    A turbofan gas turbine engine includes a forward fan section with a row of fan rotor blades, a core engine, and a fan bypass duct downstream of the forward fan section and radially outwardly of the core engine. The forward fan section has only a single stage of variable fan guide vanes which are variable fan outlet guide vanes downstream of the forward fan rotor blades. An exemplary embodiment of the engine includes an afterburner downstream of the fan bypass duct between the core engine and an exhaust nozzle. The variable fan outlet guide vanes are operable to pivot from a nominal OGV position at take-off to an open OGV position at a high flight Mach Number which may be in a range of between about 2.5-4+.     Anti-bacterial, Anti-oxidant and other Phytochemical Properties of Datura innoxia leaves

    To investigate the phytochemicals present in the leaves of Datura innoxia and to assess its antioxidant and antibacterial properties in different organic solvents, leaf extracts were exposed to different standardized techniques. Folin–Ciocalteu method and Aluminium chloride method proved that the ethanolic extract has maximum phenolic content (72.35± 0.52 mg GAE/g) and flavonoid content (29.21± 1.25 mg EQ/g) respectively. The highest DPPH radical scavenging activity with IC50 value 91.398 µg/ml also was in the ethanolic extract as compared to methanol, hexane and chloroform extracts. Free radical scavenging and antioxidant property of extracts were observed in the sequence of ethanol>methanol>hexane>chloroform. There was a strong correlation between antioxidant activity with total phenolic (DPPH, R2 = 0.41; PPM, R2 = 0.25) and total flavonoid contents (DPPH, R2 = 0.39; PPM, R2=0.23). Ethanolic and methanolic extracts showed antibacterial potential against the tested pathogenic strains; Staphylococcus aureus and Escherichia coli with a zone of inhibition ranging between 16± 0.9 to 27.5±0.8 mm. This study has proved that ethanolic leaf extract of D. innoxia showed bacterial inhibition and antioxidant activities and this herb can be assessed as a potential therapeutic species

    Socio-economic patterning of cardiometabolic risk factors in rural and peri-urban India: Andhra Pradesh children and parents study (APCAPS).

    AIM: To assess the prevalence of cardiometabolic risk factors by socio-economic position (SEP) in rural and peri-urban Indian population. SUBJECTS AND METHODS: Cross-sectional survey of 3,948 adults (1,154 households) from Telangana (2010-2012) was conducted to collect questionnaire-based data, physical measurements and fasting blood samples. We compared the prevalence of risk factors and their clustering by SEP adjusting for age using the Mantel Hansel test. RESULTS: Men and women with no education had higher prevalence of increased waist circumference (men: 8 vs. 6.4 %, P < 0.001; women: 20.9 vs. 12.0 %, P = 0.01), waist-hip ratio (men: 46.5 vs. 25.8 %, P = 0.003; women: 58.8 vs. 29.2 %, P = 0.04) and regular alcohol intake (61.7 vs. 32.5 %, P < 0.001; women: 25.7 vs. 3.8 %, P < 0.001) than educated participants. Unskilled participants had higher prevalence of regular alcohol intake (men: 57.7 vs. 38.7 %, P = 0.001; women: 28.3 vs. 7.3 %, P < 0.001). In contrast, participants with a higher standard of living index had higher prevalence of diabetes (top third vs. bottom third: men 5.2 vs. 3.5 %, P = 0.004; women 5.5 vs. 2.4 %, P = 0.003), hyperinsulinemia (men 29.5 vs. 16.3 %, P = 0.002; women 31.1 vs. 14.3 %, P < 0.001), obesity (men 23.3 vs. 10.6 %, P < 0.001; women 25.9 vs. 12.8 %, P < 0.001), and raised LDL (men 16.8 vs. 11.4 %, P = 0.001; women 21.3 vs. 14.0 %, P < 0.001). CONCLUSIONS: Cardiometabolic risk factors are common in rural India but do not show a consistent association with SEP except for higher prevalence of smoking and regular alcohol intake in lower SEP group. Strategies to address the growing burden of cardiometabolic diseases in urbanizing rural India should be assessed for their potential impact on social inequalities in health

    Effects of the cranial parasite Tylodelphys sp. on the behavior and physiology of puye Galaxias maculatus (Jenyns, 1842)

    Diplostomatid digeneans are well-known manipulators of the behavior of their intermediate hosts. Unencysted metacercariae of Tylodelphys sp. inhabit the cranial cavity of the fish Galaxias maculatus; however, to date they have not been documented to alter their host behavior. The goal of this study was to evaluate the potential effects of Tylodelphys sp. inhabiting the cranial cavity of Galaxias maculatus on host physiology and swimming behavior as well as its reaction to a simulated predation attempt. Blind experiments in the lab were carried out on 56 fish that were filmed individually. The Fulton condition factor (K) was used as an approximation of nutritional status and a respirometry chamber was used to evaluate oxygen consumption rates of fish. Of the 56 fish, 21 were parasitized by Tylodelphys sp. (mean intensity = 30, range from 1 to 101). Parasitized and non-parasitized fish were similar in condition factor and oxygen consumption rates. Furthermore, the oxygen consumption rate of G. maculatus was not correlated with the abundance of Tylodelphys sp. However, parasitized fish more frequently swam close to the water surface, whereas non-parasitized fish more frequently swam at intermediate depths. When faced with a simulated predator attack, unparasitized fish showed more frequent fleeing behavior as well as a more intense post-fleeing activity. Collectively, these results suggest that Tylodelphys sp. inhabiting the cranial cavity of fish may alter their behavior predisposing them to predation by birds

    The Andhra Pradesh Children and Parents Study (APCAPS) was originally established to study the long-term effects of early-life undernutrition on risk of cardiovascular disease. Its aims were subsequently expanded to include trans-generational influences of other environmental and genetic factors on chronic diseases in rural India. It builds on the Hyderabad Nutrition Trial (HNT) conducted in 1987-90 to compare the effects on birthweight of a protein-calorie supplement for pregnant women and children. The index children of HNT and their mothers were retraced and examined in 2003-05, and the children re-examined as young adults aged 18-21 years in 2009-10. The cohort was expanded to include both parents and siblings of the index children in a recently completed follow-up conducted in 2010-12 (N=∼6225 out of 10,213 participants). Recruitment of the remaining residents of these 29 villages (N=∼55,000) in Ranga Reddy district of Andhra Pradesh is now under way. Extensive data on socio-demographic, lifestyle, medical, anthropometric, physiological, vascular and body composition measures, DNA, stored plasma, and assays of lipids and inflammatory markers on APCAPS participants are available.     A randomized controlled trial of nonoperative treatment versus open reduction and internal fixation for stable, displaced, partial articular fractures of the radial head: The RAMBO trial

    Background: The choice between operative or nonoperative treatment is questioned for partial articular fractures of the radial head that have at least 2 millimeters of articular step-off on at least one radiograph (defined as displaced), but less than 2 millimeter of gap between the fragments (defined as stable) and that are not associated with an elbow dislocation, interosseous ligament injury, or other fractures. These kinds of fractures are often classified as Mason type-2 fractures. Retrospective comparative studies suggest that operative treatment might be better than nonoperative treatment, but the long-term results of nonoperative treatment are very good. Most experts agree that problems like reduced range of motion, painful crepitation, nonunion or bony ankylosis are infrequent with both nonoperative and operative treatment of an isolated displaced partial articular fracture of the radial head, but determining which patients will have problems is difficult. A prospective, randomized comparison would help minimize bias and determine the balance between operative and nonoperative risks and benefits. Methods/Design. The RAMBO trial (Radial Head - Amsterdam - Amphia - Boston - Others) is an international prospective, randomized, multicenter trial. The primary objective of this study is to compare patient related outcome defined by the 'Disabilities of Arm, Shoulder and Hand (DASH) score' twelve months after injury between operative and nonoperative treated patients. Adult patients with partial articular fractures of the radial head that comprise at least 1/3rd of the articular surface, have ≥ 2 millimeters of articular step-off but less than 2 millimeter of gap between the fragments will be enrolled. Secondary outcome measures will be the Mayo Elbow Performance Index (MEPI), the Oxford Elbow Score (OES), pain intensity through the 'Numeric Rating Scale', range of motion (flexion arc and rotational arc), radiographic appearance of the fracture (heterotopic ossification, radiocapitellar and ulnohumeral arthrosis, fracture healing, and signs of implant loosening or breakage) and adverse events (infection, nerve injury, secondary interventions) after one year. Discussion. The successful completion of this trial will provide evidence on the best treatment for stable, displaced, partial articular fractures of the radial head. Trial registration. The trial is registered at the Dutch Trial Register: NTR3413

    Prediction model for diabetes mellitus using machine learning algorithms for enhanced diagnosis and prognosis in healthcare

    Diabetes mellitus (DM) affects the hormone insulin, which causes improper glucose metabolism and raises the body’s blood sugar levels. With 4.2 million fatalities in 2019, DM is one of the top 10 global causes of mortality. Early detection of DM will aid in its treatment and avert complications. There must be a quick and simple technique to diagnose it. Such diseases can be managed and human lives can be saved with early diagnosis. Smart prediction techniques like Machine Learning (ML) have produced encouraging outcomes in predictive classifications. There has been a lot of interest in ML-based decision-support platforms for the prediction of chronic illnesses to provide improved diagnosis and prognosis help to medical professionals and the general population. By building predictive models using diagnostic medical datasets gathered from DM patients, ML algorithms efficiently extract knowledge that helps predict diabetic individuals. The association between DM and a healthy lifestyle is used in the model. In this study, the NHANES (National Health and Nutrition Examination Survey) data set is utilized, and five ML methods such as Artificial Neural Networks (ANN), CATBoost, XGBoost, XGBoost-histogram, and Light GBM to predict DM. The results of the experiment demonstrate that the XGB-h model outperformed other ML methods regarding area under the receiver operating characteristic curve (AUC-ROC), and accuracy. The most effective XGB-h framework can be used in a mobile app and a website to rapidly forecast DM. Real-time prediction using details delivered by the model at runtime can be developed as a whole bundle as a product. Clinicians can quickly determine who is likely to get diabetes using the proposed strategy, which will facilitate prompt intervention and caring
