    A Co-moving Coordinate System for Relativistic Hydrodynamics

    The equations of relativistic hydrodynamics are transformed so that steps forward in time preserves local simultaneity. In these variables, the space-time coordinates of neighboring points on the mesh are simultaneous according to co-moving observers. Aside from the time step varying as a function of the location on the mesh, the local velocity gradient and the local density then evolve according to non-relativistic equations of motion. Analytic solutions are found for two one-dimensional cases with constant speed of sound. One solution has a Gaussian density profile when mapped into the new coordinates. That solution is analyzed for the effects of longitudinal acceleration in relativistic heavy ion collisions at RHIC, especially in regards to two-particle correlation measurements of the longitudinal size

    Pacific abyssal transport and mixing: Through the Samoan Passage versus around the Manihiki Plateau

The definitive version was published in Journal of Physical Oceanography 49(6), (2019): 1577-1592, doi:10.1175/JPO-D-18-0124.1.The main source feeding the abyssal circulation of the North Pacific is the deep, northward flow of 5–6 Sverdrups (Sv; 1 Sv ≡ 106 m3 s−1) through the Samoan Passage. A recent field campaign has shown that this flow is hydraulically controlled and that it experiences hydraulic jumps accompanied by strong mixing and dissipation concentrated near several deep sills. By our estimates, the diapycnal density flux associated with this mixing is considerably larger than the diapycnal flux across a typical isopycnal surface extending over the abyssal North Pacific. According to historical hydrographic observations, a second source of abyssal water for the North Pacific is 2.3–2.8 Sv of the dense flow that is diverted around the Manihiki Plateau to the east, bypassing the Samoan Passage. This bypass flow is not confined to a channel and is therefore less likely to experience the strong mixing that is associated with hydraulic transitions. The partitioning of flux between the two branches of the deep flow could therefore be relevant to the distribution of Pacific abyssal mixing. To gain insight into the factors that control the partitioning between these two branches, we develop an abyssal and equator-proximal extension of the “island rule.” Novel features include provisions for the presence of hydraulic jumps as well as identification of an appropriate integration circuit for an abyssal layer to the east of the island. Evaluation of the corresponding circulation integral leads to a prediction of 0.4–2.4 Sv of bypass flow.     Uptake of Direct Acting Antivirals for Hepatitis C Virus in a New England Medicaid Population, 2014-2017

    Introduction Introduction of the direct acting antiviral (DAA) sofosbuvir (SOV) in 2013 offered significant improvement over previous options for hepatitis C virus (HCV) treatment. Initial uptake was low in Medicaid and other populations, perhaps in part due to high drug cost and prior authorization (PA) restrictions related to fibrosis stage, prescribing provider specialty, and sobriety. Both the subsequent introduction of ledipasvir/sofosbuvir (LDV/SOV), an all-oral regimen for most genotypes, and lifting of PA restrictions were expected to increase overall uptake, but little is known about recent prescribing patterns. We examined trends in DAA uptake in a Medicaid population and identified the effect of these two events on treatment initiation. Study Design An interrupted time series (ITS) design utilized enrollment, medical, and pharmacy claims from Medicaid enrollees in three New England states, 12/2013-12/2017. Trends in treatment uptake, defined as 1+ pharmacy claim for a DAA, were examined overall, by demographic characteristics, and prior to and after two time points: 10/2014 (LDV/SOV approval date) and 7/2016 (date PA restrictions affecting two-thirds of members were lifted). Chi-square evaluated demographic differences, segmented regression models examined trends. Study Population The population included members ages 18-64 years with HCV (2+ claims with ICD-9/10 code for HCV or 1+ claim for chronic HCV). Eligible individuals remained in the sample until treatment initiation or Medicaid disenrollment. Findings The analytic sample averaged 30,433 members with HCV per month, mean age 42.9 years, 60% male. In 2014 3.3% of eligible members initiated treatment, increasing to 7.7% in 2017 (p = Conclusion While initial uptake of DAAs was low in this multi-state Medicaid population, treatment initiation among eligible members increased through 2017. Introduction of new medications and lifting of PA restrictions led to an immediate increase in uptake followed by relatively flat monthly utilization. Policy implications Sharp increases in uptake after LDV/SOV introduction may indicate warehousing of members in anticipation of LDV/SOV approval; increases after PA restrictions were lifted indicates demand for treatment among those affected by restrictions. As a large percentage of the Medicaid HCV population remains untreated, planned provider interviews will help to understand barriers and facilitators of treatment for HCV

    A Cross-Sectional Study Investigating the Relationship Between Alexithymia and Suicide, Violence, and Dual Harm in Male Prisoners

One suggested risk factor for both behaviors is alexithymia. Alexithymia describes a deficit in identifying and describing feelings and is also related to externally oriented thinking. This study aimed to explore the relationship between alexithymia, suicide, violence and dual harm in male prisoners. Methods: Eighty male prisoners were recruited from three prisons. Participants were asked to complete a battery of questionnaires including measures of alexithymia (TAS-20), suicide ideation (ASIQ), suicide behavior, violence ideation (SIV), violence behavior, depression (BDI-II), hopelessness (BHS), impulsivity (DII) and anger (NAS-PI). Regression analyses and ANOVAS were conducted to assess the association between alexithymia (and its subcomponents) with six outcomes; suicide ideation, suicide behavior, violence ideation, violence behavior, dual harm ideation and dual harm behavior. Results: Alexithymia was a univariate predictor of suicide ideation, though was not a significant predictor when considered in a multivariate model. Alexithymia was a significant multivariate predictor of suicide behavior. Alexithymia was not a significant multivariate predictor of violence ideation or behavior. There were no significant differences in alexithymia or subscales between those with suicide ideation/behavior alone, violence ideation/behavior alone and those with dual harm ideation/behavior. Conclusion: In male prisoners, alexithymia appears an important univariate predictor of suicide and violence, though the current study suggests no significant contribution above other well-known correlates of suicide and violence

    The large-scale shock in the cluster of galaxies Hydra A

    We analyzed a deep XMM-Newton observation of the cluster of galaxies Hydra A, focusing on the large-scale shock discovered as a surface brightness discontinuity in Chandra images. The shock front can be seen both in the pressure map and in temperature profiles in several sectors. The Mach numbers determined from the temperature jumps are in good agreement with the Mach numbers derived from EPIC/pn surface brightness profiles and previously from Chandra data and are consistent with M~1.3. The estimated shock age in the different sectors using a spherically symmetric point explosion model ranges between 130 and 230 Myr and the outburst energy between 1.5 and 3e61 ergs. The shape of the shock seen in the pressure map can be approximated with an ellipse centered 70 kpc towards the NE from the cluster center. We aimed to develop a better model that can explain the offset between the shock center and the AGN and give a consistent result on the shock age and energy. To this end, we performed 3D hydrodynamical simulations in which the shock is produced by a symmetrical pair of AGN jets launched in a spherical galaxy cluster. As an explanation of the observed offset of the shock center, we consider large-scale bulk flows in the intracluster medium. The simulation successfully reproduces the size, ellipticity, and average Mach number of the observed shock front. The predicted age of the shock is 160 Myr and the total input energy 3e61 erg. Both values are within the range determined by the spherically symmetric model. Matching the observed 70 kpc offset of the shock ellipse from the cluster center requires large-scale coherent motions with a high velocity of 670 km/s.     Patients presenting with miliaria while wearing flame resistant clothing in high ambient temperatures: a case series

    <p>Abstract</p> <p>Introduction</p> <p>Clothing can be a cause of occupational dermatitis. Frequent causes of clothing-related dermatological problems can be the fabric itself and/or chemical additives used in the laundering process, friction from certain fabrics excessively rubbing the skin, or heat retention from perspiration-soaked clothing in hot working environments. To the best of our knowledge, these are the first reported cases of miliaria rubra associated with prolonged use of flame resistant clothing in the medical literature.</p> <p>Case presentation</p> <p>We report 18 cases (14 men and 4 women, with an age range of 19 to 37 years) of moderate to severe skin irritation associated with wearing flame resistant clothing in hot arid environments (temperature range: 39 to 50°C, 5% to 25% relative humidity). We describe the medical history in detail of a 23-year-old Caucasian woman and a 31-year-old African-American man. A summary of the other 16 patients is also provided.</p> <p>Conclusions</p> <p>These cases illustrate the potential serious nature of miliaria with superimposed <it>Staphylococcus </it>infections. All 18 patients fully recovered with topical skin treatment and modifications to their dress ensemble. Clothing, in particular blend fabrics, must be thoroughly laundered to adequately remove detergent residue. While in hot environments, individuals with sensitive skin should take the necessary precautions such as regular changing of clothing and good personal hygiene to ensure that their skin remains as dry and clean as possible. It is also important that they report to their health care provider as soon as skin irritation or rash appears to initiate any necessary medical procedures. Miliaria rubra can take a week or longer to clear, so removal of exposure to certain fabric types may be necessary.</p

    Co-production of the quality of patient-centered outcomes research partnerships instrument for people with mental health conditions

    Mounting scientific evidence over the past decades in the field of psychiatry has shown community engagement in research produces more relevant research, increased uptake of research findings, and better clinical outcomes. Despite the need for the integration of community engagement methodologies into the scientific method, doctoral and master\u27s level competencies in the field of psychiatry commonly do not include dedicated training or coursework on community engagement methodologies. Without appropriate training or research experience, attempts to facilitate community engagement are often ineffective and burdensome and leave stakeholders feeling disenfranchised. The goal of this study was to co-produce an instrument designed to improve the quality of community engagement research practices by measuring the degree to which researchers have partnered with psychiatric patient stakeholders. The development of the Quality of Patient-Centered Outcomes Research Partnerships Instrument included an iterative co-production process with psychiatric patient stakeholders and scientists, including item formulation, followed by two phases of cognitive interviews with psychiatric patient stakeholders to assess and refine instrument items. A pilot study was conducted to assess acceptability and feasibility. The pilot study of the Patient-Centered Outcomes Research Partnerships Instrument suggested feasibility and acceptability among psychiatric patient stakeholders. The Quality of Patient-Centered Outcomes Research Partnerships Instrument may be a valuable tool to enhance the quality of community engagement research practices within the field of psychiatry.     The context, influences and challenges for undergraduate nurse clinical education: Continuing the dialogue

    Introduction – Approaches to clinical education are highly diverse and becoming increasingly complex to sustain in complex milieu Objective – To identify the influences and challenges of providing nurse clinical education in the undergraduate setting and to illustrate emerging solutions. Method: A discursive exploration into the broad and varied body of evidence including peer reviewed and grey literature. Discussion - Internationally, enabling undergraduate clinical learning opportunities faces a range of challenges. These can be illustrated under two broad themes: (1) Legacies from the past and the inherent features of nurse education and (2) Challenges of the present, including, population changes, workforce changes, and the disconnection between the health and education sectors. Responses to these challenges are triggering the emergence of novel approaches, such as collaborative models.
