Comparing Haemophilus influenzae type b Conjugate Vaccine Schedules: Systematic Review and Meta-Analysis of Vaccine Trials
Background: The optimal schedule and the need for a booster dose are unclear for
Haemophilus influenzae type b (Hib) conjugate vaccines. We systematically reviewed relative
effects of Hib vaccine schedules.
Methods: We searched 21 databases to May 2010 or June 2012 and selected randomized
controlled trials (RCTs) or quasi-RCTs that compared different Hib schedules (three primary
doses with no booster dose [3p+0], 3p+1 and 2p+1) or different intervals in primary schedules
and between primary and booster schedules. Outcomes were clinical efficacy, nasopharyngeal
carriage and immunological response. Results were combined in random-effects meta-analysis.
Results: Twenty trials from 15 countries were included; 16 used vaccines conjugated to tetanus
toxoid (PRP-T). No trials assessed clinical or carriage outcomes. Twenty trials examined
immunological outcomes and found few relevant differences. Comparing PRP-T 3p+0 with 2p+0
there was no difference in seropositivity at the 1.0ÎŒg/ml threshold by six months after the last
primary dose (combined risk difference -0.02, 95%CI -0.10, 0.06). Only small differences were
seen between schedules starting at different ages, with different intervals between primary
doses, or with different intervals between primary and booster doses. Individuals receiving a
booster were more likely to be seropositive than those at the same age who did not.
Conclusions: There is no clear evidence from trials that any 2p+1, 3p+0 or 3p+1 schedule of Hib
conjugate vaccine is likely to provide better protection against Hib disease than other schedules.
Until more data become available, scheduling is likely to be determined by epidemiological and
programmatic considerations in individual settings
Brownian Carnot engine
The Carnot cycle imposes a fundamental upper limit to the efficiency of a
macroscopic motor operating between two thermal baths. However, this bound
needs to be reinterpreted at microscopic scales, where molecular bio-motors and
some artificial micro-engines operate. As described by stochastic
thermodynamics, energy transfers in microscopic systems are random and thermal
fluctuations induce transient decreases of entropy, allowing for possible
violations of the Carnot limit. Despite its potential relevance for the
development of a thermodynamics of small systems, an experimental study of
microscopic Carnot engines is still lacking. Here we report on an experimental
realization of a Carnot engine with a single optically trapped Brownian
particle as working substance. We present an exhaustive study of the energetics
of the engine and analyze the fluctuations of the finite-time efficiency,
showing that the Carnot bound can be surpassed for a small number of
non-equilibrium cycles. As its macroscopic counterpart, the energetics of our
Carnot device exhibits basic properties that one would expect to observe in any
microscopic energy transducer operating with baths at different temperatures.
Our results characterize the sources of irreversibility in the engine and the
statistical properties of the efficiency -an insight that could inspire novel
strategies in the design of efficient nano-motors.Comment: 7 pages, 7 figure
Downward pumping of magnetic flux as the cause of filamentary structures in sunspot penumbrae
The structure of a sunspot is determined by the local interaction between magnetic fields and convection near the Sun's surface. The dark central umbra is surrounded by a filamentary penumbra, whose complicated fine structure has only recently been revealed by high-resolution observations. The penumbral magnetic field has an intricate and unexpected interlocking-comb structure and some field lines, with associated outflows of gas, dive back down below the solar surface at the outer edge of the spot. These field lines might be expected to float quickly back to the surface because of magnetic buoyancy, but they remain submerged. Here we show that the field lines are kept submerged outside the spot by turbulent, compressible convection, which is dominated by strong, coherent, descending plumes. Moreover, this downward pumping of magnetic flux explains the origin of the interlocking-comb structure of the penumbral magnetic field, and the behaviour of other magnetic features near the sunspot
DNA-based Self-Assembly of Chiral Plasmonic Nanostructures with Tailored Optical Response
Surface plasmon resonances generated in metallic nanostructures can be
utilized to tailor electromagnetic fields. The precise spatial arrangement of
such structures can result in surprising optical properties that are not found
in any naturally occurring material. Here, the designed activity emerges from
collective effects of singular components equipped with limited individual
functionality. Top-down fabrication of plasmonic materials with a predesigned
optical response in the visible range by conventional lithographic methods has
remained challenging due to their limited resolution, the complexity of
scaling, and the difficulty to extend these techniques to three-dimensional
architectures. Molecular self-assembly provides an alternative route to create
such materials which is not bound by the above limitations. We demonstrate how
the DNA origami method can be used to produce plasmonic materials with a
tailored optical response at visible wavelengths. Harnessing the assembly power
of 3D DNA origami, we arranged metal nanoparticles with a spatial accuracy of 2
nm into nanoscale helices. The helical structures assemble in solution in a
massively parallel fashion and with near quantitative yields. As a designed
optical response, we generated giant circular dichroism and optical rotary
dispersion in the visible range that originates from the collective
plasmon-plasmon interactions within the nanohelices. We also show that the
optical response can be tuned through the visible spectrum by changing the
composition of the metal nanoparticles. The observed effects are independent of
the direction of the incident light and can be switched by design between left-
and right-handed orientation. Our work demonstrates the production of complex
bulk materials from precisely designed nanoscopic assemblies and highlights the
potential of DNA self-assembly for the fabrication of plasmonic nanostructures.Comment: 5 pages, 4 figure
Pollutant dispersion in a developing valley cold-air pool
Pollutants are trapped and accumulate within cold-air pools, thereby affecting air quality. A numerical model is used to quantify the role of cold-air-pooling processes in the dispersion of air pollution in a developing cold-air pool within an alpine valley under decoupled stable conditions. Results indicate that the negatively buoyant downslope flows transport and mix pollutants into the valley to depths that depend on the temperature deficit of the flow and the ambient temperature structure inside the valley. Along the slopes, pollutants are generally entrained above the cold-air pool and detrained within the cold-air pool, largely above the ground-based inversion layer. The ability of the cold-air pool to dilute pollutants is quantified. The analysis shows that the downslope flows fill the valley with air from above, which is then largely trapped within the cold-air pool, and that dilution depends on where the pollutants are emitted with respect to the positions of the top of the ground-based inversion layer and cold-air pool, and on the slope wind speeds. Over the lower part of the slopes, the cold-air-pool-averaged concentrations are proportional to the slope wind speeds where the pollutants are emitted, and diminish as the cold-air pool deepens. Pollutants emitted within the ground-based inversion layer are largely trapped there. Pollutants emitted farther up the slopes detrain within the cold-air pool above the ground-based inversion layer, although some fraction, increasing with distance from the top of the slopes, penetrates into the ground-based inversion layer.Peer reviewe
Exposure and impact of a mass media campaign targeting sexual health amongst Scottish men who have sex with men: an outcome evaluation
Background:
This paper explores the exposure and impact of a Scottish mass media campaign: Make Your Position Clear. It ran from October 2009 to July 2010, targeted gay men and other men who have sex with men (MSM), and had two key aims: to promote regular sexual health and HIV testing every 6Â months, and to promote the use of appropriate condoms and water-based lubricant with each episode of anal intercourse.
Methods: A cross-sectional survey (anonymous and self-report) was conducted 10Â months after the campaign was launched (July 2010). Men were recruited from commercial venues. Outcome measures included use of lubricant, testing for sexually transmitted infections and HIV, and intentions to seek HIV testing within the following six months. Linear-by-linear chi-square analysis and binary logistic regressions were conducted to explore the associations between the outcome measures and campaign exposure.
Results:
The total sample was 822 men (62.6% response rate). Men self-identifying as HIV positive were excluded from the analysis (nâ=â38). Binary logistic analysis indicated that those with mid or high campaign exposure were more likely to have been tested for HIV in the previous six months when adjusted for age, area of residence and use of the âgay sceneâ (AORâ=â1.96, 95% CIâ=â1.26 to 3.06, pâ=â.003), but were not more likely to be tested for STIs (AORâ=â1.37, 95% CIâ=â0.88 to 2.16, pâ=â.167). When adjusted for previous HIV testing, those with mid or high campaign exposure were not more likely to indicate intention to be tested for HIV in the following six months (AORâ=â1.30, 95% CIâ=â0.73 to 2.32, pâ=â.367). Those with no campaign exposure were less likely than those with low exposure to have used appropriate lubricant with anal sex partners in the previous year (AORâ=â0.42, 95% CIâ=â0.23 to 0.77, pâ=â.005).
Conclusions:
The campaign had demonstrable reach. The analysis showed partial support for the role of mass media campaigns in improving sexual health outcomes. This suggests that a role for mass media campaigns remains within combination HIV prevention
Alimentary fluoride intake in preschool children
<p>Abstract</p> <p>Background</p> <p>The knowledge of background alimentary fluoride intake in preschool children is of utmost importance for introducing optimal and safe caries preventive measures for both individuals and communities. The aim of this study was to assess the daily fluoride intake analyzing duplicate samples of food and beverages. An attempt was made to calculate the daily intake of fluoride from food and swallowed toothpaste.</p> <p>Methods</p> <p>Daily alimentary fluoride intake was measured in a group of 36 children with an average age of 4.75 years and an average weight of 20.69 kg at baseline, by means of a double plate method. This was repeated after six months. Parents recorded their child's diet over 24 hours and collected duplicated portions of food and beverages received by children during this period. Pooled samples of food and beverages were weighed and solid food samples were homogenized. Fluoride was quantitatively extracted from solid food samples by a microdiffusion method using hexadecyldisiloxane and perchloric acid. The content of fluoride extracted from solid food samples, as well as fluoride in beverages, was measured potentiometrically by means of a fluoride ion selective electrode.</p> <p>Results</p> <p>Average daily fluoride intake at baseline was 0.389 (SD 0.054) mg per day. Six months later it was 0.378 (SD 0.084) mg per day which represents 0.020 (SD 0.010) and 0.018 (SD 0.008) mg of fluoride respectively calculated per kg bw/day.</p> <p>When adding the values of unwanted fluoride intake from the toothpaste shown in the literature (0.17-1.21 mg per day) the estimate of the total daily intake of fluoride amounted to 0.554-1.594 mg/day and recalculated to the child's body weight to 0.027-0.077 mg/kg bw/day.</p> <p>Conclusions</p> <p>In the children studied, observed daily fluoride intake reached the threshold for safe fluoride intake. When adding the potential fluoride intake from swallowed toothpaste, alimentary intake reached the optimum range for daily fluoride intake. These results showed that in preschool children, when trying to maximize the benefit of fluoride in caries prevention and to minimize its risk, caution should be exercised when giving advice on the fluoride containing components of child's diet or prescribing fluoride supplements.</p

Health-related quality of life in Huntingtonâs Disease patients: a comparison of proxy assessment and patient self-rating using the disease-specific Huntingtonâs Disease health-related quality of life questionnaire (HDQoL)
Huntingtonâs disease (HD) is a fatal, neurodegenerative disease for which there is no known cure. Proxy evaluation is relevant for HD as its manifestation might limit the ability of persons to report their health-related quality of life (HrQoL). This study explored patientâproxy ratings of HrQoL of persons at different stages of HD, and examined factors that may affect proxy ratings. A total of 105 patientâproxy pairs completed the Huntingtonâs disease health-related quality of life questionnaire (HDQoL) and other established HrQoL measures (EQ-5D and SF-12v2). Proxyâpatient agreement was assessed in terms of absolute level (mean ratings) and intraclass correlation. Proxiesâ ratings were at a similar level to patientsâ self-ratings on an overall Summary Score and on most of the six Specific Scales of the HDQoL. On the Specific Hopes and Worries Scale, proxies on average rated HrQoL as better than patientsâ self-ratings, while on both the Specific Cognitive Scale and Specific Physical and Functional Scale proxies tended to rate HrQoL more poorly than patients themselves. The patientâs disease stage and mental wellbeing (SF-12 Mental Component scale) were the two factors that primarily affected proxy assessment. Proxy scores were strongly correlated with patientsâ self-ratings of HrQoL, on the Summary Scale and all Specific Scales. The patientâproxy correlation was lower for patients at moderate stages of HD compared to patients at early and advanced stages. The proxy report version of the HDQoL is a useful complementary tool to self-assessment, and a promising alternative when individual patients with advanced HD are unable to self-report
Regional Genetic Structure in the Aquatic Macrophyte Ruppia cirrhosa Suggests Dispersal by Waterbirds
The evolutionary history of the genus Ruppia has been shaped by hybridization, polyploidisation and vicariance that have resulted in a problematic taxonomy. Recent studies provided insight into species circumscription, organelle takeover by hybridization, and revealed the importance of verifying species identification to avoid distorting effects of mixing different species, when estimating population connectivity. In the present study, we use microsatellite markers to determine population diversity and connectivity patterns in Ruppia cirrhosa including two spatial scales: (1) from the Atlantic Iberian coastline in Portugal to the Siculo-Tunisian Strait in Sicily and (2) within the Iberian Peninsula comprising the Atlantic-Mediterranean transition. The higher diversity in the Mediterranean Sea suggests that populations have had longer persistence there, suggesting a possible origin and/or refugial area for the species. The high genotypic diversities highlight the importance of sexual reproduction for survival and maintenance of populations. Results revealed a regional population structure matching a continent-island model, with strong genetic isolation and low gene flow between populations. This population structure could be maintained by waterbirds, acting as occasional dispersal vectors. Spanish medical studentsâ attitudes and views towards Mental Health and Psychiatry: a multicentric cross-sectional study.
Objective The aim of this study is to investigate the attitudes
towards mental illness and psychiatry among fifth year
Spanish medical students.
Methods The study included 171 students from three medical
schools located in different areas of Spain: CĂĄdiz; UCA (n=
113), Madrid; San Pablo-CEU (n=22), and Barcelona; UAB
(n=36). They responded, prior to their undergraduate medical
course in psychiatry, to the AMI questionnaire to measure the
attitudes towards mental illness and to Balonâs adapted questionnaire
to investigate their view towards psychiatry.
Results The students (93.4 %) had a positive attitude towards
mental illness (AMI). Attitudes towards psychiatry were fairly
positive with a few negative views, specifically regarding the
role of psychiatrists (items 11 and 13) and the prestige of the
specialty (item 16). There were some statistically significant
differences between the three medical schools in the perception
of psychiatry as a medical discipline. A better attitude
towards mental illness was associated with a better view of the
overall merits of psychiatry.
Conclusions Findings suggest that Spanish medical students
do not have a negative attitude towards mental illness and they
have a good perception of psychiatry, although there are still
some misconceptions about this specialty. These studentâs
attitudes could favor an appropriate management of patients
suffering from mental illness
