    Monstrous predatory vampires and beneficent fairy-godmothers: British post-war colonial development in Africa

    This thesis explores the concept of colonial development, as enacted by the Attlee government during the immediate post-war period. It focuses on Africa, reflecting the ‘second colonial occupation’ of the continent during this period, and examines both economic and social welfare development initiatives. Post-war colonial development in the British African territories had two main aims: firstly, to increase the production of raw materials, to aid the reconstruction of the metropole and earn dollars on the international markets; and secondly, to improve the standard of living among colonial populations. This thesis explores the contradictions inherent in these two types of development. It can be seen that, although Britain was largely unsuccessful in this period with economic development programmes in Africa, it had some modest success with colonial social-welfare initiatives. The thesis also examines the extent to which Arthur Creech Jones, Colonial Secretary 1946-1950, shaped colonial policy in Africa based on his Fabian beliefs. It examines how far British colonial policy in this period can be characterised as ‘socialist’, and how far metropolitan and colonial populations were separated by narratives of progress and development in this period. This thesis also argues that colonial development in Africa in this period was shaped, rhetorically, ideologically and pragmatically, by the context of British reconstruction under the Marshall Plan. Colonial development was an arena in which Britain’s relationships with western Europe and the United States (the ‘special relationship’) could be explored, strengthened and sometimes challenged. The incipient Cold War imbued British policy in Africa with specific tensions, particularly relating to American ‘anti-imperialism’ and the threat of Soviet communist expansion across the continent. Colonial development, and the negotiation of such against the pressures exerted by Britain’s international political role, can thus be used as a lens, through which to view British foreign policy under the Attlee government

    Intermittent aeration to improve wastewater treatment efficiency in pilot-scale constructed wetland

    Forced aeration of horizontal subsurface flow constructed wetlands (HSSF CWs) is nowadays a recognized method to improve treatment efficiency, mainly in terms of ammonium removal. While numerous investigations have been reported testing constant aeration, scarce information can be found about the efficiency of intermittent aeration. This study aims at comparing continuous and intermittent aeration, establishing if there is an optimal regime that will increase treatment efficiency of HSSF CWs whilst minimizing the energy requirement. Full and intermittent aeration were tested in a pilot plant of three HSSF CWs (2.64 m2 each) fed with primary treated wastewater. One unit was fully aerated; one intermittently aerated (i.e. by setting a limit of 0.5 mg/L dissolved oxygen within the bed) with the remaining unit not aerated as a control. Results indicated that intermittent aeration was the most successful operating method. Indeed, the coexistence of aerobic and anoxic conditions promoted by the intermittent aeration resulted in the highest COD (66%), ammonium (99%) and total nitrogen (79%) removals. On the other hand, continuous aeration promotes ammonium removal (99%), but resulted in nitrate concentrations in the effluent of up to 27 mg/L. This study demonstrates the high potential of the intermittent aeration to increase wastewater treatment efficiency of CWs providing an extreme benefit in terms of the energy consumption

    Pleomorphic adenocarcinoma of the lacrimal gland with multiple intracranial and spinal metastases

    BACKGROUND: Pleomorphic adenoma of the lacrimal gland is known to undergo malignant transformation when incompletely excised. Even if such a malignant change occurs, intracranial direct invasion and leptomeningeal seeding are seldom encountered. CASE PRESENTATION: A 50-year-old woman presented with malignant transformation associated with both intracranial invasion and multiple intracranial and spinal disseminations in the third recurrence of pleomorphic adenoma of the lacrimal gland, 6 years after initial treatment. MRI demonstrated increased extent of orbital mass, extending to the cavernous sinus. The patient underwent intensity-modulated radiation therapy (IMRT) and Gamma Knife radiosurgery. Follow-up MRI showed multiple leptomeningeal disseminations to the intracranium and spine. CONCLUSION: It is important to recognize that leptomeningeal intracranial and spinal disseminations of pleomorphic adenocarcinoma can occur, although it is extremely rare. To our knowledge, we report the first case of pleomorphic adenocarcinoma of the lacrimal gland presumably metastasizing to the intracranium and spine

    Prediction or causality? A scoping review of their conflation within current observational research.

    Etiological research aims to uncover causal effects, whilst prediction research aims to forecast an outcome with the best accuracy. Causal and prediction research usually require different methods, and yet their findings may get conflated when reported and interpreted. The aim of the current study is to quantify the frequency of conflation between etiological and prediction research, to discuss common underlying mistakes and provide recommendations on how to avoid these. Observational cohort studies published in January 2018 in the top-ranked journals of six distinct medical fields (Cardiology, Clinical Epidemiology, Clinical Neurology, General and Internal Medicine, Nephrology and Surgery) were included for the current scoping review. Data on conflation was extracted through signaling questions. In total, 180 studies were included. Overall, 26% (n = 46) contained conflation between etiology and prediction. The frequency of conflation varied across medical field and journal impact factor. From the causal studies 22% was conflated, mainly due to the selection of covariates based on their ability to predict without taking the causal structure into account. Within prediction studies 38% was conflated, the most frequent reason was a causal interpretation of covariates included in a prediction model. Conflation of etiology and prediction is a common methodological error in observational medical research and more frequent in prediction studies. As this may lead to biased estimations and erroneous conclusions, researchers must be careful when designing, interpreting and disseminating their research to ensure this conflation is avoided

    Lessons learnt when accounting for competing events in the external validation of time-to-event prognostic models.

    BACKGROUND: External validation of prognostic models is necessary to assess the accuracy and generalizability of the model to new patients. If models are validated in a setting in which competing events occur, these competing risks should be accounted for when comparing predicted risks to observed outcomes. METHODS: We discuss existing measures of calibration and discrimination that incorporate competing events for time-to-event models. These methods are illustrated using a clinical-data example concerning the prediction of kidney failure in a population with advanced chronic kidney disease (CKD), using the guideline-recommended Kidney Failure Risk Equation (KFRE). The KFRE was developed using Cox regression in a diverse population of CKD patients and has been proposed for use in patients with advanced CKD in whom death is a frequent competing event. RESULTS: When validating the 5-year KFRE with methods that account for competing events, it becomes apparent that the 5-year KFRE considerably overestimates the real-world risk of kidney failure. The absolute overestimation was 10%age points on average and 29%age points in older high-risk patients. CONCLUSIONS: It is crucial that competing events are accounted for during external validation to provide a more reliable assessment the performance of a model in clinical settings in which competing risks occur

    Differential expression of collectins in human placenta and role in inflammation during spontaneous Labor.

In the present study, we aimed to investigate their placental transcript synthesis, labor associated differential expression and localization at feto-maternal interface, and their functional implication in spontaneous labor. The study involved using feto-maternal interface (placental/decidual tissues) from two groups of healthy pregnant women at term (≥37 weeks of gestation), undergoing either elective C-section with no labor ('NLc' group, n = 5), or normal vaginal delivery with spontaneous labor ('SLv' group, n = 5). The immune function of SP-D, on term placental explants, was analyzed for cytokine profile using multiplexed cytokine array. SP-A, SP-D and MBL transcripts were observed in the term placenta. The 'SLv' group showed significant up-regulation of SP-D (p = 0.001), and down-regulation of SP-A (p = 0.005), transcripts and protein compared to the 'NLc' group. Significant increase in 43 kDa and 50 kDa SP-D forms in placental and decidual tissues was associated with the spontaneous labor (p<0.05). In addition, the MMP-9-cleaved form of SP-D (25 kDa) was significantly higher in the placentae of 'SLv' group compared to the 'NLc' group (p = 0.002). Labor associated cytokines IL-1α, IL-1β, IL-6, IL-8, IL-10, TNF-α and MCP-1 showed significant increase (p<0.05) in a dose dependent manner in the placental explants treated with nSP-D and rhSP-D.     ZNF804a Regulates Expression of the Schizophrenia-Associated Genes PRSS16, COMT, PDE4B, and DRD2

    ZNF804a was identified by a genome-wide association study (GWAS) in which a single nucleotide polymorphism (SNP rs1344706) in ZNF804a reached genome-wide statistical significance for association with a combined diagnosis of schizophrenia (SZ) and bipolar disorder. Although the molecular function of ZNF804a is unknown, the amino acid sequence is predicted to contain a C2H2-type zinc-finger domain and suggests ZNF804a plays a role in DNA binding and transcription. Here, we confirm that ZNF804a directly contributes to transcriptional control by regulating the expression of several SZ associated genes and directly interacts with chromatin proximal to the promoter regions of PRSS16 and COMT, the two genes we find upregulated by ZNF804a. Using immunochemistry we establish that ZNF804a is localized to the nucleus of rat neural progenitor cells in culture and in vivo. We demonstrate that expression of ZNF804a results in a significant increase in transcript levels of PRSS16 and COMT, relative to GFP transfected controls, and a statistically significant decrease in transcript levels of PDE4B and DRD2. Furthermore, we show using chromatin immunoprecipitation assays (ChIP) that both epitope-tagged and endogenous ZNF804a directly interacts with the promoter regions of PRSS16 and COMT, suggesting a direct upregulation of transcription by ZNF804a on the expression of these genes. These results are the first to confirm that ZNF804a regulates transcription levels of four SZ associated genes, and binds to chromatin proximal to promoters of two SZ genes. These results suggest a model where ZNF804a may modulate a transcriptional network of SZ associated genes

    Reporting of prognostic markers: current problems and development of guidelines for evidence-based practice in the future

    Prognostic markers help to stratify patients for treatment by identifying patients with different risks of outcome (e.g. recurrence of disease), and are important tools in the management of cancer and many other diseases. Systematic review and meta-analytical approaches to identifying the most valuable prognostic markers are needed because (sometimes conflicting) evidence relating to markers is often published across a number of studies. To investigate the practicality of this approach, an empirical investigation of a systematic review of tumour markers for neuroblastoma was performed; 260 studies of prognostic markers were identified, which considered 130 different markers
