonant waveparticle interactions necessary to precipitate plasma sheet electrons into the ionosphere, producing the diffuse Aurora. Previous work has demonstrated ECH waves dominate electron scattering at L shells >8, while whistler chorus dominates scattering at L shells L 1, consistent with electron betatron acceleration. Here, however, we nd whistler chorus emissions throughout an interval of fast ows where Te,/Te,||< 1. Parallel electron beams account for the enhanced parallel electron temperature and serve as the instability mechanism for the whistler chorus. The parallel electron beams and associated cigarshaped distributions are consistent with Fermi acceleration at dipolarizations in fast ows. We demonstrate that the scattering efciency of the whistler chorus exceeds that of ECH waves, which THEMIS also detects during the fast ows. The obliquity of the whistler waves permits efcient scattering of lowerenergy electrons into the diffuse aurora. We conclude that Fermi acceleration of electrons provides one important freeenergy source for the waveparticle interactions responsible for coupling plasma sheet electrons into the diffuse aurora during substorm conditions

    Is BMI alone a sufficient outcome to evaluate interventions for child obesity?

    BACKGROUND: BMI is often used to evaluate the effectiveness of childhood obesity interventions, but such interventions may have additional benefits independent of effects on adiposity. We investigated whether benefits to health outcomes following the Mind, Exercise, Nutritionā€¦Do It! (MEND) childhood obesity intervention were independent of or associated with changes in zBMI. METHODS: A total of 79 obese children were measured at baseline; 71 and 42 participants were followed-up at 6 and 12 months respectively, and split into four groups depending on magnitude of change in zBMI. Differences between groups for waist circumference, cardiovascular fitness, physical and sedentary activities, and self-esteem were investigated. RESULTS: Apart from waist circumference and its z-score, there were no differences or trends across zBMI subgroups for any outcome. Independent of the degree of zBMI change, benefits in several parameters were observed in children participating in this obesity intervention. CONCLUSION: We concluded that isolating a single parameter like zBMI change and neglecting other important outcomes is restrictive and may undermine the evaluation of childhood obesity intervention effectiveness
