B.M.T. is supported by an individual postdoctoral grant from the Fundacion Alfonso Martin Escudero.Context: Bile acids (BA) are known for their role in intestinal lipid absorption and can also play a role as signaling molecules to control energy metabolism. Prior evidence suggests that alterations in circulating BA levels and in the pool of circulating BA are linked to an increased risk of obesity and a higher incidence of type 2 diabetes in middleaged adults. Objective: We aimed to investigate the association between plasma levels of BA with cardiometabolic risk factors in a cohort of well-phenotyped, relatively healthy young adults. Methods: Body composition, brown adipose tissue, serum classical cardiometabolic risk factors, and a set of 8 plasma BA (including glyco-conjugated forms) in 136 young adults (age 22.1 ± 2.2 years, 67% women) were measured. Results: Plasma levels of chenodeoxycholic acid (CDCA) and glycoursodeoxycholic acid (GUDCA) were higher in men than in women, although these differences disappeared after adjusting for body fat percentage. Furthermore, cholic acid (CA), CDCA, deoxycholic acid (DCA), and glycodeoxycholic acid (GDCA) levels were positively, yet weakly associated, with lean body mass (LBM) levels, while GDCA and glycolithocholic acid (GLCA) levels were negatively associated with 18F-fluorodeoxyglucose uptake by brown adipose tissue. Interestingly, glycocholic acid (GCA), glycochenodeoxycholic acid (GCDCA), and GUDCA were positively associated with glucose and insulin serum levels, HOMA index, low-density lipoprotein cholesterol, tumor necrosis factor alpha, interleukin (IL)-2, and IL-8 levels, but negatively associated with high-density lipoprotein cholesterol, ApoA1, and adiponectin levels, yet these significant correlations partially disappeared after the inclusion of LBM as a confounder.     Acetazolamide in the resolution of cerebrospinal fluid cutaneous fistula after peridural analgesia: Case report1

    AbstractBackgroundCerebrospinal fluid cutaneous fistula following spinal anaesthesia is a serious and rare complication which requires a prompt diagnosis. However, the treatment management is not well defined.Clinical caseFemale aged 50 with a stage IIB cervical carcinoma, who had a peridural catheter inserted at lumbar level. Three days after surgery she refers severe headache, which was corroborated by leakage of cerebrospinal fluid through the puncture. She was prescribed antibiotics and acetazolamide 250mg every 8hours for five days with a favourable outcome.ConclusionIn this case, management with acetazolamide and suture of the fistula inhibits cerebrospinal fluid leakage, without the need for a blood patch

    Sustainable development of an Ultra-High Performance Fibber Reinforced Concrete (UHPFRC): towards partial replacement of cement by slags

    The global production of concrete represents, every year, more than 5% of the anthropogenic emissions of carbon dioxide, mainly from the production of cement. This negative factor can be improved by incorporating supplementary cementitious materials in order to replace cement. In the last few decades, research has been conducted on what it is known as Ultra High Performance Fibber Reinforced Concrete (UHPFRC). The term includes a broad range of materials such as defect-free, dense particle, engineered composite, multi-scale particle and fibber-reinforced cementitious materials, with enhanced properties. UHPFRC has better mechanical and durability properties compared to normal strength concrete. Other benefits of using UHPFRC on a structure includes the reduction of concrete sections, concrete formwork, labour, equipment and time of construction. Despite of the benefits associated to this material, the UHPFRC is still struggling to be universally applied, mainly due to its high cost and its high environmental impact. UHPFRC cost is higher than normal concrete, due to a very high powder content and steel fibber addition. However, the production of UHPFRC using locally available materials, under normal curing conditions, should reduce its cost and turned it into a more attractive construction product. In this paper, the fresh and hardened properties of a specific UHPFRC composition are presented. The mixture replaces a significant percentage of cement by slags, and the results reveal the viability of the proposed mix. The environmental performance of the mixture confirmed the improvement on the material sustainability and allowed the identification of some potential future studies.Universidad de Málaga.     La presente monografía tuvo como propósito resaltar la importancia tener un modelo de Direccionamiento Estratégico desde la Gestión del Talento Humano como factor clave de éxito de las organizaciones, catalogando a la persona como un instrumento que posee habilidades y características que le dan vida, movimiento y acción a toda organización; factor que se logra mediante el fortalecimiento y desarrollo de las competencias requeridas para el logro de los objetivos establecidos en sus planes estratégicos y de desarrollo, tanto a corto como a mediano y largo plazo. Para esto se identificaron 25 herramientas que complementan el enfoque como factor decisivo en el proceso innovador que garantiza la supervivencia y prosperidad de las Organizaciones. Adicionalmente se realizaron y analizaron 15 encuestas de empresas exitosas, cuyo objetivo era demostrar una vez más que éste modelo de Direccionamiento Estratégico facilitaría a la organización el desglose de competencias, el fortalecimiento de las mismas y por ende el incremento de la productividad empresarial.The following Monograph had as a main purpose to highlight the importance of having a Strategic Addressing from Human Talent Managing as a main factor for Organizations to succeed, thinking about people as an instrument who have abilities and features and those abilities give the organizations movement, action and life. That factor is acquired by strengthening and developing the right capacities to obtain the established targets mentioned on its strategic and development plans, caring them also as short, medium and long term. To do that, 25 toolkits were identified which complete the approach as a decisive factor in the innovating process which guarantees the surviving and prosperity of every Organization.
