    Neonatal Intensive Care Units: Is the Level of Utilization Still Paralleled by Infant Mortality?

    Background. This research investigated the relationship of maternal place of residence with the utilization of neonatal intensive care units (NICU) and whether maternal residence was related to infant mortality in two contiguous Swiss cantons, Vaud and Valais, relying on the sante tertiary NICU. Previous works have shown that Infant mortality is close to the Swiss rate in Vaud, but elevated in Valals. Methods. Analyses were based on linked birth arid death certificates relating to the 57962 single Fivebirths to mothers resident in Vaud or Valais delivered in hospitals over the 1979-1985 period. Data on utilization were retrospectively collected from NICU admissions registers and linked to birth certificates. Results. Results of logistic regression models pointed to a large difference for all blrthweight groups, in NICU utilization between Vaud and Valais after adjustment for risk factors abstracted from the birth certificate, when tertiary NICIJ beds were considered. When the definition of NICU utilization was enlarged to all beds of an Identified NICU, irrespective of the location of the unit and of the intensity of care provided to admitted newborns, there was a clear reduction in geographical variations. Neonatal and infant mortality were significantly higher in the lower NICU utilization region only among newborns weighing ≫ 2500 g at birth. Conclusion. We found no difference in infant mortality (odds ratio 1.0) among < 2500 g newborns residing in regions characterized by a large difference in tertiary NICU utilization. Nevertheless, the limited sample size did not permit specific analyses of lower birthweight (e.g. < 1500 g) newborns. Replication of such research in other settings based on larger samples is neede

    The Roots of the Widening Racial Wealth Gap: Explaining the Black-White Economic Divide

    Growing concerns about wealth inequality and the expanding racial wealth gap have in recent years become central to the debate over whether our nation is on a sustainable economic path. This report provides critical new information about what has fueled the racial wealth gap and points to policy approaches that will set our country in a more equitable and prosperous direction.Looking at the same set of families over a 25-year period (1984-2009), our research offers key insight into how policy and the real, lived-experience of families in schools, communities, and at work affect wealth accumulation. Tracing the same households during that period, the total wealth gap between white and African-American families nearly triples, increasing from 85,000in1984to85,000 in 1984 to 236,500 in 2009. To discover the major drivers behind this dramatic $152,000 increase, we tested a wide range of possible explanations, including family, labor market, and wealth characteristics. This allowed us, for the first time, to identify the primary forces behind the racial wealth gap. Our analysis found little evidence to support common perceptions about what underlies the ability to build wealth, including the notion that personal attributes and behavioral choices are key pieces of the equation. Instead, the evidence points to policy and the configuration of both opportunities and barriers in workplaces, schools, and communities that reinforce deeply entrenched racial dynamics in how wealth is accumulated and that continue to permeate the most important spheres of everyday life

    Rainbow scattering in the gravitational field of a compact object

    We study the elastic scattering of a planar wave in the curved spacetime of a compact object such as a neutron star, via a heuristic model: a scalar field impinging upon a spherically symmetric uniform density star of radius R and mass M. For R<rc, there is a divergence in the deflection function at the light-ring radius rc ¼ 3GM=c2, which leads to spiral scattering (orbiting) and a backward glory; whereas for R>rc, there instead arises a stationary point in the deflection function which creates a caustic and rainbow scattering. As in nuclear rainbow scattering, there is an Airy-type oscillation on a Rutherford-like cross section, followed by a shadow zone. We show that, for R ∼ 3.5GM=c2, the rainbow angle lies close to 180°, and thus there arises enhanced backscattering and glory. We explore possible implications for gravitational wave astronomy and dark matter models
