    Epidemiology of West Nile in Europe and in the Mediterranean Basin

    In the last 30 years several cases of West Nile (WN) virus infection were reported in horses and humans in Europe and in the Mediterranean Basin. Most of them were determined by strains of the Lineage 1 included in the European Mediterranean/Kenyan cluster. Strains of this cluster are characterised by a moderate pathogenicity for horses and humans and limited or no pathogenicity for birds. In recent years, however, WN cases determined by strains grouped in the Israeli/American cluster of Lineage 1 or in the lineage 2 have been reported in Hungary and Austria. The role of migrating birds in introducing new viruses to Europe has been often demonstrated. The migratory birds, which may be infected in their African wintering places, carry the virus northward to European sites during spring migrations. In the past, the virus introduction determined occasional cases of WN. In the recent years, new epidemiological scenarios are developing. In few occasions it has been evidenced the capability of WNV strains of overwintering by using local birds and mosquitoes. Species of Culex amongst mosquitoes and magpies (Pica pica), carrion crows (Corvus corone) and rock pigeons (Columba livia) amongst resident birds are the most probable species involved in this hypothetical WND endemic cycle

    Plasma levels of TNF-α, IFN-γ, IL-4 and IL-10 during a course of experimental contagious bovine pleuropneumonia

    <p>Abstract</p> <p>Background</p> <p>Contagious Bovine Pleuropneumonia (CBPP), caused by <it>Mycoplasma mycoides </it>subsp. <it>mycoides</it>, is widespread in sub-Saharan Africa. The current live vaccine T1/44 has limited efficacy and occasionally leads to severe side effects in the animals. A better understanding of the immune responses triggered by <it>Mycoplasma mycoides </it>subsp. <it>mycoides </it>and their role in disease progression will help to facilitate the design of a rational vaccine. Currently, knowledge of cytokines involved in immunity and immunopathology in CBPP is rather limited. The aim of this study was to characterize the <it>in vivo </it>plasma concentrations of the cytokines TNF-α, IFN-γ, IL-4, IL-10 and the overall role of CD4<sup>+ </sup>T cells in the development of cytokine levels during a primary infection. Plasma cytokine concentrations in two groups of cattle (CD4<sup>+ </sup>T cell-depleted and non-depleted cattle) experimentally infected with <it>Mycoplasma mycoides </it>subsp. <it>mycoides </it>were measured and their relationship to the clinical outcomes was investigated.</p> <p>Results</p> <p>Plasma cytokine concentrations varied between animals in each group. Depletion of CD4<sup>+ </sup>T cells did not induce significant changes in plasma levels of TNF-α, IL-4, and IL-10, suggesting a minor role of CD4<sup>+ </sup>T cells in regulation or production of the three cytokines during the time window of depletion (1-2 weeks post depletion). Unexpectedly, the IFN-γ concentrations were slightly, but statistically significantly higher in the depleted group (p < 0.05) between week three and four post infection. Three CD4<sup>+ </sup>T cell-depleted animals that experienced severe disease, had high levels of TNF-α and IFN-γ. Only one severely diseased non-depleted animal showed a high serum concentration of IL-4 post infection.</p> <p>Conclusions</p> <p>Comparison of most severely diseased animals, which had to be euthanized prior to the expected date, versus less severe diseased animals, irrespective of the depletion status, suggested that high TNF-α levels are correlated with more severe pathology in concomitance with high IFN-γ levels.</p

    Novel mutation in the ligand‐binding domain of the androgen receptor gene (1790p) associated with complete androgen insensitivity syndrome

    Mutations in the X-linked androgen receptor (AR) gene cause androgen insensitivity syndrome (AIS), resulting in an impaired embryonic sex differentiation in 46,XY genetic men. Complete androgen insensitivity (CAIS) produces a female external phenotype, whereas cases with partial androgen insensitivity (PAIS) have various ambiguities of the genitalia. Mild androgen insensitivity (MAIS) is characterized by undermasculinization and gynecomastia. Here we describe a 2-month-old 46,XY female patient, with all of the characteristics of CAIS. Defects in testosterone (T) and dihydrotestosterone (DHT) synthesis were excluded. Sequencing of the AR gene showed the presence in exon 6 of a T to C transition in the second base of codon 790, nucleotide position 2369, causing a novel missense Leu790Pro mutation in the ligand-binding domain of the AR protein. The identification of a novel AR mutation in a girl with CAIS provides significant information due to the importance of missense mutations in the ligand-binding domain of the AR, which are able to induce functional abnormalities in the androgen binding capability, stabilization of active conformation, or interaction with coactivators

    Multispacer sequence typing of Coxiella burnetii from milk and hard tick samples from ruminant farms in Lebanon

    his study was carried out to detect and characterize Coxiella burnetii in ruminant milk samples and in different tick species from seropositive farms in four Lebanese regions. Milk and tick samples were screened for C. burnetii presence by quantitative real-time PCR (qPCR) targeting IS1111 region followed by multispacer sequence typing (MST). The overall positive percentages of 9.6% (27/282) and 95.45% (84/88) for C. burnetii were recorded in ruminant milk and tick samples, respectively. In detail, the C. burnetii DNA was recorded in 52/54 (96.3%) of Rhipicephalus annulatus, 20/21 (95.24%) of Rhipicephalus turanicus, 6/6 (100%) of Hyalomma anatolicum, 5/6 (83.3%) of Rhipicephalus sanguineus and 1/1 of Rhipicephalus bursa. After genotyping of some IS1111-positive samples (17/111), different MST genotypes were identified. Out of 15 positive ticks, 10 were infected with MST2 genotype, 4 were infected with MST7 genotype and 1 was infected with MST57. Moreover, genotypes MST20 and MST58 were found in one cow and one goat milk samples, respectively. The present study confirmed the high genetic diversity of C. burnetii in Lebanon

    Rise of pediatric robotic surgery in Italy: a multicenter observational retrospective study

    open15noAbstract Background: The minimally invasive surgery (MIS) in term of robot-assisted surgery changed in a dramatic way the surgical approach either in adults or children. For many specialties (urology, gynecology, general surgery) robotic surgery rapidly became the gold standard for some procedures, while the experience in pediatric population is not wide for some reasons. The aim of this study is to retrospective analyze trends of application of robotic surgery in pediatric patients across the country, focusing on indications, limitations, development, and training acquired by national experience and in comparison to the literature. Methods: We made a retrospective multicenter study on behalf of Italian Society of Pediatric Surgery. We performed a census among all pediatric surgery units in the country to enroll those performing robotic surgery on children between 2013 and 2019. Results: We enrolled 7 pediatric surgery referral Centers (Ancona, Bologna, Brescia, Genova, Pavia, Pescara, Siena). A total of 303 patients were included in the study, 164 males (54%) and 139 females (46%). The most commonly performed interventions for each anatomic area were respectively atypical pulmonary resection (38%), pyeloplasty (49%), and fundoplication (30%).     West Nile virus Epidemic in Horses, Tuscany Region, Italy

    During the late summer of 1998, veterinary authorities in Tuscany, Italy, received reports of cases of neurologic disease among horses residing in a large wetland area located in the provinces of Florence and Pistoia. West Nile virus was isolated from two of the six horses that died or were euthanized. A retrospective epidemiologic study identified 14 clinical neurologic cases that occurred from August 20 to October 6 (attack rate of 2.8%). A serologic survey conducted over a 700-km2 area in stables with and without apparent clinical cases confirmed a wider spread of the infection, with an overall seroprevalence rate of 38% in the affected area. No significant differences in age-specific prevalence were observed, suggesting that the horses residing in the area had not been exposed previously to West Nile virus and supporting the hypothesis of its introduction in the wetland area during the first half of 1998

    In the last ten years, the risk of introduction and spread of several major viral epidemics of list A diseases has been constantly increasing worldwide. Italy, due both to its geographical position, in the middle of the Mediterranean basin, and to it being one of the world’s largest importing countries of live animals and commodities of animal origin, is particularly at risk. Another important predisposing factor is the complete susceptibility of Italian animal populations. All the available evidence shows that speed is vital in handling outbreaks of infectious diseases. This necessitates planning of a high order and an executive empowered by a wide acceptance of the strategies to be adopted. This project aims to improve the managing skills of Veterinary Services in the event of an exotic disease outbreak, developing an integrated management system, of which one of the main sections is an information system able to guide and coordinate the services’ activities while containing the outbreak. The system has been initially tested in 4 Italian regions, markedly different as far as geographical features and farming practices are concerned: this has allowed us to develop a system better fitting the remarkably uneven nature of Italian territory and livestock rearing. Presently, the system is in the adaptation stage to the entire national territory; new functions are being introduced, especially where report activities are concerned. The following objectives have been achieved: 1. Contingency plans for list A diseases 2. Collection of relevant legislation 3. Development of a geographical information system (GIS), linked with the National Animal Identification System 4. Link with the European System for Animal Movements (ANIMO) 5. Implementation of a telematic system to support epidemics management 6. Training of personnel This system provides the NHS with an information system for the management of disease epidemics; the system can be used as a decision tool by every level of the NHS: central (Ministry of Health), regional (Regional Veterinary Agencies) and local (Veterinary Services operating locally).     L'Istituto Zooprofilattico Sperimentale dell'Abruzzo e del Molise è impegnato da anni nella ricerca e sperimentazione mirate ad approfondire la conoscenza delle malattie esotiche, cioè di quelle malattie che non sono presenti sul territorio nazionale. Per poter raggiungere tali obiettivi, è importante impegnarsi nella creazione di una rete di collaborazioni con Laboratori ed Enti di ricerca a livello nazionale ed internazionale. I rapporti che l'Istituto ha instaurato soprattutto con la Namibia e più di recente Botswana si stanno rivelando di notevole aiuto per la reciproca crescita tecnico-scientifica. L'Istituto coopera con i Servizi Veterinari della Namibia fornendo supporto tecnico-scientifico e nel 2005, ha allestito un proprio Laboratorio di Virologia, presso il Central Veterinary Laboratory di Windhoek, nel quale, personale dell'Istituto e namibiano, in unita' di intenti, condividono le competenze diagnostiche e la ricerca scientifica.     The Istituto Zooprofilattico Sperimentale dell’Abruzzo e del Molise ‘G. Caporale’ (IZS A&M) has been engaged for many years in research and studies designed to increase knowledge and expertise when dealing with ‘exotic diseases’ namely, those diseases which are not present in a country. To achieve these objectives, it is important to create cooperation networks with laboratories and research organisations at national and international levels. The relationship between the IZS A&M with Namibia in particular and, more recently with Botswana, are proving to be very valuable to mutual technical/scientific growth. In 2005, the National Reference Centre for the Study of Exotic Diseases set up its own Virology Laboratory at the Windhoek Central Veterinary Laboratory where the Namibian and IZS A&M personnel, working towards common goals, share diagnostic responsibilities and scientific research. The authors describe the activities involved in this joint project
