    Assessment of safety and efficacy of a dietary supplement KaraLiv™ in supporting liver health: a double-blind, parallel, placebo-controlled randomized clinical trial

    Background: The liver is responsible for many critical functions within the body. If the liver becomes diseased or injured, loss of those critical functions can cause significant damage to the body. KaraLivTM is a novel herbal formulation which contains a blend of different herbal extract ingredients. The current study tested the safety and efficacy of KaraLivTM versus a placebo control in supporting liver function.Methods: The study is a randomized, double-blind, parallel, and placebo-controlled study. A total of 60 patients were divided into 2 groups of 30 each. One group was given KaraLivTM and the other group was given a placebo for a period of 56 days. Treatment results were assessed by evaluating the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), bilirubin, and alkaline phosphatase (ALP) in both groups.Results: The herbal supplement KaraLivTM significantly supported healthy liver function compared to the placebo following the 56 days of treatment. The treatment (KaraLivTM) group showed a statistically significant improvement in assessed liver enzyme levels compared to the placebo group.Conclusions: The all-natural herbal supplement KaraLivTM is a safe and effective product that can significantly help support healthy liver function

    Background: Hyperlipidemia is a condition involving abnormally high levels of lipids in the blood. Hyperlipidemia is a major risk factor for cardiovascular diseases and refers to either high levels of triglycerides (TGL) or cholesterol. Herbal supplements have been used in the management of cholesterol levels in Ayurveda, a complete medical system originating in India. KaraHeart™ is a multi-herbal extract synergistic blend that may help in the management of healthy cholesterol levels. The current study tested the efficacy, tolerability, and safety of KaraHeart™ versus a placebo in the management of cholesterol levels of patients with mild hyperlipidemia.Methods: This was a randomized, double-blind, parallel, and placebo-controlled study. A total of 100 patients were divided into two groups. One group was given KaraHeart™ and the other group was given a placebo for 120 days. Treatment results were assessed by checking the lipid profile parameters such as total cholesterol, high-density lipoprotein (HDL), low-density lipoprotein (LDL), very-low-density lipoprotein (VLDL), and TGL.Results: The study found that the herbal supplement KaraHeart™ significantly reduced levels of LDL, VLDL, TGL, and total cholesterol, while increasing the levels of HDL in the blood. Additionally, the study concluded that KaraHeart™ was safe to use.Conclusions: KaraHeart™ was shown to be safe and effective in the management of cholesterol levels

    Surgical treatment of necrotizing enterocolitis: single-centre experience from Saudi Arabia

    Aim The aim of this study was to investigate our indications of laparotomy, surgical procedures and their results, and to compare our results with those of others. In necrotizing enterocolitis (NEC), indications of surgery, surgical strategy and results vary.Materials and methods This study was conducted at the regional tertiary care referral centre. The study design was a retrospective one. Case records of 24  patients with advanced NEC who underwent laparotomy over a 7-year period were analysed. Demographic data, clinical features, laboratory and skiagram findings,  indication for surgery, operative findings, procedures performed, immediate surgery-related complications and postoperative survival were studied.Results The mean age was 20.29 days (range = 3–82 days). The mean birth weight was 1810.5 g (range = 660–3000 g). Seventy-five percent of babies were premature. Indications of surgery were as follows: pneumoperitoneum (16), failure to improve with adequate medical treatment (three), abdominal tenderness and rigidity (two), abdominal wall oedema and erythema (two), and acute intestinal obstruction (one). Peritoneal drainage was carried out in five patients, of whom two (40%) survived after laparotomy. Thirteen (54.17%) patients had focal, nine (37.5%)  multifocal and two (8.33%) had panintestinal NEC. Six patients underwent resection anastomosis (RA) and 16 underwent enterostomy. Surgery-related complications occurred in six patients. The overall mortality was eight (33.33%). Two (33.33%) of the six RA patients, four (25%) of 16 stoma patients and two (100%) with  corporation panintestinal disease died.Conclusion The most common indication for laparotomy in NEC was  pneumoperitoneum. We performed laparotomy in drain-managed patients when  stable; 40% of such patients survived. We preferred gangrene resection and enterostomy to RA. Mortality and morbidity in the RA group were higher than that in the stoma group; dead RA patients had multifocal disease.     Background: Osteoarthritis is common among the aging population worldwide. The current techniques to manage osteoarthritis focus on relieving pain and slowing the progression of the disease. Herbal or natural supplements have shown promise in achieving both these treatment goals. Two new proprietary herbal extract blends, Karallief® Easy ClimbTM (KEC) and herbal extracts with glucosamine (HEG), are combinations of several natural products shown to be effective in the treatment of knee osteoarthritis. The current study tested the efficacy and safety of KEC and HEG versus a placebo control.Methods: This is a randomized, double-blind and placebo-controlled study. A total of 120 patients were divided into 3 groups and were given KEC, HEG and Placebo in the ratio 1:1:1. Treatment results were assessed using the 30 second chair stand test, WOMAC test, knee flexion test and joint space measurement using X-rays of the knee joint.Results: The study found that the herbal supplements HEG and KEC significantly reduced osteoarthritis-related knee pain and increased joint mobility and were safe to use during 120 days of treatment. Both supplements resulted in an improvement in the 30 second chair stand test results, WOMAC pain scores, knee flexion, and joint space width as measured by X-ray, as compared to the placebo.Conclusions: Natural supplements such as HEG and KEC improve knee osteoarthritis symptoms and can be a safe and effective treatment option for patients with osteoarthritis

    Comparative antioxidant and bioavailability studies of Vitamin C in Phyllanthus emblica Linn. and its combinations with Piper nigrum Linn. and Zingiber officinale Roscoe

    Phyllanthus emblica Linn. (amla) is used in Ayurveda, the ancient Indian system of medicine and its major constituent is vitamin C which has effective free radical scavenging property. The purpose of this study was to evaluate the in vitro antioxidant activity and the bioavailability profile of vitamin C in amla and its combinations with piperine and ginger in comparison to synthetic vitamin C using New Zealand rabbits. In vitro antioxidant activity studies of synthetic vitamin C, amla, amla with piperine and amla with ginger were carried out using different models such as 2,2-Diphenyl-1-picrylhydrazyl, Nitric Oxide, Hydrogen peroxide scavenging methods, Total reductive capability and Oxygen Radical Absorbance Capacity estimation. The study results showed that synthetic vitamin C, amla, amla with piperine and amla with ginger possess significant in vitro antioxidant activity. For bioavailability studies, synthetic vitamin C, amla, amla with piperine and amla with ginger 100 mg/kg, were administered orally and the serum samples were analyzed by HPLC at 0, 1, 2, 3, 4, 6, 8, 10, 12 and 24 hours. Bioavailability studies revealed that amla with piperine combination has higher concentration of vitamin C when compared to synthetic vitamin C. This is probably due to presence of piperine, which is a bioavailability enhancer. The present study supports the fact that amla with piperine combination can be an alternative to synthetic vitamin C.Phyllanthus emblica Linn. O propósito deste estudo foi avaliar a atividade antioxidante in vitro e o perfil de biodisponibilidade da vitamina C na amla e suas combinações com piperina e gengibre em comparação com a vitamina C sintética, utilizando coelhos da Nova Zelândia. Os estudos de atividade antioxidante in vitro de vitamina C sintética, amla, amla com piperina e amla com gengibre foram realizados utilizando-se diferentes modelos para sequestrantes, como 2,2-difenil-1-picrilidrazil, óxido nítrico, peróxido de hidrogênio, capacidade redutiva total e a estimativa da capacidade de absorvância do radical oxigênio. Os resultados do estudo mostraram que vitamina C sintética, amla, amla com piperina e amla com gengibre possuem atividade antioxidante in vitro significativa. Para os estudos de biodisponibilidade, administraram-se oralmente vitamina C sintética, amla, amla com piperina e amla com gengibre 100 mg/kg e as amostras de soro foram analisadas por CLAE em 0, 1, 2, 3, 4, 6, 8, 10, 12 e 24 horas. Os estudos de biodisponibilidade revelaram que a associação de amla com piperina tem maior concentração de vitamina C, quando comparada com a vitamina C sintética. Este efeito é provavelmente devido à presença de piperina, que é intensificador de biodisponibilidade. O presente estudo apoia o fato de que a associação de amla e piperina pode ser uma alternativa para a vitamina C sintética

    Antiobesogenic and Antiatherosclerotic Properties of Caralluma fimbriata Extract

    There is evidence that the principles present in the widely consumed Indian food plant C. fimbriata extract (CFE) suppress appetite, and provide antiobesogenic and metabolic benefits. The Diet-Induced Obesity (DIO) rat model was used to investigate CFE's anorexigenic effects. Rats were randomly divided into three groups: (i) untreated control (C), (ii) control for cafeteria diet (CA), and (iii) cafeteria diet fed + CFE treated. Rats in the test group received cafeteria diet and CFE from day one onwards. CFE was administered by gavage at three doses (25, 50, 100 mg/Kg BW per day) for 90 days. The antiobesogenic effects of CFE were evaluated by monitoring changes in feed intake, body weight, serum lipid and hormonal (leptin) profiles, fat pads, and liver weight. Antiatherosclerotic effects were measured by histology. CFE induced significant and dose-dependent inhibition of food intake, with dose-related prevention of gains in body weight, liver weight, and fat pad mass. Alterations in serum lipid profiles associated with weight gain were similarly inhibited, as were the typical increases in serum leptin levels. These data substantiate CFE's reported anorexigenic effects. CFE treatment also conferred protection against atherogenesis. We conclude that CFE possesses antiobesogenic and antiatherosclerotic properties

    The Acquired Immuno Deficiency Syndrome (AIDS) considered to be great deal of courage to face the disease that seems to have no cure as yet. Many of the people living with HIV (PLHIV), have the intense feelings of hopelessness, loneliness, fear, anxiety and depression. The Highly Active Anti Retroviral Therapy (HAART) has effectively reduced HIV related morbidity and mortality for PLHIV. Even they are living longer with ART, but significant side-effects like anorexia, nausea, vomiting, fatigue and stress related problems are unavoidable. The National Institute of Health and World Health Organization recommends that, yoga makes to minimize the side effects ART and stress related problems of PLHIV. Among many branches of yoga, Hatha yoga enhances the capacity of the physical body through the use of series body postures, movements (Asanas) and breathing techniques (Pranayama). It’s a form of mind-body fitness that involves the combination of muscular activity and an internally focus on the mind for awareness of the self, breath and energy aids to boost immunity and relax the mind. Many research studies supported that, this behavior modification of yoga is potentially safe, effective and this low-cost management help to ‘detoxify’ the body, mitigate chronic fatigue, enhance endurance, improve organ and immune functions makes to enhance overall well being and quality of life of PLHIV

    Dynamics of Cell Area and Force during Spreading

    AbstractExperiments on human pulmonary artery endothelial cells are presented to show that cell area and the force exerted on a substrate increase simultaneously, but with different rates during spreading; rapid-force increase systematically occurred several minutes past initial spreading. We examine this theoretically and present three complementary mechanisms that may accompany the development of lamellar stress during spreading and underlie the observed behavior. These include: 1), the dynamics of cytoskeleton assembly at the cell basis; 2), the strengthening of acto-myosin forces in response to the generated lamellar stresses; and 3), the passive strain-stiffening of the cytoskeleton

    Root canal sealers that possess good antimicrobial property can prevent residual and recurrent infection and contribute to successful endodontic therapy. This study evaluated the antimicrobial activity of four endodontic sealers, AH Plus, Tubliseal EWT, EndoRez, and iRoot SP, against three different microorganisms, E. faecalis, C. albicans, and S. aureus, by direct contact test. 10 μL microbial suspensions were allowed to directly contact the four endodontic sealers for 1 hr at 37°C. Subsequently microbial growth was measured spectrophotometrically every 30 min for 18 hours. The microbial suspensions were simultaneously tested to determine the antimicrobial effect of components which are capable of diffusing into the medium. The results revealed that AH Plus and iRootSP had significantly higher antimicrobial activity against E. faecalis. AH Plus and Tubliseal EWT showed significantly higher antimicrobial activity against C. albicans and S. aureus compared to iRoot SP and EndoRez. EndoRez showed the least antimicrobial activity against all the three microorganisms. Inhibition of microbial growth is related to the direct contact of microorganisms with the sealers. In conclusion AH Plus had significantly higher antimicrobial activity against E. faecalis, C. albicans, and S. aureus
