Background.Total knee replacement is a procedure of choice in the management of severe osteoarthritis. Currently two types of prosthesis are widely used, cruciate retaining and cruciate substituting. Experts have not yet reached agreement regarding which one is better. This study is aimed to compare functional outcome between cruciate retaining and substituting prosthesis. Materials and methods. This study was a randomized single-blinded clinical trial. Patients with severe osteoarthritis were divided into two groups and evaluated before operation. First group underwent cruciate retaining total knee replacement, and the other cruciate substituting total knee replacement. Functional outcome was evaluated in 3 and 6 months after operation, regarding the knee flexion angle and International Knee Documentation Committee score.Results.There were 14 patients. Mean knee flexion angle of cruciate retaining in 3 and 6 months were 105.0 degrees and 113.3 degrees respectively, whereas cruciate substituting were 118.1 degrees and 126.2 degrees. International Knee Documentation Committee score of cruciate retaining in 3 and 6 months were 49.0 and 59.4 respectively, while cruciate substituting were 52.6 and 63.8. There were significant differences in flexion angles between cruciate retaining and cruciate substituting groups at 3 months (p=0.006) and 6 months (p=0.018). No significant difference was demonstrated in International Knee Documentation Committee score between cruciate retaining and cruciate substituting groups at 3 months (p=0.053) and 6 months (p=0.240).Conclusions.Functional outcome regarding the knee flexion angle of cruciate substituting group was 13.1 degrees better in 3 months and 12.9 degrees in 6 months compared to cruciate retaining group. Functional outcome based on International Knee Documentation Committee score score did not show significant differences between two groups whether 3 or 6 months after operation

Multiscale Modeling of Thiol Overoxidation in Peroxiredoxins by Hydrogen Peroxide

In this work, we employ a multiscale quantum-classical mechanics (QM/MM) scheme to investigate the chemical reactivity of sulfenic acids toward hydrogen peroxide, both in aqueous solution and in the protein environment of the peroxiredoxin alkyl hydroperoxide reductase E from Mycobacterium tuberculosis (MtAhpE). The reaction of oxidation of cysteine with hydrogen peroxides, catalyzed by peroxiredoxins, is usually accelerated several orders of magnitude in comparison with the analogous reaction in solution. The resulting cysteine sulfenic acid is then reduced in other steps of the catalytic cycle, recovering the original thiol. However, under some conditions, the sulfenic acid can react with another equivalent of oxidant to form a sulfinic acid. This process is called overoxidation and has been associated with redox signaling. Herein, we employed a multiscale scheme based on density function theory calculations coupled to the classical AMBER force field, developed in our group, to establish the molecular basis of thiol overoxidation by hydrogen peroxide. Our results suggest that residues that play key catalytic roles in the oxidation of MtAhpE are not relevant in the overoxidation process. Indeed, the calculations propose that the process is unfavored by this particular enzyme microenvironment.Fil: Semelak, Jonathan Alexis. Consejo Nacional de Investigaciones Científicas y Técnicas. Recent technology advances based on smart devices have improved the medical facilities and become increasingly popular in association with real-time health monitoring and remote/personals health-care. Healthcare organisations are still required to pay more attention for some improvements in terms of cost-effectiveness and maintaining efficiency, and avoid patients to take admission at hospital. Sickle cell disease (SCD) is one of the most challenges chronic obtrusive disease that facing healthcare, affects a large numbers of people from early childhood. Currently, the vast majority of hospitals and healthcare sectors are using manual approach that depends completely on patient input, which can be slowly analysed, time consuming and stressful. This work proposes an alert system that could send instant information to the doctors once detects serious condition from the collected data of the patient. In addition, this work offers a system that can analyse datasets automatically in order to reduce error rate. A machine-learning algorithm was applied to perform the classification process. Two experiments were conducted to classify SCD patients from normal patients using machine learning algorithm in which 99 % classification accuracy was achieved using the Instance-based learning algorithm

Evaluation of FHIT gene alterations in ovarian cancer.

    Get PDF
The FHIT gene, recently cloned and mapped on chromosome 3p14.2, has frequently been found to be abnormal in several established cancer cell lines and primary tumours. As alterations of chromosome 3p are common events in ovarian cancers with breakpoint sites at 3p14.2, we decided to investigate the role of FHIT in human ovarian tumorigenesis. Fifty-four primary ovarian carcinomas were studied by reverse transcription of FHIT mRNA followed by polymerase chain reaction (PCR) amplification and sequencing of products. The same tumours and matched normal tissues were also investigated for loss of heterozygosity using three microsatellite markers located inside the gene. We found an abnormal transcript of the FHIT gene in two cases (4%) and allelic losses in eight cases (15%). Twelve (22%) of the 54 tumours investigated belonged to young patients with a family history of breast/ovarian cancer. In none of these cases was the FHITgene found to be altered. Our results indicate that FHITplays a role in a small proportion of ovarian carcinomas
