    Identities in Motion: Cyberspace and Myanmar Men having Sex with Men

    This paper explores how Myanmar men-having-sex-with-men (MSM) create identity through lived and mediated realities, by using cyber social networks, 30 in-depth interviews were conducted through snowballing among MSM cyber social network members. Content analysis was guided by a social network analysis framework. Networking positively affected connectedness and social support however norms and values were challenged. Age, experience, sex-roles, and peer-relationships seem to influence self-efficacy.  Interpersonal dynamics were driven by online-cruising, dating, cyber-sex and off-line encounters. In conclusion, access to social networking offers an outlet for socio-cultural and legal oppressed Myanmar MSM, however building life-skills among younger users’ needs attention.     A Review of What is Known about Impacts of Coastal Pollution on Childhood Disabilities and Adverse Pregnancy Outcomes

    Coastal pollution is getting global attention for its enormous impacts on human health by the means of consumptions of seafood, involvement of risky jobs and exposure to water related disastrous events. Critical review of published and unpublished literatures and documents show the effects of heavy metals, chemicals, and radioactive substances are considered as long termed and deadly, and children and pregnant women are the most vulnerable population to such exposures and at risk of adversely affecting their development. The physiological systems of children and fetuses are developing fast and usually sensitive to disruptions induced by environmental pollutants and exposures in utero increase the risk of future toxic insults. Almost sixty percent of the world‘s population is at risk of this contamination and coastal developing countries are facing significant challenges by this form of pollution. The main objective of this review was to explore the situation of coastal water pollution and its impact on child health and pregnancy outcomes globally. However, these observations are indecisive due to limitations of evidence to support. Therefore, further epidemiological studies are required to confirm these initial observations.     Fostering a Paradigm Shift in the Roles of Health Promotion Education in Southeast Asia

    In the context of an evolving domain and the complexity globalization adds to the situation, health promotion practice in Southeast Asia face challenges posed by the growing gaps between practice needs, human resource development needs, and educational program development needs. One of the challenges is how to foster a much needed paradigm shift among those responsible for workforce production in health promotion. In this paper, we provide practical proposals for action that provide leverage in thinking differently about health promotion practice. These proposals reflect the authors’ perspectives and experiences in competencies relevant to health promotion key action areas: empowerment, health services, partnerships and alliances, environments, and health and policy. We first describe the developments in the health promotion domain; summarize competency frameworks for health promotion; to arrive at a comparison of Southeast Asia education programs for health promotion with programs in socio-economic advanced regions. We suggest proposals on the way forward aimed at fueling the required paradigm shift in capacity building for health promotion in Southeast Asia; and conclude by considering the role national and international alliances can play in implementing these proposals and improving workforce production for health promotion in Southeast Asia.     Mass Gatherings and Public Health: Case Studies from the Hajj to Mecca

    Background: Many new and challenging risks can be introduced during mass gatherings. The Hajj, as one of the largest mass gatherings, provides an excellent annual opportunity to reflect on the public health risk posed by international and multicultural crowds and the value of mitigation strategies. Objectives: To identify the gap between preparation and training taken before being exposed to the mass gathering and postexposure experiences, and the breach between the expectations and reality of the holy place. Methods: This was a qualitative study with in-depth interviews using semistructured questionnaires among Hajjis from 4 different countries (Bangladesh, Pakistan, Myanmar, and New Zealand). Purposive sampling was done. The present study was also supported by literature review. Findings: Findings pointed to weaknesses in implementation and enforcement of law, for both the custodian country and countries of origin of Hajjis. Disparities among developed and developing countries were also noticeable. Conclusions: From a global health and human security perspective, strengthening of core capacities in managing mass gatherings as well as researching risks posed by such gatherings are paramount to safeguard the public’s health.     A preliminary study of skin bleaching and factors associated with skin bleaching among women living in Zimbabwe

    Background: Skin bleaching was reported to be commonly practiced among women and Africa was reported to be one of the most affected yet the subject is not given much attention in public health research in Zimbabwe despite the adverse effects of skin bleaching on health. Method: This study was an exploratory cross-sectional survey to explore skin bleaching, skin bleaching patterns and factors associated with skin bleaching among women living in Zimbabwe. An online self-administered questionnaire was sent out to women on social network i.e. WhatsApp, Facebook, LinkedIn and Twitter. Findings: A total number of 260 respondents, mean age 31.69 (SD, 8.12) years participated in the survey. The prevalence of skin bleaching among the participants was 31.15%. The major reason reported for skin bleaching was to have smooth and healthy skin alongside other factors such as beauty, gaining social favours for example getting married and good jobs. Occupation, complexion and marital status were associated with skin bleaching. The odds of skin bleaching for participants who were employed was 1.45(95% confidence interval [CI],0.32-1.91);p-value 0.02, dark skinned participants 2.56(95% CI, 0.76-2.87);p-value 0.01 and unmarried participants 2.87(95% CI,0.29-3.58);p-value 0.03. Conclusion: Evidence from the research suggests skin bleaching might be common among women living in Zimbabwe and possibly poses serious health threats to the women. Skin bleaching seems to be deep rooted in colourism. The colourism seems to be taken advantage of by the cosmetic industry which produce the potentially hazardous products which promise the revered light skin to women but which comes with a price. However, the study provides a base for future studies to explore more on skin bleaching practices among women living in Zimbabwe

    Relation between Red Cell Distribution Width and Fibroblast Growth Factor 23 Cleaving in Patients with Chronic Kidney Disease and Heart Failure

    In chronic kidney disease (CKD), both anemia and deregulated phosphate metabolism are common and predictive of adverse outcome. Previous studies suggest that iron status influences phosphate metabolism by modulating proteolytic cleavage of FGF23 into C-terminal fragments. Red cell distribution width (RDW) was recently identified as a strong prognostic determinant for cardiovascular morbidity and mortality, independently of iron status. We assessed whether RDW is associated with FGF23 cleaving in CKD patients with heart failure.The associations between RDW and either intact FGF23 (iFGF23), C-terminal FGF23 (cFGF23, reflecting iFGF23 and C-terminal fragments together) and the iFGF23/cFGF23 ratio were analyzed in 52 patients with CKD (eGFR 34,9 ± 13.9 ml/min/1.73m2) and chronic heart failure (CHF). Associations between RDW and FGF23 forms were studied by linear regression analysis adjusted for parameters of renal function, iron metabolism, phosphate metabolism and inflammation.Median cFGF23 levels were 197.5 [110-408.5] RU/ml, median iFGF23 levels were 107.3 [65.1-162.2] pg/ml and median FGF23 ratio was 0.80 [0.37-0.86]. Mean RDW was 14.1 ± 1.2%. cFGF23 and RDW were associated (β = 1.63 x 10(-3), P < 0.001), whereas iFGF23 and RDW were not (β = -1.38 x 10(-3), P = 0.336). The iFGF23/cFGF23 ratio was inversely associated with RDW. The difference between cFGF23 and iFGF23 (cFGF23- iFGF23) was positively associated with RDW (β = 1.74 x 10(-3), P < 0.001). The association between cFGF23 and RDW persisted upon multivariable linear regression analysis, adjusted for parameters of renal function, phosphate metabolism, iron metabolism and inflammation (β = 0.97 x 10(-3), P = 0.047).RDW is associated with cFGF23 but not with iFGF23 levels in patients with CKD and CHF. This suggests a connection between RDW and FGF23 catabolism, independent of iron status and inflammation. Future studies are needed to unravel underlying mechanisms and whether these pertain to the link between RDW and outcome

    Recent 210 Pb, 137 Cs and 241 Am accumulation in an ombrotrophic peatland from Amsterdam Island (Southern Indian Ocean)

    Over the past 50 years, 210Pb, 137Cs and 241Am have been abundantly used in reconstructing recent sediment and peat chronologies. The study of global aerosol-climate interaction is also partially depending on our understanding of 222Rn-210Pb cycling, as radionuclides are useful aerosol tracers. However, in comparison with the Northern Hemisphere, few data are available for these radionuclides in the Southern Hemisphere, especially in the South Indian Ocean. A peat core was collected in an ombrotrophic peatland from the remote Amsterdam Island (AMS) and was analyzed for 210Pb, 137Cs and 241Am radionuclides using an underground ultra-low background gamma spectrometer. The 210Pb Constant Rate of Supply (CRS) model of peat accumulations is validated by peaks of artificial radionuclides (137Cs and 241Am) that are related to nuclear weapon tests. We compared the AMS 210Pb data with an updated 210Pb deposition database. The 210Pb flux of 98 ± 6 Bq·m−2·y−1 derived from the AMS core agrees with data from Madagascar and South Africa. The elevated flux observed at such a remote location may result from the enhanced 222Rn activity and frequent rainfall in AMS. This enhanced 222Rn activity itself may be explained by continental air masses passing over southern Africa and/or Madagascar. The 210Pb flux at AMS is higher than those derived from cores collected in coastal areas in Argentina and Chile, which are areas dominated by marine westerly winds with low 222Rn activities. We report a 137Cs inventory at AMS of 144 ± 13 Bq·m−2 (corrected to 1969). Our data thus contribute to the under-represented data coverage in the mid-latitudes of the Southern Hemisphere

    Soil networks become more connected and take up more carbon as nature restoration progresses

    Soil organisms have an important role in aboveground community dynamics and ecosystem functioning in terrestrial ecosystems. However, most studies have considered soil biota as a black box or focussed on specific groups, whereas little is known about entire soil networks. Here we show that during the course of nature restoration on abandoned arable land a compositional shift in soil biota, preceded by tightening of the belowground networks, corresponds with enhanced efficiency of carbon uptake. In mid- and long-term abandoned field soil, carbon uptake by fungi increases without an increase in fungal biomass or shift in bacterial-to-fungal ratio. The implication of our findings is that during nature restoration the efficiency of nutrient cycling and carbon uptake can increase by a shift in fungal composition and/or fungal activity. Therefore, we propose that relationships between soil food web structure and carbon cycling in soils need to be reconsidered

    Preoperative risk stratification in endometrial cancer (ENDORISK) by a Bayesian network model: A development and validation study

    Background: Bayesian networks (BNs) are machine-learning-based computational models that visualize causal relationships and provide insight into the processes underlying disease progression, closely resembling clinical decision-making. Preoperative identification of patients at risk for lymph node metastasis (LNM) is challenging in endometrial cancer, and although several biomarkers are related to LNM, none of them are incorporated in clinical practice. The aim of this study was to develop and externally validate a preoperative BN to predict LNM and outcome in endometrial cancer patients.Methods and findings: Within the European Network for Individualized Treatment of Endometrial Cancer (ENI-TEC), we performed a retrospective multicenter cohort study including 763 patients, median age 65 years (interquartile range [IQR] 58-71), surgically treated for endometrial cancer between February 1995 and August 2013 at one of the 10 participating European hospitals. A BN was developed using score-based machine learning in addition to expert knowledge. Our main outcome measures were LNM and 5-year disease-specific survival (DSS). Preoperative clinical, histopathological, and molecular biomarkers were included in the network. External validation was performed using 2 prospective study cohorts: the Molecular Markers in Treatment in Endometrial Cancer (MoMaTEC) study cohort, including 446 Norwegian patients, median age 64 years (IQR 59-74), treated between May 2001 and 2010; and the PIpelle Prospective ENDOmetrial carcinoma (PIPENDO) study cohort, including 384 Dutch patients, median age 66 years (IQR 60-73), treated between September 2011 and December 2013. A BN called ENDORISK (preoperative risk stratification in endometrial cancer) was developed including the following predictors: preoperative tumor grade; immunohistochemical expression of estrogen receptor (ER), progesterone receptor (PR), p53, and L1 cell adhesion molecule (L1CAM); cancer antigen 125 serum level; thrombocyte count; imaging results on lymphadenopathy; and cervical cytology. In the MoMaTEC cohort, the area under the curve (AUC) was 0.82 (95% confidence interval [CI] 0.76-0.88) for LNM and 0.82 (95% CI 0.77-0.87) for 5-year DSS. In the PIPENDO cohort, the AUC for 5-year DSS was 0.84 (95% CI 0.78-0.90). The network was well-calibrated. In the MoMaTEC cohort, 249 patients (55.8%) were classified with Conclusions: In this study, we illustrated how BNs can be used for individualizing clinical decision-making in oncology by incorporating easily accessible and multimodal biomarkers. The network shows the complex interactions underlying the carcinogenetic process of endometrial cancer by its graphical representation. A prospective feasibility study will be needed prior to implementation in the clinic.</div
