    A thesis submitted for the degree of Master of Philosophy of the University of BedfordshireYouth justice in England and Wales is a highly politicised area of government policy and youth justice provision has always been a highly contested issue. The discourse of diversion stems from debates about the purpose and effectiveness of various types of penal regimes, and particularly their effect on children and young people in trouble with the law. The process of diversion aims to remove children and young people from the formal sanctions of the criminal justice system or minimize their penetration into it, and failing that it aims to avoid incarceration. Over the years diversion has taken many forms and the extent to which children have been diverted has varied. This thesis explores the various types of diversionary practice and how they have changed over time. It explores the political, administrative and professional conditions under which diversion has been a priority and those under which it has been effective. Bernard (1992) has argued that there is a ‘cycle of youth justice’ in which responses to youth crime move from the harsh to the more lenient before swinging back again. The thesis suggests that there is a ‘spiral’ of youth justice in which different paradigms are sometimes entangled together leading to the often contradictory and complex realities of youth justice and diversion without necessarily returning to the place of origin. It concludes that, given the current fiscal climate, there is a distinct likelihood that diversion policies will gain ascendancy. However, any developments will be fragile and susceptible to unintended consequences if the ‘real’ outcomes for children and young people are not part of the motivation for reform

    How effective are on-farm mitigation measures for delivering an improved water environment? A systematic map

    Background\ud Agricultural activities are estimated to contribute 70% of nitrates, 28% of phosphates and 76% of sediments measured in UK rivers. Catchments dominated by agriculture also have elevated levels of pesticides and bacterial pathogens. European member states have a policy commitment to tackle this pollution through the water framework directive. Here we report on the results of a systematic map to investigate and describe the nature and coverage of research pertaining to the effectiveness of 6 on-farm mitigation measures, slurry storage, cover/catch crops, woodland creation; controlled trafficking, subsoiling and vegetated buffer strips for delivering an improved water environment in terms of a reduction in nitrogen (N), phosphorus (P), sediment, pesticides and faecal indicator organisms (FIOs) or pathogens from faecal material.\ud Methods\ud Research evidence for the effectiveness of the 6 on-farm mitigation measures for delivering an improved water environment (as detailed above) was collated using English language search terms for temperate farming systems in Europe, Canada, New Zealand and northern states of the United States of America. Searches for literature were made from online publication databases, search engines, specialist websites and bibliographies of topic specific reviews. Recognised experts, authors and practitioners were also contacted to identify unpublished literature. Articles were screened for relevance at title, abstract and full text using predefined inclusion criteria set out in an a priori published protocol. All relevant articles were mapped in a searchable database using pre-defined coding and critically appraised for relevance and reliability. Articles reporting the same study were removed. All full text studies without confounding factors were identified and coded for in a separate searchable database.\ud Results\ud A total of 718 articles were included in the database. Buffer strips were the most commonly studied intervention followed by cover crops and slurry storage. Little evidence was found for woodland creation and sub-soiling. No studies were found for controlled trafficking on grassland. Nitrogen was most frequently measured, followed by P, sediment, pesticides and FIOs or pathogens from faecal material.\ud Conclusions\ud The majority of the evidence collated in this map investigated the effectiveness of buffer strips and cover crops for improving water quality. This evidence was predominantly focussed on reducing N pollution. An evidence gap exists for the impact of cover/catch crops in reducing leaching of pesticides, FIOs and pathogens, and for organic forms of N and P. There was limited research investigating the effectiveness of buffer strips for reducing leaching of organic forms of N or P, or for pesticides that are currently authorised for use/commonly used in UK agriculture. Further, long term studies across different seasons with controls, pre and post water quality measurements and multiple sampling points from both field and rivers would improve the evidence base.     Inequalities in ventilation tube insertion procedures between Aboriginal and non-Aboriginal children in New South Wales, Australia: a data linkage study

    OBJECTIVES Australian Aboriginal children experience earlier, more frequent and more severe otitis media, particularly in remote communities, than non-Aboriginal children. Insertion of ventilation tubes is the main surgical procedure for otitis media. Our aim was to quantify inequalities in ventilation tube insertion (VTI) procedures between Australian Aboriginal and non-Aboriginal children, and to explore the influence of birth characteristics, socioeconomic background and geographical remoteness on this inequality. DESIGN Retrospective cohort study using linked hospital and mortality data from July 2000 to December 2008. SETTING AND PARTICIPANTS A whole-of-population cohort of 653 550 children (16 831 Aboriginal and 636 719 non-Aboriginal) born in a New South Wales hospital between 1 July 2000 and 31 December 2007 was included in the analysis. OUTCOME MEASURE First VTI procedure. RESULTS VTI rates were lower in Aboriginal compared with non-Aboriginal children (incidence rate (IR), 4.3/1000 person-years; 95% CI 3.8 to 4.8 vs IR 5.8/1000 person-years; 95% CI 5.7 to 5.8). Overall, Aboriginal children were 28% less likely than non-Aboriginal children to have ventilation tubes inserted (age-adjusted and sex-adjusted rate ratios (RRs) 0.72; 95% CI 0.64 to 0.80). After adjusting additionally for geographical remoteness, Aboriginal children were 19% less likely to have ventilation tubes inserted (age-adjusted and sex-adjusted RR 0.81; 95% CI 0.73 to 0.91). After adjusting separately for private patient/health insurance status and area socioeconomic status, there was no significant difference (age-adjusted and sex-adjusted RR 0.96; 95% CI 0.86 to 1.08 and RR 0.93; 95% CI 0.83 to 1.04, respectively). In the fully adjusted model, there were no significant differences in VTI rates between Aboriginal and non-Aboriginal children (RR 1.06; 95% CI 0.94 to 1.19). CONCLUSIONS Despite a much higher prevalence of otitis media, Aboriginal children were less likely to receive VTI procedures than their non-Aboriginal counterparts; this inequality was largely explained by differences in socioeconomic status and geographical remoteness.This work was supported by the National Health and Medical Research Council (NHMRC; grant number 573113).     Platinum bis(phosphine) complexes of 1,8-naphthosultam

    The work in this project was supported by the Engineering and Physical Sciences Research Council (EPSRC).A series of bis(phosphine) platinum complexes 1-4 and 6-8 that bear the 1,8-naphthosultam ligand (L) have been synthesised. The nitrogen atom in L was deprotonated with sodium tert-butoxide to form the sodium salt. Metathetical reaction of the sodium salt (1 eq.) with the appropriate cis-dichlorobis(phosphine) platinum (1 eq.) in THF resulted in the formation of platinum complexes [Pt(PR3)2(L)Cl] (R3 = Ph3; 1, Ph2Me; 2, PhMe2; 3, Me3; 4), whilst reaction with [Pt(COD)Cl2] afforded [Pt(COD)(L)Cl] (5). The corresponding reaction employing two equivalents of L, two equivalents of NaOtBu and one equivalent of [Pt(PR3)2Cl2]/[Pt(COD)Cl2] yielded complexes [Pt(PR3)2(L)2] (R3 = Ph2Me; 6, PhMe2; 7, Me3; 8) and [Pt(COD)(L)2] (9). L, 1, 5 and 9 have been fully characterised, principally by multinuclear magnetic resonance and IR spectroscopy and mass spectrometry, the remaining members of the series were analysed by 31P NMR only. Unsymmetrical complexes 1-4 provide examples of AX spin systems, with appropriate satellites attributed to 31P NMR-195Pt coupling. X-ray structures determined for 1,8-naphthosultam L, and complexes 1, 5 and 9 and where appropriate the platinum metal geometry, N-S distance and ligand distortions were compared.PostprintPeer reviewe

    Systematic mapping of food safety outbreaks in the hospitality sector in the Dominican Republic

    Purpose: The hospitality sector underpins the Dominican Republic’s (DR) economy but may be a setting where foodborne disease outbreaks (FBDOs) can occur. The purpose of this research is to conduct a systematic mapping exercise on the available scientific literature related to FBDOs in hospitality in the DR and their link to reported food safety and hygienic practices. Design/methodology/approach: A predefined search protocol applied the principles of PRISMA guidance. Publications (n= 2,793) from databases (e.g. Web of Science, PubMed) were identified, and systematically selected for relevance. A full-text assessment based on the inclusion criteria led to the identification of a refined list of studies and academic publications (n=22) included in this review. The descriptive analysis of the collated data is then presented graphically. Findings: A low rate of reporting highlights a knowledge gap on FBDOs, the related food safety hazards and how they are mitigated by stakeholders and local health authorities in the DR. Improving government and other stakeholder capacity to report, investigate and understand FBDOs and the practices involved is essential. Research limitations/implications: The research has implications for Government, businesses and public health officials and managers in the hospitality sector in the DR. A potential research limitation is that the search strategies could miss some relevant articles. Originality/value: To the best of our knowledge this is the first systematic mapping research assessing evidence of FBDOs affecting hospitality in the DR. Practical implications: The findings provide a framing for improved risk analysis in implementing food safety management strategies for FBDOs

    Implementing a Mobile Health System to Integrate the Treatment of Addiction Into Primary Care: A Hybrid Implementation-Effectiveness Study

    Background: Despite the near ubiquity of mobile phones, little research has been conducted on the implementation of mobile health (mHealth) apps to treat patients in primary care. Although primary care clinicians routinely treat chronic conditions such as asthma and diabetes, they rarely treat addiction, a common chronic condition. Instead, addiction is most often treated in the US health care system, if it is treated at all, in a separate behavioral health system. mHealth could help integrate addiction treatment in primary care. Objective: The objective of this paper was to report the effects of implementing an mHealth system for addiction in primary care on both patients and clinicians. Methods: In this implementation research trial, an evidence-based mHealth system named Seva was introduced sequentially over 36 months to a maximum of 100 patients with substance use disorders (SUDs) in each of three federally qualified health centers (FQHCs; primary care clinics that serve patients regardless of their ability to pay). This paper reports on patient and clinician outcomes organized according to the Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) framework. Results: The outcomes according to the RE-AIM framework are as follows: Reach—Seva reached 8.31% (268/3226) of appropriate patients. Reach was limited by our ability to pay for phones and data plans for a maximum of 100 patients per clinic. Effectiveness—Patients who were given Seva had significant improvements in their risky drinking days (44% reduction, (0.7-1.25)/1.25, P=.04), illicit drug-use days (34% reduction, (2.14-3.22)/3.22, P=.01), quality of life, human immunodeficiency virus screening rates, and number of hospitalizations. Through Seva, patients also provided peer support to one another in ways that are novel in primary care settings. Adoption—Patients sustained high levels of Seva use—between 53% and 60% of the patients at the 3 sites accessed Seva during the last week of the 12-month implementation period. Among clinicians, use of the technology was less robust than use by patients, with only a handful of clinicians using Seva in each clinic and behavioral health providers making most referrals to Seva in 2 of the 3 clinics. Implementation—At 2 sites, implementation plans were realized successfully; they were delayed in the third. Maintenance—Use of Seva dropped when grant funding stopped paying for the mobile phones and data plans. Two of the 3 clinics wanted to maintain the use of Seva, but they struggled to find funding to support this. Conclusions: Implementing an mHealth system can improve care among primary care patients with SUDs, and patients using the system can support one another in their recovery. Among clinicians, however, implementation requires figuring out how information from the mHealth system will be used and making mHealth data available in the electronic health (eHealth) record. M.B. wishes to thank EaStCHEM and the University of St Andrews for support.Three related series of peri‐substituted bis(tellurides) bearing naphthalene, acenaphthene and acenaphthylene backbones (Nap/Acenap/Aceyl(TeY)2 (Nap=naphthalene‐1,8‐diyl N ; Acenap=acenaphthene‐5,6‐diyl A ; Aceyl=acenaphthylene‐5,6‐diyl Ay ; Y=Ph 1 ; Fp 2 ; Tol 3 ; An‐p­ 4 ; An‐o­ 5 ; Tp 6 ; Mes 7 ; Tip 8 ) have been synthesised and their solid‐state structures determined by X‐ray crystallography. Molecular conformations were classified as a function of the two C9‐C‐Te‐C(Y) dihedral angles (θ); in the solid all members adopt AB or CCt configurations, with larger Te(aryl) moieties exclusively imposing the CCt variant. Exceptionally large J(125Te,125Te) spin–spin coupling constants between 3289–3848 Hz were obtained for compounds substituted by bulky Te(aryl) groups, implying these species are locked in a CCt‐type conformation. In contrast, compounds incorporating smaller Te(aryl) moieties are predicted to be rather dynamic in solution and afford much smaller J values (2050–2676 Hz), characteristic of greater populations of AB conformers with lower couplings. This conformational dependence of through‐space coupling is supported by DFT calculations.PostprintPeer reviewe

    Sustainable food consumption across Western and Non-Western cultures: A scoping review considering the theory of planned behaviour

    The theory of planned behaviour (TPB) states that food consumption is preceded by an intention, which is shaped by behavioural beliefs and attitudes. To mitigate criticism of the TPB's lack of cultural context, researchers have tested extended models with culturally specific variables included. This scoping review maps the use of the extended TPB across Western and Non-Western cultures in the context of sustainable food consumption, which includes meat consumption, food waste and organic food purchases. 3924 abstracts and 241 articles were screened. The final review included 95 articles. The number of Western and Non-Western studies was similar, but sample sizes were larger in Western cultures. Generally, the inclusion of culturally specific variables improved models that predicted organic food purchases and food waste, but not for meat consumption. The current findings highlight a lack of consensus regarding the selection of culturally specific variables. Instead, future cross-cultural research that explores similar factors could facilitate the development of a universal model of sustainable food. This model is required to drive a global approach towards encouraging sustainable diets. Incorporating cultural nuances and targeting common core values and attitudes may improve generalisability and efficacy of subsequent interventions that target sustainable food consumption across cultures
