“We’ve got a lack of family values”: an examination of how teachers formulate and justify their SRE approach
Sex and Relationship Education (SRE) in England has been the focus of critical scrutiny on several occasions, but there has been little attention paid to how teachers formulate their provision, especially given their crucial role in determining what is taught in the classroom. While current policy suggests that provision should be inclusive of sexual diversity, it simultaneously gives educators the scope to determine the form this takes. This is an important issue given the substantial impact that teachers’ views and discourses have on what is taught. Using a discourse analytical framework, this study sought to examine how teachers of SRE formulate and account for their provision, with a particular focus on how their assumptions about young people’s sexual health needs underpin their actions. Initially, teachers sought to formulate their activities in terms of an overall ethos, providing legitimacy for the key elements of their programme being aligned with official government health promotion strategy, as opposed to other areas such as pleasure and diversity. This was supported by their constructions of young people, particularly young women and individuals from ‘at risk’ communities, as being particularly vulnerable
Ibuprofen Blunts Ventilatory Acclimatization to Sustained Hypoxia in Humans.
Ventilatory acclimatization to hypoxia is a time-dependent increase in ventilation and the hypoxic ventilatory response (HVR) that involves neural plasticity in both carotid body chemoreceptors and brainstem respiratory centers. The mechanisms of such plasticity are not completely understood but recent animal studies show it can be blocked by administering ibuprofen, a nonsteroidal anti-inflammatory drug, during chronic hypoxia. We tested the hypothesis that ibuprofen would also block the increase in HVR with chronic hypoxia in humans in 15 healthy men and women using a double-blind, placebo controlled, cross-over trial. The isocapnic HVR was measured with standard methods in subjects treated with ibuprofen (400 mg every 8 hrs) or placebo for 48 hours at sea level and 48 hours at high altitude (3,800 m). Subjects returned to sea level for at least 30 days prior to repeating the protocol with the opposite treatment. Ibuprofen significantly decreased the HVR after acclimatization to high altitude compared to placebo but it did not affect ventilation or arterial O2 saturation breathing ambient air at high altitude. Hence, compensatory responses prevent hypoventilation with decreased isocapnic ventilatory O2-sensitivity from ibuprofen at this altitude. The effect of ibuprofen to decrease the HVR in humans provides the first experimental evidence that a signaling mechanism described for ventilatory acclimatization to hypoxia in animal models also occurs in people. This establishes a foundation for the future experiments to test the potential role of different mechanisms for neural plasticity and ventilatory acclimatization in humans with chronic hypoxemia from lung disease
The Evolution of the Optical and Near-Infrared Galaxy Luminosity Functions and Luminosity Densities to z~2
Using Hubble Space Telescope and ground-based U through K- band photometry
from the Great Observatories Origins Deep Survey (GOODS), we measure the
evolution of the luminosity function and luminosity density in the rest-frame
optical (UBR) to z ~ 2, bridging the poorly explored ``redshift desert''
between z~1 and z~2. We also use deep near-infrared observations to measure the
evolution in the rest-frame J-band to z~1. Compared to local measurements from
the SDSS, we find a brightening of the characteristic magnitude, (M*), by ~2.1,
\~0.8 and ~0.7 mag between z=0.1 and z=1.9, in U, B, and R bands, respectively.
The evolution of M* in the J-band is in the opposite sense, showing a dimming
between redshifts z=0.4 and z=0.9. This is consistent with a scenario in which
the mean star formation rate in galaxies was higher in the past, while the mean
stellar mass was lower, in qualitative agreement with hierarchical galaxy
formation models. We find that the shape of the luminosity function is strongly
dependent on spectral type and that there is strong evolution with redshift in
the relative contribution from the different spectral types to the luminosity
density.
We find good agreement in the luminosity function derived from an R-selected
and a K-selected sample at z~1, suggesting that optically selected surveys of
similar depth (R < 24) are not missing a significant fraction of objects at
this redshift relative to a near-infrared-selected sample. We compare the
rest-frame B-band luminosity functions from z~0--2 with the predictions of a
semi-analytic hierarchical model of galaxy formation, and find qualitatively
good agreement. In particular, the model predicts at least as many optically
luminous galaxies at z~1--2 as are implied by our observations.Comment: 43 pages; 15 Figures; 5 Tables, Accepted for publication in Ap.
Exploration of potential objective and subjective daily indicators of sleep health in normal sleepers
Purpose: While the concept of "sleep health" has only recently been defined, how it relates to both subjective and objective sleep parameters is yet to be determined. The current study aimed to identify potential indicators of poorer sleep health, from subjective and objective daily sleep characteristics, in normal sleepers.
Participants and methods: Eighty-three individuals aged 18-65 years with no history of sleep disorders, chronic physical or psychiatric illnesses, or substance misuse were recruited from the North of England. Secondary analysis of a series of standardized studies, which included psychometrics, actigraphy, and an in-lab polysomnography (PSG) component, was undertaken. Questions from several psychometric sleep scales were combined to create an aggregate measure of sleep health status. Subjective sleep continuity was assessed by 2-week sleep diary. Objective measures comprised two continuous weeks of actigraphy and two nights of in-lab PSG.
Results: Significant negative correlations were evident between sleep health scores and both diary-derived subjective sleep latency (SL; diary) and actigraphy-derived SL (actigraphy). This was reflected by independent samples t-test between high and low sleep health groups. No relationships between sleep health and PSG parameters were observed. Regression analyses indicated sleep latencies from both the sleep diary and actigraphy as significant predictors, explaining 28.2% of the variance in sleep health.
Conclusion: Perceived increases in SL appear to be a primary indicator of declining sleep health in normal sleepers. The majority of objective sleep parameters, including gross PSG sleep parameters, appear not to be sensitive to sleep health status in normal sleepers. Future research is needed to understand the physical and psychological correlates of sleep health in larger samples
Ophthalmic signs in Ugandan adults with HIV-associated cryptococcal meningitis: A nested analysis of the ASTRO-CM cohort.
Cryptococcal meningitis is a leading cause of morbidity and mortality among HIV-infected persons, accounting for 15% of AIDS-related deaths. Visual disturbance is commonly reported, and a wide range of ophthalmic signs may be present on examination. There is limited published literature to date describing the range and incidence of ophthalmic signs in HIV-associated cryptococcal meningitis. Nested within the Adjunctive Sertraline for the Treatment of HIV-Associated Cryptococcal Meningitis (ASTRO-CM) trial (ClinicalTrials.gov number: NCT01802385), we conducted an observational study of 696 Ugandan adults with HIV-associated cryptococcal meningitis. Patients were screened for visual disturbance and external ophthalmic signs at initial presentation and at follow-up appointments over 18 weeks. Assessment comprised simple clinical history and basic examination and required no specialist equipment. More than a quarter of our cohort demonstrated ocular signs or symptoms, which were observed throughout the study period. A broad range of ocular signs were demonstrated: these included neurological signs (10.9%), localized ocular pathology (4.5%), and evidence of concurrent systemic disease (12.9%). The range of signs observed demonstrates the complexities of case management in patients with advanced HIV and cryptococcosis and also the importance of basic ocular examination in low resource settings. There remains an urgent need for studies conducting comprehensive ocular examination in patients with HIV-associated cryptococcal meningitis; these studies should include formal assessment of visual acuity, slit lamp examination and dilated indirect ophthalmoscopy. Prospective studies should investigate whether there is a correlation between reported visual disturbance and objective signs, in order to further clarify the underlying mechanisms and to guide effective diagnosis, follow-up and management
Using HCAHPS data to model correlates of medication understanding at hospital discharge
Background: Hospitals are challenged to improve hospital transitions to home and are held accountable through public reporting. Design: This cross-sectional study used patients’ self-reported experience data from the publicly reported Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey to describe correlates of medication understanding at hospital discharge, using data collected from adult patients discharged from one Midwestern community hospital (N=154).
Results: The final logistic regression model included four correlates of medication understanding: 1) nurse always communicates well, 2) physician always communicates well, 3) new prescriptions during hospital stay, and 4) very good or better mental health, and these classified 72.6% of the cases. Significant correlates of the patient strongly agreeing that they understood discharge medications were the “nurse always communicates well” (odds ratio =3.10, 95% confidence interval: 1.25, 7.66) and “very good or better self-perceived mental health” (odds ratio =2.17, 95% confidence interval: 1.02, 4.64).
Conclusion: HCAHPS data can be used to model correlates of medication understanding, which are then useful for evaluating intervention effects following quality improvement
Emotional Intelligence and Decision-Making in Higher Education Administrators in Post-Secondary Institutions in Eastern Tennessee
This purpose of this phenomenological study was to understand the impact of emotional intelligence on decision-making in higher education administrators. A qualitative research design was used for this study. Data was collected in three phases from the following sources; survey, case study submissions, and open-ended face-to-face interviews. The research data, about emotional intelligence and decision-making, were collected form ten higher education administrators. Constant comparative method was an important part of the data collection process for this study. Range of participant leadership experience was 3-8+ years and participant roles were either, Department Chair, Program Director, Dean, Provost, and President. The findings from this research study indicate that decision-making is impacted by emotional intelligence of higher education leaders. Empathy, self-awareness, and relationship management were influential components in navigating difficult situations, conflict resolution, and the decision-making process
