    HLB e a psila-africana-dos citrinos: um novo problema e novas soluções

    Os citrinos são uma cultura importante na Península Ibérica, fazendo parte da dieta mediterrânica e contribuindo para a economia dos dois países. Atualmente a citricultura desta região está ameaçada por uma doença devastadora, o huanglongbing, uma vez que a psila-africana-dos-citrinos, vetor da bactéria que causa esta doença, já se encontra em quase toda a região costeira de Portugal e também no norte de Espanha. A bactéria não foi até agora detetada, mas para travar o vetor, tem sido usada a luta biológica com um parasitóide da psila. A gravidade desta ameaça à nossa citricultura obriga a que se adotem novas abordagens de proteção fitossanitária, incluindo estratégias coletivas, abrangendo várias explorações agrícolas.info:eu-repo/semantics/publishedVersio

    Dissertação de Mestrado em Segurança Alimentar apresentada à Faculdade de FarmáciaUma das estratégias no combate a doenças cardiovasculares passa pela introdução de frutas, como a framboesa na alimentação diária. No entanto sendo a framboesa, uma fruta perecível, frágil e sazonal, é preciso arranjar métodos de consumo acessíveis, em qualquer mercado mantendo os seus valores nutricionais. Estudos in vitro e in vivo revelaram vários mecanismos através dos quais antocianinas e elagitaninos presentes em extratos de framboesa vermelha (ou a fruta inteira) poderiam reduzir o risco ou até mesmo reverter pato fisiologias metabolicamente associadas.Os compostos fenólicos são compostos bioativos que possuem propriedades antioxidantes e anti-inflamatórias. Têm sido referidos pelo efeito inibitório no desenvolvimento de doenças crónicas. Um consumo de polifenóis por um longo período, pode contribuir para efeitos benéficos na saúde como a prevenção de distúrbios inflamatórios, doenças cardiovasculares ou efeitos protetores para reduzir o risco de vários tipos de cancro, ou seja, doenças não transmissíveis que surgem de um organismo inflamado, onde se gera stresse oxidativo, suscetível de promover a libertação de mediadores inflamatórios e de espécies reativas adicionais, capaz de amplificar os efeitos pró inflamatórios.A capacidade antioxidante das framboesas, à semelhança de outros frutos vermelhos, está relacionada com vários compostos bioativos que possuem propriedades antioxidantes e que contribuem para a redução de espécies reativas de oxigénio, no entanto a biodisponibilidade dos mesmos ainda não está totalmente descoberta.Embora existam várias evidências para o benefício desses alimentos, ainda existe a necessidade de estudos mais controlados de longo prazo para se ter uma clareza melhor.One of the strategies in cardiovascular diseases is the introduction of fruits, such as raspberries in the daily diet. However, being a raspberry, a perishable, fragile and seasonal fruit, it is necessary to find methods of consumption, in any market, maintaining its nutritional values. In vitro and in vivo studies have revealed several mechanisms through which anthocyanins and ellagitannins present in red raspberry extracts (or the whole fruit) reduce the risk or even reverse metabolically associated duck physiologies.Phenolic compounds are bioactive compounds that have antioxidant and anti-inflammatory properties. They have been concluded by the inhibitory effect on the development of chronic diseases. A long-term consumption of polyphenols can contribute to beneficial health effects such as the prevention of inflammatory disorders, cardiovascular diseases or protective effects to reduce the risk of various types of cancer, that is, non-communicable diseases that arise from an inflamed organism, where oxidative stress is generated, capable of promoting the release of inflammatory mediators and additional reactive species, capable of amplifying the pro-inflammatory effects.The antioxidant capacity of raspberries, like other red fruits, is related to several bioactive compounds that have antioxidant properties and that contribute to the reduction of reactive oxygen species, however their bioavailability is not yet fully discovered.Although there is a lot of evidence for the benefit of these foods, there is still a need for more controlled, long-term studies to have better clarity

    Severe bradycardia and asystole associated with sugammadex: case report

    Background and objectives: Sugamadex is a modified gamma-cyclodextrin, the first selective agent for reversal of neuromuscular blockade induced by steroidal non-depolarizing muscle relaxants, with greater affinity for rocuronium. In this article we present a case of severe bradycardia and asystole following sugammadex administration. Case report: A 54-year-old male patient, ASA II, with a history of hypertension, dyslipidemia and obesity, who underwent an emergency umbilical herniorrhaphy under balanced general anesthesia. Intraoperative muscle relaxation was maintained with rocuronium. At the end of the surgery, the patient maintained a neuromuscular block with two TOF responses, and sugammadex (200 mg) was administered. About thirty seconds after its administration, the patient developed marked bradycardia (HR 30 bpm) followed by asystole. Conclusions: Documented bradycardia and asystole were attributed to the administration of sugammadex. This case shows that, although rare, cardiac arrest is a possible adverse effect of this drug, and that the knowledge of this situation can be determinant for the patient's evolution. Resumo: Justificativa e objetivos: O sugamadex é uma gama ciclodextrina modificada, o primeiro agente seletivo para reversão do bloqueio neuromuscular induzido pelos relaxantes musculares não despolarizantes do tipo esteroide, com maior afinidade para o rocurônio. Neste artigo apresentamos um caso de bradicardia grave e assistolia após administração de sugamadex. Relato do caso: Doente do sexo masculino, 54 anos, ASA II, com antecedentes pessoais de hipertensão, dislipidemia e obesidade, submetido à herniorrafia umbilical em regime de urgência sob anestesia geral balanceada. O relaxamento muscular intraoperatório foi mantido com rocurônio. No fim da cirurgia o doente mantinha bloqueio neuromuscular com duas respostas no TOF, pelo que foi administrado sugamadex 200 mg. Cerca de trinta segundos após a sua administração, o doente desenvolveu bradicardia acentuada (FC 30 bpm) seguida de período de assistolia. Conclusões: A bradicardia e a assistolia documentadas foram atribuídas à administração do sugamadex. Este caso evidencia que, apesar de rara, a parada cardíaca é um efeito adverso possível desse fármaco e que o conhecimento dessa situação pode ser determinante para a evolução do doente.     Intraoperative transfusion practices in Europe

    Transfusion of allogeneic blood influences outcome after surgery. Despite widespread availability of transfusion guidelines, transfusion practices might vary among physicians, departments, hospitals and countries. Our aim was to determine the amount of packed red blood cells (pRBC) and blood products transfused intraoperatively, and to describe factors determining transfusion throughout Europe. We did a prospective observational cohort study enrolling 5803 patients in 126 European centres that received at least one pRBC unit intraoperatively, during a continuous three month period in 2013. The overall intraoperative transfusion rate was 1.8%; 59% of transfusions were at least partially initiated as a result of a physiological transfusion trigger- mostly because of hypotension (55.4%) and/or tachycardia (30.7%). Haemoglobin (Hb)- based transfusion trigger alone initiated only 8.5% of transfusions. The Hb concentration [mean (sd)] just before transfusion was 8.1 (1.7) g dl and increased to 9.8 (1.8) g dl after transfusion. The mean number of intraoperatively transfused pRBC units was 2.5 (2.7) units (median 2). Although European Society of Anaesthesiology transfusion guidelines are moderately implemented in Europe with respect to Hb threshold for transfusion (7-9 g dl), there is still an urgent need for further educational efforts that focus on the number of pRBC units to be transfused at this threshold
