    Nutritional strategies for post-exercise recovery: a review

    Finding the optimal nutrition regimen for enhanced recovery is fundamental in enhancing exercise training and performance. Therefore, research has aimed to examine post-exercise nutritional strategies for optimal recovery. Because muscle glycogen is the primary substrate utilised during high-intensity exercise, it must be replenished. Recent research has examined the effectiveness on recovery of adding protein to a post-exercise carbohydrate beverage. This review summarises and analyses the literature on nutritional strategies aimed at enhancing various indicators of post-exercise recovery: glycogen resynthesis, muscle damage and performance. Furthermore, the literature on Medline and Pubmed comparing the effectiveness of carbohydrate-only (CHO) beverage with a carbohydrate:protein (CHO:PRO) beverage on maximising recovery was reviewed. The methods and results of studies regarding post-exercise nutritional strategies for recovery were analysed. primary results of this review suggest that the optimal timing in regard to post-exercise nutritional strategies for maximal glycogen resynthesis is within the first 30 minutes after exercise. The literature suggests that 1.0 - 1.5 g.kg-1h-1 of carbohydrate ingested at 2-hour intervals after exercise for up to 6 hours may be optimal for recovery. The addition of protein to a post-exercise meal may supply additional amino acids necessary for muscle repair creating an anabolic condition

    Heat storage in upper and lower body during high-intensity exercise in athletes with spinal cord injuries

    Background: The thermophysiology of athletes with spinal cord injuries (SCI) is not well understood. Spinal cord lesions impact muscle mass, thermoregulatory neural signals and circulatory function. Understanding SCI thermoregulation physiology would benefit exercise function. Therefore, this study was designed to describe heat storage in the upper and lower bodies of SCI and able-bodied (AB) athletes. Procedure: Seven SCI and 8 AB athletes (matched for arm-crank VO2 peak) performed a ramp protocol in an environment similar to an indoor competitive environment (21˚C±1.5˚C, 55±3% relative humidity).Results: SCI athletes experienced similar upper-body heat storage of 0.82±0.59 J.g-1 and lower-body heat storage of 0.47±0.33 J.g-1 compared with that of AB athletes at 0.80±0.61 J.g-1 and 0.27±0.22 J.g-1 for upper and lower body, respectively. There were no significant differences between groups for rectal temperature (Trec) or oesophageal temperature (Tes). However, mean skin temperature (Msk) was significantly higher for SCI throughout the exercise bout (p=0.006). Conclusions: The results of this study suggest that SCI and AB athletes appear to thermoregulate in a similar manner, though SCI tend to store slightly more heat

    A Novel Pathogenic Mechanism of Highly Pathogenic Avian Influenza H5N1 Viruses Involves Hemagglutinin Mediated Resistance to Serum Innate Inhibitors

    In this study, the effect of innate serum inhibitors on influenza virus infection was addressed. Seasonal influenza A(H1N1) and A(H3N2), 2009 pandemic A(H1N1) (H1N1pdm) and highly pathogenic avian influenza (HPAI) A(H5N1) viruses were tested with guinea pig sera negative for antibodies against all of these viruses as evaluated by hemagglutination-inhibition and microneutralization assays. In the presence of serum inhibitors, the infection by each virus was inhibited differently as measured by the amount of viral nucleoprotein produced in Madin-Darby canine kidney cells. The serum inhibitors inhibited seasonal influenza A(H3N2) virus the most, while the effect was less in seasonal influenza A(H1N1) and H1N1pdm viruses. The suppression by serum inhibitors could be reduced by heat inactivation or treatment with receptor destroying enzyme. In contrast, all H5N1 strains tested were resistant to serum inhibitors. To determine which structure (hemagglutinin (HA) and/or neuraminidase (NA)) on the virus particles that provided the resistance, reverse genetics (rg) was applied to construct chimeric recombinant viruses from A/Puerto Rico/8/1934(H1N1) (PR8) plasmid vectors. rgPR8-H5 HA and rgPR8-H5 HANA were resistant to serum inhibitors while rgPR8-H5 NA and PR8 A(H1N1) parental viruses were sensitive, suggesting that HA of HPAI H5N1 viruses bestowed viral resistance to serum inhibition. These results suggested that the ability to resist serum inhibition might enable the viremic H5N1 viruses to disseminate to distal end organs. The present study also analyzed for correlation between susceptibility to serum inhibitors and number of glycosylation sites present on the globular heads of HA and NA. H3N2 viruses, the subtype with highest susceptibility to serum inhibitors, harbored the highest number of glycosylation sites on the HA globular head. However, this positive correlation cannot be drawn for the other influenza subtypes

    Practical nutritional recovery strategies for elite soccer players when limited time separates repeated matches

    Specific guidelines that aim to facilitate the recovery of soccer players from the demands of training and a congested fixture schedule are lacking; especially in relation to evidence-based nutritional recommendations. The importance of repeated high level performance and injury avoidance while addressing the challenges of fixture scheduling, travel to away venues, and training commitments requires a strategic and practically feasible method of implementing specific nutritional strategies. Here we present evidence-based guidelines regarding nutritional recovery strategies within the context of soccer. An emphasis is placed on providing practically applicable guidelines for facilitation of recovery when multiple matches are played within a short period of time (i.e. 48 h). Following match-play, the restoration of liver and muscle glycogen stores (via consumption of ~1.2 gkg-1h-1 of carbohydrate) and augmentation of protein synthesis (via ~40 g of protein) should be prioritised in the first 20 minutes of recovery. Daily intakes of 6-10 gkg-1 body mass of carbohydrate are recommended when limited time separates repeated matches while daily protein intakes of >1.5 gkg-1 body mass should be targeted; possibly in the form of multiple smaller feedings (e.g., 6 x 20-40 g). At least 150% of the body mass lost during exercise should be consumed within 1 h and electrolytes added such that fluid losses are ameliorated. Strategic use of protein, leucine, creatine, polyphenols and omega-3 supplements could also offer practical means of enhancing post-match recovery.     Postpartum psychiatric disorders

    Pregnancy is a complex and vulnerable period that presents a number of challenges to women, including the development of postpartum psychiatric disorders (PPDs). These disorders can include postpartum depression and anxiety, which are relatively common, and the rare but more severe postpartum psychosis. In addition, other PPDs can include obsessive–compulsive disorder, post-traumatic stress disorder and eating disorders. The aetiology of PPDs is a complex interaction of psychological, social and biological factors, in addition to genetic and environmental factors. The goals of treating postpartum mental illness are reducing maternal symptoms and supporting maternal–child and family functioning. Women and their families should receive psychoeducation about the illness, including evidence-based discussions about the risks and benefits of each treatment option. Developing effective strategies in global settings that allow the delivery of targeted therapies to women with different clinical phenotypes and severities of PPDs is essential

    A short history of the 5-HT2C receptor: from the choroid plexus to depression, obesity and addiction treatment

    This paper is a personal account on the discovery and characterization of the 5-HT2C receptor (first known as the 5- HT1C receptor) over 30 years ago and how it translated into a number of unsuspected features for a G protein-coupled receptor (GPCR) and a diversity of clinical applications. The 5-HT2C receptor is one of the most intriguing members of the GPCR superfamily. Initially referred to as 5-HT1CR, the 5-HT2CR was discovered while studying the pharmacological features and the distribution of [3H]mesulergine-labelled sites, primarily in the brain using radioligand binding and slice autoradiography. Mesulergine (SDZ CU-085), was, at the time, best defined as a ligand with serotonergic and dopaminergic properties. Autoradiographic studies showed remarkably strong [3H]mesulergine-labelling to the rat choroid plexus. [3H]mesulergine-labelled sites had pharmacological properties different from, at the time, known or purported 5-HT receptors. In spite of similarities with 5-HT2 binding, the new binding site was called 5-HT1C because of its very high affinity for 5-HT itself. Within the following 10 years, the 5-HT1CR (later named 5- HT2C) was extensively characterised pharmacologically, anatomically and functionally: it was one of the first 5-HT receptors to be sequenced and cloned. The 5-HT2CR is a GPCR, with a very complex gene structure. It constitutes a rarity in theGPCR family: many 5-HT2CR variants exist, especially in humans, due to RNA editing, in addition to a few 5-HT2CR splice variants. Intense research led to therapeutically active 5-HT2C receptor ligands, both antagonists (or inverse agonists) and agonists: keeping in mind that a number of antidepressants and antipsychotics are 5- HT2CR antagonists/inverse agonists. Agomelatine, a 5-HT2CR antagonist is registered for the treatment of major depression. The agonist Lorcaserin is registered for the treatment of aspects of obesity and has further potential in addiction, especially nicotine/ smoking. There is good evidence that the 5-HT2CR is involved in spinal cord injury-induced spasms of the lower limbs, which can be treated with 5-HT2CR antagonists/inverse agonists such as cyproheptadine or SB206553. The 5-HT2CR may play a role in schizophrenia and epilepsy. Vabicaserin, a 5-HT2CR agonist has been in development for the treatment of schizophrenia and obesity, but was stopped. As is common, there is potential for further indications for 5-HT2CR ligands, as suggested by a number of preclinical and/or genome-wide association studies (GWAS) on depression, suicide, sexual dysfunction, addictions and obesity. The 5-HT2CR is clearly affected by a number of established antidepressants/antipsychotics and may be one of the culprits in antipsychotic-induced weight gain

    FUELING STRATEGIES IN ULTRA TRAIL RUNNERS

Perez, C Metzler, K Henninger, K.L Pritchett, N Kumar, R.C Pritchett Central Washington University, Ellensburg It has been suggested that ultra-runners may fail to meet hourly carbohydrate intake guidelines during training and competition due to improper knowledge around fueling, higher reliance on fats as fuel, lack of dietary strategies, and gastrointestinal stress (Stellingwerff 2016, Costa 2013). However; the consequences could include poor performances as well as an increase the risk of low energy availability (LEA) in ultra-endurance athletes. PURPOSE: The purpose of this study was to examine self-reported fueling habits during training and competition in male and female ultra-trail runners. METHODS: Trail runners between the age of 18-40 (males: n= 510) completed a Qualtrics survey that included training and racing characteristics. Questions regarding carbohydrate intake during training and competition were compared to current evidence-based recommendations. RESULTS: 45.6% male and 47.6% female reported incorporating fueling with carbohydrates during their workouts. 47.5% of males and 45.2% only sometimes incorporate carbohydrate fueling into their workouts. 7.1% of female athletes and 6.9% of male athletes do not incorporate any Carbohydrates into their workouts. 25% of male ultra-trail runners intentionally eat less during their easy training days and 36% of female’s athletes. However, 35.9% of males intentionally train in a fasted state and only 19.7 % of females train in a fasted state. Furthermore, 36.2% of female ultra-trail runners intentionally eat less on their easy days compared to only 25.7% of male ultra-trail runners intentionally restrict fueling during their easy days. CONCLUSION: Athletes should seek guidance from a sports dietitian to help with fueling plans that meet carbohydrate needs during training runs and races. The guidance should be focused on both male and female athletes
