    Joint British Diabetes Societies for Inpatient Care (JBDS-IP) Clinical Guideline Inpatient care of the frail older adult with diabetes: an Executive Summary

    We present an Executive Summary of a guideline produced by a Joint British Diabetes Societies for Inpatient Care Writing Group for managing frail older inpatients with diabetes. This represents a multidisciplinary stakeholder consensus document providing more than 100 recommendations in eight areas: functional assessment and detection of frailty; preventative care: assessing risk factors and avoiding hospital admissions; general inpatient management principles; managing therapy choices for the frail older inpatient with diabetes; managing associated comorbidities and concerns; pre-operative assessment and care; discharge planning and principles of follow-up; and end of life care. The document is intended to guide effective clinical decision-making in an inpatient setting and is supported by four appendices: Appendix 1, STOPPFRAIL criteria; Appendix 2, Acute care toolkit 3-Royal College of Physicians; Appendix 3, a description of physical performance and frailty measures for routine NHS application; and Appendix 4, Inpatient Frailty Care Pathway-template. This document is expected to enhance clinical outcomes and overall health status for this vulnerable inpatient population of older people with diabetes.     Detecting and Characterizing Small Dense Bipartite-like Subgraphs by the Bipartiteness Ratio Measure

    We study the problem of finding and characterizing subgraphs with small \textit{bipartiteness ratio}. We give a bicriteria approximation algorithm \verb|SwpDB| such that if there exists a subset SS of volume at most kk and bipartiteness ratio θ\theta, then for any 0<ϵ<1/20<\epsilon<1/2, it finds a set SS' of volume at most 2k1+ϵ2k^{1+\epsilon} and bipartiteness ratio at most 4θ/ϵ4\sqrt{\theta/\epsilon}. By combining a truncation operation, we give a local algorithm \verb|LocDB|, which has asymptotically the same approximation guarantee as the algorithm \verb|SwpDB| on both the volume and bipartiteness ratio of the output set, and runs in time O(ϵ2θ2k1+ϵln3k)O(\epsilon^2\theta^{-2}k^{1+\epsilon}\ln^3k), independent of the size of the graph. Finally, we give a spectral characterization of the small dense bipartite-like subgraphs by using the kkth \textit{largest} eigenvalue of the Laplacian of the graph.Comment: 17 pages; ISAAC 201

    First Demonstration of a Pixelated Charge Readout for Single-Phase Liquid Argon Time Projection Chambers

    Liquid Argon Time Projection Chambers (LArTPCs) have been selected for the future long-baseline Deep Underground Neutrino Experiment (DUNE). To allow LArTPCs to operate in the high-multiplicity near detector environment of DUNE, a new charge readout technology is required. Traditional charge readout technologies introduce intrinsic ambiguities, combined with a slow detector response, these ambiguities have limited the performance of LArTPCs, until now. Here, we present a novel pixelated charge readout that enables the full 3D tracking capabilities of LArTPCs. We characterise the signal to noise ratio of charge readout chain, to be about 14, and demonstrate track reconstruction on 3D space points produced by the pixel readout. This pixelated charge readout makes LArTPCs a viable option for the DUNE near detector complex.Comment: 13 pages, 9 figure

    Do bilinguals have different concepts? The case of shape and material in Japanese L2 users of English

    An experiment investigated whether Japanese speakers’ categorisation of objects and substances as shape or material is influenced by acquiring English, based on Imai and Gentner (1997). Subjects were presented with an item such as a cork pyramid and asked to choose between two other items that matched it for shape (plastic pyramid) or for material (piece of cork). The hypotheses were that for simple objects the number of shape-based categorisations would increase according to experience of English and that the preference for shape and material-based categorisations of Japanese speakers of English would differ from mono¬lingual speakers of both languages. Subjects were 18 adult Japanese users of English who had lived in English-speaking countries between 6 months and 3 years (short-stay group), and 18 who had lived in English-speaking countries for 3 years or more (long-stay group). Both groups achieved above criterion on an English vocabulary test. Results were: both groups preferred material responses for simple objects and substances but not for complex objects, in line with Japanese mono¬linguals, but the long-stay group showed more shape preference than the short-stay group and also were less different from Americans. These effects of acquiring a second language on categorisation have implications for conceptual representation and methodology

    Perfect Lattice Actions for Staggered Fermions

    We construct a perfect lattice action for staggered fermions by blocking from the continuum. The locality, spectrum and pressure of such perfect staggered fermions are discussed. We also derive a consistent fixed point action for free gauge fields and discuss its locality as well as the resulting static quark-antiquark potential.     Epstein-Barr virus (EBV) causes infectious mononucleosis and is associated with various malignancies, including Burkitt's lymphoma and nasopharyngeal carcinoma. Like all herpesviruses, the EBV life cycle alternates between latency and lytic replication. During latency, the viral genome is largely silenced by host-driven methylation of CpG motifs and, in the switch to the lytic cycle, this epigenetic silencing is overturned. A key event is the activation of the viral BRLF1 gene by the immediate-early protein Zta. Zta is a bZIP transcription factor that preferentially binds to specific response elements (ZREs) in the BRLF1 promoter (Rp) when these elements are methylated. Zta's ability to trigger lytic cycle activation is severely compromised when a cysteine residue in its bZIP domain is mutated to serine (C189S), but the molecular basis for this effect is unknown. Here we show that the C189S mutant is defective for activating Rp in a Burkitt's lymphoma cell line. The mutant is compromised both in vitro and in vivo for binding two methylated ZREs in Rp (ZRE2 and ZRE3), although the effect is striking only for ZRE3. Molecular modeling of Zta bound to methylated ZRE3, together with biochemical data, indicate that C189 directly contacts one of the two methyl cytosines within a specific CpG motif. The motif's second methyl cytosine (on the complementary DNA strand) is predicted to contact S186, a residue known to regulate methyl-ZRE recognition. Our results suggest that C189 regulates the enhanced interaction of Zta with methylated DNA in overturning the epigenetic control of viral latency. As C189 is conserved in many bZIP proteins, the selectivity of Zta for methylated DNA may be a paradigm for a more general phenomenon

    Synthesis of a 12R-type hexagonal perovskite solid solution Sr3NdNb3-xTixO12-delta and the influence of acceptor doping on electrical properties

    A solid solution forms for Sr3NdNb3−xTixO12−δ with approximate limits 0 ≤ x ≤ 0.06. The system crystallizes with a 12R-type hexagonal perovskite structure in the space group R[3 with combining macron], as determined by neutron diffraction and selected area electron diffraction. The electrical properties of the end members have been investigated by impedance spectroscopy in the temperature range 550–800 °C under various gas atmospheres and as a function of oxygen and water-vapour partial pressure. Proton transport dominates under wet oxidising conditions in the temperature range 550–700 °C, as confirmed by the H+/D+ isotope effect. Acceptor doping considerably enhances proton conductivity with a value of 3.3 × 10−6 S cm−1 for the bulk response of x = 0.06 at 700 °C in moistened air. The presence of a −¼ slope for both doped and undoped samples in the range 10−19 ≤ pO2 ≤ 10−8 atm at 900 °C indicates n-type transport under reducing conditions following the extrinsic model attributable to acceptor centres. The conductivity is essentially independent of pO2 at 600 °C under dry oxidising conditions, consistent with oxide-ion transport; a positive power-law dependence at higher temperature indicates extrinsic behaviour and a significant electron–hole contribution. The dielectric constant at RT of nominally stoichiometric Sr3NdNb3O12 is εr ∼ 37, with a moderately high quality factor of Q × f ∼ 16 400 GHz at fr ∼ 6.4 GHz. The temperature coefficient of resonant frequency of x = 0 is τf ∼ 12 ppm °C−1, which lowers to −3 ppm °C−1 for the Ti-doped phase x = 0.06
