Tuberous sclerosis complex is a genetic disorder with multisystem involvement that poses significant challenges to the affected child and family. Caregiver knowledge in the South African population has not previously been reported. A prospective study of the parents of 21 children with tuberous sclerosis complex was undertaken. Median parental age was 38 (interquartile range 34.5-45) years. Parents were randomly allocated to receive written information about the condition, or to receive verbal counselling already established in clinic. A significant difference (P Π.001) was observed in the change in the mean knowledge scores for the parent group that received written information (34.2 at baseline, 51.7 at the second visit. This impact was higher in parents with an education level of at least grade 8 (P Π.003). Parental understanding of tuberous sclerosis complex can be improved by provision of written information and should be routinely available in a readily understandable format

Through the Open-Access Publishing Program, the DFG supports research universities like the Free University of Berlin in financing publication fees which arise from the publication of articles in open-access journals, provided that members of the university are classified as "submitting author" or "corresponding author". The program objective is the formation of lasting, reliable structures for financing open-access publications. For that reason, universities submitting proposals must contribute a fixed share of the requested funding volume and explain in the proposal how the publication fund will be perpetuated. Based on statistical data and comprehensive explanations of the infrastructural measures, the present application presents how the Free University of Berlin contributes to the continuation of the program

The Berlin Open Access Strategy claims: "It is a long-term goal to establish a Berlin-wide open access infrastructure for monographs, anthologies and periodicals – as a cooperation of infrastructure service providers located at research institutions as well as Berlin-based publishers and publishing service providers." A Berlin-wide open access publishing infrastructure should be based on the existing infrastructural and scholarly expertise of several, cooperating institutions in Berlin and result in high-quality publications at fair costs. A working group "Open Access Publishing Platforms" consisting of representatives of several Berlin research institutions was set up and commissioned to carry out the necessary preliminary work. The first part reports on existing publication infrastructures run by Berlin research institutions. The second part describes three possible model for re- structuring and setting up a Berlin-wide open access publishing infrastructure for monographs, anthologies and journals

Diagnosis, monitoring and treatment of tuberous sclerosis complex: A South African consensus response to international guidelines

    Get PDF
Tuberous sclerosis complex (TSC) is a genetic disorder with multi-system manifestations and a high burden of disease. In 2013, an international panel of TSC experts revised the guidelines for the diagnosis, surveillance and treatment of the disorder. In South Africa (SA), a local multidisciplinary group of healthcare professionals and TSC researchers reviewed the international guidelines to generate an SA consensus clinical update on the identification, diagnosis, treatment and lifelong monitoring of individuals who live with TSC. We strongly endorse dissemination and use of the international guidelines for the assessment, monitoring and treatment of TSC. In addition, we strongly support access to genetic testing and to mTOR (mammalian target of rapamycin) inhibitors to treat subependymal giant cell astrocytomas not amenable to surgery and renal angiomyolipomas larger than 3 cm, and as adjunctive treatment for refractory focal seizures. We await with interest results from mTOR inhibitor trials of skin and TSC-associated neuropsychiatric disorders (TAND). With regard to training, we recommend the inclusion of TSC in undergraduate and postgraduate medical and health sciences curricula, and the promotion of other continuing professional development events to raise awareness about TSC. We also support the development of a TSC user/carer/parent organisation to provide an informal support network for families across SA. We acknowledge that some progress has been made in recent years in SA, but much remains to be done. We hope that this SA onsensus clinical update based on the international guidelines will make a positive contribution to increase knowledge and improve clinical care for all patients who live with TSC in SA, and their families

Millennial-scale fluctuations in Saharan dust supply across the decline of the African Humid Period

    Get PDF
The Sahara is the world's largest dust source with significant impacts on trans-Atlantic terrestrial and large-scale marine ecosystems. Contested views about a gradual or abrupt onset of Saharan aridity at the end of the African Humid Period dominate the current scientific debate about the Holocene Saharan desiccation. In this study, we present a 19.63 m sediment core sequence from Lake Sidi Ali (Middle Atlas, Morocco) at the North African desert margin. We reconstruct the interaction between Saharan dust supply and Western Mediterranean hydro-climatic variability during the last 12,000 yr based on analyses of lithogenic grain-sizes, XRF geochemistry and stable isotopes of ostracod shells. A robust chronological model based on AMS 14C dated pollen concentrates supports our multi-proxy study. At orbital-scale there is an overall increase in southern dust supply from the Early Holocene to the Late Holocene, but our Northern Saharan dust record indicates that a gradual Saharan desiccation was interrupted by multiple abrupt dust increases before the ‘southern dust mode‘ was finally established at 4.7 cal ka BP. The Sidi Ali record features millennial peaks in Saharan dust increase at about 11.1, 10.2, 9.4, 8.2, 7.3, 6.6, 6.0, and 5.0 cal ka BP. Early Holocene Saharan dust peaks coincide with Western Mediterranean winter rain minima and North Atlantic cooling events. In contrast, Late Holocene dust peaks correspond mostly with prevailing positive phases of the North Atlantic Oscillation. By comparing with other North African records, we suggest that increases in Northern Saharan dust supply do not solely indicate sub-regional to regional aridity in Mediterranean Northwest Africa but might reflect aridity at a trans-Saharan scale. In particular, our findings support major bimillennial phases of trans-Saharan aridity at 10.2, 8.2, 6.0 and 4.2 cal ka BP. These phases coincide with North Atlantic cooling and a weak African monsoon.Christoph Zielhofer, Steffen Mischke and William Fletcher as principal investigators thank the German Research Foundation (DFG, ZI 721/9-1), the Federal Ministry of Education and Research (BMBF, 01DH17020) and the Natural Environment Research Council (New Investigator Award to W Fletcher, NE/K000608/1, and NERC RCF dating awards, 1765.1013 and 1809.0414) for generous funding of the fieldwork and lab analyses.Peer Reviewe
