    Radio signatures of solar energetic particles during the 23rd solar cycle

    We present the association rates between solar energetic particles (SEPs) and the radio emission signatures in the corona and IP space during the entire solar cycle 23. We selected SEPs associated with X and M-class flares from the visible solar hemisphere. All SEP events are also accompanied by coronal mass ejections. Here, we focus on the correlation between the SEP events and the appearance of radio type II, III and IV bursts on dynamic spectra. For this we used the available radio data from ground-based stations and the Wind/WAVES spacecraft. The associations are presented separately for SEP events accompanying activity in the eastern and western solar hemisphere. We find the highest association rate of SEP events to be with type III bursts, followed by types II and IV. Whereas for types III and IV no longitudinal dependence is noticed, these is a tendency for a higher SEP-association rate with type II bursts in the eastern hemisphere.     Spatial Centrality in the Finance-Growth Nexus: Panel Data Evidence from EAC and SADC Countries.

    Finance-growth nexuses through extensively studied, the empirical observations are still varies across countries and regions. The central place theory and the theory of money and space provide some prospects for harmonised solutions. This study examines the finance-growth nexus in Southern African Development Community (SADC) and East Africa Community (EAC) countries based on three panel error correction ARDL models and one Panel Smooth Transition Regression (PSTR) model where centrality was used as regime switching variable. Aggregate data was obtained from the World Development Indicators. The findings suggest that the finance-growth nexus are generally negative in centre countries and positive in the periphery. At the regional integration level, it is clear that finance-growth causal direction is unidirectional regardless of whether a country uses broad money growth or domestic credit to the private sector except for SADC and centrally located countries. The regime switching effect of centrality is statistically significant with finance-growth nexus being negative in centre countries and positive in the periphery an observation that supports the Convergent Growth Hypothesis (CGH) that countries in the extreme periphery are catching up to centres within their respective regions in terms of GDP growth through FSD. Based on these observations EAC countries can positively benefit from expanded monetary base while SADC require improved economic infrastructure to benefit from the same.     Using Community-Owned Resource Persons to Provide Early Diagnosis and Treatment and Estimate Malaria Burden at Community Level in North-Eastern Tanzania.

    Although early diagnosis and prompt treatment is an important strategy for control of malaria, using fever to initiate presumptive treatment with expensive artemisinin combination therapy is a major challenge; particularly in areas with declining burden of malaria. This study was conducted using community-owned resource persons (CORPs) to provide early diagnosis and treatment of malaria, and collect data for estimation of malaria burden in four villages of Korogwe district, north-eastern Tanzania.In 2006, individuals with history of fever within 24 hours or fever (axillary temperature ≥37.5°C) at presentation were presumptively treated using sulphadoxine/pyrimethamine. Between 2007 and 2010, individuals aged five years and above, with positive rapid diagnostic tests (RDTs) were treated with artemether/lumefantrine (AL) while under-fives were treated irrespective of RDT results. Reduction in anti-malarial consumption was determined by comparing the number of cases that would have been presumptively treated and those that were actually treated based on RDTs results. Trends of malaria incidence and slide positivity rates were compared between lowlands and highlands. Of 15,729 cases attended, slide positivity rate was 20.4% and declined by >72.0% from 2008, reaching <10.0% from 2009 onwards; and the slide positivity rates were similar in lowlands and highlands from 2009 onwards. Cases with fever at presentation declined slightly, but remained at >40.0% in under-fives and >20.0% among individuals aged five years and above. With use of RDTs, cases treated with AL decreased from <58.0% in 2007 to <11.0% in 2010 and the numbers of adult courses saved were 3,284 and 1,591 in lowlands and highlands respectively. Malaria incidence declined consistently from 2008 onwards; and the highest incidence of malaria shifted from children aged <10 years to individuals aged 10-19 years from 2009. With basic training, supervision and RDTs, CORPs successfully provided early diagnosis and treatment and reduced consumption of anti-malarials. Progressively declining malaria incidence and slide positivity rates suggest that all fever cases should be tested with RDTs before treatment. Data collected by CORPs was used to plan phase 1b MSP3 malaria vaccine trial and will be used for monitoring and evaluation of different health interventions. The current situation indicates that there is a remarkable changing pattern of malaria and these areas might be moving from control to pre-elimination levels

    Statistical Evidence for Contributions of Flares and Coronal Mass Ejections to Major Solar Energetic Particle Events

    Solar energetic particle (SEP) events are related to flares and coronal mass ejections (CMEs). This work is a new investigation of statistical relationships between SEP peak intensities - deka-MeV protons and near-relativistic electrons - and characteristic quantities of the associated solar activity. We consider the speed of the CME and quantities describing the flare-related energy release: peak flux and fluence of soft X-ray (SXR) emission, fluence of microwave emission. The sample comprises 38 SEP events associated with strong SXR bursts (classes M and X) in the western solar hemisphere between 1997 and 2006, and where the flare-related particle acceleration is accompanied by radio bursts indicating electron escape to the interplanetary space. The main distinction of the present statistical analysis from earlier work is that besides the classical Pearson correlation coefficient the partial correlation coefficients are calculated in order to disentangle the effects of correlations between the solar parameters themselves. The classical correlation analysis shows the usual picture of correlations with broad scatter between SEP peak intensities and the different parameters of solar activity, and strong correlations between the solar activity parameters themselves. The partial correlation analysis shows that the only parameters that affect significantly the SEP intensity are the CME speed and the SXR fluence. The SXR peak flux and the microwave fluence have no additional contribution. We conclude that these findings bring statistical evidence that both flare acceleration and CME shock acceleration contribute to the deka-MeV proton and near-relativistic electron populations in large SEP events.Comment: The final publication is available at Springer via http://dx.doi.org/10.1007/s11207-014-0628-

    The Perceived Impact of Disclosure of Pediatric HIV Status on Pediatric Antiretroviral Therapy Adherence, Child Well-Being, and Social Relationships in a Resource-Limited Setting

We sought to describe how parents and guardians of HIV-infected children view the impact of disclosing a child’s HIV status, particularly for children’s antiretroviral therapy (ART) adherence. A qualitative study was conducted using involving focus groups and interviews with parents and guardians of HIV-infected children receiving ART in western Kenya. Interviews covered multiple aspects of the experience of having children take medicines. Transcribed interview dialogues were coded for analysis. Data were collected from 120 parents and guardians caring for children 0–14 years (mean 6.8 years, standard deviation [SD] 6.4); 118 of 120 had not told the children they had HIV. Children’s caregivers (parents and guardians) described their views on disclosure to children and to others, including how this information-sharing impacted pediatric ART adherence, children’s well-being, and their social relationships. Caregivers believed that disclosure might have benefits such as improved ART adherence, especially for older children, and better engagement of a helping social network. They also feared, however, that disclosure might have both negative psychological effects for children and negative social effects for their families, including discrimination. In western Kenya, caregivers’ views on the risks and benefits to disclosing children’s HIV status emerged a key theme related to a family’s experience with HIV medications, even for families who had not disclosed the child’s status.     Rain-Fed Farming System at a Crossroads in Semi-Arid Areas of Tanzania: What Roles do Climate Variability and Change Play?

    Positive changes, like adopting drought resistant crop varieties, in the rain-fed farming system (RFFS) in response to climate variability and change enhance system’s ability to support people’s living as opposed to negative changes, like lack of pastures, which put the system at risk of failure in supporting the living. Using participatory rural appraisal (PRA) and household survey, this paper examined the roles of climate variability and change in triggering changes in RFFS. Specifically, the paper: (i) assessed dominant crop and livestock farming system; (ii) assessed the change element of crop and livestock production systems; and (iii) examined factors for the changes in RFFS. A random sample of 388 households was used. Qualitative data analysis was done through content analysis. Binary logistic regression was used to assess factors that explain changes on RFFS. The results showed that dominant crops were different in each village. Secondly, some changes in crop varieties and in livestock grazing arrangements were noted in response to climate variability and change. Unlike the hypothesis (P&gt;0.05), the results demonstrated that warming (ß = -10.61, Wald = 36.26, P ? 0.001) showed highest significant impact on likelihood of adopting new crop varieties relative to other factors. Similarly, drought (ß = 2.16, Wald = 6.82, P ? 0.009) showed highest impact on the likelihood of changing a grazing place. Yet, the changes were constrained by factors like natural resources protective policies, failure of crop varieties to withstand warming and drought, and poor land use management. Therefore, the RFFS was at a crossroads with implications on system sustainability and livelihoods. The government and private interventions should support farmers and agro-pastoralists to manage risks related to the changes in RFFS in response to climate variability and change.     Epidemiology of Malaria in an Area Prepared for Clinical Trials in Korogwe, North-eastern Tanzania.

    Site preparation is a pre-requesite in conducting malaria vaccines trials. This study was conducted in 12 villages to determine malariometric indices and associated risk factors, during long and short rainy seasons, in an area with varying malaria transmission intensities in Korogwe district, Tanzania. Four villages had passive case detection (PCD) of fever system using village health workers. Four malariometric cross-sectional surveys were conducted between November 2005 and May 2007 among individuals aged 0-19 years, living in lowland urban, lowland rural and highland strata. A total of 10,766 blood samples were collected for malaria parasite diagnosis and anaemia estimation. Blood smears were stained with Giemsa while haemoglobin level was measured by HaemoCue. Socio-economic data were collected between Jan-Apr 2006. Adjusting for the effect of age, the risk of Plasmodium falciparum parasitaemia was significantly lower in both lowland urban, (OR = 0.26; 95%CI: 0.23-0.29, p < 0.001) and highlands, (OR = 0.21; 95%CI: 0.17-0.25, p < 0.001) compared to lowland rural. Individuals aged 6-9 years in the lowland rural and 4-19 years in both lowland urban and highlands had the highest parasite prevalence, whilst children below five years in all strata had the highest parasite density. Prevalence of splenomegaly and gametocyte were also lower in both lowland urban and highlands than in lowland rural. Anaemia (Hb <11 g/dl) prevalence was lowest in the lowland urban. Availability of PCD and higher socio-economic status (SES) were associated with reduced malaria and anaemia prevalence. Higher SES and use of bed nets in the lowland urban could be the important factors for low malaria infections in this stratum. Results obtained here were used together with those from PCD and DSS in selecting a village for Phase 1b MSP3 vaccine trial, which was conducted in the study area in year 2008

    Existing techniques for characterization of longitudinal recording media using remanence measurements are extended to perpendicular media, in particular to Alumite, and correction for demagnetizing fields is taken into account. It is found that these techniques have limited value because of the sensitivity of the analysis to the correction factor used. Measurement of the recoil lines is investigated as an alternative method of probing the reversal processes

    Antimicrobial resistance predicts death in Tanzanian children with bloodstream infections: a prospective cohort study

    Bloodstream infection is a common cause of hospitalization, morbidity and death in children. The impact of antimicrobial resistance and HIV infection on outcome is not firmly established. We assessed the incidence of bloodstream infection and risk factors for fatal outcome in a prospective cohort study of 1828 consecutive admissions of children aged zero to seven years with signs of systemic infection. Blood was obtained for culture, malaria microscopy, HIV antibody test and, when necessary, HIV PCR. We recorded data on clinical features, underlying diseases, antimicrobial drug use and patients' outcome. The incidence of laboratory-confirmed bloodstream infection was 13.9% (255/1828) of admissions, despite two thirds of the study population having received antimicrobial therapy prior to blood culture. The most frequent isolates were klebsiella, salmonellae, Escherichia coli, enterococci and Staphylococcus aureus. Furthermore, 21.6% had malaria and 16.8% HIV infection. One third (34.9%) of the children with laboratory-confirmed bloodstream infection died. The mortality rate from Gram-negative bloodstream infection (43.5%) was more than double that of malaria (20.2%) and Gram-positive bloodstream infection (16.7%). Significant risk factors for death by logistic regression modeling were inappropriate treatment due to antimicrobial resistance, HIV infection, other underlying infectious diseases, malnutrition and bloodstream infection caused by Enterobacteriaceae, other Gram-negatives and candida. Bloodstream infection was less common than malaria, but caused more deaths. The frequent use of antimicrobials prior to blood culture may have hampered the detection of organisms susceptible to commonly used antimicrobials, including pneumococci, and thus the study probably underestimates the incidence of bloodstream infection. The finding that antimicrobial resistance, HIV-infection and malnutrition predict fatal outcome calls for renewed efforts to curb the further emergence of resistance, improve HIV care and nutrition for children
