Comparison of conventional nighttime with automatic or manual daytime CPAP titration in unselected sleep apnea patients: study of the usefulness of daytime titration studies
AbstractDaytime CPAP titration studies with full polysomnography have been successfully performed in patients with severe sleep apneaâhypopnea syndrome (SAHS). The implementation of daytime studies in unselected SAHS patients could help to reduce the waiting lists for CPAP titrations. The main purpose of this study was to compare the effectiveness of conventional versus manual or automatic daytime CPAP titration in unselected patients with SAHS.Ninety-three consecutive patients with SAHS in whom CPAP was indicated were assigned to conventional titration or to manual or automatic (AutoSet) daytime CPAP titration, after sleep deprivation. The number of valid studies, sleep architecture, final pressure selected and mean pressure in the different sleep stages were compared. Changes in sleepiness (Epworth sleepiness score) and hours of CPAP use were assessed after 3 months of treatment.Four patients did not sleep (3 AutoSet, 1 conventional daytime groups). Sleep latency was shorter during automatic daytime titration whereas REM latency was shorter in daytime studies; the percentage of sleep stages was similar during all types of titration. CPAP requirements were significantly higher during REM sleep in conventional and manual daytime titrations while mean pressure was unchanged throughout sleep stages during AutoSet titration. CPAP pressure selected with conventional or daytime manual titration (7.5(2.2)cm H2O and 7.4(1.5)cm H2O, ns) were significantly lower (P<0.001) than with AutoSet (9.4(1.6)cm H2O. All groups showed similar decrease of sleepiness and hours of use of CPAP at 3 months of follow-up.Automatic and manual daytime PSG studies after sleep deprivation are useful for CPAP titration in unselected patients with SAHS. Pressure selected with AutoSet is significantly higher than with conventional daytime or nighttime titration, although not significant in terms of treatment compliance and symptom improvement
Ultrasensitive multiplex optical quantification of bacteria in large samples of biofluids
Efficient treatments in bacterial infections require the fast
and accurate recognition of pathogens, with concentrations as
low as one per milliliter in the case of septicemia. Detecting
and quantifying bacteria in such low concentrations is
challenging and typically demands cultures of large samples of
blood (~1 milliliter) extending over 24-72 hours. This delay
seriously compromises the health of patients. Here we
demonstrate a fast microorganism optical detection system for
the exhaustive identification and quantification of pathogens in
volumes of biofluids with clinical relevance (~1 milliliter) in
minutes. We drive each type of bacteria to accumulate antibody
functionalized SERS-labelled silver nanoparticles. Particle
aggregation on the bacteria membranes renders dense arrays of
inter-particle gaps in which the Raman signal is exponentially
amplified by several orders of magnitude relative to the
dispersed particles. This enables a multiplex identification of
the microorganisms through the molecule-specific spectral
fingerprints
Effects of stress and REM sleep deprivation on the patterns of avoidance learning and brain acetylcholine in the mouse
Pure bred DBA/2J mice were subjected to varying degrees of stress and âwater tankâ REM sleep deprivation using one-animal cages, large platforms and/or poles. Subsequently, the animals were studied for acquisition, short-term and long-term retention using a conditioned avoidance procedure in a Warner type automated shuttle box. In addition, various groups of animals subjected to the same stressful procedures were sacrificed and brain acetylcholine (ACh) was measured by the frog rectus abdominus muscle bioassay. It was found that stress and REM sleep deprivation had no significant effect on acquisition and short-term retention but did impair long term retention of conditioned behavior. Moderate environmental stress also produced significant behavioral changes. Long Term Eff ects of Fiscal Policy on the Size and the Distribution of the Pie in the UK
This paper provides a joint analysis of the output and distributional long term effects of various fiscal
policies in the UK, using a Vector Autoregression approach. Our findings suggest that the output
effects of fiscal policies are consistent with the Keynesian paradigm for both direct and indirect taxes
but not for public spending. The estimated long term impact on GDP of increasing all type of
expenditure and taxes analysed is negative and especially strong in the case of current expenditure. We
also find significant distributional effects associated to fiscal policies, indicating that an increase in
public spending and direct taxes reduces inequality while a raise in indirect taxes increases income
inequality. Finally, the relationship between inequality and output is also explored

By the careful survey questions to be cared of the researcher, the progress of the study will be able to come up with the collection of patterns and relations from the responses of the participants due to the establish virtue of the thematic analysis. Furthermore, from the given set of data responded by the participants, by the virtue of grounded theory, the researcher will establish the guided principles in making the holistic approach of parental guidance for the enthusiasts in mobile games and to be well â versed for the solutions to make in balancing the time and effort of the parents and their children who are learners and gamers.Leading them into a Paretto principle will make these enthusiasts in mobile games will make them holistically well in their personalities and characters to immediate improvements in their academics
