    AYURVEDIC MANAGEMENT OF POLYCYSTIC OVARIAN SYNDROME BY PALASADI BASTI AND PATHADI CHOORNA: A CASE REPORT

    Polycystic Ovarian Syndrome is the most common disease in the female population among adolescence and reproductive age group mainly due to the adoption of westernized culture. Polycystic Ovarian Syndrome (PCOS) is a physiological disorder that causes many negative effects involving a variety of systems in the body, such as the endocrine, metabolic, psychological, and reproductive systems. The complex symptomatology of PCOS makes very difficult to treat as a whole. In Ayurveda also no disease can be compared directly with PCOS. Some of clinical symptoms of PCOS may simulate Granthibhoota Artava dushti and if not treated early lead to the full manifestations and complications of PCOS. A 26 year old female came to OPD of Streeroga of IPGT & RA, having the complaints of irregular cycle, weight gain and failure to conceive since 2 years of active married life with a previous history of abortion. Sonography revealed bulky ovaries with multiple small follicles. Based on clinical findings along with sonological evidences Polycystic Ovarian Syndrome was diagnosed and a treatment protocol was selected for managing the PCOS and finally to achieve conception. Palasadi basti was selected followed by Pathadi choorna orally with warm water as internal medicine. The treatment was done for 3 months and during follow up the cycles became regular and finally the patient got conceived within 2 months

    Nebivolol: haemodynamic effects and clinical significance of combined beta-blockade and nitric oxide release.

    Nebivolol is a third-generation beta-adrenergic receptor antagonist (beta-blocker) with high selectivity for beta(1)-adrenergic receptors. In addition, it causes vasodilatation via interaction with the endothelial L-arginine/nitric oxide (NO) pathway. This dual mechanism of action underlies many of the haemodynamic properties of nebivolol, which include reductions in heart rate and blood pressure (BP), and improvements in systolic and diastolic function. With respect to BP lowering, the NO-mediated effects cause a reduction in peripheral vascular resistance and an increase in stroke volume with preservation of cardiac output. Flow-mediated dilatation and coronary flow reserve are also increased during nebivolol administration. Other haemodynamic effects include beneficial effects on pulmonary artery pressure, pulmonary wedge pressure, exercise capacity and left ventricular ejection fraction. In addition, nebivolol does not appear to have adverse effects on lipid metabolism and insulin sensitivity like traditional beta-blockers. The documented beneficial haemodynamic effects of nebivolol are translated into improved clinical outcomes in patients with hypertension or heart failure. In patients with hypertension, the incidence of bradycardia with nebivolol is often lower than that with other currently available beta-blockers. This, along with peripheral vasodilatation and NO-induced benefits such as antioxidant activity and reversal of endothelial dysfunction, should facilitate better protection from cardiovascular events. In addition, nebivolol has shown an improved tolerability profile, particularly with respect to events commonly associated with beta-blockers, such as fatigue and sexual dysfunction. Data from SENIORS (Study of the Effects of Nebivolol Intervention on Outcomes and Rehospitalization in Seniors with Heart Failure) showed that significantly fewer nebivolol versus placebo recipients experienced the primary endpoint of all-cause mortality or cardiovascular hospitalization. The benefits of nebivolol therapy were shown to be cost effective. Thus, nebivolol is an effective and well tolerated agent with benefits over and above those of traditional beta-blockade because of its effects on NO release, which give it unique haemodynamic effects, cardioprotective activity and a good tolerability profile

    Pengembangan kawasan Minapolitan bertujuan untuk mendorong percepatan pengembangan wilayah dengan kegiatan perikanan sebagai kegiatan utama dalam meningkatkan pendapatan dan kesejahteraan masyarakat salah satunya pada Kabupaten Bolaang Mongondow Timur yang hampir sebagian besar daerahnya berada dibagian pesisir yang kaya akan hasil alamnya pada bidang perikanan. Oleh karena itu penelitian ini dilakukan untuk melihat bagaimana ketersediaan prasarana sarana dan kriteria khusus kawasan minapolitan di Kecamatan Nuangan Bolaang Mongondow Timur. Penelitian ini bertujuan untuk mengidentifikasi ketersediaan prasarana sarana pendukung serta menganalisis ketersediaan dan mengetahui tingkat ketersediaan prasarana sarana terhadap kriteria khusus pengembangan kawasan minapolitan di Kecamatan Nuangan. Metode analasisi yang deskriptif kuantitatif dan dalam analisis data menggunakan analisis skala likert. Dari hasil penelitian disimpulkan menjadi 2 dari hasil identifikasi di kategorikan sedang dan rendah di karenakan prasarana sarana yang ada saat ini pada lokasi penelitian masih dalam tahap pengembangan dan prasarana yang di butuhkan yaitu pembangunan jaringan air bersih dapat di kembangkan, untuk jaringan telekomunikasi perlu ditambah tower untuk jarinagan hp pada beberapa desa seperti pada desa Mata Bulu dan Jiko Belanga dan kebutuhan dermaga yang belum ada. Sedangkan sarana yaitu kebutuhan lembaga masyarakat (kelompok tani/nelayan), TPI, industri pengolahan ikan, lapangan penjemuran, pabrik es, lembaga keuangan, SPBU/SPDN, gedung pengolahan/pengepakkan, penyediaan benih, lemari pendingin dan bengkel perahu.Kata Kunci : Sarana, Prasarana, Minapolitan, Nuanga

    Bleomycin-induced pneumonitis in a young Ghanaian male with Hodgkin’s Lymphoma

    We report a case of a young Ghanaian male who developed Bleomycin Induced Pneumonitis (BIP) after being treated for Hodgkin’s Lymphoma. Pulmonary toxicity is the most feared complication of bleomycin therapy despite its effectiveness in achieving cure in patients with Hodgkin’s lymphoma and germ cell tumors. BIP has a significant mortality rate if detected late and a high index of suspicion is required in all patients on bleomycin-based therapies with sudden onset of respiratory symptoms

    Integrated smart system for energy audit: methodology and application

The system is set up as a "black box" consisting of a hardware aimed at logging both energy and environmental parameters and a software for the assessment of building behavior and the management of energy flows.     Unambiguous Characterization of p-Cresyl Sulfate, a Protein-Bound Uremic Toxin, as Biomarker of Heart and Kidney Disease

    p-Cresyl sulfate is one of the bound uremic toxins whose level increases in the sera of patients with the severity of chronic kidney disease and is therefore used as a standard for clinical investigations. Our first attempts to obtain p-cresyl sulfate led exclusively to the product of sulfonation of the aromatic ring instead of sulfation on the OH moiety. Nevertheless, this initial discouraging result allowed us to handle both p-cresyl sulfate and 2-hydroxy-5-methylbenzenesulfonic acid obtained by different synthetic pathways. Interestingly, the comparison between the two isomers pointed out that the two molecules show the same fragmentation pattern and are indistinguishable by mass spectrometry. They cannot be separated on several commercially available columns. The only difference between the two compounds is a 10-fold higher ionization yield under negative ion electrospray ionization. NMR spectral studies definitely confirmed the different molecular structures. We present here an unambiguous biomimetic synthetic route for p-cresyl sulfate and the spectroscopic characterization of both the compounds by nuclear magnetic resonance and mass spectrometry
