Pathologies of tumor origin are more frequent in the older adult population, however said population has fewer opportunities to access diagnostic studies and therapeutic interventions, not only due to the age and stage of the cancer, but also because they have conditions of aging such as decreased functional reserve, multiple pathologies, cognitive disorders, or lack of social support that impact on the clinical response to the different therapies that exist to treat cancer. In addition to this, the risk of toxicity conferred by the various cancer treatments, which leads to a more careful evaluation of the type of therapy, the dose, and the benefit of starting it. With the discovery of new therapies for cancer management, we not only face the risk of toxicity with traditional chemotherapy, but also the schemes developed in recent times, such as immunotherapy or targeted therapy, which can mainly generate toxicity to a lesser degree. cardiovascular, gastrointestinal and skin hematological. Taking into account the above, an attempt has been made to advance in the prediction of the risk of toxicity to chemo-radiotherapy, counting to date with different models based on clinical and paraclinical calculations, many of these criteria that come out of these tools are derived from the assessment geriatric (GV), we find tools to evaluate functional performance, cognitive status, social support, which provides valuable and comprehensive information for decision-making not only therapeutic, as well as models that include clinical and paraclinical parameters such as the CRASH tool which predicts non-hematological and joint hematological toxicity and the CARG tool that predicts joint toxicity. However, globally, these toxicity risk prediction tools have several limitations, such as the lack of precision when calculating the risk of severe toxicity associated with chemo-radiotherapy, many are not validated, so many studies conclude that models are needed more accurate when predicting such risk. However, it has been shown that combining various tools (functional and clinical) improves results on many occasions by modifying the care plan for the elderly patient with cancer.Revista Nacional - Indexad

Background: Obstructive sleep apnea (OSA) is prevalent in pregnancy and it is associated with adverse pregnancy-related outcomes such as gestational diabetes, pre-eclampsia, and low birth weight. Maternal systemic inflammation is proposed to be one of the main intermediate mechanisms. However, the effects of OSA on systemic inflammation are unknown in normal pregnancy. Methods: Women in the 3rd trimester underwent hospital polysomnography to evaluate whether OSA increases systemic inflammation in normal pregnancy and its potential association with adverse fetal outcomes. OSA was defined as an apnea–hypopnea index (AHI) of = 5 h-1. Plasma cytokines levels (TNF-a, IL-1ß, IL-6, IL-8, and IL-10) were determined by multiple immunoassays. Results: We included 11 patients with OSA and 22 women with AHI < 5 h-1, who were homogeneous in age, and body mass index (BMI). Women with OSA had significant higher levels of TNF-a, IL-1ß, IL-8, and IL-10. We found significant correlations between AHI during REM and TNF-a (r = 0.40), IL-1ß (r = 0.36), IL-6 (r = 0.52), IL-8 (r = 0.43), between obstructive apnea index and TNF-a (r = 0.46) and between AHI and IL-1ß (r = 0.43). We also found that CT90% was related to IL-8 (r = 0.37). There were no significant differences in neonatal characteristics; however, we found inverse correlations between TNF-a and IL-8 with birth weight (both r = -0.48), while IL-8 showed a significant inverse relationship with neonatal gestational age (r = -0.48). Conclusions: OSA in our normal pregnancy population was associated with higher systemic inflammation, which was related to obstructive events, especially during REM sleep. Moreover, systemic inflammation was inversely correlated with neonatal birth weight and age

Consumption of caffeinated beverages and kidney function decline in an elderly Mediterranean population with metabolic syndrome

It remains unclear whether caffeinated beverages could have deleterious renal effects in elderly population with underlying comorbid conditions. We investigated the associations between coffee, tea, or caffeine intake and 1-year changes in glomerular filtration rate (eGFR) in a large Spanish cohort of overweight/obese elderly with metabolic syndrome (MetS). This prospective analysis includes 5851 overweight/obese adults (55-75 years) with MetS from the PREDIMED-Plus study. We assessed coffee, tea, and caffeine consumption from a validated food-frequency questionnaire and creatinine-based eGFR using the Chronic Kidney Disease Epidemiology Collaboration equation. Multivariate-adjusted regression models were applied to test associations between baseline coffee, tea, or caffeine intake and 1-year eGFR changes. Caffeinated coffee (> 2 cups/day) and tea (at least 1 cup/day) drinkers had 0.88 and 0.93 mL/min/1.73 m2 greater eGFR decrease respectively, compared to those with less than 1 cup/day of coffee consumption or non-tea drinkers. Furthermore, caffeinated coffee consumption of > 2 cups/day was associated with 1.19-fold increased risk of rapid eGFR decline > 3 mL/min/1.73 m2 (95% CI 1.01-1.41). Similarly, individuals in the highest (median, 51.2 mg/day) tertile of caffeine intake had a 0.87 mL/min/1.73 m2 greater eGFR decrease. Decaffeinated coffee was not associated with eGFR changes. In conclusion, higher consumption of caffeinated coffee, tea, and caffeine was associated with a greater 1-year eGFR decline in overweight/obese adults with MetS

Long daytime napping is associated with increased adiposity and type 2 diabetes in an elderly population with metabolic syndrome

Get PDF
Research examining associations between objectively-measured napping time and type 2 diabetes (T2D) is lacking. This study aimed to evaluate daytime napping in relation to T2D and adiposity measures in elderly individuals from the Mediterranean region. A cross-sectional analysis of baseline data from 2190 elderly participants with overweight/obesity and metabolic syndrome, in the PREDIMED-Plus trial, was carried out. Accelerometer-derived napping was measured. Prevalence ratios (PR) and 95% confidence intervals (CI) for T2D were obtained using multivariable-adjusted Cox regression with constant time. Linear regression models were fitted to examine associations of napping with body mass index (BMI) and waist circumference (WC). Participants napping ≥90 min had a higher prevalence of T2D (PR 1.37 (1.06, 1.78)) compared with those napping 5 to <30 min per day. Significant positive associations with BMI and WC were found in those participants napping ≥30 min as compared to those napping 5 to <30 min per day. The findings of this study suggest that longer daytime napping is associated with higher T2D prevalence and greater adiposity measures in an elderly Spanish population at high cardiovascular risk

No full text
In the present work is proposed to develop a communications plan for the Crossfit Tropa gym in the city of Bogota in order to improve communication through the organization’s digital channels for the capitalization of target audiences and strengthening of the community of members. In order to achieve this goal, a diagnosis was carried out with the aim of identifying all types of communication failures, both internally and externally. This established the need to plan communication strategies to specifically address and improve their external communication practices. The development of this project consists of exploratory and descriptive research of mostly quantitative methods. However, there are spaces in which qualitative data justify part of the proposed strategies and tactics. The sources of information were primary: the founder of the gym, employees, athletes and members of the place and, finally, a massive survey was conducted of people who exercise in other spaces or practice different disciplines. Likewise, people who live near the establishment area were included to learn about their exercise habits. On the other hand, secondary sources, books, internet research and physical and digital files. This work takes 6 fitness centers located in Bogotá as a research reference, especially in places that are close to the Tropa gym, one in the city of Medellín and other establishments that provide aesthetic services. This analysis in order to know and compare the communication strategies, especially in social networks, that they have developed to attract customers. The foregoing, in order to obtain references and make new proposals to the Bogotá Tropa strategy. In addition, the communication strategies of these physical conditioning centers against the conjuncture of COVID - 19 with their clients and target audiences were evaluated.Comunicación Social y PeriodismoComunicador Social y Periodist
