Is Mirror Therapy an Effective Treatment for Reducing Pain Associated with Phantom Limb Syndrome in Unilateral Amputees?

OBJECTIVE: The objective of this selective EBM review is to determine if “Mirror therapy is an effective treatment for reducing pain associated with phantom limb syndrome in unilateral amputees.” STUDY DESIGN: Review of three randomized controlled trials published between 2017 and 2018, with selection based on patient-oriented outcomes and contributing to development of an answer to the clinical question. DATA SOURCES: All three randomized controlled trials were found using searches within PubMed, published in English in peer-reviewed journals. OUTCOMES MEASURED: Each randomized controlled trial assessed changes in severity of pain using a survey known as the visual analog scale, where patients reported their pain being between zero, which means no pain at all, up to 10, the most intense pain they have ever felt. RESULTS: Both Finn et al. (Front Neurol. 2017;8:267. doi:10.3389/fneur.2017.00267) and Ramadugu et al. (Indian J Psychiatry. 2017;59(4):457-464. doi:10.4103/psychiatry.IndianJPsychiatry_259_16) demonstrated that mirror therapy for 15 minutes daily for four weeks reduced both severity of pain and daily pain time in phantom limb syndrome patients versus controls. Ol et al. (Scand J Pain. 2018;18(4):603-610. doi:10.1515/sjpain-2018-0042) concluded that mirror therapy for 10 minutes daily, especially when utilized in addition to other methods like tactile therapy, produced more than a 50% decrease in visual analog scale scores measuring severity of pain associated with phantom limb syndrome. CONCLUSIONS: Mirror therapy has been shown across multiple randomized controlled trials to be effective in reducing both pain severity and duration of pain episodes associated with unilateral amputees who report having phantom limb syndrome

Increased blood lacate levels: an important warning signal in surgical practice

Both in emergency and elective surgical patients increased blood lactate levels warn the physician that the patient is at risk of increased morbidity and decreased changes of survival. Prompt therapeutic measures to restore the balance between oxygen demand and supply are warranted in these patients

Climate readiness indicators for agriculture

Countries vary in their institutional technical and financial abilities to prepare for climate change in agriculture and to balance food security, adaptation, and mitigation goals.Indicators for climate readiness provide guidance to countries and enable monitoring progress. Readiness assessments can enable donors, investors and national decision-makers to identify where investments are needed or likely to be successful. Examples of climate readiness indicators are provided for five work areas: 1. governance and stakeholder engagement, 2. knowledge and information services, 3. climate-smart agricultural strategy and implementation frameworks, 4. national and subnational capabilities and 5. national information and accounting systems

O PODER PREDITIVO DOS MODELOS COM APRENDIZADO DE MÁQUINA É SUPERIOR AOS MODELOS TRADICIONAIS PARA ANÁLISE DO RISCO DE CRÉDITO?

O objetivo desse trabalho foi desenvolver e analisar, para o risco de crédito, se modelos com uso de aprendizado de máquina apresentam melhor poder preditivo comparado aos tradicionais e aplicar técnicas de interpretabilidade ao de melhor performance. A metodologia adotada corresponde a pesquisa empírica econométrica com o uso das técnicas de aprendizado supervisionado. O público-alvo foram empresas do segmento atacado. Para as variáveis do modelo foram utilizados indicadores econômicos e financeiros, retirados das demonstrações contábeis das empresas, e variáveis macroeconômicas. Os resultados indicam que o modelo de melhor capacidade preditiva foi o XGBoost, com curva ROC de 0.99 e acurácia de 0.98, na base teste. Na análise de interpretabilidade, via sharp value, os resultados corroboram o sentido econômico das variáveis. Do mesmo modo, a interpretabilidade via interações mostrou a influência da interação entre variáveis para melhora preditiva do modelo. Estes resultados corroboram a tendência de crescimento do uso de modelos com técnicas de machine learning na área econômica

É de notório saber que os baixos níveis de investimento criaram uma lacuna considerável na infraestrutura na América Latina e Caribe (ALC). Pesquisas empíricas mostram uma correlação positiva entre crescimento e investimento em infraestrutura nestes países e sugerem a necessidade da ALC investir cerca de 5% do PIB em infraestrutura por um período prolongado de tempo para superar sua lacuna atual

GNOSE: A arte Drag através das lentes do Design de Moda

Design.A existência de um corpo dentro da nossa realidade está fadada a transmitir informações que correspondem a uma série de interpretações, dependendo do contexto em que se está inserido. Essas percepções individuais podem ser transgibilizadas através do Design, uma área do saber que oferece diversas ferramentas para compreender e representar o universo, principalmente no processo de construção de imagem. Utilizando a metodologia Fashion Design Thinking foi possível estabelecer uma conexão entre Moda, Design e Arte Drag dentro dos contextos sociais e temporais, alcançando uma potente forma de expressão individual de autenticidade através de características conceituais e materiais, capazes de gerar significado. Com essa metodologia foi possível investigar o contexto que permeia o projeto, coletando informações e delimitando o que se desejava transmitir através do desenvolvimento de três looks. O projeto convida o leitor a exercer o questionamento das concepções pré-estabelecidas, a fim de verificar se estamos vivendo nosso autoconhecimento e autenticidade em plenitude. Ao final foram obtidos três looks que representavam a completa necessidade de expressão do autor, dois quais um foi escolhido para ser desenvolvido em um protótipo de alta fidelidade utilizando os materiais e oportunidades disponíveis.The existence of a body within our reality is bound to transmit information that corresponds to a series of interpretations, depending on the context in which one is inserted. These individual perceptions can be made more transparent through Design, an area of knowledge that offers several tools to understand and represent the universe, especially in the process of image construction. Using the Fashion Design Thinking methodology it was possible to establish a connection between Fashion, Design and Drag Art within the social and temporal contexts, achieving a powerful form of individual expression of authenticity through conceptual and material characteristics, capable of generating meaning. With this methodology it was possible to investigate the context that permeates the project, collecting information and delimiting what was desired to be transmitted through the development of three looks. The project invites the reader to question pre-established conceptions, in order to verify if we are living our self-knowledge and authenticity in fullness. In the end, three looks were obtained that represented the author's complete need for expression, two of which were chosen to be developed into a high fidelity prototype using the materials and opportunities available

Hyalohyphomycosis in an orthopedic patient: case report.

Hyalohyphomycosis are opportunistic fungal infections caused by saprobic fungi. Such mycoses usually occur in immunosuppressed patients. The most affected organ is usually the lungs. A male patient has arrived at the Orthopedics and Traumatology Service complaining of a mass on the dorsal surface of his right foot. On physical examination, a subcutaneous, elastic, non-mobile mass was observed on the dorsal surface of the right foot. Ulcerated lesion, without drainage of secretion and with pain on palpation. He reported focal increase in the dorsal surface of the right foot for 7 years. Among other diagnostic possibilities, imaging studies indicated the hypothesis of a giant cell tumor with a tendon sheath. The patient was then submitted to biopsy of the lesion. The anatomopathological study showed chronic suppurative inflammation with granulation tissue and the presence of septate hyaline hyphae compatible with hyalohyphomycosis. Surgical resection of the lesion and systemic treatment were then performed. Patient evolved well after the procedure

A controversa questão probatória da “Lei Seca”, seus conflitos principiológicos e a edição da Lei nº 12.760/2012

Monografia (graduação)—Universidade de Brasília, Faculdade de Direito, 2013.Neste trabalho analisamos a controversa questão probatória do crime de embriaguez ao volante estabelecida com a edição da Lei nº 11.705/2008,“Lei Seca”, e a tentativa de sua retificação com a promulgação da Lei nº 12.760/2012. Após uma breve análise do tipo penal do art. 306, do Código de Transito Brasileiro, faremos uma incursão no campo das provas, para afirmar que a atual legislação não respeita princípios importantes do Processo Penal,como a presunção de inocência e o nenotenetur se detegere – problema que não encontrou solução com entrada em vigor da lei nº 12.760/2012. Em conclusão tem-se que a tipificação da embriaguez ao volante, na forma como descrita na Lei atual, além de violar a Constituição Brasileira, mostra-se menos eficaz que a resposta administrativa ao mesmo problema
