Prevention of nasal trauma due to CPAP in a preterm newborn: case report
The administration of continuous positive airway pressure through nasal CPAP has been used in the treatment of neonatal respiratory diseases as a means of providing ventilatory support to preterm infants, producing adequate gas exchange, decreasing the complications induced by invasive mechanical ventilation and reducing bronchopulmonary dysplasia. The nasal prong is the most common device used to connect to the airways with the purpose of providing mechanical ventilation. Nasal prongs are available in different sizes and made of lightweight and flexible material. Despite its advantages, this device can cause trauma to the nose causing nasal discomfort and disfigurement. We report the case of an extremely low birth weight preterm who used a system of nasal CPAP developed by the nurses of the Neonatal Intensive Care Unit of the HCPA to protect the skin, mucosa, and nasal septum. Overview of ASDEX Upgrade results
Recent results from the ASDEX Upgrade experimental campaigns 2001 and 2002 are presented. An improved understanding of energy and particle transport emerges in terms of a 'critical gradient' model for the temperature gradients. Coupling this to particle diffusion explains most of the observed behaviour of the density profiles, in particular, the finding that strong central heating reduces the tendency for density profile peaking. Internal transport barriers (ITBs) with electron and ion temperatures in excess of 20 keV (but not simultaneously) have been achieved. By shaping the plasma, a regime with small type II edge localized modes (ELMs) has been established. Here, the maximum power deposited on the target plates was greatly reduced at constant average power. Also, an increase of the ELM frequency by injection of shallow pellets was demonstrated. ELM free operation is possible in the quiescent H-mode regime previously found in DIII-D which has also been established on ASDEX Upgrade. Regarding stability, a regime with benign neoclassical tearing modes (NTMs) was found. During electron cyclotron current drive (ECCD) stabilization of NTMs, ÎČN could be increased well above the usual onset level without a reappearance of the NTM. Electron cyclotron resonance heating and ECCD have also been used to control the sawtooth repetition frequency at a moderate fraction of the total heating power. The inner wall of the ASDEX Upgrade vessel has increasingly been covered with tungsten without causing detrimental effects on the plasma performance. Regarding scenario integration, a scenario with a large fraction of noninductively driven current (â„50%), but without ITB has been established. It combines improved confinement (ÏE/ÏITER98 â 1.2) and stability (ÎČN †3.5) at high Greenwald fraction (ne/nGW â 0.85) in steady state and with type II ELMy edge and would offer the possibility for long pulses with high fusion power at reduced current in ITER

Este trabalho tem como objetivo analisar os principais dispositivos de discurso que estabelecem fronteiras e hierarquias entre prĂĄticas sexuais. Pretende-se analisar em que medida a utilização de alguns conceitos da PsicanĂĄlise, da Antropologia e do Direito sĂŁo evocados para definir fronteiras entre sexualidades normais e desviantes. Para isto, serĂŁo destacadas: a construção, por aqueles discursos, do dispositivo "diferença sexual" (de hierarquia entre os sexos e de exclusĂŁo da homossexualidade) e a noção de "ordem procriativa" (atualizadora de um modelo biolĂłgico de filiação). A partir dessas premissas, nota-se que a trĂade heterossexualidade-casamento-filiação permanece como a Ășnica referĂȘncia possĂvel para pensar a cultura ou a sociedade, sendo que a visibilidade ou o reconhecimento civil do laço afetivo e sexual homossexual se transforma numa ameaça de apagamento de fronteiras ou de transgressĂ”es de limites.<br>This study aims to analyze the main discursive devices that establish borders and hierarchies among sexual practices. The objective is to analyze to what extent the utilization of certain concepts from psychoanalysis, anthropology, and law are invoked to define borders between normal and deviant sexualities. The following points are highlighted: the construction, by these discourses, of the "sexual difference" device (hierarchy between the sexes and exclusion of homosexuality) and the notion of "procreative order" (updating a biological model of filiation). Based on these premises, one notes that the heterosexuality-marriage-filiation triad remains as the only possible reference to conceive of culture or society, while the visibility or civil recognition of homosexual affective and sexual ties threatens to erase borders or transgress limits
ITER plasma control system final design and preparation for first plasma
The ITER plasma control system (PCS) has successfully completed its final design for the first plasma (FP) and engineering operations (EOs) phase and plant system commissioning has begun as ITER prepares for this first operation phase. Commissioning of the essential plant systems will continue as each plant system is completed and made ready for operation. Tokamak assembly has begun with the base and lower cylinder of the cryostat and the lower most poloidal field (PF) coil installed in the tokamak pit. The first vacuum vessel (VV) sector and accompanying two toroidal field (TF) coils are being prepared for transfer to the pit. Once the tokamak is assembled, the cryostat top lid is closed and pump down begins, this will start approximately one year of integrated commissioning (IC) to prepare all of the relevant plant systems for FP operation. After a scheduled one month of plasma operation with the goal of achieving a plasma current > 100 kA for at least 100 ms, there will be about six months of EO to complete commissioning of the superconducting central solenoid, PF, and TF magnet systems to full current, without plasma, to complete this initial ITER operation phase. The PCS final design for FP will be described as well as the IC sequence of the main plant systems required for this operations phase. The plans for the FP operation campaign will be described, including specific challenges present on ITER due to large VV eddy currents, issues associated with electron cyclotron heating (ECH) assist, neutral pressure and impurities. This will be followed by the EO phase to commission full current operation of the superconducting magnets, possibly including plasma operation at full TF of 5.3 T to have improved conditions both for Ohmic plasma initiation and ECH absorption
