Proprioceptive changes impair balance control in individuals with chronic obstructive pulmonary disease

However, the specific use of proprioception, which is of primary importance during balance control, has not been studied in individuals with COPD. The objective was to determine the specific proprioceptive control strategy during postural balance in individuals with COPD and healthy controls, and to assess whether this was related to inspiratory muscle weakness. Methods: Center of pressure displacement was determined in 20 individuals with COPD and 20 age/gender-matched controls during upright stance on an unstable support surface without vision. Ankle and back muscle vibration were applied to evaluate the relative contribution of different proprioceptive signals used in postural control. Results: Individuals with COPD showed an increased anterior-posterior body sway during upright stance (p=0.037). Compared to controls, individuals with COPD showed an increased posterior body sway during ankle muscle vibration (p=0.047), decreased anterior body sway during back muscle vibration (p=0.025), and increased posterior body sway during simultaneous ankle-muscle vibration (p=0.002). Individuals with COPD with the weakest inspiratory muscles showed the greatest reliance on ankle muscle input when compared to the stronger individuals with COPD (p=0.037). Conclusions: Individuals with COPD, especially those with inspiratory muscle weakness, increased their reliance on ankle muscle proprioceptive signals and decreased their reliance on back muscle proprioceptive signals during balance control, resulting in a decreased postural stability compared to healthy controls. These proprioceptive changes may be due to an impaired postural contribution of the inspiratory muscles to trunk stability. Impaired Postural Control Reduces Sit-to-Stand-to-Sit Performance in Individuals with Chronic Obstructive Pulmonary Disease
Abstract
Background: Functional activities, such as the sit-to-stand-to-sit (STSTS) task, are often impaired in individuals with chronic
obstructive pulmonary disease (COPD). The STSTS task places a high demand on the postural control system, which has
been shown to be impaired in individuals with COPD. It remains unknown whether postural control deficits contribute to
the decreased STSTS performance in individuals with COPD.
Methods: Center of pressure displacement was determined in 18 individuals with COPD and 18 age/gender-matched
controls during five consecutive STSTS movements with vision occluded. The total duration, as well as the duration of each
sit, sit-to-stand, stand and stand-to-sit phase was recorded.
Results: Individuals with COPD needed significantly more time to perform five consecutive STSTS movements compared to healthy controls (1966 vs. 1364 seconds, respectively; p = 0.001). The COPD group exhibited a significantly longer stand phase (p = 0.028) and stand-to-sit phase (p = 0.001) compared to the control group. In contrast, the duration of the sit phase (p = 0.766) and sit-to-stand phase (p = 0.999) was not different between groups.
Conclusions: Compared to healthy individuals, individuals with COPD needed significantly more time to complete those phases of the STSTS task that require the greatest postural control. These findings support the proposition that suboptimal postural control is an important contributor to the decreased STSTS performance in individuals with COPD
Does alpha phase modulate visual target detection? Three experiments with tACS-phase-based stimulus presentation
In recent years, the influence of alpha (7–13 Hz) phase on visual processing has received a lot of attention. Magneto‐/encephalography (M/EEG) studies showed that alpha phase indexes visual excitability and task performance. Studies with transcranial alternating current stimulation (tACS) aim to modulate oscillations and causally impact task performance. Here, we applied right occipital tACS (O2 location) to assess the functional role of alpha phase in a series of experiments. We presented visual stimuli at different pre‐determined, experimentally controlled, phases of the entraining tACS signal, hypothesizing that this should result in an oscillatory pattern of visual performance in specifically left hemifield detection tasks. In experiment 1, we applied 10 Hz tACS and used separate psychophysical staircases for six equidistant tACS‐phase conditions, obtaining contrast thresholds for detection of visual gratings in left or right hemifield. In experiments 2 and 3, tACS was at EEG‐based individual peak alpha frequency. In experiment 2, we measured detection rates for gratings with (pseudo‐)fixed contrast. In experiment 3, participants detected brief luminance changes in a custom‐built LED device, at eight equidistant alpha phases. In none of the experiments did the primary outcome measure over phase conditions consistently reflect a one‐cycle sinusoid. However, post hoc analyses of reaction times (RT) suggested that tACS alpha phase did modulate RT for specifically left hemifield targets in both experiments 1 and 2 (not measured in experiment 3). This observation requires future confirmation, but is in line with the idea that alpha phase causally gates visual inputs through cortical excitability modulation
Resonant backward scattering of light by a two-side-open subwavelength metallic slit
The backward scattering of TM-polarized light by a two-side-open
subwavelength slit in a metal film is analyzed. We show that the reflection
coefficient versus wavelength possesses a Fabry-Perot-like dependence that is
similar to the anomalous behavior of transmission reported in the study [Y.
Takakura, Phys. Rev. Lett. \textbf{86}, 5601 (2001)]. The open slit totally
reflects the light at the near-to-resonance wavelengths. In addition, we show
that the interference of incident and resonantly backward-scattered light
produces in the near-field diffraction zone a spatially localized wave whose
intensity is 10-10 times greater than the incident wave, but one order of
magnitude smaller than the intra-cavity intensity. The amplitude and phase of
the resonant wave at the slit entrance and exit are different from that of a

Continuous sedation until death: The everyday moral reasoning of physicians, nurses and family caregivers in the UK, The Netherlands and Belgium

reproduction in any medium, provided the original work is properly credited.Background - Continuous sedation is increasingly used as a way to relieve symptoms at the end of life. Current research indicates that some physicians, nurses, and relatives involved in this practice experience emotional and/or moral distress. This study aims to provide insight into what may influence how professional and/or family carers cope with such distress.
Methods - This study is an international qualitative interview study involving interviews with physicians, nurses, and relatives of deceased patients in the UK, The Netherlands and Belgium (the UNBIASED study) about a case of continuous sedation at the end of life they were recently involved in. All interviews were transcribed verbatim and analysed by staying close to the data using open coding. Next, codes were combined into larger themes and categories of codes resulting in a four point scheme that captured all of the data. Finally, our findings were compared with others and explored in relation to theories in ethics and sociology.
Results - The participants’ responses can be captured as different dimensions of ‘closeness’, i.e. the degree to which one feels connected or ‘close’ to a certain decision or event. We distinguished four types of ‘closeness’, namely emotional, physical, decisional, and causal. Using these four dimensions of ‘closeness’ it became possible to describe how physicians, nurses, and relatives experience their involvement in cases of continuous sedation until death. More specifically, it shined a light on the everyday moral reasoning employed by care providers and relatives in the context of continuous sedation, and how this affected the emotional impact of being involved in sedation, as well as the perception of their own moral responsibility.
Conclusion - Findings from this study demonstrate that various factors are reported to influence the degree of closeness to continuous sedation (and thus the extent to which carers feel morally responsible), and that some of these factors help care providers and relatives to distinguish continuous sedation from euthanasia.The Economic and Social Research

Effect of diet type on serum and faecal concentration of S100/calgranulins in the captive cheetah
Gastrointestinal disease is omnipresent in captive cheetahs (Acinonyx jubatus), in contrast to its freeranging populations. The current study aimed to evaluate the effect of diet type (meat-only whole prey) on gastrointestinal health in captive cheetahs by measuring faecal and serum concentrations of S100/calgranulins. This paper reports faecal S100A12 and calprotectin concentrations in 12 captive cheetahs fed supplemented beef versus whole rabbit for one month in a cross-over design. Also, serum S100A12 and calprotectin concentrations were determined in four cheetahs fed whole rabbit and six cheetahs fed supplemented beef, and these were compared to the faecal concentrations of the respective marker proteins. Both the immunoassay for canine calprotectin and canine S100A12 were precise and reproducible for use with serum samples and faecal extracts. Whereas the assay for S100A12 was linear and accurate, an inconsistent linearity of the canine calprotectin assay was observed and could be indicative of an insufficient cross-reactivity of the specific antibody used for this assay. Serum concentrations of S100A12 and calprotectin were not altered by diet type, and were not correlated with the respective concentrations in faeces. Numerically (P=0.241) greater calprotectin concentrations and greater (P=0.041) faecal S100A12 concentrations were detected in cheetahs fed supplemented beef compared with whole rabbit. These findings demonstrate that whole prey feeding may decrease intestinal inflammation in the captive cheetah. Consequently, the relation between diet type and intestinal inflammatory conditions in the captive cheetah warrants further investigation
Fermentation of animal components in strict carnivores: a comparative study with cheetah fecal inoculum
The natural diet of felids contains highly digestible animal tissues but also fractions resistant to small intestinal digestion, which enter the large intestine where they may be fermented by the resident microbial population. Little information exists on the microbial degradability of animal tissues in the large intestine of felids consuming a natural diet. This study aimed to rank animal substrates in their microbial degradability by means of an in vitro study using captive cheetahs fed a strict carnivorous diet as fecal donors. Fresh cheetah fecal samples were collected, pooled, and incubated with various raw animal substrates (chicken cartilage, collagen, glucosamine-chondroitin, glucosamine, rabbit bone, rabbit hair, and rabbit skin; 4 replicates per substrate) for cumulative gas production measurement in a batch culture technique. Negative (cellulose) and positive (casein and fructo-oligosaccharides; FOS) controls were incorporated in the study. Additionally, after 72 h of incubation, short-chain fatty acids (SCFA), including branched-chain fatty acids (BCFA), and ammonia concentrations were determined for each substrate. Glucosamine and glucosamine-chondroitin yielded the greatest OM cumulative gas volume (OMCV) among animal substrates (P < 0.05), whereas total SCFA production was greatest for collagen (P < 0.05). Collagen induced an acetate production comparable to FOS and a markedly high acetate-to-propionate ratio (8.41:1) compared to all other substrates (1.67:1 to 2.97:1)
Lifetime measurements of Triaxial Strongly Deformed bands in Tm
With the Doppler Shift Attenuation Method, quadrupole transition moments,
, were determined for the two recently proposed Triaxial Strongly Deformed
(TSD) bands in Tm. The measured moments indicate that the
deformation of these bands is larger than that of the yrast, signature
partners. However, the measured values are smaller than those predicted by
theory. This observation appears to be valid for TSD bands in several nuclei of
