Differential expression of salivary glycoproteins in aggressive and chronic periodontitis
OBJECTIVES:The aim of this study was to compare the pattern of secretion and the expression of mucin glycoprotein-2 (MG2) and lactoferrin in individuals with or without periodontitis. MATERIAL AND METHODS: Five individuals with aggressive periodontitis (APG), 5 with generalized chronic periodontitis (CPG) and 5 without periodontitis (CG) were enrolled after informed consent. Non-stimulated and stimulated submandibular and sublingual saliva was collected and samples analyzed by Western blot probed with specific antibodies. RESULTS: Stimulated and non-stimulated salivary flow rates did not differ among groups. Western blot analysis revealed that stimulation led to: an increase in MG2 expression in all groups, and to lactoferrin expression in APG and CPG. In non-stimulated saliva, CG exhibited the highest expression of both glycoproteins. In stimulated saliva, CG exhibited the highest expression of MG2, whereas APG the highest of lactoferrin. CONCLUSIONS: The pattern of secretion of MG2 and lactoferrin in health and disease is complex. Although the present study analyzed samples from a limited number of participants, the reduced expression of MG2 and lactoferrin in APG and CPG under non-stimulated condition, the predominant circumstance of salivary secretion during the day, suggests that these salivary constituents may play a role in the etiopathogenesis of these diseases
Abordagem oncoplástica no tratamento conservador do câncer mamário: análise de custos
PURPOSE: To analyze the direct costs of conservative surgical treatment of breast cancer, performed in a university hospital, to the Brazilian National Health Care Public System (SUS), checking the impact of the oncoplastic approach on these costs. METHODS: One hundred thirty eight breast cancer patients who had undergone conservative treatment with oncoplastic approach (n=36) or not (control group, n=102), in the period from 2005 to 2010, were enrolled. Sociodemographic and clinical data were recorded. The direct costs of the surgical procedure were obtained and analyzed. RESULTS: Groups did not differ in regard to age (p=0.963), and patients in oncoplastic group had a longer time of hospital stay (p=0.000). The median direct cost for the oncoplastic group was R229.00 (p=0.000). CONCLUSION: The oncoplastic approach has generated higher direct costs in conservative surgical treatment of breast cancer to SUS.OBJETIVO: Analisar os custos diretos do tratamento cirúrgico conservador do câncer mamário, realizado pelo Sistema Único de Saúde (SUS) em um hospital universitário, verificando o impacto da abordagem oncoplástica sobre estes custos. RESULTADOS: Não houve diferença entre os grupos quanto à idade (p=0.963), e o tempo de internação hospitalar foi maior no grupo oncoplástica (p=0,000). A mediana dos custos diretos do grupo oncoplástica foi de R229,00 (p=0,000). A multibiomarker approach in the caged neotropical fish to assess the environment health in a river of central Brazilian Cerrado
Embargo until 19 August 2022.Water safety is a world-wide concern and several efforts have been made in order to ensure the conservation of aquatic ecosystems. Water quality monitoring must be performed with an integrated approach using biomonitor organisms allied to water parameters. Nonetheless, very few studies have focused on biomarker responses in neotropical fish, especially in the freshwater ecosystem of Brazilian Cerrado savanna. In present study, the active biomonitoring of the João Leite river (central Brazilian Cerrado river) was performed through the evaluation of biomarker responses in caged Astyanax lacustris in combination with land use classification and analysis of water parameters. Caged fish were exposed for seven days at four sites along the river and two control groups were kept in a tank under controlled conditions. Results showed that pasture was the predominant land use in the João Leite river basin (54.07%), followed by natural vegetation (34.92%) and other kind of land use (11.01%). Water analyses showed metal concentrations (Mn and Fe) above the maximum allowed by Brazilian regulation, with particularly higher concentrations at Site 2 (near to pasture area). Biomarker responses did not show significant differences for somatic and mutagenic biomarkers between sites. However, the comet assay showed high DNA damage at Sites 2 and 3, indicating genotoxic effects in caged fish at pasture areas. Histopathological analysis showed highest frequency of leukocyte infiltration in liver of fish from Site 2, confirming the ecotoxic effects on A. lacustris in streams impacted by grazing activities. DNA damage and leukocyte infiltration in fish hepatic tissues were sensitive biomarkers in the neotropical fish A. lacustris to assess the environment health of the Cerrado river. These results showed the importance of using a multibiomarker approach in environmental risk assessment, especially in areas more at risk from anthropogenic pollution.acceptedVersio
TRANSTORNOS PSIQUIÁTRICOS ASSOCIADOS A CIRURGIA BARIÁTRICA E SEUS DESAFIOS NO SÉCULO XXI
Objective: Review the literature on mental disorders related to bariatric surgery. Methods: This is an integrative literature review. The electronic survey was carried out in the LILACS, Pubmed, Scielo, VHL and CAPES journal portal databases. You descriptors in health sciences (DeC) “bariatric surgery” and “mental disorders” were applied as search terms. Results: 9 studies were selected for analysis qualitative, 6 from the Pubmed platform, 2 from Scielo and 1 from the LILACS platform. It was observed that patients who are candidates for bariatric surgery have a high prevalence of disorders psychological problems, such as depression, anxiety and binge eating disorder, especially when associated with post-surgical weight recovery. It has also been discussed that bariatric surgery is an effective technique to promote improvement in the symptoms of anxiety and binge eating. Final Considerations: Strong correlation was found between the development of depression and anxiety disorders and weight gain after surgery. Psychological treatment of obese patients with psychological disorders can be carried out with cognitive and behavioral therapies, with a higher success rate if initiated after surgery, before weight gain.Objetivo: Revisar a literatura sobre transtornos mentais relacionados à cirurgia bariátrica. Métodos: Trata-se de uma revisão integrativa da literatura. O levantamento electrónico foi realizado nas bases de dados LILACS, Pubmed, Scielo, BVS e portal de periódicos CAPES. Os descritores em ciências da saúde (DeC) “cirurgia bariátrica” e “transtornos mentais” foram aplicados como termos de busca. Resultados: foram selecionados 9 estudos para análise qualitativa, sendo 6 da plataforma Pubmed, 2 da Scielo e 1 da plataforma LILACS. Observou-se que pacientes candidatos à cirurgia bariátrica apresentam alta prevalência de distúrbios psicológicos, como depressão, ansiedade e transtorno da compulsão alimentar periódica, principalmente quando associados à recuperação de peso pós-cirúrgica. Também tem sido discutido que a cirurgia bariátrica é uma técnica eficaz para promover melhora dos sintomas de ansiedade e compulsão alimentar. Considerações Finais: Foi encontrada forte correlação entre o desenvolvimento de transtornos de depressão e ansiedade e o ganho de peso após a cirurgia. O tratamento psicológico de pacientes obesos com distúrbios psicológicos pode ser realizado com terapias cognitivas e comportamentais, com maior taxa de sucesso se iniciadas após a cirurgia, antes do ganho de peso
Pervasive gaps in Amazonian ecological research
Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4
While the increasing availability of global databases on ecological communities has advanced our knowledge
of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In
the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of
Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus
crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced
environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian

Goodbye Hartmann trial: a prospective, international, multicenter, observational study on the current use of a surgical procedure developed a century ago
Background: Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann's procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods: This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results: 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Severe complications (Clavien-Dindo ≥ 3b) were higher in the HP group (P < 0.001). 30-day mortality was higher in HP patients (13.7%), especially in case of bowel perforation and diffused peritonitis. 1-year follow-up showed no differences on ostomy reversal rate between HP and RPA. (P = 0.127). A backward likelihood logistic regression model showed that RPA was preferred in younger patients, having low ASA score (≤ 3), in case of large bowel obstruction, absence of colonic ischemia, longer time from admission to surgery, operating early at the day working hours, by a surgeon who performed more than 50 colorectal resections. Conclusions: After 100 years since the first Hartmann's procedure, HP remains the most common treatment for left-sided colorectal emergencies. Treatment's choice depends on patient characteristics, the time of surgery and the experience of the surgeon. RPA should be considered as the gold standard for surgery, with HP being an exception

Diretriz da Sociedade Brasileira de Cardiologia sobre Diagnóstico e Tratamento de Pacientes com Cardiomiopatia da Doença de Chagas
This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.
Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.
