    New species of Dolichopoda BolĂ­var, 1880 (Orthoptera, Rhaphidophoridae) from the Aegean Islands of Andros, Paros and Kinaros (Greece)

    In this paper two new species of Dolichopoda Bolívar, 1880 from the Cyclades islands of Andros and Paros and one from the Dodecanissos island of Kinaros, are described, increasing the total number of Greek species to 32. The new species from Paros (D. kikladica Di Russo & Rampini, n. sp.) and Kinaros (D. margiolis Di Russo & Rampini, n. sp.) show strong similarities with D. naxia Boudou-Saltet, 1972 from Naxos forming an homogenous group limited to the central Aegean islands. On the other hand the species D. christos-nifoni Di Russo & Rampini, n. sp. from Andros shows a combination of morphological characters that partly resemble characters found in Evvian Dolichopoda, and partly in species from the Cyclades. Relationships among these three new taxa and the other adjacent Dolichopoda species are discussed on the basis of the paleogeological and paleoclimatic events that shaped the present geography of the Aegean area

    In this paper a new synonymy Hellerina Galvagni, 2006 = Dolichopoda Bolivar, 1880 and a new combination Hellerina lycia Galvagni, 2006 = Dolichopoda lycia (Galvagni, 2006). A redescription of male morphology and female description are reported. The affinity with the other Anatolian Dolichopoda and with some species of the sub genus Petrochilosina Boudou-Saltet, 1980, characterized by a bifurcated epiphallus, are discussed

    Genetic divergence in the cave cricket Troglophilus neglectus (Orthoptera Rhaphidophoridae): mitochondrial and nuclear DNA data.

    In this study we used sequence polymorphisms at one mitochondrial and one nuclear gene (Cytochrome Oxidase subunit I and Internal Transcribed Spacer 1, respectively) to assess the degree of genetic divergence among 21 populations of the cave cricket Troglophilus neglectus (Orthoptera, Rhaphidophoridae), a species whose currently known range extends from the Balkan Peninsula to Southern Bavaria. Nineteen populations were sampled in Northern Italy, Slovenia, Croatia and Bosnia and Herzegovina, while two populations came from Germany (Lower Saxony) and Czech Republic, thus well outside the species range. Molecular data revealed a high level of fragmentation, with most of the study populations bearing exclusive haplotypes, the sole exception being the German and Czech specimens, which carried haplotypes also occurring at Slovenian locations. Spatial distribution of genetic heterogeneity and pattern of genetic divergence argue in favor of a recent origin of the two Central European populations, possibly through man-mediated dispersal event(s). These populations being not considered, our data are in remarkable agreement with a previous study based on allozymes conducted on a subset of populations of the same species and, more generally, with what is known on the population genetics of peri-Mediterranean Rhaphidophorids

    The cave crickets of the genus Dolichopoda from Evvia and Skyros islands: formal description of D. octhoniai and D. saraolacosi (Orthoptera: Rhaphidophoridae)

    Two species, D. octhoniai from Evvia and D. saraolacosi from Skyros island (Greece) are morphologically described. These two species were collected for the first time at the end of the '70s but they were recorded only as nomina nuda without any formal taxonomic description. D. octhoniai is very similar to the other Evvian species D. makrikapa and to D. vandeli and D. petrochilosi from Viotia and Attiki respectively, differing from them only by a few morphological characters. On the other hand D. saraolacosi is very different from all the other species of Central Greece and West Aegean showing some affinity only with the Attiki species D. insignis and with the South Evvian species D. cassagnaui. Relationships among the species inhabiting caves of this area of Greece are discussed in relation to the complex geological history of the West Aegean area and the adjacent mainland

    Molecular divergence and evolutionary relationships among Aemodogryllinae from Southern China, Laos and Thailand (Orthoptera, Rhaphidophoridae)

    In this study we screened for sequence polymorphisms at one mitochondrial (Cytochrome Oxidase subunit I) and one nuclear (Internal Transcribed Spacer 1) gene 33 populations of the cave cricket genera Diestrammena, Paradiestrammena, Eutachycines and Paratachycines from Southern China (three Provinces: Jiangxi, Guangdong and Guizhou), Laos and Thailand. Twenty-five of these populations were assigned to the genus Diestrammena, subgenus Gymnaeta, while the remaining eight belonged to the genera Paradiestrammena (3), Eutachycines (3) and Paratachycines (2). The degree of troglomorphosis varies among them; some populations are blind and depigmented, some have fully developed eyes, while some others show intermediate characteristics. Phylogenetic searches carried out on the two gene partitions separately revealed multiple cases of incongruence but only three of them were statistically significant and were hence removed from the subsequent analyses based on the combined data set. Our data do not support Diestrammena as monophyletic while representatives of Paradiestrammena, Eutachycines and Paratachycines were clustered together; the validity of some nominal species was confirmed molecularly but we also revealed a large number of deeply divergent lineages. Populations with the same degree of troglomorphosis do not cluster together. We identified five major clades; divergence among them (and in a few circumstances also within them) is always higher than the DNA barcode threshold for intraspecific comparisons in insects. In two circumstances, the same clades (III and V) are co-distributed in geographically distinct areas (Provinces). This geographical distribution might be explained by envisioning an evolutionary scenario based on zones of secondary admixture following epigean dispersal among lineages that diverged in allopatry

    Infected pancreatic necrosis: outcomes and clinical predictors of mortality. A post hoc analysis of the MANCTRA-1 international study

    : The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135-15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359-5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138-5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184-5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598-9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090-6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286-5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p < 0.001; 95% CI 1.912-7.442; aOR 3.772), whereas endoscopic drainage of pancreatic necrosis (p = 0.018; 95% CI 0.138-0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143-0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible.     Goodbye Hartmann trial: a prospective, international, multicenter, observational study on the current use of a surgical procedure developed a century ago

    Background: Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann's procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods: This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results: 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Severe complications (Clavien-Dindo ≄ 3b) were higher in the HP group (P < 0.001). 30-day mortality was higher in HP patients (13.7%), especially in case of bowel perforation and diffused peritonitis. 1-year follow-up showed no differences on ostomy reversal rate between HP and RPA. (P = 0.127). A backward likelihood logistic regression model showed that RPA was preferred in younger patients, having low ASA score (≀ 3), in case of large bowel obstruction, absence of colonic ischemia, longer time from admission to surgery, operating early at the day working hours, by a surgeon who performed more than 50 colorectal resections. Conclusions: After 100 years since the first Hartmann's procedure, HP remains the most common treatment for left-sided colorectal emergencies. Treatment's choice depends on patient characteristics, the time of surgery and the experience of the surgeon. RPA should be considered as the gold standard for surgery, with HP being an exception

    Association of kidney disease measures with risk of renal function worsening in patients with type 1 diabetes

    Background: Albuminuria has been classically considered a marker of kidney damage progression in diabetic patients and it is routinely assessed to monitor kidney function. However, the role of a mild GFR reduction on the development of stage 653 CKD has been less explored in type 1 diabetes mellitus (T1DM) patients. Aim of the present study was to evaluate the prognostic role of kidney disease measures, namely albuminuria and reduced GFR, on the development of stage 653 CKD in a large cohort of patients affected by T1DM. Methods: A total of 4284 patients affected by T1DM followed-up at 76 diabetes centers participating to the Italian Association of Clinical Diabetologists (Associazione Medici Diabetologi, AMD) initiative constitutes the study population. Urinary albumin excretion (ACR) and estimated GFR (eGFR) were retrieved and analyzed. The incidence of stage 653 CKD (eGFR < 60 mL/min/1.73 m2) or eGFR reduction > 30% from baseline was evaluated. Results: The mean estimated GFR was 98 \ub1 17 mL/min/1.73m2 and the proportion of patients with albuminuria was 15.3% (n = 654) at baseline. About 8% (n = 337) of patients developed one of the two renal endpoints during the 4-year follow-up period. Age, albuminuria (micro or macro) and baseline eGFR < 90 ml/min/m2 were independent risk factors for stage 653 CKD and renal function worsening. When compared to patients with eGFR > 90 ml/min/1.73m2 and normoalbuminuria, those with albuminuria at baseline had a 1.69 greater risk of reaching stage 3 CKD, while patients with mild eGFR reduction (i.e. eGFR between 90 and 60 mL/min/1.73 m2) show a 3.81 greater risk that rose to 8.24 for those patients with albuminuria and mild eGFR reduction at baseline. Conclusions: Albuminuria and eGFR reduction represent independent risk factors for incident stage 653 CKD in T1DM patients. The simultaneous occurrence of reduced eGFR and albuminuria have a synergistic effect on renal function worsening

    The study of the biogeography of the eight species of Isopoda Trichoniscidae from Sardegna brings to consider the faunistic relationships happened in the past between the island and the Pyrenees. Two of the species are eutroglophilous and rather widespread; the other six are troglobic and endemic to Sardinia. Five of the latter show strong affinity to Pyrenees forms. A detailed description is given of the distribution of the various species. This distribution makes possible some observations on the history of the fauna of the island
