Kosten verschiedener Behandlungsstrategien der Schizophrenie in Österreich

Die vorliegende Diplomarbeit beschäftigt sich mit den Kosten verschiedener Behandlungsstrategien von Schizophrenie in Österreich. Im theoretischen Teil der Arbeit wird ein Überblick über das Krankheitsbild der Schizophrenie, den Krankheitsverlauf und die Häufigkeit des Auftretens der Krankheit gegeben. Des Weiteren wird die Therapie bei Schizophrenie beschrieben und die mit der Krankheit verbundenen direkten und indirekten volkswirtschaftlichen Kosten erläutert. Auch das im Zusammenhang mit der Krankheit Schizophrenie besonders wichtige Thema der Compliance wird betrachtet. Im zweiten, empirischen Teil der Arbeit werden die Kosten verschiedener Behandlungsstrategien von Schizophrenie in Österreich miteinander verglichen. Zudem werden die finanziellen Auswirkungen der Einführung von Paliperidone in Depotform am österreichischen Markt analysiert. Es werden die direkten Kosten von Schizophrenie vor und nach der Einführung von Paliperidone in Depotform in Österreich berechnet.The present diploma thesis deals with costs of different treatment strategies of schizophrenia in Austria. In the theoretical part an overview about the disease pattern of schizophrenia, the course of the disease and the frequency of occurrence is given. Furthermore the therapy of schizophrenia and the direct and indirect economic costs of schizophrenia are discussed. As well the important topic of compliance is considered. In the second, empirical part of the diploma thesis costs of different treatment strategies of schizophrenia in Austria are compared with each other. Furthermore the budget-impact of the introduction of paliperidone palmitate on the Austrian market is analysed. Relation between crystal structure and optical properties in the correlated blue pigment YIn_1−xMn_xO₃

A material's properties and functionalities are determined by its chemical constituents and the atomic arrangement in which they crystallize. For the recently discovered pigment YIn1−xMnxO3, for instance, it had been surmised that its bright blue color owes to an unusual, trigonal bipyramidal, oxygen coordination of the manganese impurities. Here, we demonstrate that, indeed, a direct correspondence between details of the local Mn environment and the pigment's blue color holds: Combining realistic many-body calculations (dynamical mean-field theory to treat the quasi-atomic Mn-multiplets at low doping x=8%) with an effective medium description (Kubelka-Munk model to describe scattering in a milled pigment sample), we find that only a Mn-coordination polyhedra consisting of two distorted oxygen pyramids results in a diffuse reflectance commensurate with the experimental blue color. We motivate that the distortion of the bipyramid helps circumventing atomic selection rules, allowing for dipolar d−d transitions and creating the desired two-peak absorption profile

"Ὑγίαινε, φίλον ἦτορ!" - "Hygiaine, philon ētor!"

Die vorliegende Diplomarbeit befasst sich anhand der Briefe des Gelehrten der neugriechischen Aufklärung Konstantinos M. Koumas (1777-1836) mit dem Thema der griechischen Epistolographie. Zunächst wird ein Überblick über die Geschichte der Epistolographie in griechischer Sprache von der Antike bis ins 19. Jahrhundert gegeben, wobei der Schwerpunkt auf dem Mittelalter und der Neuzeit liegt. Nach einer kurzen Biographie von Koumas wird ein Überblick über die Briefpartner seiner gesamten erhaltenen Korrespondenz geboten. Weiters werden die Briefe in die beiden Kategorien „Private Briefe“ und „an eine größere Öffentlichkeit gerichtete Briefe“ und in der Folge in weitere Untergruppen unterteilt, anhand derer versucht wird, erste Schlüsse zum Zusammenhang zwischen Stilniveau und Zugehörigkeit eines Briefes zu einer bestimmten Kategorie zu ziehen. In der Folge werden die Briefe nach verschiedenen genauer differenziert und anhand von Textbeispielen stilistisch analysiert. Das vierte Kapitel beschäftigt sich mit den verschiedenen Einflüssen, durch die Koumas’ Briefstil geprägt wurde. Koumas vielseitiger Briefstil, der die im Zusammenhang mit der Rezeption der Briefe der Kirchenväter stehende traditionelle rhetorische Schreibweise genauso beinhaltet wie Korais’ Sprachtheorie des „Mittleren Weges“ und eine von der philhellenischen Antike-Begeisterung inspirierte archaisierende Schreibweise, kann, da er diese verschiedenen Strömungen in sich vereint, als repräsentativ für die griechischsprachige Epistolographie seiner Zeit bezeichnet werden. Da bisher keine Gesamtedition der Briefe von Koumas existiert, werden im Anhang Informationen zur Überlieferungslage von Koumas’ Briefen gegeben und die erhaltene Korrespondenz nach verschiedenen Kriterien statistisch ausgewertet. Schließlich findet sich ein Regestenteil mit den 174 von mir recherchierten, erhaltenen Briefen von und an Koumas.The present diploma thesis deals with the subject of Greek epistolography by means of the letters by the scholar of the Modern Greek enlightenment Konstantinos M. Koumas (1777-1836). The first chapter gives an outline of the history of Greek epistolography from antiquity to the 19th century with its focus on the Middle Ages and the early modern period. In the second chapter a short biography of Koumas and an overview of the correspondents of all his preserved correspondence are to be found. Moreover, the letters are divided into the two categories “private letters” and “letters directed to a wider public” and subsequently into some further sub-categories, by means of which I try to draw some primary conclusions regarding the relation between style level and the belonging of a letter to a certain category. Then the letters are distinguished in more detail and are analyzed stylistically on the basis of text samples. The fourth chapter deals with the various influences that shaped Koumas’ style of letter writing. Koumas’ multifaceted style contains the traditional rhetoric style of writing linked to the reception of the letters of the fathers of the church as well as Korais’ language theory of the “middle way” and an archaizing style of writing inspired by the philhellenic enthusiasm for antiquity. As it unifies these different approaches it can be characterized as representative of the Greek epistolography of its time. Since a complete edition of Koumas’ letters doesn’t exist so far the appendix gives information concerning the preservation of Koumas’ letters. Finally, there is a register containing summaries of the 174 preserved letters to or from Koumas

Entwicklung und Implementierung klinischer Ethikkomitees

Die vorliegende Diplomarbeit befasst sich mit der Entwicklung und Implementierung klinischer Ethikkomitees in Deutschland und Österreich. In Krankenhäusern müssen häufig schwierige ethische Entscheidungen wie z.B. im Zusammenhang mit Behandlungsbegrenzung und Behandlungsabbruch oder Aufklärung und Einwilligung getroffen werden. Klinische Ethikkomitees bieten Unterstützung im Umgang mit solchen ethischen Konfliktsituationen. Sie sind interdisziplinär zusammengesetzt. Der pflegerische Bereich ist neben dem ärztlichen Bereich zahlenmäßig am stärksten in klinischen Ethikkomitees vertreten. Die Aufgaben klinischer Ethikkomitees umfassen neben der Einzelfallberatung und Leitlinienentwicklung auch die Fort- und Weiterbildung von Mitarbeitern. Der Anwendungsbereich von Ethikkomitees erstreckt sich nicht nur auf Krankenhäuser, sondern beispielsweise auch auf Altenpflegeheime. Dort stehen überwiegend pflegeethische Fragestellungen im Mittelpunkt der Arbeit der Komitees. In Österreich steht die Etablierung klinischer Ethikkomitees noch ganz am Beginn. Nur vereinzelt (z.B. am LKH-Universitätsklinikum Graz oder im St. Josef-Krankenhaus Wien) wurden klinische Ethikkomitees eingerichtet. Deutschland ist in der Entwicklung wesentlich weiter als Österreich, da die Etablierung klinischer Ethikkomitees dort bereits in den 1990er Jahren begann. Die Erfahrungen aus Deutschland haben gezeigt, dass klinische Ethikkomitees einen wichtigen Beitrag zur Unterstützung von Mitarbeitern, Patienten und Angehörigen im Umgang mit schwierigen ethischen Fragestellungen leisten können, wenn gewisse Voraussetzungen erfüllt werden. Es ist beispielsweise wesentlich, dass ein Ethikkomitee sowohl von den Mitarbeitern als auch von der Klinikleitung mitgetragen wird. Die Pflege nahm bei der Entwicklung klinischer Ethikkomitees in Deutschland im Vergleich zu anderen Berufsgruppen bisher eher eine untergeordnete Rolle ein. Es gibt jedoch auch positive Beispiele wie die Gründung des klinischen Ethikkomitees am Westküstenklinikum Heide (die Initiative ging von Pflegepersonen aus) oder den Ethiktreff Pflege am Universitätsklinikum Tübingen. Für die Pflege können klinische Ethikkomitees ein Schritt auf dem Weg zur Professionalisierung sein. Derzeit ist jedoch in Deutschland zu beobachten, dass die Themen der Pflege in klinischen Ethikkomitees marginalisiert werden.The present diploma thesis deals with the development and implementation of healthcare ethics committees in Germany and in Austria. Difficult ethical decisions frequently have to be made in hospitals, for example in connection with limiting or stopping treatment or in connection with the education and compliance of patients. Healthcare ethics committees provide support in dealing with such ethical dilemmas. These committees are composed in an interdisciplinary way. Along with the medical domain, the domain of nursing is the most represented in healthcare ethics committees. Besides individual case consultation and development of guidelines, the activities of healthcare ethics committees also cover further education for the staff. The application area for healthcare ethics committees is not limited to hospitals but also extends, for instance, to nursing homes for the elderly. The work of an ethics committee in a nursing home is predominated by ethical questions of nursing. In Austria the implementation of healthcare ethics committees is still at the beginning. Healthcare ethics committees (e.g. at the university hospital of Graz or at the St. Josef´s hospital in Vienna) have only been introduced sporadically. In Germany, the development is far more advanced than in Austria since the implementation of healthcare ethics committees has already started in the 1990s. The experiences in Germany made it clear, that healthcare ethics committees can provide beneficial support for staff, patients and their families in dealing with difficult ethical decisions if certain requirements are given. It is, for example, very important, that a healthcare ethics committee is supported by the staff as well as by the hospital management. In the development of healthcare ethics committees in Germany, nurses played a rather subordinate role in comparison to other occupational groups. But there are also positive examples, like the implementation of a healthcare ethics committee at the hospital of Heide, which was initiated by nurses, or the ethical nursing meeting at the university hospital of Tübingen. Healthcare ethics committees can contribute significantly to the professionalization of nurses. However, it can be seen in Germany, that, currently, the issues of nurses in healthcare ethics committees are marginalized
