    A cien años de la administración científica. Análisis de las aportaciones de Taylor

    Con la entrada del cronómetro en el taller como dispositivo simbólico de control, quedaba claro que, con miras a la consolidación del capitalismo industrial, la propiedad del tiempo y de los movimientos era expropiada al trabajador para pasar a manos del management que, con la aplicación de los principios de la administración científica, se encargaría de codificar, medir, fragmentar todo ese savoir-faire, antes patrimonio exclusivo del artesano, para regresarle un único método, one best way, que incluye los movimientos y las herramientas idóneas con tiempos controlados para la minimización del tiempo y el esfuerzo requeridos en la realización de la tarea. Este hecho representó un cambio organizacional que, inevitablemente, llevó a transformaciones de orden económico, ideológico, social, político, laboral y sindical, ecológico y, por supuesto, en términos de los inicios de la concepción de la administración como ciencia. Si bien los Principios de la administración científica son el retrato de un momento histórico en el que el capitalismo industrial comenzaba a consolidarse, no podemos dejar de reconocer que el principio fundacional se mantiene en la era del capitalismo postindustrial. La lógica del control y de la medición lograría el sueño dorado de toda tecnocracia: la abolición del conflicto y de toda resistencia. Al análisis de las consecuencias de la aparición del taylorismo en diversos ámbitos, dedicamos este trabajo. Finalizamos exponiendo cómo es que hoy, más que nunca, el pensamiento de Taylor cobra vigencia en un sector tan delicado como el de la gestión de las organizaciones públicas según esquemas organizacionales que incentivan la eficiencia por encima de otros valores, como la legitimidad y la eficacia política. Este tipo de acciones, indudablemente, encuentra sus cimientos en el taylorismo.With the introduction of the chronometer in the workroom as a symbolic control device, it was clear that the ownership of the time and people performance was expropiated from workforce to management, towards capitalism consolidation. With the principles of Scientific Administration, the former savoir-faire, once owned by workers, was to be codified, measured, and put into separated pieces. The goal was to take the workers back a unique method, one best way, which includes proper procedures and tools in order to control time for minimizing time and effort applied to achieve specific tasks. All the above led to changes in many different fields, such as economic, ideologies, social, politic, labour and work unions and ecological transformations. These changes impacted the beginning of the concept of Administration as a science. We must recognize that the main principle of the “Scientific Administration” is still the post-industrial capitalism era, but these principles are the portrait of an historical moment in which industrial capitalism began to consolidate itself. Through the logics of control and measurement, technocracy was achieving its golden dream: abolition of conflict and resistance. These pages are dedicated to the consequences of taylorism in different fields. We will finalize showing how Taylor´s thoughts impact today more than ever in a field as special as public organizations Management, under organizacional schemata that lead efficency above other values like legitimity and politic effectiveness.     Extended WKB method, resonances and supersymmetric radial barriers

    Semiclassical approximations are implemented in the calculation of position and width of low energy resonances for radial barriers. The numerical integrations are delimited by t/T<<8, with t the period of a classical particle in the barrier trap and T the resonance lifetime. These energies are used in the construction of `haired' short range potentials as the supersymmetric partners of a given radial barrier.     Diastolic shock index and clinical outcomes in patients with septic shock

    Background: Loss of vascular tone is a key pathophysiological feature of septic shock. Combination of gradual diastolic hypotension and tachycardia could reflect more serious vasodilatory conditions. We sought to evaluate the relationships between heart rate (HR) to diastolic arterial pressure (DAP) ratios and clinical outcomes during early phases of septic shock. Methods: Diastolic shock index (DSI) was defined as the ratio between HR and DAP. DSI calculated just before starting vasopressors (Pre-VPs/DSI) in a preliminary cohort of 337 patients with septic shock (January 2015 to February 2017) and at vasopressor start (VPs/DSI) in 424 patients with septic shock included in a recent randomized controlled trial (ANDROMEDA-SHOCK; March 2017 to April 2018) was partitioned into five quantiles to estimate the relative risks (RR) of death with respect to the mean risk of each population (assumed to be 1). Matched HR and DAP subsamples were created to evaluate the effect of the individual components of the DSI on RRs. In addition, time-course of DSI and interaction between DSI and vasopressor dose (DSI*NE.dose) were compared between survivors and non-survivors from both populations, while ROC curves were used to identify variables predicting mortality. Finally, as exploratory observation, effect of early start of vasopressors was evaluated at each Pre-VPs/DSI quintile from the preliminary cohort. Results: Risk of death progressively increased at gradual increments of Pre-VPs/DSI or VPs/DSI (One-way ANOVA, p < 0.001). Progressive DAP decrease or HR increase was associated with higher mortality risks only when DSI concomitantly increased. Areas under the ROC curve for Pre-VPs/DSI, SOFA and initial lactate were similar, while mean arterial pressure and systolic shock index showed poor performances to predict mortality. Time-course of DSI and DSI*NE.dose was significantly higher in non-survivors from both populations (repeated-measures ANOVA, p < 0.001). Very early start of vasopressors exhibited an apparent benefit at higher Pre-VPs/DSI quintile. Conclusions: DSI at pre-vasopressor and vasopressor start points might represent a very early identifier of patients at high risk of death. Isolated DAP or HR values do not clearly identify such risk. Usefulness of DSI to trigger or to direct therapeutic interventions in early resuscitation of septic shock need to be addressed in future studies

    Too little but not too late: Results of a literature review to improve routine immunization programs in developing countries

    <p>Abstract</p> <p>Background</p> <p>Globally, immunization services have been the center of renewed interest with increased funding to improve services, acceleration of the introduction of new vaccines, and the development of a health systems approach to improve vaccine delivery. Much of the credit for the increased attention is due to the work of the GAVI Alliance and to new funding streams. If routine immunization programs are to take full advantage of the newly available resources, managers need to understand the range of proven strategies and approaches to deliver vaccines to reduce the incidence of diseases. In this paper, we present strategies that may be used at the sub-national level to improve routine immunization programs.</p> <p>Methods</p> <p>We conducted a systematic review of studies and projects reported in the published and gray literature. Each paper that met our inclusion criteria was rated based on methodological rigor and data were systematically abstracted. Routine-immunization – specific papers with a methodological rigor rating of greater than 60% and with conclusive results were reported.</p> <p>Results</p> <p>Greater than 11,000 papers were identified, of which 60 met our inclusion criteria and 25 papers were reported. Papers were grouped into four strategy approaches: bringing immunizations closer to communities (n = 11), using information dissemination to increase demand for vaccination (n = 3), changing practices in fixed sites (n = 4), and using innovative management practices (n = 7).</p> <p>Conclusion</p> <p>Immunization programs are at a historical crossroads in terms of developing new funding streams, introducing new vaccines, and responding to the global interest in the health systems approach to improving immunization delivery. However, to complement this, actual service delivery needs to be strengthened and program managers must be aware of proven strategies. Much was learned from the 25 papers, such as the use of non-health workers to provide numerous services at the community level. However it was startling to see how few papers were identified and in particular how few were of strong scientific quality. Further well-designed and well-conducted scientific research is warranted. Proposed areas of additional research include integration of additional services with immunization delivery, collaboration of immunization programs with new partners, best approaches to new vaccine introduction, and how to improve service delivery.</p

    Effects of very early start of norepinephrine in patients with septic shock: a propensity score-based analysis

    BACKGROUND: Optimal timing for the start of vasopressors (VP) in septic shock has not been widely studied since it is assumed that fluids must be administered in advance. We sought to evaluate whether a very early start of VP, even without completing the initial fluid loading, might impact clinical outcomes in septic shock. METHODS: A total of 337 patients with sepsis requiring VP support for at least 6 h were initially selected from a prospectively collected database in a 90-bed mixed-ICU during a 24-month period. They were classified into very-early (VE-VPs) or delayed vasopressor start (D-VPs) categories according to whether norepinephrine was initiated or not within/before the next hour of the first resuscitative fluid load. Then, VE-VPs (n = 93) patients were 1:1 propensity matched to D-VPs (n = 93) based on age; source of admission (emergency room, general wards, intensive care unit); chronic and acute comorbidities; and lactate, heart rate, systolic, and diastolic pressure at vasopressor start. A risk-adjusted Cox proportional hazard model was fitted to assess the association between VE-VPs and day 28 mortality. Finally, a sensitivity analysis was performed also including those patients requiring VP support for less than 6 h. RESULTS: Patients subjected to VE-VPs received significantly less resuscitation fluids at vasopressor starting (0[0-510] vs. 1500[650-2300] mL, p < 0.001) and during the first 8 h of resuscitation (1100[500-1900] vs. 2600[1600-3800] mL, p < 0.001), with no significant increase in acute renal failure and/or renal replacement therapy requirements. VE-VPs was related with significant lower net fluid balances 8 and 24 h after VPs. VE-VPs was also associated with a significant reduction in the risk of death compared to D-VPs (HR 0.31, CI95% 0.17-0.57, p < 0.001) at day 28. Such association was maintained after including patients receiving vasopressors for < 6 h. CONCLUSION: A very early start of vasopressor support seems to be safe, might limit the amount of fluids to resuscitate septic shock, and could lead to better clinical outcomes

Pero abarcó mucho más: reguló la actividad aseguradora, incursionó en el contrato de seguro, reguló el sistema previsional y delineó el régimen bursátil. Así mismo, anticipó lineamientos de lo que sería, un año después, la consagración de tales actividades -financiera, aseguradora, bursátil y previsional- como de rango constitucional. Este libro recoge las reflexiones de expertos sobre las diversas temáticas abordadas en dicha ley, con la visión enriquecida derivada del transcurso de 20 años, de varias legislaciones adicionales y de algunas crisis nacionales y foráneas. Escrutar en su veintenario es un ejercicio válido y oportuno, tanto más como que al haber renovado los arquetipos de la legislación precedente, sirvió además como referente de empeños de enmienda normativas de otros países de la región y de los organismos multilaterales

    The agricultural Internet of Things (Ag-IoT) paradigm has tremendous potential in transparent integration of underground soil sensing, farm machinery, and sensor-guided irrigation systems with the complex social network of growers, agronomists, crop consultants, and advisors. The aim of the IoT in agricultural innovation and security chapter is to present agricultural IoT research and paradigm to promote sustainable production of safe, healthy, and profitable crop and animal agricultural products. This chapter covers the IoT platform to test optimized management strategies, engage farmer and industry groups, and investigate new and traditional technology drivers that will enhance resilience of the farmers to the socio-environmental changes. A review of state-of-the-art communication architectures and underlying sensing technologies and communication mechanisms is presented with coverage of recent advances in the theory and applications of wireless underground communications. Major challenges in Ag-IoT design and implementation are also discussed

    Can the intake of antiparasitic secondary metabolites explain the low prevalence of hemoparasites among wild Psittaciformes?

    Background: Parasites can exert selection pressure on their hosts through effects on survival, on reproductive success, on sexually selected ornament, with important ecological and evolutionary consequences, such as changes in population viability. Consequently, hemoparasites have become the focus of recent avian studies. Infection varies significantly among taxa. Various factors might explain the differences in infection among taxa, including habitat, climate, host density, the presence of vectors, life history and immune defence. Feeding behaviour can also be relevant both through increased exposure to vectors and consumption of secondary metabolites with preventative or therapeutic effects that can reduce parasite load. However, the latter has been little investigated. Psittaciformes (parrots and cockatoos) are a good model to investigate these topics, as they are known to use biological control against ectoparasites and to feed on toxic food. We investigated the presence of avian malaria parasites (Plasmodium), intracellular haemosporidians (Haemoproteus, Leucocytozoon), unicellular flagellate protozoans (Trypanosoma) and microfilariae in 19 Psittaciformes species from a range of habitats in the Indo-Malayan, Australasian and Neotropical regions. We gathered additional data on hemoparasites in wild Psittaciformes from the literature. We considered factors that may control the presence of hemoparasites in the Psittaciformes, compiling information on diet, habitat, and climate. Furthermore, we investigated the role of diet in providing antiparasitic secondary metabolites that could be used as self-medication to reduce parasite load. Results: We found hemoparasites in only two of 19 species sampled. Among them, all species that consume at least one food item known for its secondary metabolites with antimalarial, trypanocidal or general antiparasitic properties, were free from hemoparasites. In contrast, the infected parrots do not consume food items with antimalarial or even general antiparasitic properties. We found that the two infected species in this study consumed omnivorous diets. When we combined our data with data from studies previously investigating blood parasites in wild parrots, the positive relationship between omnivorous diets and hemoparasite infestation was confirmed. Individuals from open habitats were less infected than those from forests. Conclusions: The consumption of food items known for their secondary metabolites with antimalarial, trypanocidal or general antiparasitic properties, as well as the higher proportion of infected species among omnivorous parrots, could explain the low prevalence of hemoparasites reported in many vertebrates

    HTLV-1 infection in solid organ transplant donors and recipients in Spain

    HTLV-1 infection is a neglected disease, despite infecting 10-15 million people worldwide and severe illnesses develop in 10% of carriers lifelong. Acknowledging a greater risk for developing HTLV-1 associated illnesses due to immunosuppression, screening is being widely considered in the transplantation setting. Herein, we report the experience with universal HTLV testing of donors and recipients of solid organ transplants in a survey conducted in Spain. All hospitals belonging to the Spanish HTLV network were invited to participate in the study. Briefly, HTLV antibody screening was performed retrospectively in all specimens collected from solid organ donors and recipients attended since the year 2008. A total of 5751 individuals were tested for HTLV antibodies at 8 sites. Donors represented 2312 (42.2%), of whom 17 (0.3%) were living kidney donors. The remaining 3439 (59.8%) were recipients. Spaniards represented nearly 80%. Overall, 9 individuals (0.16%) were initially reactive for HTLV antibodies. Six were donors and 3 were recipients. Using confirmatory tests, HTLV-1 could be confirmed in only two donors, one Spaniard and another from Colombia. Both kidneys of the Spaniard were inadvertently transplanted. Subacute myelopathy developed within 1 year in one recipient. The second recipient seroconverted for HTLV-1 but the kidney had to be removed soon due to rejection. Immunosuppression was stopped and 3 years later the patient remains in dialysis but otherwise asymptomatic. The rate of HTLV-1 is low but not negligible in donors/recipients of solid organ transplants in Spain. Universal HTLV screening should be recommended in all donor and recipients of solid organ transplantation in Spain. Evidence is overwhelming for very high virus transmission and increased risk along with the rapid development of subacute myelopathy
