    Process of nursing care in a post-operative patient of complete correction of Tetralogy of Fallot. Case study and literature review.

    Tetralogy of Fallot is a congenital heart disease cyanotic common in childhood, whose surgical treatment has a high rate of success in the short and long term. Complications of surgical treatment may and should be corrected promptly, being crucial the nursing intervention in the evolution of the patient: sign valuations of right ventricular dysfunction, implementation of interventions for minimize the effects of this dysfunction, collaboration in the treatment of the entity and the constant evaluation of the patient. A case of a patient of 1 year and 5 months, is presented where after a cardiac valuation and an electrocardiogram is diagnosed with Tetralogy of Fallot, and you are subsequently underwent surgery without reporting no complications during the surgery. They were handled several diagnoses nursing, with objectives to be followed and specific interventions carried out according to the presentation of the manifestations of the patient in the study.La Tetralogía de Fallot es una cardiopatía congénita cianógena frecuente en la infancia, cuyo tratamiento quirúrgico presenta alta tasa de éxito a corto y largo plazos. Las complicaciones del tratamiento quirúrgico pueden y deben ser corregidas oportunamente, siendo crucial la intervención de enfermería en la evolución del paciente: valoración de signos de disfunción ventricular derecha, ejecución de intervenciones para minimizar los efectos de dicha disfunción, colaboración en el tratamiento de la entidad y la evaluación constante del paciente. Se presenta un caso de un paciente de 1 año y 5 meses, donde después de una valoración cardiológica y un electrocardiograma se le diagnostica Tetralogía de Fallot, y posteriormente fue intervenido quirúrgicamente sin reportarse ninguna complicación durante la cirugía. Se manejaron diagnósticos de enfermería, con objetivos a seguir e intervenciones específicas que se realizaron de acuerdo con la presentación de las manifestaciones del paciente en estudio

    Hemophilia is the most common of the severe bleeding disorders and if not properly managed since early infancy can lead to chronic disease and lifelong disabilities. However, it enjoys the most efficacious and safe treatment among the most prevalent monogenic disorders. Hemophilia should be considered in the neonatal period in the case of unusual bleeding or in the case of positive family history. Later, hemophilia should be suspected mainly in males because of abnormal bruising/bleeding or unusual bleeding following invasive procedures—for example, tonsillectomy or circumcision. Prophylactic treatment that is started early with clotting-factor concentrates has been shown to prevent hemophilic arthropathy and is, therefore, the gold standard of care for hemophilia A and B in most countries with adequate resources. Central venous access catheters and arterovenous fistulas play an important role in the management of hemophilia children requiring repeated and/or urgent administration of coagulation factor concentrates. During childhood and adolescence, personalized treatment strategies that suit the patient and his lifestyle are essential to ensure optimal outcomes. Physical activity is important and can contribute to better coordination, endurance, flexibility and strength.     Aims: Gut microbiota significantly impacts human health and is influenced by dietary changes. We evaluated the effects of diets naturally rich in polyphenols (PP) and/or long-chain n-3 polyunsaturated fatty acids (LCn3) on microbiota composition in an ancillary analysis of a randomized controlled trial in individuals at high cardiometabolic risk. Methods: Seventy-eight individuals with high waist circumference and at least one additional component of the metabolic syndrome were randomized to an isoenergetic 8-week diet: (a) low LCn3 and PP; (b) high LCn3; (c) high PP; or (d) high LCn3 and PP. Microbiota analysis was performed on feces collected before and after the intervention. DGGE analysis of the predominant bacteria, Eubacterium rectale and Blautia coccoides group (Lachnospiraceae, EREC), Clostridium leptum (Ruminococcaceae, CLEPT), Bacteroides spp., Bifidobacteria, and Lactobacillus group was performed. A quantitative real-time PCR was performed for the same group, additionally including Atopobium cluster (Coriobatteriaceae). Before and after the intervention, participants underwent a 75 g OGTT and a high-fat test meal to evaluate glucose and lipid response. Results: Adherence to the four diets was optimal. PP significantly increased microbial diversity (p = 0.006) and CLEPT (p = 0.015), while it reduced EREC (p = 0.044). LCn3 significantly increased the numbers of Bifidobacteria (p = 0.041). Changes in CLEPT numbers correlated with changes in early insulin secretion (r = 0.263, p = 0.030). Changes in Atopobium numbers correlated with postprandial triglycerides in plasma (r = 0.266, p = 0.026) and large VLDL (r = 0.313, p = 0.009), and cholesterol in large VLDL (r = 0.319, p = 0.008). Conclusions: Diets naturally rich in PP or LCn3 influenced gut microbiota composition in individuals at high cardiometabolic risk. These modifications were associated with changes in glucose/lipid metabolism

    Intrinsic Capacity and Active and Healthy Aging Domains Supported by Personalized Digital Coaching: Survey Study Among Geriatricians in Europe and Japan on eHealth Opportunities for Older Adults

    BackgroundThe worldwide aging trend requires conceptually new prevention, care, and innovative living solutions to support human-based care using smart technology, and this concerns the whole world. Enabling access to active and healthy aging through personalized digital coaching services like physical activity coaching, cognitive training, emotional well-being, and social connection for older adults in real life could offer valuable advantages to both individuals and societies. A starting point might be the analysis of the perspectives of different professionals (eg, geriatricians) on such technologies. The perspectives of experts in the sector may allow the individualization of areas of improvement of clinical interventions, supporting the positive perspective pointed out by the intrinsic capacity framework. ObjectiveThe overall aim of this study was to explore the cross-national perspectives and experiences of different professionals in the field of intrinsic capacity, and how it can be supported by eHealth interventions. To our knowledge, this is the first study to explore geriatric care providers’ perspectives about technology-based interventions to support intrinsic capacity. MethodsA survey involving 20 geriatricians or clinical experts in the fields of intrinsic capacity and active and healthy aging was conducted in Italy, France, Germany, and Japan between August and September 2021. ResultsThe qualitative findings pointed out relevant domains for eHealth interventions and provided examples for successful practices that support subjective well-being under the intrinsic capacity framework (the benefits offered by personalized interventions, especially by promoting health literacy but avoiding intrusiveness). Moreover, eHealth interventions could be used as a bridge that facilitates and enables social engagement; an instrument that facilitates communication between doctors and patients; and a tool to enrich the monitoring actions of medical staff. ConclusionsThere is an unexplored and significant role for such geriatric perspectives to help the development process and evaluate the evidence-based results on the effectiveness of technologies for older people. This is possible only when clinicians collaborate with data scientists, engineers, and developers in order to match the complex daily needs of older adults

    We analyzed 97 Fanconi anemia patients from a clinic/biological database for genotype, somatic, and hematologic phenotype, adverse hematological events, solid tumors, and treatment. Seventy-two patients belonged to complementation group A. Eighty percent of patients presented with mild/moderate somatic phenotype and most with cytopenia. No correlation was seen between somatic/hematologic phenotype and number of missense mutations of FANCA alleles. Over follow-up, 33% of patients improved or maintained mild/moderate cytopenia or normal blood count, whereas remaining worsened cytopenia. Eleven patients developed a hematological adverse event (MDS, AML, pathological cytogenetics) and three developed solid tumors. 10 years cumulative risk of death of the whole cohort was 25.6% with median follow-up 5.8 years. In patients eligible to hematopoietic stem cell transplantation because of moderate cytopenia, mortality was significantly higher in subjects transplanted from matched unrelated donor over nontransplanted subjects, whereas there was no significant difference between matched sibling donor transplants and nontransplanted patients. In patients eligible to transplant because of severe cytopenia and clonal disease, mortality risk was not significantly different in transplanted from matched unrelated versus matched sibling donor versus nontransplanted subjects. The decision to transplant should rely on various elements including, type of donor, HLA matching, patient comorbidities, impairment, and clonal evolution of hematopoiesis.     Comprehensive analysis of local and nonlocal amplitudes in the B0K0μ+μB^0\rightarrow K^{*0}\mu^+\mu^- decay

    International audienceA comprehensive study of the local and nonlocal amplitudes contributing to the decay B0K0(K+π)μ+μB^0\rightarrow K^{*0}(\to K^+\pi^-) \mu^+\mu^- is performed by analysing the phase-space distribution of the decay products. The analysis is based on \proton\proton collision data corresponding to an integrated luminosity of 8.4fb1^{-1} collected by the LHCb experiment. This measurement employs for the first time a model of both one-particle and two-particle nonlocal amplitudes, and utilises the complete dimuon mass spectrum without any veto regions around the narrow charmonium resonances. In this way it is possible to explicitly isolate the local and nonlocal contributions and capture the interference between them. The results show that interference with nonlocal contributions, although larger than predicted, only has a minor impact on the Wilson Coefficients determined from the fit to the data. For the local contributions, the Wilson Coefficient C9C_9, responsible for vector dimuon currents, exhibits a 2.1σ2.1\sigma deviation from the Standard Model expectation. The Wilson Coefficients C10C_{10}, C9C_{9}' and C10C_{10}' are all in better agreement than C9C_{9} with the Standard Model and the global significance is at the level of 1.5σ1.5\sigma. The model used also accounts for nonlocal contributions from B0K0[τ+τμ+μ]B^{0}\to K^{*0}\left[\tau^+\tau^-\to \mu^+\mu^-\right] rescattering, resulting in the first direct measurement of the bsττb s\tau\tau vector effective-coupling C9τC_{9\tau}
