I segnali delle sue forme, delle sue relazioni e delle sue identit\ue0 sono gi\ue0 evidenti in alcune citt\ue0 del presente ed ad essi sono dedicate numerose ricerche urbanistiche, sociologiche ed economiche. Ma i segnali sono evidenti e trasmettono ispirazioni e stimoli anche a chi osserva la citt\ue0 per mestiere di progettista, di pianificatore, di stratega dello sviluppo. Il XXI secolo sar\ue0 l\u2019era indiscussa delle citt\ue0 e su di esse si misurer\ue0 lo sviluppo delle nazioni. Per la prima volta, pi\uf9 della met\ue0 della popolazione mondiale vivr\ue0 nelle citt\ue0, in Europa oggi la cifra \ue8 gi\ue0 di oltre il 75%, e nei paesi in via di sviluppo raggiunger\ue0 velocemente il 50%.     The effects of home-based exercise therapy for breast cancer-related fatigue induced by radical radiotherapy

    Background: Radiotherapy (RT) can lead to cancer-related fatigue (CRF) and decreased health-related quality of life (HRQoL) in breast cancer patients. The purpose of this trial was to examine the feasibility and efficacy of a home-based resistance and aerobic exercise intervention for reducing CRF and improving HRQoL in breast cancer patients during RT. Methods: Women with breast cancer (N = 106) commencing RT were randomized to 12 weeks of home-based resistance and aerobic exercise (EX) or usual care/control (CON). The primary endpoint was CRF, with secondary endpoints of HRQoL, sleep duration and quality, and physical activity. Measurements were undertaken prior to RT, at completion of RT (~ 6 weeks), at completion of the intervention (12 weeks), and 6 and 12 months after RT completion, while CRF was also measured weekly during RT. Results: Eighty-nine women completed the study (EX = 43, CON = 46). Over the 12-week intervention, EX completed 1 – 2 resistance training sessions and accumulated 30–40 min of aerobic exercise weekly. For CRF, EX had a quicker recovery both during and post-RT compared to CON (p \u3c 0.05). Moreover, there was a significant difference in HRQoL between groups at RT completion, with HRQoL unchanged in CON and higher in EX (p \u3c 0.05). There was no change in sleep duration or quality for either group and there were no exercise-related adverse effects. Conclusions: Home-based resistance and aerobic exercise during RT is safe, feasible, and effective in accelerating CRF recovery and improving HRQoL. Improvements in CRF and HRQoL for these patients can be achieved with smaller exercise dosages than stated in the generic recommendations for breast cancer

    Immediate versus delayed exercise in men initiating androgen deprivation: effects on bone density and soft tissue composition

    OBJECTIVES: To examine whether it is more efficacious to commence exercise medicine in men with prostate cancer at the onset of androgen-deprivation therapy (ADT) rather than later on during treatment to preserve bone and soft-tissue composition, as ADT results in adverse effects including: reduced bone mineral density (BMD), loss of muscle mass, and increased fat mass (FM). PATIENTS AND METHODS: In all, 104 patients with prostate cancer, aged 48-84 years initiating ADT, were randomised to immediate exercise (IMEX, n = 54) or delayed exercise (DEL, n = 50) conditions. The former consisted of 6 months of supervised resistance/aerobic/impact exercise and the latter comprised 6 months of usual care followed by 6 months of the identical exercise programme. Regional and whole body BMD, lean mass (LM), whole body FM and trunk FM, and appendicular skeletal muscle (ASM) were assessed by dual X-ray absorptiometry, and muscle density by peripheral quantitative computed tomography at baseline, and at 6 and 12 months. RESULTS: There was a significant time effect (P \u3c 0.001) for whole body, spine and hip BMD with a progressive loss in the IMEX and DEL groups, although lumbar spine BMD was largely preserved in the IMEX group at 6 months compared with the DEL group (-0.4% vs -1.6%). LM, ASM, and muscle density were preserved in the IMEX group at 6 months, declined in the DEL group at 6 months (-1.4% to -2.5%) and then recovered at 12 months after training. FM and trunk FM increased (P \u3c 0.001) over the 12-month period in the IMEX (7.8% and 4.5%, respectively) and DEL groups (6.5% and 4.3%, respectively). CONCLUSIONS: Commencing exercise at the onset of ADT preserves lumbar spine BMD, muscle mass, and muscle density. To avoid treatment-related adverse musculoskeletal effects, exercise medicine should be prescribed and commenced at the onset of ADT

    Exercise Preserves Physical Function in Prostate Cancer Patients with Bone Metastases

    The presence of bone metastases has excluded participation of cancer patients in exercise interventions and is a relative contraindication to supervised exercise in the community setting due to concerns of fragility fracture. We examined the efficacy and safety of a modular multi-modal exercise program in prostate cancer patients with bone metastases.Between 2012 and 2015, 57 prostate cancer patients (70.0±8.4 years; BMI 28.7±4.0 kg/m) with bone metastases (pelvis 75.4%, femur 40.4%, rib/thoracic spine 66.7%, lumbar spine 43.9%, humerus 24.6%, other sites 70.2%) were randomised to multi-modal supervised aerobic, resistance and flexibility exercises undertaken thrice weekly (EX, n=28) or usual care (CON, n=29) for 3 months. Physical function subscale of the SF-36 was the primary endpoint as an indicator of patient-rated physical functioning. Secondary endpoints included objective measures of physical function, lower body muscle strength, body composition and fatigue. Safety was assessed by recording the incidence and severity of any adverse events, skeletal complications, and bone pain throughout the intervention.There was a significant difference between groups for self-reported physical functioning (3.2 points, 95% CI 0.4-6.0 points; p=0.028) and lower body muscle strength (6.6 kg, 95% CI 0.6-12.7; p =0.033) at 3 months favouring EX. However, there was no difference between groups for lean mass (p=0.584), fat mass (p=0.598), or fatigue (p=0.964). There were no exercise-related adverse events or skeletal fractures and no differences in bone pain between EX and CON (p=0.507).Multi-modal modular exercise in prostate cancer patients with bone metastases led to self-reported improvements in physical function and objectively measured lower body muscle strength with no skeletal complications or increased bone pain.This is an open-access article distributed under the terms of the Creative Commons Attribution-Non Commercial-No Derivatives License 4.0 (CCBY-NC-ND), where it is permissible to download and share the work provided it is properly cited.     Determining Material Response for Polyvinyl Butyral (PVB) in Blast Loading Situations

    Protecting structures from the effect of blast loads requires the careful design of all building components. In this context, the mechanical properties of Polyvinyl Butyral (PVB) are of interest to designers as the membrane behaviour will affect the performance of laminated glass glazing when loaded by explosion pressure waves. This polymer behaves in a complex manner and is difficult to model over the wide range of strain rates relevant to blast analysis. In this study, data from experimental tests conducted at strain rates from 0.01 s−1 to 400 s−1 were used to develop material models accounting for the rate dependency of the material. Firstly, two models were derived assuming Prony series formulations. A reduced polynomial spring and a spring derived from the model proposed by Hoo Fatt and Ouyang were used. Two fits were produced for each of these models, one for low rate cases, up to 8 s−1, and one for high rate cases, from 20 s−1. Afterwards, a single model representing all rates was produced using a finite deformation viscoelastic model. This assumed two hyperelastic springs in parallel, one of which was in series with a non-linear damper. The results were compared with the experimental results, assessing the quality of the fits in the strain range of interest for blast loading situations. This should provide designers with the information to choose between the available models depending on their design needs

    The history, genome and biology of NCTC 30: a non-pandemic Vibrio cholerae isolate from World War One.

    The sixth global cholera pandemic lasted from 1899 to 1923. However, despite widespread fear of the disease and of its negative effects on troop morale, very few soldiers in the British Expeditionary Forces contracted cholera between 1914 and 1918. Here, we have revived and sequenced the genome of NCTC 30, a 102-year-old Vibrio cholerae isolate, which we believe is the oldest publicly available live V. cholerae strain in existence. NCTC 30 was isolated in 1916 from a British soldier convalescent in Egypt. We found that this strain does not encode cholera toxin, thought to be necessary to cause cholera, and is not part of V. cholerae lineages responsible for the pandemic disease. We also show that NCTC 30, which predates the introduction of penicillin-based antibiotics, harbours a functional β-lactamase antibiotic resistance gene. Our data corroborate and provide molecular explanations for previous phenotypic studies of NCTC 30 and provide a new high-quality genome sequence for historical, non-pandemic V. cholerae

    Ground reaction force differences in the countermovement jump in girls with different levels of performance

    Purpose: The aim of this study was to ascertain the biomechanical differences between better and poorer performers of the vertical jump in a homogeneous group of children. Method: Twenty-four girls were divided into low-scoring (LOW; M age = 6.3 ± 0.8 years) and high-scoring (HIGH; M age = 6.6 ± 0.8 years) groups based on their performance on the vertical jump. The force-, velocity-, displacement-, and rate of force development (RFD)-time curves of vertical jumps were analyzed to determine the differences between groups. Results: The analysis of the data showed differences in the pattern of the ensemble mean curves of the HIGH and LOW groups, although the majority of the differences occurred during the eccentric contraction phase of the jump. The differences in the HIGH group with respect to the LOW group were: lower force at the beginning of the movement, higher speed and RFD during the eccentric phase, high force at the beginning of the concentric phase, higher velocity during the concentric phase, and a higher position at takeoff. Conclusion: The results showed that the HIGH group achieved a higher jump height than did the LOW group by increasing the effectiveness of the countermovement and achieving a more advantageous position at takeoff.Centro de Investigación en Rendimiento Físico y Deportiv

    Cancer survivors’ experiences of a community-based cancer-specific exercise programme: results of an exploratory survey

    Purpose Exercise levels often decline following cancer diagnosis despite growing evidence of its benefits. Treatment side-effects, older age, lack of confidence and opportunity to exercise with others in similar circumstances influence this. Our study explored the experiences of people attending a cancer-specific community-based exercise programme (CU Fitter™). Methods A survey distributed to those attending the programme gathered demographic/clinical information, self-reported exercise levels, information provision and barriers to/benefits of exercise. Results Sixty surveys were evaluable from 65/100 returned (62% female, 68% >60yrs, 66% breast/prostate cancer). Most (68%) were receiving treatment. 68% attended classes once or twice weekly. 55% received exercise advice after diagnosis, usually from their hospital doctor/nurse. More (73%) had read about exercising, but less used the internet to source information (32%). Self-reported exercise levels were higher currently than before diagnosis (p=0.05). 48% said their primary barrier to exercising was the physical impact of cancer/treatment. Improving fitness/health (40%) and social support (16%) were the most important gains from the programme. Many (67%) had made other lifestyle changes and intented to keep (50%), or increase (30%) exercising. Conclusions This community-based cancer-specific exercise approach engaged people with cancer and showed physical, psychological, and social benefits. Implications for cancer survivors Community grown exercise initiatives bring cancer survivors together creating their own supportive environment. Combining this with instructors familiar with the population and providing an open-ended service may prove particularly motivating and beneficial.
