    Neural Dynamics Variations Observer Designed for Robot Manipulator Control Using a Novel Saturated Control Technique

    (is work presents a novel controller for the dynamics of robots using a dynamic variations observer. (e proposed controller uses a saturated control law based on sin(tg− 1(.)) function instead of tanh(.). Besides, this function is an alternative to the use of tanh(.) in saturation control, since it reaches its maximum value more gradually than the hyperbolic tangent function. Using this characteristic, the transition between states is smoother, with similar accuracy to tanh(.). (e controller is designed using a saturated SMC (sliding mode controller) and a dynamic variations observer based on GRNN (general regression neural network). (e originality of this work is the use of a combination of adaptive GRNN with a sliding mode controller (SMC) including a new saturation function.     Prognostic significance of FLT3-ITD length in AML patients treated with intensive regimens

    FLT3-ITD mutations are detected in approximately 25% of newly diagnosed adult acute myeloid leukemia (AML) patients and confer an adverse prognosis. The FLT3-ITD allelic ratio has clear prognostic value. Nevertheless, there are numerous manuscripts with contradictory results regarding the prognostic relevance of the length and insertion site (IS) of the FLT3-ITD fragment. We aimed to assess the prognostic impact of these variables on the complete remission (CR) rates, overall survival (OS) and relapse-free survival (RFS) of AML patients with FLT3-ITDmutations. We studied the FLT3-ITD length of 362 adult AML patients included in the PETHEMA AML registry. We tried to validate the thresholds of ITD length previously published (i.e., 39 bp and 70 bp) in intensively treated AML patients (n = 161). We also analyzed the mutational profile of 118 FLT3-ITD AML patients with an NGS panel of 39 genes and correlated mutational status with the length and IS of ITD. The AUC of the ROC curve of the ITD length for OS prediction was 0.504, and no differences were found when applying any of the thresholds for OS, RFS or CR rate. Only four out of 106 patients had ITD IS in the TKD1 domain. Our results, alongside previous publications, confirm that FLT3-ITD length lacks prognostic value and clinical applicability. © 2021, The Author(s)

    Chemotherapy or allogeneic transplantation in high-risk Philadelphia chromosome–negative adult lymphoblastic leukemia

    The need for allogeneic hematopoietic stem cell transplantation (allo-HSCT) in adults with Philadelphia chromosome–negative (Ph−) acute lymphoblastic leukemia (ALL) with high-risk (HR) features and adequate measurable residual disease (MRD) clearance remains unclear. The aim of the ALL-HR-11 trial was to evaluate the outcomes of HR Ph− adult ALL patients following chemotherapy or allo-HSCT administered based on end-induction and consolidation MRD levels. Patients aged 15 to 60 years with HR-ALL in complete response (CR) and MRD levels (centrally assessed by 8-color flow cytometry) <0.1% after induction and <0.01% after early consolidation were assigned to receive delayed consolidation and maintenance therapy up to 2 years in CR. The remaining patients were allocated to allo-HSCT. CR was attained in 315/348 patients (91%), with MRD <0.1% after induction in 220/289 patients (76%). By intention-to-treat, 218 patients were assigned to chemotherapy and 106 to allo-HSCT. The 5-year (±95% confidence interval) cumulative incidence of relapse (CIR), overall survival (OS), and event-free survival probabilities for the whole series were 43% ± 7%, 49% ± 7%, and 40% ± 6%, respectively, with CIR and OS rates of 45% ± 8% and 59% ± 9% for patients assigned to chemotherapy and of 40% ± 12% and 38% ± 11% for those assigned to allo-HSCT, respectively. Our results show that avoiding allo-HSCT does not hamper the outcomes of HR Ph− adult ALL patients up to 60 years with adequate MRD response after induction and consolidation. Better postremission alternative therapies are especially needed for patients with poor MRD clearance

    Role of age and comorbidities in mortality of patients with infective endocarditis

    [Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality. [Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65 years,65 to 80 years,and ≥ 80 years.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk. [Results]: A total of 3120 patients with IE (1327 < 65 years;1291 65-80 years;502 ≥ 80 years) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients ≥80 years who underwent surgery were significantly lower compared with other age groups (14.3%,65 years; 20.5%,65-79 years; 31.3%,≥80 years). In-hospital mortality was lower in the <65-year group (20.3%,<65 years;30.1%,65-79 years;34.7%,≥80 years;p < 0.001) as well as 1-year mortality (3.2%, <65 years; 5.5%, 65-80 years;7.6%,≥80 years; p = 0.003).Independent predictors of mortality were age ≥ 80 years (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32–3.34), CCI ≥ 3 (HR:1.62; 95% CI:1.39–1.88),and non-performed surgery (HR:1.64;95% CI:11.16–1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65 years(p < 0.001) for both in-hospital and 1-year mortality. [Conclusion]: There were no differences in the clinical presentation of IE between the groups. Age ≥ 80 years, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group

    Applicability of probabilistic graphical models for early detection of SARS-CoV-2 reactive antibodies after SARS-CoV-2 vaccination in hematological patients

    Prior studies of antibody response after full SARS-CoV-2 vaccination in hematological patients have confirmed lower antibody levels compared to the general population. Serological response in hematological patients varies widely according to the disease type and its status, and the treatment given and its timing with respect to vaccination. Through probabilistic machine learning graphical models, we estimated the conditional probabilities of having detectable anti-SARS-CoV-2 antibodies at 3–6 weeks after SARS-CoV-2 vaccination in a large cohort of patients with several hematological diseases (n= 1166). Most patients received mRNA-based vaccines (97%), mainly Moderna® mRNA-1273 (74%) followed by Pfizer-BioNTech® BNT162b2 (23%). The overall antibody detection rate at 3 to 6 weeks after full vaccination for the entire cohort was 79%. Variables such as type of disease, timing of anti-CD20 monoclonal antibody therapy, age, corticosteroids therapy, vaccine type, disease status, or prior infection with SARS-CoV-2 are among the most relevant conditions influencing SARS-CoV-2-IgG-reactive antibody detection. A lower probability of having detectable antibodies was observed in patients with B-cell non-Hodgkin’s lymphoma treated with anti-CD20 monoclonal antibodies within 6 months before vaccination (29.32%), whereas the highest probability was observed in younger patients with chronic myeloproliferative neoplasms (99.53%). The Moderna® mRNA-1273 compound provided higher probabilities of antibody detection in all scenarios. This study depicts conditional probabilities of having detectable antibodies in the whole cohort and in specific scenarios such as B cell NHL, CLL, MM, and cMPN that may impact humoral responses. These results could be useful to focus on additional preventive and/or monitoring interventions in these highly immunosuppressed hematological patients.REDCap is developed and supported by Vanderbilt Institute for Clinical and Translational Research.     Analiza las enciclopedias multimedia de carácter general y su aplicación didáctica en la enseñanza. Recoge los elementos que conforman la comunicación multimedia, sus principales características, los problemas que plantea al usuario y cómo resolverlos. Estudia el mercado de la edición electrónica en España, para lo que se ha seleccionado una muestra de 570 títulos multimedia, enciclopédicos, temáticos y monográficos (susceptibles de ser utilizados en educación), que se han ordenado mediante una clasificación propuesta que facilita su integración didáctica en la enseñanza. Presenta distintos modelos de evaluación de productos multimedia y propone uno específicamente desarrollado para las enciclopedias multimedia. Además, se estudian, analizan y comparan tres enciclopedias multimedia en soporte DVD ROM: la Enciclopedia Universal 2003 de Micronet, la Biblioteca de Consulta Encarta 2003 de Microsoft y la Enciclopedia Multimedia Larousse 2003, del grupo editorial del mismo nombre. En cuanto a la aplicación didáctica de las enciclopedias multimedia en educación. Se distinguen tres espacios de integración (habitación individual, aula ordinaria y aula de informática) y se describen posibles actividades, a la vez que se ofrecen orientaciones para su uso. Una de las aportaciones del estudio es la teoría del conocimiento relacional, y cómo las enciclopedias multimedia potencian este modelo de aprendizaje, a través del desarrollo de contenidos procedimentales y actitudinales, convirtiéndose en un eje (contenido) transversal, y mediante los hiperenlaces hacia dentro de la propia enciclopedia (in) y hacia fuera, Internet (out).MadridES

    Todo Trabajo Fin de Máster supone siempre la integración y puesta en práctica de los contenidos asimilados durante el periodo de estudio y prácticas. En este caso, los conocimien-tos del Máster en Dirección y Gestión de Centros Educativos se concretan en el colegio Ntra. Sra. de las Escuelas Pías, centro privado concertado en todos los niveles educativos (desde In-fantil a Bachillerato), ubicado en Madrid. Su modelo de gestión, basado en los equipos de tra-bajo, sirve como referente para conocer los órganos colegiados y unipersonales que podemos encontrar en cualquier centro educativo. El funcionamiento paralelo de siete planes de innova-ción son el ejemplo de una apuesta por la renovación en la enseñanza y la calidad, más allá de la implantación del modelo EFQM. El modelo pedagógico basado en el Estilo Educativo Escolapias centra la visión, misión y los valores del Proyecto Educativo, que se caracteriza por una escuela humanizadora, inno-vadora y evangelizadora, desde una perspectiva cristiana de la educación. Integrado, junto con otros siete colegios, en la Fundación Escolapias Montal, titular del centro, participa de un mo-delo de gestión económica integrada en el que se respeta la autonomía en la propuesta y ejecu-ción de los presupuestos y la gestión económica. Aunque el centro manifiesta una estructura consolidada, el Trabajo Fin de Máster desarrollado realiza más de cuarenta y cinco propuesta de innovación y mejora, que podrían constituir las líneas de acción en un Proyecto de Direc-ción, y que pone de manifiesto el margen de crecimiento que aún tiene el colegio
