    A Survey of Practitioner’s Knowledge of Psychiatric Medication Costs

    Introduction. Escalating medical costs continue to be an issue facing contemporary medicine. One factor contributing to this escalation may be physicians’ knowledge of medication costs. As physicians increasingly face opportunities to treat a variety of symptoms and conditions in a single patient, including co-morbid psychiatric disorders or complications, accurate knowledge of medication costs becomes increasingly important. Methods. Resident and attending physicians (N = 16) across the disciplines of internal medicine, psychiatry, and combined internal medicine/psychiatry from a large, mid-western medical school were surveyed on the costs of several medications that are used to manage physical and psychiatric symptoms. Results. Differences were found in the perceived estimated cost of medications among practitioners particularly with specialty internal medicine training as compared to those with additional psychiatric training/experience. Trends also were noted across practitioners with psychiatric and internal medicine/psychiatry training. Conclusions. The breadth of training and experience can affect accuracy in estimating anticipated costs of medication regimens

    Insulin-like growth factor 2 (IGF2) protects against Huntington's disease through the extracellular disposal of protein aggregates

    Impaired neuronal proteostasis is a salient feature of many neurodegenerative diseases, highlighting alterations in the function of the endoplasmic reticulum (ER). We previously reported that targeting the transcription factor XBP1, a key mediator of the ER stress response, delays disease progression and reduces protein aggregation in various models of neurodegeneration. To identify disease modifier genes that may explain the neuroprotective effects of XBP1 deficiency, we performed gene expression profiling of brain cortex and striatum of these animals and uncovered insulin-like growth factor 2 (Igf2) as the major upregulated gene. Here, we studied the impact of IGF2 signaling on protein aggregation in models of Huntington's disease (HD) as proof of concept. Cell culture studies revealed that IGF2 treatment decreases the load of intracellular aggregates of mutant huntingtin and a polyglutamine peptide. These results were validated using induced pluripotent stem cells (iPSC)-derived medium spiny neurons from HD patients and spinocerebellar ataxia cases. The reduction in the levels of mutant huntingtin was associated with a decrease in the half-life of the intracellular protein. The decrease in the levels of abnormal protein aggregation triggered by IGF2 was independent of the activity of autophagy and the proteasome pathways, the two main routes for mutant huntingtin clearance. Conversely, IGF2 signaling enhanced the secretion of soluble mutant huntingtin species through exosomes and microvesicles involving changes in actin dynamics. Administration of IGF2 into the brain of HD mice using gene therapy led to a significant decrease in the levels of mutant huntingtin in three different animal models. Moreover, analysis of human postmortem brain tissue and blood samples from HD patients showed a reduction in IGF2 level. This study identifies IGF2 as a relevant factor deregulated in HD, operating as a disease modifier that buffers the accumulation of abnormal protein species

    Unveiling Lived Experiences: Exploring the Health and Lifestyle Effects of COVID-19 on Healthcare Workers

    The COVID-19 pandemic brought about significant life disruptions among healthcare workers (HCWs), including changes in weight, eating habits, and physical activity. This qualitative study sought to evaluate the initial and longitudinal effects of health habits among HCWs throughout the pandemic. Data were collected through Qualtrics surveys at three points over a 2-year period with questions asking participants (n = 234) to describe whether they experienced changes in weight, eating behaviors, and physical activity and why they believe these changes occurred. The open-ended responses were analyzed following the summative content analysis approach. Four key themes emerged: (1) problematic eating patterns and habits, (2) disruptions in physical activity, (3) alterations in work environment and schedule, and (4) declines in mental health. Respondent reflections highlight the immediate and long-term pandemic-related effects on weight status for some, attributed to alterations in routines and health habits. Other HCWs reported a “reset” or indicated their habits may have been initially disrupted but normalized or improved over the 2-year time span. Findings underscore the need for strategies that support the physical and mental health of healthcare workers

    A Study of United States Registered Dietitian Nutritionists during COVID-19: From Impact to Adaptation

    The COVID-19 pandemic introduced an unprecedented health crisis, requiring many Registered Dietitian Nutritionists (RDNs) to expand their duties and services, while other RDNs faced unemployment, reduced hours, and changes to their work environment. This study evaluated whether the pandemic impacted RDNs’ weight, eating behaviors, and psychological factors, and whether professional training as an RDN was perceived as a protective factor in maintaining healthy habits. A 57-item, cross-sectional, online questionnaire including open-ended questions was distributed to RDNs residing in the United States. Over two months (January 2021 to February 2021), 477 RDNs completed the questionnaire. Among RDNs, 68.5% reported no weight change, 21.4% reported weight gain greater than 5 pounds, and 10.3% reported weight loss greater than 5 pounds. Approximately 75% (n = 360) reported their RDN professional training equipped them with the skills needed to maintain healthy eating behaviors. Reduced physical activity and mental health were the top qualitative themes that emerged regarding reasons for weight change. These findings suggest that RDN professional practice skills may have conferred some personal health benefits, as evidenced by smaller weight gains, the maintenance of healthy habits, and fewer reporting psychological effects relative to the general population and other health professionals, thereby limiting the impact of pandemic-induced work and life disruptions

    COVID-19 Pandemic-related Changes in Weight, Health Behaviors, and Professional Practice: A Study of Dietitians

    Learning Outcome Describe how the COVID-19 Pandemic-impacted weight, health behaviors, and professional practice of RDNs. Background The COVID-19 pandemic remains an unprecedented health crisis requiring many RDNs to expand their duties and services, while other RDNs faced unemployment, reduced hours, and changes to their work environment. We evaluated how COVID-19 impacted RDNs\u27 weight and eating behaviors, the relationship between psychological factors and weight change, and whether professional training as an RDN was perceived as a protective factor in maintaining healthy habits. Methods A 57-item cross-sectional online survey distributed among RDNs residing in the U.S. captured health behaviors and tools to measure anxiety (General Anxiety Disorder-7) and insomnia (Insomnia Severity Index). Adjusting for the effects of age, ANCOVA was used to determine differences in anxiety or insomnia symptoms in those who lost, gained, or remained weight neutral. Results 477 RDNs (96% female, 94% white) completed the questionnaire. 68.5% of RDNs reported no weight change, 21.4% reported weight gain greater than 5 pounds, and 10.3% reported weight loss greater than 5 pounds. There were no statistically significant differences between anxiety, insomnia, and weight change. Although 360 (75.5%) reported their RDN professional training equipped them with the skills needed to maintain healthy eating behaviors, an additional 17.6% reported that circumstances made it difficult to put these skills into practice consistently. Conclusion These findings suggest that the professional practice skills of the RDN may have conferred some personal health benefits, as evidenced by smaller weight gains experienced by RDNs relative to the general population and other health professionals, thereby limiting the impact of pandemic-induced work and life disruptions.     Introduction: Essential workers serving the healthcare sector experienced significantly higher burdens and life demands due to working under strenuous and uncertain conditions during the COVID-19 pandemic. Chronic stress and lifestyle disruption can lead to changes in eating patterns, weight, and physical activity which can be associated with health outcomes. This study aimed to quantify how psychological stress assessed during the height of the pandemic (spring 2020) was associated with workers\u27 self-reported weight change 6 months later. Methods: 242 workers completed online questionnaires focused on psychological stress at baseline and weight and lifestyle six-months later. Using one-way ANOVA, differences in depression (PHQ-2), anxiety (GAD-7), insomnia (ISI), and post-traumatic stress (IES-R) symptoms were compared across groups who lost weight (\u3c5 pounds), gained weight (\u3e5 pounds), and those whose weight did not change. Results: Of the sample, 14% reported weight loss greater than five pounds, 46% reported no weight change and 40% report weight gain greater than five pounds. Compared to those with no weight change, people who reported weight gain suffered from significantly greater insomnia (mean difference=2.54, SE=0.82; p=.007). Higher anxiety tended to be reported in those who lost weight (p=.15). No other differences in psychological symptoms by weight change were found. Discussion: These findings showed insomnia is particularly problematic in individuals with weight gain and anxiety may contribute to unintentional weight loss. These study findings contribute to understanding how psychological stress relates to pandemic-related changes in health behaviors and weight change among essential workers

    Empowering Community Leadership: Perspectives of Peer Mentors Facilitating a Food Pantry-Based Nutrition Education Program

    Peer Mentors (PMs) are valuable health educators within food-insecure communities; however, little is known about PMs’ perspectives and experiences after serving in their peer mentor role. Therefore, this qualitative study explored PMs’ (n = 10) perceptions and analyzed data using thematic analysis based on descriptive phenomenology. Four themes emerged: (1) Successes and Struggles in Sharing Nutrition Knowledge; (2) Establishing a Conducive Learning Environment; (3) The Peer Mentor and Mentee Connection: Impact of Shared Experiences; (4) Empowerment of the Peer Mentor Experience. PMs have many advantages; however, more research is needed to evaluate the sustainability and efficacy of PMs within food-insecure communities

    Learning Environment and Evidence among Professionals and Students Satisfaction (LEAPS), Experienced During the COVID-19 Pandemic

    The COVID-19 pandemic required schools to transition courses to an online platform. This shift to Emergency Remote Teaching (ERT) created gaps in the literature about its impact on students. The purpose of this study was to test the relationship between learner and instructional attributes and learner satisfaction with ERT. A modified version of the Student Satisfaction Survey assessed learner and instructional attributes and learner satisfaction among a convenience sample of 12 graduate and 83 undergraduate nursing students. Open-ended questions assessed students\u27 responses to their satisfaction with ERT. Multiple regression analysis was used to test associations of learner and instructional attributes with student satisfaction. Overall satisfaction with ERT was neutral with a mean of 2.76 on a 1 to 5 scale; students rated instructional attributes higher with a mean of 3.64. Instructional engagement/technology use (single factor) and learner technology competence were associated with student satisfaction, beta = 0.93(0.09), p = .008, respectively. Between-class technology use and prior experience with online courses were not associated with student satisfaction, beta = -0.08(0.09), p = .379, beta = 0.26(0.15), p = .079, respectively. Qualitative findings revealed faculty engagement was a major determinant in learner satisfaction with ERT. Supporting faculty competence for the use of technology may increase learner satisfaction with ERT
