Manejo endoscópico de la coledocolitiasis gigante en pacientes atendidos en 4 hospitales de la ciudad de Managua, entre enero 2010 y diciembre 2016

La coledocolitiasis es frecuente en nuestro medio y se desconocen las cifras reales. Su espectro clínico va desde asintomático hasta las consecuencias graves, pudiendo ser fatales, siendo devastador al manifiestarse como fuga biliar en el postquirúrgico inmediato. En cualquier caso, el manejo menos invasivo es imperativo. Los Cálculos Gigantes son un problema desafiante y la CPRE es muy útil en su resolución a través sus diferentes modalidades. El objetivo de este estudio, es conocer la efectividad del manejo endoscópico en la coledocolitiasis gigante, principalmente con el uso de la Dilatación papilar con balón grande. Se realizó un estudio observacional, descriptivo y de corte transversal, en 4 hospitales de Managua, de enero 2010 a diciembre 2016, la población de estudio fue 115 pacientes con coledocolitiasis gigante confirmada, la muestra está conformada por el mismo universo. Se realizó análisis estadístico mediante frecuencias y porcentajes para variables cualitativas, promedio y desviación estándar para las cuantitativas. Se estudió la asociación entre variables cuantitativas y cualitativas, usando Prueba de U de Mann-Withney. La edad promedio fue 66.1 años. Predominaron el sexo femenino, la población urbana, los pacientes con antecedentes de cirugía biliar. El diámetro promedio del colédoco fue 18mm, el de los cálculos fue 16.51mm. Se realizaron un total de 136 procedimientos para un promedio de 1.18 por paciente, 15 casos resolvieron con EE sola, 68 pacientes con EE+DEPBG y 26 requirieron EE+DEPBG+LM, el procedimiento fue fallido en solo 6 pacientes. El éxito general de aclaramiento fue 94.8% en dos sesiones. Se presentó 7.8% de complicaciones, ninguna grave, y predominó el sangrado, no hubo mortalidad. No hubo diferencias significativas en cuanto a procedimiento y sexo, procedimiento y hallazgos radiológicos, uso de precorte, duración del procedimiento, número de cálculos o diámetro de los mismos. Sí hubo correspondencia respecto a edad y entre tipo de procedimiento y diámetro del colédoco. Se demostró que el uso de DEPBG en el manejo de cálculos gigantes, es tanto efectivo como seguro. Se deberá garantizar condiciones, equipos y recursos humanos en los hospitales nacionales para generalizar esta técnica. Deberá promoverse la realización de más estudios prospectivos sobre este tem

Software for determining dripline irrigation pipes using a Pocket PC

This communication presents the development of a freeware for mobile devices (Smartphone and Pocket PC). It was developed with a LabVIEW program, and enables engineers and installers to calculate commercial diameters to be used in laterals of drip irrigation, without the need for being at the personal computer. Input data required: drippers flow rates, number of drippers, spacing between the drippers, medium pressure in the lateral and pressure tolerance. As results, it shows commercial diameters that can be used with diode LED, maximum and minimum pressures for each lateral, and a graphic that shows, on a comparative, wether it exceeds each lateral of the pressure tolerance or not

El monocultivo olivarero jiennense: conformación histórica, valores patrimoniales y proyección cultural-turística

Se presentan algunas características básicas del monocultivo olivarero existente en la provincia de Jaén, tales como su alcance superficial y las claves que han marcado su expansión reciente. De la misma forma, se describe la diferente realidad paisajística, agronómica y económica que subyace bajo la aparente uniformidad que concede la existencia de un impresionante y continuado tapiz arbustivo. Completeness in the Mackey topology by norming subspaces

[EN] We study the class of Banach spaces X such that the locally convex space (X, mu(X,Y)) is complete for every norming and norm-closed subspace Y subset of X*, where mu(X, Y) denotes the Mackey topology on X associated to the dual pair . Such Banach spaces are called fully Mackey complete. We show that fully Mackey completeness is implied by Efremov's property (epsilon) and, on the other hand, it prevents the existence of subspaces isomorphic to l(1)(omega(1)). This extends previous results by Guirao et al. (2017) [9] and Bonet and Cascales (2010) [3]. Further examples of Banach spaces which are not fully Mackey complete are exhibited, like C[0, omega(1)] and the long James space J(omega(1)). Finally, by assuming the Continuum Hypothesis, we construct a Banach space with w*-sequential dual unit ball which is not fully Mackey complete. A key role in our discussion is played by the (at least formally) smaller class of Banach spaces X such that (Y, w*) has the Mazur property for every norming and norm-closed subspace Y subset of X*. Analysis of the presence of sustainability in Higher Education Degrees of the Spanish university system

This paper analyzes the extent to which sustainability is present in the curricula of the 16 Education Degree programs belonging to the EDINSOST project: 6 Early Childhood Education Degrees, 7 Primary Education Degrees, 2 Pedagogy Degrees and 1 Social Education Degree. The results obtained suggest that sustainability is present in all Degrees, but not uniformly so. A great disparity is observed in the number of subjects that develop sustainability, with an average of 22.63 subjects per Degree. The competency most present is the ‘ Application of ethical principles related to the values of sustainability in personal and professional behaviors, ’ while the least present is ‘ Sustainable use of resources and prevention of negative impacts on the natural and social environment. ’Sustainability is not developed uniformly in the different universities either. Three universities (UAM, UCA and UIC) develop sustainability competencies at 100%, while others such as the USAL do so at only 50

Some general data of the brilliant academic trajectory of the autor, who lived long periods in the town mentioned, are offered. Especially, attention is drawn to the fact that he was able to take advantage of the resources of anthropology to give the reader an enormous amount of verisimilitude proofs throughout the book, which he titled with the name of its protagonist and to which he added the epithet Biography of a Tzotzil

El laberinto urbano de José Revueltas

On these essay José Revueltas’ aesthetic ideas about the creation of characters, applied on the novel Los días terrenales (1949), are associated with the deformation and monstrosity in the ancient indigenous art, refered at El arte o de la monstruosidad by Edmundo O’Gorman, and at Cosmogonía antigua Mexicana by Rubén Bonifaz Nuño. Reference is also made to the literary criticism poured on the work of Revueltas, and the adversity he had to face, since the Mexican State judged him as a seditious intellectual, and at the same time some leftist militants considered him a petit bourgeois writer, a revisionist intellectual.En este trabajo se asocian las ideas estéticas de José Revueltas presentes en Losdías terrenalesrespecto a la creación de personajes con la deformación en el arte indígena antiguo. ¿Influye la gestión de los empleados en su decisión de seguir utilizando el teletrabajo después de la pandemia?

[ENG] This paper analyses the intention of Spanish companies to continue using telework after the end of the COVID-19 pandemic. With this aim, we consider the influence that the management of several variables related to one of the key stakeholders, employees, has on this intention. This line of study is of great interest, given the implications not only for the management of companies, but also for key aspects such as the goals of the 2030 Agenda for Sustainable Development. To this end, we start with a review of the relevant literatura on this topic, which allows us to raise various research hypotheses. This is followed by an analysis of a sample of 1,879 Spanish SMEs though a binary logistic regression model, which shows the positive influence of managers’ university education and companies’ orientation towards innovation on the use of telework after the pandemic. [SPA] Este trabajo analiza la intención de las empresas españolas de seguir utilizando el teletrabajo tras el fin de la pandemia del COVID-19. Con este objetivo, consideramos la influencia que tiene en esta intención la gestión de diversas variables relacionadas con uno de los principales grupos de interés, los empleados. McCLEC, a robust and stable enzymatic based microreactor platform

A microfluidic chip for cross-linked enzyme crystals (McCLEC) is presented and demonstrated to be a stable, reusable and robust biocatalyst-based device with very promising biotechnological applications. The cost-effective microfluidic platform allows in situ crystallization, cross-linking and enzymatic reaction assays on a single device.
