    Synchronous collecting duct carcinoma and papillary renal cell carcinoma: A case report and review of the literature

    The coexistence of multiple and synchronous primary neoplasms in the same organ (including kidney) has only rarely been described in the literature. We herein present a case of collecting duct carcinoma (CDC) combined with papillary renal carcinoma (RCC) having a 57-month disease-free survival. CDC is a rather rare and aggressive neoplasm of the kidney. Sharing probably the same embryological origin, synchronous or metachronous association with in situ or papillary transitional cell carcinoma (TCC) may be found; association with RCC has been only once reported in the literature. The high incidence of c-erbB-2 oncogene amplification in CDC further characterizes this tumor as a separate entity from renal cell carcinoma, and shows some genetic characteristics in common with TCC. The histohgical diagnosis of Bellini CDC can be confirmed by the positive immuno-histochemical staining with a collecting duct marker and distal tubule marker and negative staining with a proximal tubule marker

    Produção e Geração de Renda na Agricultura Familiar: Um Diagnóstico do Assentamento Rural Lagoa Grande em Dourados, Mato Grosso do Sul

In this perspective, this study aimed to identify the major factors related to productive activities and income generation in Lagoa Grande rural settlement, in the Dourados City (MS). The methodology consisted of a field survey, with the completion of data collection through a census, with questionnaires to residents of lots of seating. This analysis identified the need for greater diversification in productive activities, soil amendment, reform of pastures and technical assistance, as well as the need to improve the conditions of transport, management of properties, knowledge of public policy and alternative generating income in order to provide satisfactory production and thus avoid abandonment of lots by the settlers.http://dx.doi.org/10.5902/2179460X11064Viver e criar estratégias de subsistência para permanecer nos lotes em assentamentos rurais, em terras pouco produtivas, distantes dos centros urbanos e carentes de infraestrutura de serviços básicos, tem sido um grande desafio para os agricultores familiares em todo o Brasil, especialmente no estado de Mato Grosso do Sul (MS). Nesta perspectiva, este estudo objetivou identificar os principais fatores relacionados às atividades produtivas e à geração de renda no assentamento rural Lagoa Grande, localizado no município de Dourados (MS). A metodologia utilizada constou de uma pesquisa de campo, com a realização de coleta de dados por meio de um Censo, com a aplicação de questionários aos moradores dos lotes do assentamento. Este diagnóstico identificou a necessidade de maior diversificação nas atividades produtivas, de correção do solo, de reforma das pastagens e assistência técnica, bem como a necessidade de melhoria das condições de transporte, de gestão das propriedades, de conhecimento de políticas públicas e alternativas geradoras de renda, a fim de proporcionar produção satisfatória e, dessa forma, evitar o abandono dos lotes por parte dos assentados.

    Intersections and identities in the Brazilian legal academy

    O artigo analisa as mudanças na docência do direito, relacionando a maior participação das mulheres e da diferença no professorado, com a pulverização das instituições de ensino, a distribuição regional, os regimes de trabalho e o maior de grau de titulação docente. Investiga como esses deslocamentos se refletem nas identificações profissionais e em suas intersecções, bem como no jogo para produzir o centro e o descentramento dos grupos estabelecidos e do cânone jurídico.     Longitudinal follow‐up of patients with thalassaemia intermedia who started transfusion therapy in adulthood: a cohort study

    SummaryWe longitudinally evaluated the effects of regular blood transfusions (BTs), in the real‐life context of the Myocardial Iron Overload in Thalassaemia network, in patients with thalassaemia intermedia (TI). We considered 88 patients with TI (52 females) who started regular BTs after the age of 18 years. Magnetic resonance imaging was used to quantify iron overload and biventricular function. For 56·8% of the patients there were more than two indications for the transition to regular BTs, with anaemia present in 94·0% of the cases. A significant decrease in nucleated red blood cells, platelets, lactate dehydrogenase, bilirubin, and uric acid levels was detected 6 months after starting regular BTs. After the transition to the regular BT regimen there was a significant increase only in the frequency of hypothyroidism and osteopenia, and a significant decrease in liver iron and cardiac index. The percentage of chelated patients increased significantly after starting regular BTs. The decision to regularly transfuse patients with TI may represent a way to prevent or slow down the natural progression of the disease, despite the more complex initial management

    PDGFRA defines the mesenchymal stem cell Kaposi's sarcoma progenitors by enabling KSHV oncogenesis in an angiogenic environment

    Kaposi’s sarcoma (KS) is an AIDS-defining cancer caused by the KS-associated herpesvirus (KSHV). Unanswered questions regarding KS are its cellular ontology and the conditions conducive to viral oncogenesis. We identify PDGFRA(+)/SCA-1(+) bone marrow-derived mesenchymal stem cells (Pα(+)S MSCs) as KS spindle-cell progenitors and found that pro-angiogenic environmental conditions typical of KS are critical for KSHV sarcomagenesis. This is because growth in KS-like conditions generates a de-repressed KSHV epigenome allowing oncogenic KSHV gene expression in infected Pα(+)S MSCs. Furthermore, these growth conditions allow KSHV-infected Pα(+)S MSCs to overcome KSHV-driven oncogene-induced senescence and cell cycle arrest via a PDGFRA-signaling mechanism; thus identifying PDGFRA not only as a phenotypic determinant for KS-progenitors but also as a critical enabler for viral oncogenesis.Fil: Naipauer, Julian.     Single Molecule Analysis of Replicated DNA Reveals the Usage of Multiple KSHV Genome Regions for Latent Replication

    Kaposi's sarcoma associated herpesvirus (KSHV), an etiologic agent of Kaposi's sarcoma, Body Cavity Based Lymphoma and Multicentric Castleman's Disease, establishes lifelong latency in infected cells. The KSHV genome tethers to the host chromosome with the help of a latency associated nuclear antigen (LANA). Additionally, LANA supports replication of the latent origins within the terminal repeats by recruiting cellular factors. Our previous studies identified and characterized another latent origin, which supported the replication of plasmids ex-vivo without LANA expression in trans. Therefore identification of an additional origin site prompted us to analyze the entire KSHV genome for replication initiation sites using single molecule analysis of replicated DNA (SMARD). Our results showed that replication of DNA can initiate throughout the KSHV genome and the usage of these regions is not conserved in two different KSHV strains investigated. SMARD also showed that the utilization of multiple replication initiation sites occurs across large regions of the genome rather than a specified sequence. The replication origin of the terminal repeats showed only a slight preference for their usage indicating that LANA dependent origin at the terminal repeats (TR) plays only a limited role in genome duplication. Furthermore, we performed chromatin immunoprecipitation for ORC2 and MCM3, which are part of the pre-replication initiation complex to determine the genomic sites where these proteins accumulate, to provide further characterization of potential replication initiation sites on the KSHV genome. The ChIP data confirmed accumulation of these pre-RC proteins at multiple genomic sites in a cell cycle dependent manner. Our data also show that both the frequency and the sites of replication initiation vary within the two KSHV genomes studied here, suggesting that initiation of replication is likely to be affected by the genomic context rather than the DNA sequences
