    On rigid origami II: Quadrilateral creased papers

    Miura-ori is well-known for its capability of flatly folding a sheet of paper through a tessellated crease pattern made of repeating parallelograms. Many potential applications have been based on the Miura-ori and its primary variations. Here we are considering how to generalize the Miura-ori: what is the collection of rigid-foldable creased papers with a similar quadrilateral crease pattern as the Miura-ori? This paper reports some progress. We find some new variations of Miura-ori with less symmetry than the known rigid-foldable quadrilateral meshes. They are not necessarily developable or flat-foldable, and still only have single degree of freedom in their rigid folding motion.     The early experiences of Physician Associate students in the UK: A regional cross-sectional study investigating factors associated with engagement

    Background The number of physician associates (PAs) training and working in the UK has increased over the last few years following the proliferation of postgraduate courses. Understanding early experiences and what impacts on engagement is important if we are to appropriately support this relatively new professional group. Methods This paper reports on a cross-sectional analysis of the first year of data from a prospective 10-year longitudinal cohort study. First year PA students (n = 89) were enrolled from five universities in one UK region where the training programmes were less than 2 years old. Data collected were: demographic information, wellbeing, burnout and engagement, expectations, placement experience, performance and caring responsibilities. Pearson’s correlations were used to examine relationships between variables and to select variables for a hierarchical regression analysis to understand which factors were associated with engagement. Descriptive statistics were calculated for questions relating to experience. Results The experiences of PA students during their first 3–6 months were mixed. For example, 78.7% of students felt that there were staff on placement they could go to for support, however, 44.8% reported that staff did not know about the role and 61.3% reported that staff did not know what clinical work they should undertake. Regression analysis found that their level of engagement was associated with their perceived career satisfaction, overall well-being, and caring responsibilities. Conclusions The support systems required for PAs may need to be examined as results showed that the PA student demographic is different to that of medical students and caring responsibilities are highly associated with engagement. A lack of understanding around the PA role in clinical settings may also need to be addressed in order to better support and develop this workforce

    Wave Composition, Propagation, and Polarization of Magnetohydrodynamic Turbulence within 0.3 au as Observed by Parker Solar Probe

    Turbulence, a ubiquitous phenomenon in interplanetary space, is crucial for the energy conversion of space plasma at multiple scales. This work focuses on the propagation, polarization, and wave composition properties of the solar wind turbulence within 0.3 au, and its variation with heliocentric distance at magnetohydrodynamic scales (from 10 s to 1000 s in the spacecraft frame). We present the probability density function of propagation wavevectors (PDF (k_{∥}, k_{⊥})) for solar wind turbulence within 0.3 au for the first time: (1) wavevectors cluster quasi-(anti-)parallel to the local background magnetic field for kd_{i} 0.02. Based on our wave composition diagnosis, we find that: the outward/anti-sunward Alfvén mode dominates over the whole range of scales and distances, the spectral energy density fraction of the inward/sunward fast mode decreases with distance, and the fractional energy densities of the inward and outward slow mode increase with distance. The outward fast mode and inward Alfvén mode represent minority populations throughout the explored range of distances and scales. On average, the degree of anisotropy of the magnetic fluctuations defined with respect to the minimum variation direction decreases with increasing scale, with no trend in distance at any scale. Our results provide comprehensive insight into the scenario of transport and transfer of the solar wind fluctuations/turbulence in the inner heliosphere

    IO:I, a near-infrared camera for the Liverpool Telescope

    IO:I is a new instrument that has recently been commissioned for the Liverpool Telescope, extending current imaging capabilities beyond the optical and into the near-infrared. Cost has been minimized by the use of a previously decommissioned instrument's cryostat as the base for a prototype and retrofitting it with Teledyne's 1.7-μm cutoff Hawaii-2RG HgCdTe detector, SIDECAR ASIC controller, and JADE2 interface card. The mechanical, electronic, and cryogenic aspects of the cryostat retrofitting process will be reviewed together with a description of the software/hardware setup. This is followed by a discussion of the results derived from characterization tests, including measurements of read noise, conversion gain, full well depth, and linearity. The paper closes with a brief overview of the autonomous data reduction process and the presentation of results from photometric testing conducted on on-sky, pipeline processed data. © 2016 Society of Photo-Optical Instrumentation Engineers (SPIE)

    An Abdominal Phantom with Tunable Stiffness Nodules and Force Sensing Capability for Palpation Training

    Robotic phantoms enable advanced physical examination training before using human patients. In this paper, we present an abdominal phantom for palpation training with controllable stiffness liver nodules that can also sense palpation forces. The coupled sensing and actuation approach is achieved by pneumatic control of positive-granular jammed nodules for tunable stiffness. Soft sensing is done using the variation of internal pressure of the nodules under external forces. This paper makes original contributions to extend the linear region of the neo-Hookean characteristic of the mechanical behavior of the nodules by 140% compared to no-jamming conditions and to propose a method using the organ level controllable nodules as sensors to estimate palpation position and force with a root-means-quare error (RMSE) of 4% and 6.5%, respectively. Compared to conventional soft sensors, the method allows the phantom to sense with no interference to the simulated physiological conditions when providing quantified feedback to trainees, and to enable training following current bare-hand examination protocols without the need to wear data gloves to collect data.This work was supported in part by the Engineering and Physical Sciences Research Council (EPSRC) MOTION grant EP/N03211X/2 and EP/N03208X/1, and EPSRC RoboPatient grant EP/T00603X/

    Repetitive vascular occlusion stimulus (RVOS) versus standard care to prevent muscle wasting in critically ill patients (ROSProx):a study protocol for a pilot randomised controlled trial

    Background Forty per cent of critically ill patients are affected by intensive care unit-acquired weakness (ICU-AW), to which skeletal muscle wasting makes a substantial contribution. This can impair outcomes in hospital, and can cause long-term physical disability after hospital discharge. No effective mitigating strategies have yet been identified. Application of a repetitive vascular occlusion stimulus (RVOS) a limb pressure cuff inducing brief repeated cycles of ischaemia and reperfusion, can limit disuse muscle atrophy in both healthy controls and bed-bound patients recovering from knee surgery. We wish to determine whether RVOS might be effective in mitigating against muscle wasting in the ICU. Given that RVOS can also improve vascular function in healthy controls, we also wish to assess such effects in the critically ill. We here describe a pilot study to assess whether RVOS application is safe, tolerable, feasible and acceptable for ICU patients. Methods This is a randomised interventional feasibility trial. Thirty-two ventilated adult ICU patients with multiorgan failure will be recruited within 48 h of admission and randomised to either the intervention arm or the control arm. Intervention participants will receive RVOS twice daily (except only once on day 1) for up to 10 days or until ICU discharge. Serious adverse events and tolerability (pain score) will be recorded; feasibility of trial procedures will be assessed against pre-specified criteria and acceptability by semi-structured interview. Together with vascular function, muscle mass and quality will be assessed using ultrasound and measures of physical function at baseline, on days 6 and 11 of study enrolment, and at ICU and hospital discharge. Blood and urine biomarkers of muscle metabolism, vascular function, inflammation and DNA damage/repair mechanism will also be analysed. The Health questionnaire will be completed 3 months after hospital discharge. Discussion If this study demonstrates feasibility, the derived data will be used to inform the design (and sample size) of an appropriately-powered prospective trial to clarify whether RVOS can help preserve muscle mass/improve vascular function in critically ill patients.</p
