    Ambientes de comunidades de prática virtuais como apoio ao desenvolvimento de neologismos terminológicos em língua de sinais

Porém, por um longo período da história, a Língua Brasileira de Sinais (Libras) foi desvalorizada e forçadamente excluída dos espaços sociais. Em consequência do processo histórico, hoje é comum identificar conceitos de áreas de especialidade que não possuam terminologia específica em Libras. A inexistência de signos incide diretamente sobre o acesso ao conhecimento de forma mais natural ao surdo. Neste sentido, quando se aborda o domínio da terminologia, dois problemas devem ser tratados: (1) a baixa disseminação dos registros da língua; e (2) a dificuldade de interação entre as diferentes comunidades do país, o que tem implicado em criações difusas de sinais ou mesmo compreensões equivocadas sobre conceitos. Ante as constatações, esta tese teve em vista a proposição de um framework conceitual para Comunidades de Prática Virtuais visando a apoiar a ampliação do repertório terminológico da Língua de Sinais. Para tanto, a pesquisa buscou promover um diálogo entre a Teoria de Comunidades de Prática, ao considerar o aspecto social da aprendizagem e construção do conhecimento; a Teoria da Cognição Distribuída ao considerar o uso das Tecnologias de Informação e Comunicação para fomentar a distribuição dos processos cognitivos entre pessoas e artefatos; o domínio da Terminologia por envolver o desenvolvimento de neologismos para designar conhecimentos e a Língua de Sinais enquanto modalidade de comunicação e língua de desenvolvimento dos neologismos. Além das pesquisas sobre o referencial teórico, foi realizado um estudo etnográfico em uma de uma comunidade de prática atuante na produção de neologismos terminológicos em Língua de Sinais, sendo este estudo analisado de utilizando a Teoria da Cognição Distribuída. A análise do estudo culminou na proposta do framework conceitual Términus, o qual foi utilizado como referência para a implementação de um protótipo funcional nomeado como MooBi. As avaliações da proposta validaram a aplicabilidade do framework ao problema identificado.Abstract : The communication in visuospatial modality marks all the construction of deaf?s culture. However, for a long period of history, the Brazilian Sign Language (Libras) was devalued and forcibly excluded from social spaces. As result of the historical process, it is common identify concepts in specific domains that do not have terminology in Libras. The absence of signs directly affects access to knowledge in a more natural manner to the deaf. In this sense, when discussing the field of terminology, two problems must be addressed: (1) the low dissemination of terminologies in Libras; and (2) the difficulty of interaction between the different communities of the country, which has been implicated in diffuse creations signs or even misunderstandings about concepts. In view of the findings, this research sought to propose a conceptual framework for Virtual Communities of Practice aiming to support the expansion of terminological repertoire of sign language. Therefore, the research sought to promote a dialogue between Communities of Practice Theory, considering the social aspect of learning and knowledge construction; the Distributed Cognition Theory, considering the distribution of cognitive processes among people and artifacts; the domain of Terminology which through neological processes, operates in creation of terms to designate knowledge; and Sign Language, as a mode of communication. In addition to research of the state of art, an ethnographic study was conducted in an community of practice acting in the production of terminological neologisms in sign language which was analyzed according to the Distributed Cognition Theory. The analysis of the study culminated in the proposition of Términus conceptual framework, which was used as reference for the implementation of a functional prototype named MooBi. Assessments of the prototype validated the applicability of the framework to the identified problem

    Este trabalho se propõe a analisar três sites brasileiros de compras através da internet e apresentar alternativas para acesso às informações, nestes meios, de forma acessível para o público surdo. As análises foram baseadas nas diretrizes e recomendações da World Wide Web Consortium (W3C) atendidas através das diretrizes Web Content Accessibility Guidelines (WCAG) versão 2.1, diretrizes para a criação de pistas proximais e testes automáticos baseados na WCAG 2.0. Os resultados obtidos apresentam-se como argumento sobre a importância do investimento em ferramentas que tornem ambientes de consumo online mais acessíveis.     The prime educational objectives for the remote regions course of study has been to train professional teachers of Japanese who have no phyisical access to an educational institution, or whose access to such an institution is restricted by time, location or financial resources. Recently, as a result of the increasing presence of foreign workers in local communities and companies, a number of ordinary people with no professional credentials have taken on the responsibility of teaching Japanese. The purpose of this broadcast course is not only to train professional teachers of Japanese, but also to help local citizens to learn Japanese language teaching techniques and to enable them to learn by themselves. Using as a case study Nihongo no Kyoiku to Sono Riron, which is one of the courses at the University of the Air in Japan, this paper evaluates the appropriateness of the course and course material design as a contribution to distant learning

    Prognostic assessment of 1310 patients with non–small-cell lung cancer who underwent complete resection from 1980 to 1993

    AbstractObjective: The TNM staging system of lung cancer is widely used as a guide for estimating prognosis and selecting treatment modality. In 1997, the International Union Against Cancer and the American Joint Committee on Cancer have adopted a revised stage grouping for lung cancer. However, the validity of the new stage grouping has not been fully established. We investigated the prognoses of patients who had resection of non–small-cell lung cancer to confirm the validity of the revised classification. Methods: A total of 1310 patients with non–small-cell lung cancer underwent complete resection and pathologic staging of the disease in our hospitals from 1980 through 1993. A pulmonary resection was performed with a systematic nodal dissection. The survivals were calculated with the Kaplan-Meier method on the basis of overall deaths, and the survival curves were compared by log rank test. Results: There were significant differences in survival between patients with T1 N0 M0 and T2 N0 M0 disease and between those with T1 N1 M0 and T2 N1 M0 disease. However, there was no significant difference between patients with T2 N0 M0 disease and those with T1 N1 M0 disease. No significant difference in survival was observed among patients with T2 N1 M0, T3 N0 M0, and T3 N1 M0 cancer. Patients with different invaded organs of T3 subdivision (pleura, chest wall, pericardium, or diaphragm) had a different prognosis. There was no significant difference between patients with T3 N2 M0 disease and those with stage IIIB disease. Conclusions: We supported most of the revision, such as dividing stage I, dividing stage II, and putting T3 N0 M0 to stage IIB. Furthermore, we found some candidates for a subsequent revision, such as putting T3 N1 M0 to stage IIB, putting T2 N0 M0 and T1 N1 M0 together, regarding diaphragm invasion as T4, and putting T3 N2 M0 to stage IIIB.     Evaluation of tissue response to periodontal dressings: histological study in tooth sockets of rats

    Although the use of periodontal dressings is currently limited, there are some indications for their use. Selection of any material that will have direct contact with live tissues, such as periodontal dressings, should be careful in order to allow surgical wound healing. The aim of this study was to evaluate the intensity of inflammatory response and bone formation in tooth sockets of rats after implantation of three periodontal dressings. After removal of the right maxillary incisors of 84 male rats, each tooth socket received implantation of a polyethylene tube, 63 of which were filled with non-eugenol periodontal dressing and the remaining 21 tubes remained empty (control group). Histological evaluation assessed the intensity of inflammatory response and presence and location of bone tissue formation at postoperative periods of 7, 14 and 28 days. Statistical analysis was performed by the Kruskal-Wallis test at 5% significance level.     Impact of extracellular matrix on engraftment and maturation of pluripotent stem cell-derived cardiomyocytes in a rat myocardial infarct model

    Pluripotent stem cell-derived cardiomyocytes show great promise in regenerating the heart after myocardial infarction; however, several uncertainties exist that must be addressed before clinical trials. One practical issue is graft survival following transplantation. Although a pro-survival cocktail with Matrigel has been shown to enhance graft survival, the use of Matrigel may not be clinically feasible. The purpose of this study was to test whether a hyaluronan-based hydrogel, HyStem, could be a substitute for Matrigel. Human induced pluripotent stem cell-derived cardiomyocytes diluted with HyStem alone, HyStem plus pro-survival factors, or a pro-survival cocktail with Matrigel (PSC/MG), were transplanted into a rat model of acute myocardial infarction. Histological analysis at 4 weeks post transplantation revealed that, among the three groups, recipients of PSC/MG showed the largest graft size. Additionally, the grafted cardiomyocytes in the recipients of PSC/MG had a more matured phenotype compared to those in the other two groups.     Background Insulin beneficially affects myocardial functions during myocardial ischemia. It increases glucose-derived ATP production, decreases oxygen consumption, suppresses apoptosis of cardiomyocytes, and promotes the survival of cardiomyocytes. Patients with chronic heart failure generally have high insulin resistance, which is correlated with poor outcomes. The role of insulin in acute decompensated heart failure (ADHF) remains unclear. This study aimed to investigate the prognostic value of serum insulin level at the time of admission for long-term outcomes in patients with ADHF. Methods and Results We enrolled 1074 consecutive patients who were admitted to our department for ADHF. Of these 1074 patients, we studied the impact of insulin on the prognosis of ADHF in 241 patients without diabetes mellitus. The patients were divided into groups according to low, intermediate, and high tertiles of serum insulin levels. Primary end points were all-cause death and cardiovascular death. During a mean follow-up of 21.8 months, 71 all-cause deaths and 38 cardiovascular deaths occurred. Kaplan-Meier analysis showed that all-cause and cardiovascular mortality was significantly higher in the low-insulin group than those in the intermediate- and high-insulin groups (log-rank P=0.0046 and P=0.038, respectively). Moreover, according to the multivariable analysis, low serum insulin was an independent predictor of all-cause and cardiovascular mortality (hazard ratio, 2.37 [95% CI, 1.24-4.65; P=0.009] and 2.94 [95% CI, 1.12-8.19; P=0.028], respectively). Conclusions Low serum insulin levels were associated with increased risk of all-cause and cardiovascular death in ADHF patients without diabetes mellitus.博士(医学)・甲第808号・令和4年3月15日© 2020 The Authors.     日本における非代償性急性心不全患者の30日および90日以内の心不全再入院の発生率と臨床的意義 : NARA-HF研究より

    Background: Countermeasure development for early rehospitalization for heart failure (re-HHF) is an urgent and important issue in Western countries and Japan.Methods and Results:Of 1,074 consecutive NARA-HF study participants with acute decompensated HF admitted to hospital as an emergency between January 2007 and December 2016, we excluded 291 without follow-up data, who died in hospital, or who had previous HF-related hospitalizations, leaving 783 in the analysis. During the median follow-up period of 895 days, 241 patients were re-admitted for HF. The incidence of re-HHF was the highest within the first 30 days of discharge (3.3% [26 patients]) and remained high until 90 days, after which it decreased sharply. Within 90 days of discharge, 63 (8.0%) patients were re-admitted. Kaplan-Meier analysis revealed that patients with 90-day re-HHF had worse prognoses than those without 90-day re-HHF in terms of all-cause death (hazard ratio [HR] 2.321, 95% confidence interval [CI] 1.654-3.174; P<0.001) and cardiovascular death (HR 3.396, 95% CI 2.153-5.145; P<0.001). Multivariate analysis indicated that only male sex was an independent predictor of 90-day re-HHF. Conclusions: The incidence of early re-HHF was lower in Japan than in Western countries.
