To evaluate the results of sacrospinous colpopexy surgery associated with anterior colporrhaphy for the treatment of womeN with post-hysterectomy vaginal vault prolapse. Methods This prospective study included 20women with vault prolapse, PelvicOrgan Prolapse Quantification System (POP-Q) stage ≄ 2, treated between January 2003 and February 2006, and evaluated in a follow-up review (more than one year later). Genital prolapse was evaluated qualitatively in stages and quantitatively in centimeters. Prolapse stage < 2 was considered to be the cure criterion. Statistical analysis was performed using the Wilcoxon test (paired samples) to compare the points and stages of prolapse before and after surgery. Results Evaluation of the vaginal vault after one year revealed that 95% of subjects were in stage zero and that 5% were in stage 1. For cystocele, 50% were in stage 1, 10% were in stage 0 (cured) and 40% were in stage 2. For rectocele, three women were in stage 1 (15%), one was in stage 2 (5%) and 16 had no further prolapse. The most frequent complication was pain in the right buttock, with remission of symptoms in all three cases three months after surgery. Conclusions In this retrospective study, the surgical correction of vault prolapse using a sacrospinous ligament fixation technique associatedwith anterior colporrhaphy proved effective in resolving genital prolapse. Despite the low complication rates, there was a high rate of cystocele, which may be caused by posterior vaginal shifting due to either the technique or an overvaluation by the POP-Q system. © 2016 by Thieme PublicaçÔes Ltda, Rio de Janeiro, Brazil.382778

Right circumcaval ureter and double right renal vein in the Brazilian shorthair cat (Felis catus): two case reports

Variations of the renal veins are well described in the literature, although variations concerning the ureter are considered a rare finding in cats. The circumcaval ureter is one of the rarest variations of the ureter and is characterised by a loop of the ureter posterior to the caudal vena cava. This variant is also known as preureteral vena cava and retrocaval ureter. It is thought to be caused by a deviation during embryonic development of the aforementioned vein. Due to its rarity, there are scarce reports of the circumcaval ureter in cats, and its association with two renal veins makes it less common as well. These variations should be preoperatively identified in order to avoid complications in kidney transplants, ureteral surgeries and cystoscopies, for instance. The present work aims to report two cases of a circumcaval ureter with two renal veins in two different Brazilian shorthair cats (Felis catus)

Evidence for a mixed mass composition at the `ankle' in the cosmic-ray spectrum

We report a first measurement for ultra-high energy cosmic rays of the correlation between the depth of shower maximum and the signal in the water Cherenkov stations of air-showers registered simultaneously by the fluorescence and the surface detectors of the Pierre Auger Observatory. Such a correlation measurement is a unique feature of a hybrid air-shower observatory with sensitivity to both the electromagnetic and muonic components. It allows an accurate determination of the spread of primary masses in the cosmic-ray flux. Up till now, constraints on the spread of primary masses have been dominated by systematic uncertainties. The present correlation measurement is not affected by systematics in the measurement of the depth of shower maximum or the signal in the water Cherenkov stations. The analysis relies on general characteristics of air showers and is thus robust also with respect to uncertainties in hadronic event generators. The observed correlation in the energy range around the `ankle' at lg⁡(E/eV)=18.5−19.0\lg(E/{\rm eV})=18.5-19.0 differs significantly from expectations for pure primary cosmic-ray compositions. A light composition made up of proton and helium only is equally inconsistent with observations. The data are explained well by a mixed composition including nuclei with mass A>4A > 4. Scenarios such as the proton dip model, with almost pure compositions, are thus disfavoured as the sole explanation of the ultrahigh-energy cosmic-ray flux at Earth.Comment: Published version.
