    Evidências das Pesquisas que Abordam a Tipologia de Miles e Snow no Brasil

    This paper´s objective is to analyze the references used in the publications of national management conferences that present the typologies developed by Miles and Snow. The methods used in the study were descriptive, quantitative, and the type bibliometric survey. The existence of the term "Miles and Snow" in the publication titles, abstracts or keywords was used to observe the literature referenced in articles published. From a universe of 1,258 items consisting of four databases that provide access to publications of the national management conferences electronically, there was a sample of 235 relevant articles selected. As a result of the analysis, it was found that 18.68% of the references used in the articles surveyed contained the terminology by default, and only 4.21% spoke about the typology of Miles and Snow.El objetivo de este trabajo es analizar las referencias utilizadas en las publicaciones de las conferencias nacionales que las tipologías actuales de gestión desarrolladas por Miles y Snow. El método utilizado en el estudio fue de tipo descriptivo, con abordaje cuantitativo, y el estudio bibliométrico tipo. En la selección de artículos, se tomó como base la existencia del término "Miles y Snow" en el título, resumen o palabras clave, la observación de la literatura mencionada en los artículos publicados. De un universo de 1.258 artículos, que consta de cuatro bases de datos que proporcionan acceso a las publicaciones de congresos nacionales de gestión electrónica, se seleccionó una muestra de 235 artículos pertinentes.     Latrogenic bile duct injuries : experience of a tertiary reference center

    Orientador: Elinton Adami ChaimDissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências MédicasResumo: Contexto: As lesões iatrogênicas da via biliar são de difícil manejo e requerem equipes com experiência para a reconstrução da via biliar. O objetivo desse estudo foi avaliar tardiamente os 21 anos de experiência em um centro de referência terciário das reconstruções bileodigestivas após a lesão iatrogênica da via biliar pós-colecistectomia e os fatores envolvidos na estenose da anastomose bileodigestiva. Métodos: Análise retrospectiva de 91 pacientes submetidos a hepaticojejunostomia em Y de Roux entre 1993 e 2013. Dados pré-operatórios, sintomas clínicos após a lesão, reoperações, classificação da lesão e evolução ambulatorial foram avaliados. O teste de qui-quadrado e a regressão logística foram usados para determinar os fatores envolvidos na estenose da anastomose bileodigestiva. Resultados: Mulheres eram 84,5% dos pacientes, média de idade de 43 anos. Foi observada associação entre fístula biliar e derivação bileodigestiva no serviço de origem (p=0,009). Colangite e sintomas clínicos no seguimento ambulatorial foram associadas com a classificação de Bismuth (p=0,008 e p<0,001). O aumento em uma unidade na aspartato aminotransferase em 12 meses de pós-operatório, aumentou a chance de apresentar sintomas clínicos no ambulatório em 4,8% (IC 95%=1,003-1,096). A derivação bileodigestiva no serviço de origem aumentou a chance de associação com fístula biliar em 5,9 vezes (IC 95%=1,132-31,142). Conclusão: A reconstrução da via biliar após a lesão iatrogênica no tratamento cirúrgico da colecistectomia deve ser realizado por cirurgiões experientes e, de preferência, em centros de referência terciárioAbstract: Background: Iatrogenic bile duct injuries are difficult to manage and require surgical teams with experience in bile duct repair. The aim of the study was to describe and correlate factors associated with biliary-enteric anastomosis stricture in a tertiary referral center. Methods: A retrospective analysis of 91 patients undergoing Roux-en-Y hepaticojejunostomy from 1993 to 2013 was carried out. Preoperative data, symptoms following injury, reoperations, classification of lesions and outpatient outcome were evaluated. The chi-square test and logistic regression were used to determine factors involved in biliary-enteric anastomosis stricture. Results: Women comprised 84.5% of patients (mean age: 43 years). An association between bile leakage and biliary-enteric anastomosis at the original health care facility (p=0.009) was observed. Cholangitis and symptoms in the outpatient facility were associated with Bismuth classification (p=0.008 and p<0.001). For each additional unit increase in aspartate aminotransferase values in the 12-month postoperative period, the likelihood of outpatient symptoms increased by 4.8% (95% CI=1.003-1.096). A biliary-enteric anastomosis performed at the original health care facility increased 5.9-fold the chance that a bile leakage was associated (95% CI=1.132-31.142). Conclusion: Bile duct reconstruction due to iatrogenic injury is challenging.     CONTROL OF HYPERTENSION AFTER ROUX-EN-Y GASTRIC BYPASS AMONG OBESE DIABETIC PATIENTS

    Context Hypertension is a common disorder in general practice and has a widely known association with type 2 diabetes mellitus. Low adhesion to clinical treatment may lead to poor results. Obesity surgery can bring early and relevant resolution rates of both morbidities. Objective To describe hypertension evolution after Roux-en-Y gastric bypass in patients with type 2 diabetes mellitus. Method Descriptive observational study designed as a historical cohort of 90 subjects with hypertension and diabetes who underwent Roux-en-Y gastric bypass and were evaluated before and after surgery. Results It was observed a hypertension resolution rate of 85.6% along with markedly decrease in anti-hypertensive usage. Mean resolution time was 3.2 months. Resolution was associated with homeostasis model assessment – insulin resistance, preoperative fasting insulin, anti-hypertensive usage, hypertension time, body mass index and percentage of weight loss. Resolution of hypertension was not statistically associated with diabetes remission within this sample. Conclusion Roux-en-Y gastric bypass was a safe and effective therapeutic tool to achieve hypertension resolution in patients who also had diabetes mellitus

    Nuxcell Neo® improves vaccine efficacy in antibody response

    Current vaccination protocols raise concerns about the efficacy of immunization. There is evidence that changes in the gut microbiota can impact immune response. The formation of the gut microbiota in newborns plays a crucial role in immunity. Probiotic bacteria and prebiotics present important health-promoting and immunomodulatory properties. Thus, we hypothesize that pro and prebiotic supplementation can improve the efficacy of vaccination in newborns. In this protocol, newborn mice were used and treated with a single-dose rabies vaccine combined with Nuxcell Neo® (2 g/animal/week) for 3 weeks. Samples were collected on days 7, 14, and 21 after vaccination for analysis of cytokines and concentration of circulating antibodies. Our results show an increased concentration of antibodies in animals vaccinated against rabies and simultaneously treated with Nuxcell Neo® on days 14 and 21 when compared to the group receiving only the vaccine. In the cytokine levels analysis, it was possible to observe that there weren't relevant and significant changes between the groups, which demonstrates that the health of the animal remains stable. The results of our study confirm the promising impact of the use of Nuxcell Neo® on the immune response after vaccination

    Brazilian Flora 2020: Leveraging the power of a collaborative scientific network

    International audienceThe shortage of reliable primary taxonomic data limits the description of biological taxa and the understanding of biodiversity patterns and processes, complicating biogeographical, ecological, and evolutionary studies. This deficit creates a significant taxonomic impediment to biodiversity research and conservation planning. The taxonomic impediment and the biodiversity crisis are widely recognized, highlighting the urgent need for reliable taxonomic data. Over the past decade, numerous countries worldwide have devoted considerable effort to Target 1 of the Global Strategy for Plant Conservation (GSPC), which called for the preparation of a working list of all known plant species by 2010 and an online world Flora by 2020. Brazil is a megadiverse country, home to more of the world's known plant species than any other country. Despite that, Flora Brasiliensis, concluded in 1906, was the last comprehensive treatment of the Brazilian flora. The lack of accurate estimates of the number of species of algae, fungi, and plants occurring in Brazil contributes to the prevailing taxonomic impediment and delays progress towards the GSPC targets. Over the past 12 years, a legion of taxonomists motivated to meet Target 1 of the GSPC, worked together to gather and integrate knowledge on the algal, plant, and fungal diversity of Brazil. Overall, a team of about 980 taxonomists joined efforts in a highly collaborative project that used cybertaxonomy to prepare an updated Flora of Brazil, showing the power of scientific collaboration to reach ambitious goals. This paper presents an overview of the Brazilian Flora 2020 and provides taxonomic and spatial updates on the algae, fungi, and plants found in one of the world's most biodiverse countries. We further identify collection gaps and summarize future goals that extend beyond 2020. Our results show that Brazil is home to 46,975 native species of algae, fungi, and plants, of which 19,669 are endemic to the country. The data compiled to date suggests that the Atlantic Rainforest might be the most diverse Brazilian domain for all plant groups except gymnosperms, which are most diverse in the Amazon. However, scientific knowledge of Brazilian diversity is still unequally distributed, with the Atlantic Rainforest and the Cerrado being the most intensively sampled and studied biomes in the country. In times of “scientific reductionism”, with botanical and mycological sciences suffering pervasive depreciation in recent decades, the first online Flora of Brazil 2020 significantly enhanced the quality and quantity of taxonomic data available for algae, fungi, and plants from Brazil. This project also made all the information freely available online, providing a firm foundation for future research and for the management, conservation, and sustainable use of the Brazilian funga and flora
