Abstract Introduction: Loneliness is a painful and distressing feeling that leads to a malaise where a person feels alone, despite being surrounded by people, feeling that he or she lacks support, especially of an emotional nature. It is most common in adolescence, but is also prevalent in old age. Objective: To identify the opinion of elderly persons in relation to feelings of loneliness. Methodolog y: The respondents were 73 elderly men and women living at home but attending day care/social centers. A random convenience sample was used, consisting of elderly persons who were able and wished to respond to a short, duly tested questionnaire with mixed response questions. Confidentiality was respected throughout the study. Results: In 78.1% of respondents, a significant person was a family member who was not a spouse (especially daughters and granddaughters); 79.4% reported feeling sometimes or rarely/ never feeling lonely. Widowed or divorced elderly persons were those who most frequently referred to sometimes feeling lonely. To reduce loneliness, they suggested various forms of interaction and activities in which communication predominated. Intergenerational solidarity was also cited. Conclusions: Although loneliness is described as an increasing problem for the elderly, its frequency was not high among those interviewed, perhaps because the respondents engaged in social activities. A longitudinal study aimed at elderly persons who stayed at home, with a representative sample of the elderly population in a given region, who participated or did not participate in social activities, is recommended to provide a broader perspective of the views of the elderly about loneliness

OBJECTIVE: to identify the needs of nursing students in the field of relational competencies. METHOD: qualitative study with an exploratory-descriptive nature. The random sample included 62 students in the 2nd year of the nursing undergraduate program of a school located in the central region of Portugal. The inclusion criterion was the nonexistence of clinical teaching. Data were collected through a form designed to assess relational needs; content analysis was used to analyze data. RESULTS: the results indicated that the students' concept of nursing care at this stage of their education is focused on the performance of nursing tasks and techniques instead of on scientific knowledge. Overall, they are aware that greater personal development and better self-knowledge are determinant for their personal and social well-being and for them to become good professionals. CONCLUSION: these results will support the improvement of an intervention program to be developed with these students.OBJETIVO: identificar las necesidades manifestadas por los estudiantes de enfermería en el área de las habilidades relacionales. This study aimed to investigate access and use of health services during the COVID-19 pandemic among migrants in Portugal. A mixed-methods approach was used. A community-based cross-sectional survey was conducted involving migrant communities residing in the Lisbon Metropolitan Area. Analyses of a subsample of participants (n = 929) examined factors associated with perceived worsening of access to health services during the pandemic. Semi-structured interviews with 14 migrants were conducted and thematically analyzed to further understand experiences and difficulties in health services’ use. Around 44% of surveyed participants reported worsening of access to health services since the pandemic, more frequently women, those with lower income, and those who perceived being at moderate or high risk for COVID-19 infection. Digital change in services and lack of formal and informal support during lockdowns were highlighted by interviewers as main barriers in access to healthcare for migrants. The pandemic renewed concerns about inequalities in healthcare access among migrants. It is key that in following years health systems are able to address the potential accumulated burden of disease.publishersversionpublishe

A RESPONSABILIDADE CIVIL PELA PERDA DE UMA CHANCE: UM ESTUDO À LUZ DA CIÊNCIA MÉDICA

    Get PDF

GESTAÇÃO DE SUBSTITUIÇÃO: O DIÁLOGO DAS FONTES ENTRE A RESOLUÇÃO DO CONSELHO FEDERAL DE MEDICINA Nº 2.320/2022 E O ORDENAMENTO JURÍDICO BRASILEIRO

The CgHaa1-dependent pathway mediates Candida glabrata response and tolerance to acetic acid thereby enhancing colonization of vaginal epithelium

To successfully colonize the vaginal tract Candida glabrata has to cope with various stresses including the presence of acetic acid at a low pH that is produced by the bacteria that co-colonize this niche. The genes/pathways involved in C. glabrata tolerance and response to acetic acid are largely unknown, although these are a highly interesting set of novel targets to control vaginal infections caused by this yeast. Saccharomyces cerevisae response and tolerance to acetic acid was found to be largely mediated by the ScHaa1 transcription factor [1,2,3]. In this work the involvement of CgHaa1 in C. glabrata tolerance and response to acetic acid is demonstrated. Elimination of CgHAA1 gene from C. glabrata genome dramatically increased susceptibility of this pathogenic yeast to acetic acid (30 mM at pH 4.0). Around 140 genes were found to be up-regulated, directly or indirectly, by CgHaa1 in response to acetic acid stress, based on results of a transcriptomic analysis. Functional clustering of the genes activated by CgHaa1 under acetic acid stress shows an enrichment of those involved in carbohydrate metabolism, transport, cell wall maintenance, regulation of internal pH and nucleic acid processing. At least five of the CgHaa1-regulated genes were found to increase C. glabrata tolerance to acetic acid including CgGAD1, encoding a glutamate decarboxylase; CgTPO2/3, encoding a drug efflux pump of the Major Facilitator Superfamily; CgYPS1, encoding a cell wall aspartyl protease; and CAGL0H04851 and CAGL0E03740, encoding two uncharacterized ORFs. Altogether our results are consistent with the concept that the CgHaa1- signalling pathway increases C. glabrata tolerance to acetic acid by reducing the internal accumulation of the acid and by up-regulating the activity of the plasma membrane proton pump H+-ATPase CgPma1, two essential features for a robust weak acid response. The role exerted by CgHaa1 in the ability of C. glabrata to colonize reconstituted vaginal human epithelium (RVHE) in the presence of acetic acid (30 mM at pH 4.0) was also investigated in this work. In the absence of acetic acid wild-type and DCgHaa1 mutant cells were able to colonize RVHE at a similar rate, however, in the presence of acetic acid colonization of the vaginal tissue was markedly reduced in the mutant background. The reduced colonizing capacity of DCgHaa1 mutant cells was correlated with a reduced expression of the adhesin-encoding genes EPA6, EPA7 and EPA1 and with a lower adhesiveness to the extracellular matrix proteins fibronectin and vitronectin

Mitoxantrone (MTX) is a topoisomerase II inhibitor that binds and intercalates with DNA, being used in the treatment of several cancers and multiple sclerosis. Although MTX can induce chemobrain, its neurotoxic mechanisms are poorly studied. This work aimed to identify the adverse outcome pathways (AOPs) activated in the brain upon the use of a clinically relevant cumulative dose of MTX. Three-month-old male CD-1 mice were given a biweekly intraperitoneal administration of MTX over the course of three weeks until reaching a total cumulative dose of 6 mg/kg. Controls were given sterile saline in the same schedule. Two weeks after the last administration, the mice were euthanized and their brains removed. The left brain hemisphere was used for targeted profiling of the metabolism of glutathione and the right hemisphere for an untargeted metabolomics approach. The obtained results revealed that MTX treatment reduced the availability of cysteine (Cys), cysteinylglycine (CysGly), and reduced glutathione (GSH) suggesting that MTX disrupts glutathione metabolism. The untargeted approach revealed metabolic circuits of phosphatidylethanolamine, catecholamines, unsaturated fatty acids biosynthesis, and glycerolipids as relevant players in AOPs of MTX in our in vivo model. As far as we know, our study was the first to perform such a broad profiling study on pathways that could put patients given MTX at risk of cognitive deficits.publishersversionpublishe
