An Environmental Watch System for the Andean Countries: El Observatorio Andino
An experimental Environmental Watch System, the so-called Observatorio Andino (OA), has been implemented in Venezuela, Colombia, Ecuador, Peru, Bolivia, and Chile over the past 2 yr. The OA is a collaborative and regional network that aims to monitor several environmental variables and develop accurate forecasts based on different scientific tools. Its overall goal is to improve risk assessments, set up early warning systems, support decision-making processes, and provide easily and intuitively understandable spatial maps to end users. The initiative works under the scientific and logistic coordination of the Centro de Modelado Científico (CMC) at Zulia University, Venezuela, and the Centro Internacional para la Investigación del Fenómeno “El Niño” (CIIFEN), and it is operated at a local level by the national weather services (NWSs) of the above-mentioned six Andean nations. The OA provides several freely available model outputs, including meteorological and hydrological forecasts, droughts, fire and flood indices, ecosystems dynamics, climate and health applications, and 5-day high-resolution oceanographic predictions for the eastern Pacific. This article briefly describes the current products, methodologies, and dynamical and statistical modeling outputs provided by the OA. Also, a discussion on how these sets of tools have been put together as a coordinated scientific watch and forecast system for each country and for the entire region is presented. Our experiences over the past 2 yr suggest that this initiative would significantly improve the current decision-making processes in Andean countries
Abstract: Introduction: The National Medical Examination (ENAM) is a mandatory test that students are given to complete after their medical internship. With the changes in law, it now serves as a requirement for the Rural and Urban-Marginal Health Service (SERUMS). Objective: To determine perceptions of usefulness and preparation for ENAM in ten Peruvian medical faculties. Methodology: An analytical cross-sectional study was based on 11 questions from a self-administered questionnaire, with a Cronbach Alpha of 0.65. The variables were crossed according to the type of university, year of studies, and if courses were repeated, with the generalised linear models. Results: Almost all of them (92%) knew what the ENAM is, 82% for what it served, 74% thought they would pass it, and 27% perceived that the knowledge gained in their university was sufficient to pass it. The year of studies was the variable that was most related to the knowledge and perceptions about the ENAM, but some differences were also found according to the type of university and to have failed a previous course. Discussion: The ENAM is still the most important national medical examination, as it compares the level of teaching between universities, and now serves to work in SERUMS. Most students knew what it was and what it was for, but the minority thought they could pass it or that the knowledge they received was sufficient, being occasionally influenced by the year of studies and to a lesser extent by other variables. Abstract 42: Association of Optimism with Cardiovascular Health: Results From the Hispanic Community Health Study/Study (HCHS/SOL) of Latinos Sociocultural Ancillary Study
Introduction:
There is mounting evidence linking positive psychological functioning to restorative health processes and favorable medical outcomes. However, very little is known about the relationship between optimism--an indicator of psychological functioning--and the American Heart Association (
AHA
) defined concept of cardiovascular health (
CVH
), particularly in Hispanics/Latinos of diverse backgrounds.
Hypothesis:
Persons with greater optimism will have more favorable CVH profiles.
Methods:
We analyzed data from adults ages 18-75 that participated in the Hispanic Community Health Study/Study of Latinos (
HCHS/SOL
) Sociocultural Ancillary Study in 2010-11. Optimism was assessed using the Life-Orientation Test-Revised, with scores ranging from 6-24 and higher scores indicative of greater levels of optimism. AHA classification standards were used to derive a composite CVH score with subsequent grouping into categorizes of
poor
(0-7 points),
intermediate
(8-11 points), and
ideal
(12-14 points). Metrics considered when calculating the overall CVH score included diet, body mass index, physical activity, cholesterol, blood pressure, fasting glucose, and smoking status. Multinomial logistic regression was used to examine associations of optimism with ideal and intermediate CVH (poor CVH—ref. group), after adjusting for socio-demographic factors and depressive symptoms.
Results:
Among 4,960 participants with complete data, 9.2% were categorized as having ideal CVH. In multivariable-adjusted models, participants with moderate levels of optimism were more likely to have intermediate [OR = 1.37: 95%CI = 1.08, 1.74] and ideal [OR = 1.61: 95%CI = 1.04, 2.48] CVH when compared to the least optimistic group (
Table 1
).
Conclusions:
The current study offers preliminary evidence for an association between optimism and CVH in a large heterogeneous group of Hispanic/Latino adults. Future studies should explore the concept of optimistic bias and whether exaggerated levels of optimism are detrimental to health
