A first principles simulation of rigid water
We present the results of Car-Parrinello (CP) simulations of water at ambient
conditions and under pressure, using a rigid molecule approximation. Throughout
our calculations, water molecules were maintained at a fixed intramolecular
geometry corresponding to the average structure obtained in fully unconstrained
simulations. This allows us to use larger time steps than those adopted in
ordinary CP simulations of water, and thus to access longer time scales. In the
absence of chemical reactions or dissociation effects, these calculations open
the way to ab initio simulations of aqueous solutions that require timescales
substantially longer than presently feasible (e.g. simulations of hydrophobic
solvation). Our results show that structural properties and diffusion
coefficients obtained with a rigid model are in better agreement with
experiment than those determined with fully flexible simulations. Possible
reasons responsible for this improved agreement are discussed

Phase separation in hydrogen-helium mixtures at Mbar pressures
The properties of hydrogen-helium mixtures at Mbar pressures and intermediate
temperatures (4000 to 10000 K) are calculated with first-principles molecular
dynamics simulations. We determine the equation of state as a function of
density, temperature, and composition and, using thermodynamic integration, we
estimate the Gibbs free energy of mixing, thereby determining the temperature,
at a given pressure, when helium becomes insoluble in dense metallic hydrogen.
These results are directly relevant to models of the interior structure and
evolution of Jovian planets. We find that the temperatures for the demixing of
helium and hydrogen are sufficiently high to cross the planetary adiabat of
Saturn at pressures around 5 Mbar; helium is partially miscible throughout a
significant portion of the interior of Saturn, and to a lesser extent in

Focal neuromyotonia: do I love you?
We present a rare case of focal neuromyotonia in a 73-year-old woman with a follow up of 5years. The clinical picture showed a fixed contraction of the 3rd and 4th finger of the left hand. Similar to other published cases, our patient suffered from COPD and was treated with beta-2-sympathomimetics. This clinical picture shows a rare but rather salient differential diagnosis of Dupuytren's contracture. Towards comprehensive and transparent reporting: context-specific additions to the ICF taxonomy for medical evaluations of work capacity involving claimants with chronic widespread pain and low back pain
BACKGROUND: Medical evaluations of work capacity provide key information for decisions on a claimant's eligibility for disability benefits. In recent years, the evaluations have been increasingly criticized for low transparency and poor standardization. The International Classification of Functioning, Disability and Health (ICF) provides a comprehensive spectrum of categories for reporting functioning and its determinants in terms of impairments and contextual factors and could facilitate transparent and standardized documentation of medical evaluations of work capacity. However, the comprehensiveness of the ICF taxonomy in this particular context has not been empirically examined. In this study, we wanted to identify potential context-specific additions to the ICF for its application in medical evaluations of work capacity involving chronic widespread pain (CWP) and low back pain (LBP).METHODS: A retrospective content analysis of Swiss medical reports was conducted by using the ICF for data coding. Concepts not appropriately classifiable with ICF categories were labeled as specification categories (i.e. context-specific additions) and were assigned to predefined specification areas (i.e. precision, coverage, personal factors, and broad concepts). Relevant specification categories for medical evaluations of work capacity involving CWP and LBP were determined by calculating their relative frequency across reports and setting a relevance threshold.RESULTS: Forty-three specification categories for CWP and fifty-two for LBP reports passed the threshold. In both groups of reports, precision was the most frequent specification area, followed by personal factors.CONCLUSIONS: The ICF taxonomy represents a universally applicable standard for reporting health and functioning information. However, when applying the ICF for comprehensive and transparent reporting in medical evaluations of work capacity involving CWP and LBP context-specific additions are needed. This is particularly true for the documentation of specific pain-related issues, work activities and personal factors. To ensure the practicability of the multidisciplinary evaluation process, the large number of ICF categories and context-specific additions necessary for comprehensive documentation could be specifically allocated to the disciplines in charge of their assessment.<br/
Short-Range Structural Transformations in Water at High Pressures
We report results of molecular dynamics simulations of liquid water at the
temperature T=277 K for a range of high pressure. One aim of the study was to
test the model Amoeba potential for description of equilibrium structural
properties and dynamical processes in liquid water. The comparison our
numerical results with the Amoeba and TIP5P potentials, our results of \emph{ab
initio} molecular dynamics simulations and the experimental data reveals that
the Amoeba potential reproduces correctly structural properties of the liquid
water. Other aim of our work was related with investigation of the pressure
induced structural transformations and their influence on the microscopic
collective dynamics. We have found that the structural anomaly at the pressure
Atm is related with the changes of the local, short-range
order in liquid water within first two coordination shells. This anomaly
specifies mainly by deformation of the hydrogen-bond network. We also discuss
in detail the anomalous behavior of sound propagation in liquid water at high

Evidence for plasma phase transition in high pressure hydrogen from ab-initio simulations
We have performed a detailed study of molecular dissociation in liquid hydrogen using both Born-Oppenheimer molecular dynamics with Density Functional Theory and Coupled Electron-Ion Monte Carlo simulations. We observe a range of densities where (dP/d{rho}){sub T} = 0 that coincides with sharp discontinuities in the electronic conductivity, which is clear evidence of the plasma phase transition for temperatures 600K {le} T {le} 1500K. Both levels of theory exhibit the transition, although Quantum Monte Carlo predicts higher transition pressures. Based on the temperature dependence of the discontinuity in the electronic conductivity, we estimate the critical point of the transition at temperatures slightly below 2000 K. We examine the influence of proton zero point motion by using Path Integral Molecular Dynamics with Density Functional Theory; the main effect is to shift the transition to lower pressures. Furthermore, we calculate the melting curve of molecular hydrogen up to pressures of 200 GPa, finding a reentrant melting line in good agreement with previous calculations. The melting line crosses the metalization line at 700 K and 220 GPa using density functional energetics and at 550 K and 290 GPa using Quantum Monte Carlo energetics
Detecting subgroups in social participation among individuals living with spinal cord injury:a longitudinal analysis of community survey data
STUDY DESIGN: Longitudinal community survey. OBJECTIVES: To determine subgroups in social participation of individuals living with spinal cord injury (SCI). SETTING: Community. METHODS: Data were collected in 2012 and 2017 as part of the community survey of the Swiss Spinal Cord Injury cohort. Participation was assessed using the 33-item Utrecht Scale of Evaluation of Rehabilitation-Participation evaluating frequency of, restrictions in and satisfaction with productive, leisure, and social activities. Linear mixed-effects model trees were used to distinguish subgroups in participation associated with sociodemographic and lesion characteristics. RESULTS: In all, 3079 observations were used for the analysis, of which 1549 originated from Survey 2012, 1530 from Survey 2017, and 761 from both surveys. Participants were mostly male (2012: 71.5%; 2017: 71.2%), aged on average 50 years (2012: 52.3; 2017: 56.5), with an incomplete paraplegia (2012: 37.5%; 2017: 41.8%) of traumatic origin (2012: 84.7%; 2017: 79.3%). There was limited within-person variation in participation over the 5-year period. Participation varied with age, SCI severity, education, financial strain, number of self-reported health conditions (SHCs), and disability pension level. Among modifiable parameters, the number of SHCs and disability pension level emerged as the most frequent partitioning variables, while education was most informative for participation in productive, leisure, and social activities. CONCLUSIONS: Long-term rehabilitation management and clinical practice should target people most prone to decreased participation in major life domains. Our study indicates that the alleviation of SHCs, engagement in further education, or adjusting disability pension level are promising areas to improve participation of persons living with SCI
Diabetic foot infections: current concept review
The purpose of this manuscript is to provide a current concept review on the diagnosis and management of diabetic foot infections which are among the most serious and frequent complications encountered in patients with diabetes mellitus. A literature review on diabetic foot infections with emphasis on pathophysiology, identifiable risk factors, evaluation including physical examination, laboratory values, treatment strategies and assessing the severity of infection has been performed in detail. Diabetic foot infections are associated with high morbidity and risk factors for failure of treatment and classification systems are also described. Most diabetic foot infections begin with a wound and once an infection occurs, the risk of hospitalization and amputation increases dramatically. Early identification of infection and prompt treatment may optimize the patient's outcome and provide limb salvage
