    Quality of life in patients aged 65 years or over diagnosed with SAHS with CPAP treatment

    Fundamento. El síndrome de apneas-hipopneas del sueño (SAHS) es una enfermedad crónica que aumenta su prevalencia con la edad. El tratamiento incluye dispositivos de presión positiva continua en la vía aérea (CPAP). Los estudios sobre el uso de CPAP en los ancianos son escasos. El objetivo principal de este estudio fue determinar si la CPAP contribuye a mejorar la calidad de vida relacionada con la salud (CVRS) en pacientes ancianos con SAHS. Sujetos y métodos. Estudio prospectivo, cuasi-experimental, evaluación previa / posterior a la intervención de una cohorte de pacientes ≥ 65 años de edad con diagnóstico de SAHS por polisomnografía (PSG), a los que se les evaluó la CVRS utilizando los cuestionarios SF-36 y SAQLI y la somnolencia diurna utilizando el cuestionario Epworth Sleepiness Scale (ESS), antes y después de tres meses de terapia con CPAP. Resultados. Se estudiaron 123 pacientes, el 65% (80) hombres y el 35% (43) mujeres, edad media 72,20 ± 4,53 años y un IMC medio de 33,49 ± 5,81 kg/m2. Después de tres meses de terapia, el uso promedio de CPAP fue de 6,33 ± 1,36 horas/día. Se demostró la efectividad de la CPAP para controlar el SAHS (IAH post-terapia fue de 4,90 ± 6,16; IC95% = 3,79 a 6,01; p < 0,001). Después de la intervención, todas las categorías del SF-36 y SAQLI mejoraron significativamente (p <0,001), así como las puntuaciones de ESS (diferencia de medias = 5,19 ± 4,66; IC95%: 4,36 a 6,02; p < 0,001) después de la intervención. Conclusiones. La terapia con CPAP en personas mayores con SAHS ayuda a mejorar su CVRS y reduce la somnolencia diurna.Background. Sleep apnea-hypopnea syndrome (SAHS) is a chronic illness that increases in prevalence with age. Treatment includes continuous positive airway pressure (CPAP) devices. Studies about the use of CPAP in the elderly are scarce. The main objective of this study was to determine if CPAP contributes to improvement in health-related quality of life (HRQL) in elderly patients with OSA. Material and methods. Prospective study, pre/post-intervention assessment of a cohort of patients ≥ 65 years of age with SAHS diagnosis by polysomnography (PSG) and treated with CPAP, physical independence and good cognitive status. Determined HRQL before and after three months with CPAP, using SF-36 and SAQLI questionnaires. Effect of CPAP on daytime sleepiness was assessed with Epworth Sleepiness Scale (ESS). Results. 123 patients were studied, 80 (65%) male and 43 (35%) women, mean age 72.20 ± 4.53 years and mean IMC 33.49 ± 5.81 kg/m2. After three months of therapy, the mean CPAP usage 6.33 ± 1.36 hours/day. The effectiveness of CPAP to control the SAHS was demonstrated (AHI mean post-therapy was 4.90 ± 6.16; IC95%: 3.79 to 6.01; p < 0.001). Post-intervention, the categories of the SF-36 and SAQLI improved meaningfully (p < 0.001). The ESS scores also improved significantly (difference = 5.19 ± 4.66; IC95% = 4.36 a 6.02; p < 0.001) after the intervention. Conclusions. Therapy with CPAP in the elderly with SAHS helps improve their HRQL and reduces daytime sleepiness

    On the origin of the X-ray emission from a narrow-line radio quasar at z ; 1

    We present new XMM?Newton X-ray observations of the z = 1.246 narrow-line radio quasar RX J1011.2+5545 serendipitously discovered by ROSAT. The flat X-ray spectrum previously measured by ROSAT and ASCA is shown to be the result of a steep ? 1.8 power-law spectrum seen through a moderate intrinsic absorbing column (N H ? 4 × 1021 cm?2). The position of the X-ray source is entirely coincident with the nucleus of the radio source that we have resolved in new sensitive VLA observations at 3.6 and 6 cm, implying that scattering in the radio lobes is not responsible for the bulk of X-ray emission. In the EPIC pn image, a faint patch of X-ray emission is apparent 14 arcsec to the north-east of the main X-ray source. The former is positionally coincident with an apparently extended optical object with R ? 21.9, but there is no associated radio emission, thus ruling out the possibility that this represents a hotspot in a jet emanating from the primary X-ray source. No reflection features are detected in the X-ray spectrum of the narrow-line radio quasar, although an Fe line with an equivalent width of up to 600 eV cannot be ruled out.The work reported herein is based partly on observations obtained with XMM–Newton, an ESA science mission with instruments and contributions directly funded by ESA member states and the USA (NASA).     Profibrotic role of inducible heat shock protein 90α isoform in systemic sclerosis

    Systemic sclerosis (SSc) is an autoimmune disease that affects skin and multiple internal organs. TGF-β, a central trigger of cutaneous fibrosis, activates fibroblasts with the involvement of the stress-inducible chaperone heat shock protein 90 isoform α (Hsp90α). Available evidence supports overexpression and secretion of Hsp90α as a feature in profibrotic pathological conditions. The aim of this work is to investigate the expression and function of Hsp90α in experimental models of skin fibrosis such as human fibroblasts, C57BL/6 mice, and in human SSc. For this purpose, we generated a new experimental model based on doxorubicin administration with improved characteristics with respect to the bleomycin model. We visualized disease progression in vivo by fluorescence imaging. In this work, we obtained Hsp90α mRNA overexpression in human skin fibroblasts, in bleomycin- and doxorubicin-induced mouse fibrotic skin, and in lungs of bleomycin- and doxorubicin-treated mice. Hsp90α-deficient mice showed significantly decreased skin thickness compared with wild-type mice in both animal models. In SSc patients, serum Hsp90α levels were increased in patients with lung involvement and in patients with the diffuse form of SSc (dSSc) compared with patients with the limited form of SSc. The serum Hsp90α levels of patients dSSc were correlated with the Rodnan score and the forced vital capacity variable. These results provide new supportive evidence of the contribution of the Hsp90α isoform in the development of skin fibrosis.     Bonding structure and hydrogen content in silicon nitride thin films deposited by the electron cyclotron resonance plasma method

    The bonding structure and hydrogen content of amorphous hydrogenated silicon nitride (a-SiNx:H) thin films have been investigated by infrared spectroscopy and ion beam techniques. Electron cyclotron resonance plasma enhanced chemical vapor deposition was used to produce these films under different values of gas flow ratio, deposition temperature, and microwave power. The amount of bonded hydrogen was calculated from the N-H and Si-H infrared absorption bands. An increase of the SiH4 partial pressure during deposition was found to have the same effect on the H content as an increase of the substrate temperature: both cause a decrease of the N-H bond density and an increase in the number of Si-H bonds. This is explained by a competitive process in the formation of N-H and Si-H bonds during the growth of the film, whereby Si-H bonds are favored at the expense of N-H bonds when either the SiH4 flow or the substrate temperature are increased. Such tendency to chemical order is compared with previous results in which the same behavior was induced by thermal annealing or ion beam bombardment

    Lymphedema, secondary to breast cancer (BCRL), is the abnormal accumulation of protein-rich fluid in the interstitium caused by a malfunction of the lymphatic system. It causes swelling, deficiencies in upper limb functions and structures, sensory pain and emotional alterations, which have a chronic course and affect the upper limb’s functionality. This study aims to verify the efficacy and efficiency in the upper limb´s functionality of a protocolized experimental approach based on occupational therapy, TAPA (activity-oriented proprioceptive antiedema therapy), in the rehabilitation of BCRL in stages I and II, comparing it with the conservative treatment considered as the standard, complex decongestive therapy (CDT), through a multicenter randomized clinical trial.The study has been financed in the call for competitive competition of research and innovation projects in the field of Primary Care, Regional Hospitals and High Resolution Hospital Centers of the Public Health System of Andalusia for the year 2021, of the Andalusian Public Foundation Progress and Health, according to the definitive list of funded projects, published on 23 December 2021, with EXP.     Alterations derived from lymphedema in the upper-limb secondary to breast cancer-related lymphedema (BCRL) decrease the health-related quality of life (HRQoL), but there is limited evidence of the impact of the different interventions on it. The aim of this research was to compare the effect of conventional treatment with another treatment based on Activity-Oriented Antiedema Proprioceptive Therapy (TAPA) on HRQoL in women diagnosed with BCRL.This research was funded by the Call for research and innovation projects in the field of primary care, regional hospitals and high-resolution hospital centres of the Public Health System of Andalusia in 2021 by the Progreso y Salud Foundation, of the Ministry of Health and Families of the Junta de Andalucía, with EXP.     Integrando escalas y métodos LTER para comprender la dinámica global de un espacio protegido de montaña: el Parque Nacional de Ordesa y Monte Perdido.

    Los espacios protegidos, por el hecho de albergar una gran geo-biodiversidad y asegurar una baja intervención humana, constituyen lugares muy adecuados para el seguimiento de organismos y procesos a escala ecológica, así como para la obtención de series temporales largas a escala geológica. En el marco de la red LTER-España, el Parque Nacional de Ordesa y Monte Perdido (PNOMP) y el Instituto Pirenaico de Ecología-CSIC están impulsando estudios para la detección de cambios a distintas escalas mediante variados métodos y aproximaciones. Destacamos aquí los más consolidados, entre los que se encuentran los análisis de registros de sedimentos en lagos, espeleotemas en cuevas, la dinámica de uno de los pocos glaciares activos de la Península ibérica, el análisis físico-químico de aguas corrientes e ibones de alta montaña, el registro del cambio climático actual en árboles longevos, la afección que éste ejerce sobre masas actuales de pinos en el límite superior del bosque y de abetales en zonas húmedas, la matorralización de algunos pastos y los procesos mecanicistas que subyacen, la reorganización de la diversidad florística en pastos tras el abandono paulatino o drástico de la ganadería, la biodiversidad de las comunidades alpinas y la dinámica poblacional de especies amenazadas o indicadoras de hábitats o de motores de cambio global. Los seguimientos ecológicos actuales muestran que tanto el cambio climático como el de usos del suelo están teniendo una considerable trascendencia en la fisionomía y la estructura de algunos de los ambientes más icónicos y frecuentes del parque (deterioro del glaciar, termofilización de la flora en cumbres alpinas, densificación del bosque en su límite superior, pérdida de productividad en algunos pastos supraforestales, etc.). También sugieren una importante variabilidad espacial en los procesos (por ej. en el PNOMP conviven pastos matorralizados y pastos muy estables), y evidencian que los cambios observados no siempre siguen los paradigmas establecidos (por ej., las especies amenazadas mantienen dinámicas poblacionales estables). La integración de resultados parciales proporcionados por cada aproximación relativiza la importancia de las percepciones que cada estudio destaca por separado, y permite medir los cambios actuales en el marco de referencia de los cambios a escala geológica.Predecir la resistencia y resiliencia de los ecosistemas o las poblaciones de seres vivos para enfrentarse a los futuros cambios ambientales es complicado, no sólo por la falta de conocimientos disponibles sino también porque las respuestas que observamos no siempre son tan rápidas o lineales como se espera. La modelización constituye una herramienta cada vez más utilizada, pero requiere de evidencias reales para validar sus pronósticos, por lo que la observación de los procesos que actúan en el PNOMP ha de incluir un esfuerzo continuado de monitorización multiescalar y multidisciplinar de los distintos componentes de la geo, hidro-, crio- y biosfera, sin olvidar el componente humano. Entender la complejidad supone conectar las interacciones que existen entre todos los sistemas y ponderar sus efectos según las escalas de trabajo

    Molecular basis of engineered meganuclease targeting of the endogenous human RAG1 locus

    Homing endonucleases recognize long target DNA sequences generating an accurate double-strand break that promotes gene targeting through homologous recombination. We have modified the homodimeric I-CreI endonuclease through protein engineering to target a specific DNA sequence within the human RAG1 gene. Mutations in RAG1 produce severe combined immunodeficiency (SCID), a monogenic disease leading to defective immune response in the individuals, leaving them vulnerable to infectious diseases. The structures of two engineered heterodimeric variants and one single-chain variant of I-CreI, in complex with a 24-bp oligonucleotide of the human RAG1 gene sequence, show how the DNA binding is achieved through interactions in the major groove. In addition, the introduction of the G19S mutation in the neighborhood of the catalytic site lowers the reaction energy barrier for DNA cleavage without compromising DNA recognition. Gene-targeting experiments in human cell lines show that the designed single-chain molecule preserves its in vivo activity with higher specificity, further enhanced by the G19S mutation. This is the first time that an engineered meganuclease variant targets the human RAG1 locus by stimulating homologous recombination in human cell lines up to 265 bp away from the cleavage site. Our analysis illustrates the key features for à la carte procedure in protein–DNA recognition design, opening new possibilities for SCID patients whose illness can be treated ex vivo

    Se presenta un estudio evaluativo que trata de optimizar la tutoría académica centrada en la evaluación auténtica que se orienta hacia la construcción transversal de un proyecto profesional del estudiante de nuevo ingreso en el grado en magisterio. Se aborda un proceso hacia la virtualización de las tutorías académicas analizando las necesidades del estudiante, los requisitos competenciales de estudiantes y docentes universitarios y las buenas prácticas. An evaluative study is presented that tries to optimize the academic tutoring focused on the authentic evaluation that is oriented towards the transversal construction of a professional project of the new student of degree in primary education teaching.
