    A minimum estimate for the incidence of gastric cancer in Eastern Kenya

    We documented available information concerning incident cases of gastric cancer in part of Kenya's Eastern Province between 1991 and 1993. By reviewing the records of all major health facilities in the area, 200 cases of gastric carcinoma were found giving an annual average crude incidence rate of 7.01 per 100 000 males and 3.7 for females (world age-standardised rates, 14.3 for males and 7.1 for females). There is likely to be underascertainment of cases especially among those aged over 65 years. Previous incidence estimates for the same area of Kenya were reviewed and a 10-fold increase in the recorded indirectly standardised incidence rate between the periods 1965–70 and 1991–93 was noted but this may be due to improved diagnostic facilities. The recent rates in this part of Kenya are comparable to Eastern European rates and similar to those recorded in other highland regions of Africa. © 2001 Cancer Research Campaig

    Bi-allelic Loss-of-Function CACNA1B Mutations in Progressive Epilepsy-Dyskinesia.

    The occurrence of non-epileptic hyperkinetic movements in the context of developmental epileptic encephalopathies is an increasingly recognized phenomenon. Identification of causative mutations provides an important insight into common pathogenic mechanisms that cause both seizures and abnormal motor control. We report bi-allelic loss-of-function CACNA1B variants in six children from three unrelated families whose affected members present with a complex and progressive neurological syndrome. All affected individuals presented with epileptic encephalopathy, severe neurodevelopmental delay (often with regression), and a hyperkinetic movement disorder. Additional neurological features included postnatal microcephaly and hypotonia. Five children died in childhood or adolescence (mean age of death: 9 years), mainly as a result of secondary respiratory complications. CACNA1B encodes the pore-forming subunit of the pre-synaptic neuronal voltage-gated calcium channel Cav2.2/N-type, crucial for SNARE-mediated neurotransmission, particularly in the early postnatal period. Bi-allelic loss-of-function variants in CACNA1B are predicted to cause disruption of Ca2+ influx, leading to impaired synaptic neurotransmission. The resultant effect on neuronal function is likely to be important in the development of involuntary movements and epilepsy.     We review the literature on universal computably enumerable equivalence relations, i.e. the computably enumerable equivalence relations (ceers) which are Σ10\Sigma^0_1-complete with respect to computable reducibility on equivalence relations. Special attention will be given to the so-called uniformly effectively inseparable (u.e.i.) ceers, i.e. the nontrivial ceers yielding partitions of the natural numbers in which each pair of distinct equivalence classes is effectively inseparable (uniformly in their representatives). The u.e.i. ceers comprise infinitely many isomorphism types. The relation of provable equivalence in Peano Arithmetic plays an important role in the study and classification of the u.e.i. ceers

    Winter warming in West Antarctica caused by central tropical Pacific warming

    The Pacific sector of Antarctica, including both the Antarctic Peninsula and continental West Antarctica, has experienced substantial warming in the past 30 years. An increase in the circumpolar westerlies, owing in part to the decline in stratospheric ozone concentrations since the late 1970s, may account for warming trends in the peninsula region in austral summer and autumn. The more widespread warming in continental West Antarctica (Ellsworth Land and Marie Byrd Land) occurs primarily in austral winter and spring, and remains unexplained. Here we use observations of Antarctic surface temperature and global sea surface temperature, and atmospheric circulation data to show that recent warming in continental West Antarctica is linked to sea surface temperature changes in the tropical Pacific. Over the past 30 years, anomalous sea surface temperatures in the central tropical Pacific have generated an atmospheric Rossby wave response that influences atmospheric circulation over the Amundsen Sea, causing increased advection of warm air to the Antarctic continent. General circulation model experiments show that the central tropical Pacific is a critical region for producing the observed high latitude response. We conclude that, by affecting the atmospheric circulation at high southern latitudes, increasing tropical sea surface temperatures may account for West Antarctic warming through most of the twentieth century
