    The detection of Burkholderia spp. and pathogenic Leptospira spp. in South Africa

    Leptospirosis is a zoonosis of ubiquitous distribution and causes a wide spectrum of disease. Burkholderia species are important plant and human pathogens. Little or no investigation has been performed on any clinically-relevant Burkholderia or Leptospira species in Johannesburg. Environmental samples were taken from different sites in Johannesburg along the Jukskei River. These were subjected to culturing for Burkholderia spp. and polymerase chain reaction (PCR) for Burkholderia and Leptospira spp. Human serum, animal serum and kidney samples were also subjected to PCR for both organisms. A Leptospira IgM ELISA was also performed on human serum samples. More Burkholderia spp. were isolated by culture from soil samples than water samples. The PCR yielded a significantly higher PCR positive from soil samples (p = 0.015). There was a high prevalence of pathogenic Leptospira spp. in soil samples. The ELISA yielded only 7.8% (26/332) positive samples. There were no human or animal positive PCR results for either organism. There is an environmental presence of both leptospires and Burkholderia in the area sampled. More studies are needed to establish how both organisms might affect patients with compromised immune systems, and how often both infections are incorrectly or under-diagnosed

Multiplex polymerase chain reactions (PCRs) were used to detect blaCTX-M, blaSHV and blaTEM genes in a proportion of the ESBL isolates. All isolates exhibiting reduced susceptibility to carbapenems were PCR tested for blaKPC and blaNDM-1 resistance genes. Results. Overall, 68.3% of the 2 774 isolates were ESBL-positive, showing resistance to cefotaxime, ceftazidime and cefepime. Furthermore, 46.5% of all isolates were resistant to ciprofloxacin and 33.1% to piperacillin-tazobactam. The major ESBL genes were abundantly present in the sample analysed. Most isolates (95.5%) were susceptible to the carbapenems tested, and no isolates were positive for blaKPC or blaNDM-1. There was a trend towards a decrease in susceptibility to most antibiotics. Conclusion. The high proportion of ESBL-producing K. pneumoniae isolates observed, and the prevalence of ESBL genes, are of great concern.     Decline in subarachnoid haemorrhage volumes associated with the first wave of the COVID-19 pandemic

    BACKGROUND: During the COVID-19 pandemic, decreased volumes of stroke admissions and mechanical thrombectomy were reported. The study\u27s objective was to examine whether subarachnoid haemorrhage (SAH) hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines. METHODS: We conducted a cross-sectional, retrospective, observational study across 6 continents, 37 countries and 140 comprehensive stroke centres. Patients with the diagnosis of SAH, aneurysmal SAH, ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases, 10th Revision, codes. The 3-month cumulative volume, monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before (1 year and immediately before) and during the pandemic, defined as 1 March-31 May 2020. The prior 1-year control period (1 March-31 May 2019) was obtained to account for seasonal variation. FINDINGS: There was a significant decline in SAH hospitalisations, with 2044 admissions in the 3 months immediately before and 1585 admissions during the pandemic, representing a relative decline of 22.5% (95% CI -24.3% to -20.7%, p\u3c0.0001). Embolisation of ruptured aneurysms declined with 1170-1035 procedures, respectively, representing an 11.5% (95%CI -13.5% to -9.8%, p=0.002) relative drop. Subgroup analysis was noted for aneurysmal SAH hospitalisation decline from 834 to 626 hospitalisations, a 24.9% relative decline (95% CI -28.0% to -22.1%, p\u3c0.0001). A relative increase in ruptured aneurysm coiling was noted in low coiling volume hospitals of 41.1% (95% CI 32.3% to 50.6%, p=0.008) despite a decrease in SAH admissions in this tertile. INTERPRETATION: There was a relative decrease in the volume of SAH hospitalisations, aneurysmal SAH hospitalisations and ruptured aneurysm embolisations during the COVID-19 pandemic. These findings in SAH are consistent with a decrease in other emergencies, such as stroke and myocardial infarction

    Leptospirosis is a common zoonosis worldwide. It has a ubiquitous distribution and causes a wide spectrum of disease. Leptospirosis therefore has a broad reservoir host range, and many infected species of animals excrete leptospires in their urine, which leads to contamination of soil and water. Typical descriptions of the disease include a biphasic (anicteric form) and fulminant disease in the icterohaemorrhagic form. Only a few local case reports of human leptospirosis have been published, the most recent one being in 1974. A rodent-related zoonosis study (RatZooMan) was conducted from 2003 until 2006 in three provinces (Limpopo, KwaZulu-Natal and the Eastern Cape). Of the people sampled in Cato Crest (Durban, KwaZulu-Natal Province), 43/217 (19.8%) were seropositive for leptospirosis. Of the clinical samples sent to the Special Bacterial Pathogens Reference Unit from all over the country for testing in 2009, 16/176 (9%) were IgM positive; in 2010 and January 2011 to May 2011, 14/215 (6.5%) and 12/96 (12.5%), respectively, were IgM positive. The apparent incidence of leptospirosis in the South African population is moderately high based on the detected positives in suspected cases; it is thought that the circulating infection rate may be even higher when looking at the RatZooMan results. This may be due to underreporting and undiagnosed cases. Communities in informal settlements in urban areas are especially at risk as infected rodent populations are a continuous source of transmission

    Global perspective of familial hypercholesterolaemia: a cross-sectional study from the EAS Familial Hypercholesterolaemia Studies Collaboration (FHSC)

    none724siBackground The European Atherosclerosis Society Familial Hypercholesterolaemia Studies Collaboration (FHSC) global registry provides a platform for the global surveillance of familial hypercholesterolaemia through harmonisation and pooling of multinational data. In this study, we aimed to characterise the adult population with heterozygous familial hypercholesterolaemia and described how it is detected and managed globally.Methods Using FHSC global registry data, we did a cross-sectional assessment of adults (aged 18 years or older) with a clinical or genetic diagnosis of probable or definite heterozygous familial hypercholesterolaemia at the time they were entered into the registries. Data were assessed overall and by WHO regions, sex, and index versus non-index cases.Findings Of the 61 612 individuals in the registry, 42 167 adults (21 999 [53.6%] women) from 56 countries were included in the study. Of these, 31 798 (75.4%) were diagnosed with the Dutch Lipid Clinic Network criteria, and 35 490 (84.2%) were from the WHO region of Europe. Median age of participants at entry in the registry was 46.2 years (IQR 34.3-58.0); median age at diagnosis of familial hypercholesterolaemia was 44.4 years (32.5-56.5), with 40.2% of participants younger than 40 years when diagnosed. Prevalence of cardiovascular risk factors increased progressively with age and varied by WHO region. Prevalence of coronary disease was 17.4% (2.1% for stroke and 5.2% for peripheral artery disease), increasing with concentrations of untreated LDL cholesterol, and was about two times lower in women than in men. Among patients receiving lipid-lowering medications, 16 803 (81.1%) were receiving statins and 3691 (21.2%) were on combination therapy, with greater use of more potent lipid-lowering medication in men than in women. Median LDL cholesterol was 5.43 mmol/L (IQR 4.32-6.72) among patients not taking lipid-lowering medications and 4.23 mmol/L (3.20-5.66) among those taking them. Among patients taking lipid-lowering medications, 2.7% had LDL cholesterol lower than 1.8 mmol/L; the use of combination therapy, particularly with three drugs and with proprotein convertase subtilisin-kexin type 9 inhibitors, was associated with a higher proportion and greater odds of having LDL cholesterol lower than 1.8 mmol/L. Compared with index cases, patients who were non-index cases were younger, with lower LDL cholesterol and lower prevalence of cardiovascular risk factors and cardiovascular diseases (all p<0.001).Interpretation Familial hypercholesterolaemia is diagnosed late. Guideline-recommended LDL cholesterol concentrations are infrequently achieved with single-drug therapy. Cardiovascular risk factors and presence of coronary disease were lower among non-index cases, who were diagnosed earlier. Earlier detection and greater use of combination therapies are required to reduce the global burden of familial hypercholesterolaemia. Copyright (C) 2021 Elsevier Ltd.
