    Evaluation of conservative management in uncomplicated acute appendicitis

    Background: Appendectomy has been the treatment for acute appendicitis for years based on the understanding that acute appendicitis always leads to perforation and peritonitis. However, there is growing evidence that a significant proportion of patients can be successfully managed with conservative treatment without developing gangrene or perforation. Conservative treatment avoids discomfort, surgery-related morbidities and minimizes treatment cost.Methods: 60 patients taken up for conservative management were evaluated and followed up for 6 months. Study patients received intravenous antibiotics for 2 days. Repeated clinical and TLC monitoring were done. In patients whose clinical condition did not improve, appendectomy was performed. Follow-up at 10 days, 30 days, 3 months and 6 months were carried out to assess recurrence in conservatively managed patients.Results: In this study, the mean age was 25.65 years with a standard deviation of ±8.96 years. The incidence of uncomplicated appendicitis was 63.3% in males and 36.7% in females. Mean Alvarado score was 7.75 with a standard deviation of ±1.20. Failure of conservative management (conversion to appendectomy) was observed in 11.7% of patients and 4 patients (6.6%) had recurrence within 6 months. The overall treatment efficacy was 81.7%.Conclusions: In many cases, first attack of uncomplicated acute appendicitis can be treated successfully by conservative management. Treatment failure on primary admission as well as short-term recurrence up to six months after conservative treatment is low and acceptable. Incidence of complications like perforation and abscess formation are also statistically low

    Phonon driven transport in amorphous semiconductors: Transition probabilities

    Inspired by Holstein's work on small polaron hopping, the evolution equations of localized states and extended states in presence of atomic vibrations are derived for an amorphous semiconductor. The transition probabilities are obtained for four types of transitions: from one localized state to another localized state, from a localized state to an extended state, from an extended state to a localized state, and from one extended state to another extended state. At a temperature not too low, any process involving localized state is activated. The computed mobility of the transitions between localized states agrees with the observed `hopping mobility'. We suggest that the observed `drift mobility' originates from the transitions from localized states to extended states. Analysis of the transition probability from an extended state to a localized state suggests that there exists a short-lifetime belt of extended states inside conduction band or valence band. It agrees with the fact that photoluminescence lifetime decreases with frequency in a-Si/SiO2_{2} quantum well while photoluminescence lifetime is not sensitive to frequency in c-Si/SiO2_{2} structure.Comment: 41 pages, 3 figures, submitted to Phys.     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    China and BRICS: Dealing with Power Asymmetry in a Multipolar World

    A distinctive feature of the rise of the multipolar world order in the 21st century has been the proliferation of multilateral institutions. While the raison d'être of these institutions essentially remains elevating multilateral interests, as opposed to unilateral or even for that matter, bilateral interests, national interests continue to remain paramount. Even within multilateral institutions, states undertake various approaches to secure their own national interests over and above that of others. BRICS, a multilateral grouping of Brazil, Russia, India, China and South Africa, in its conception is targeted towards reforming and filling up the space left by the West-dominated structures of global governance. However, the power asymmetry inherent among the BRICS members creates traction that the Chinese require to wield the kind of influence that should concern other members, and certainly India. The dilemma of balancing common interest and national interest within multilateral organisations like the BRICS will be an eminent feature of the varied and multiple interest sets that countries have in an interdependent world. While countries will still find purpose, in the membership of multilateral institutions, the challenge of managing power asymmetry within those organisations will mean that countries find leverage through their engagement with countries outside those groups. For China, the heavyweight within the group, BRICS has multiple purposes, one of them being the elevation of Chinese interests of opposing Western hegemony. However, for India, for which the grouping remains of pertinence (given its stress on addressing emerging and important global challenges such as those of economic development and sustainability) taking a position against the so-called dominance of the West or aligning itself with any particular country or group that stands in opposition to others is a dilemma.     Renal functional reserve in live related kidney donors

    Objectives: To determine the renal functional reserve in live related donors and compare it with that of normal controls. Patients and Methods: The study group consisted of 12 subjects who underwent donor nephrectomy at our cen-tre, more than 6 months prior to this study. The control group consisted of 12 healthy, age and sex-matched vol-unteers who had no previous history of renal disease. The test for renal, functional reserve consisted of a 2 hour base-line creatinine clearance followed by ingestion of 1.2 grains of protein per kg body weight, in the form of cooked red meat, over one hour: A subsequent 2-hour creatinine clearance starting one hour after the completion of pro-tein load was done. Urine collections were meticulously supervised and serum creatinine levels were obtained at the midpoint of each urine collection. Results: The rise in serum creatinine and creatinine clear-ance after protein loading was statistically significant in both donors and controls. The test used for estimation for renal, functional reserve was the percent change in creati-nine clearance after an acute protein load. This was 26.9 % in the donor population and 29.6 % in the controls. The difference between the 2 groups, therefore, was not statisti-cally significant.
