Linfoma angiocéntrico centrofacial
El linfoma angiocéntrico centrofacial es una neoplasia linfoide
rara, con un diagnóstico a menudo difícil, debido al cuadro clínico
inespecífico y a que muchas veces son necesarias varias
biopsias para llegar a un diagnóstico correcto.
Se trata de un linfoma no Hodgkin(LNH) agresivo, de localización
preferente en el tracto respiratorio superior (sobre todo
en cavidad nasal), y con un pronóstico ominoso, ya que la
supervivencia media es de 12-18 meses aproximadamente(1).
Predomina en orientales y sudamericanos, entre los 50-60 años
de edad, y con ligera preferencia por el sexo masculino (2:1).
Se presenta el caso de una paciente ecuatoriana que acude a
nuestro servicio con edema hemifacial, rinorrea achocolatada y
obstrucción respiratoria nasal de 1 mes de evolución, sin respuesta
al tratamiento antibiótico y antiinflamatorio, que tras realizar
varias pruebas diagnósticas se evidenció histológicamente la
presencia de un linfoma T extranodal de tipo nasal (también
llamado linfoma de células T angiocéntrico).The centrofacial angiocentric lymphoma is a rare lymphoid neoplasm,
with an often-difficult diagnosis due to the non-specific
clinical picture. On many occasions it is necessary to perform
various biopsies to reach the correct diagnosis.
This lymphoma is an aggressive Non-Hodgkin's (NHL) type,
which is normally found in the upper respiratory tract (predominantly
in the nasal cavity), and has an ominous prognosis, as
the average survival rate is between 12 and 18 months (1). It is
predominantly found in subjects of oriental and South American
extraction, who are between the ages of 50 and 60 years and
with a slight tendency towards males (2:1).
This is the case study of a female Ecuadorian patient who was
referred to our department with a hemifacial edema, chocolate-
like rhinorrhea and nasal respiratory obstruction, which had
been treated with antibiotics and anti-inflammatories for a month
without success. After performing a number of diagnostic tests,
it was found histologically that the patient had an extranodal
T-cell lymphoma of the nasal type (also known as T-cell angiocentric
lymphoma)

Stability of schizophrenia diagnosis in a ten-year longitudinal study on first episode of non-affective psychosis: conclusions from the PAFIP cohort
OBJECTIVE: To evaluate the ten-year stability of schizophrenia diagnosis in a cohort of first-episode psychosis (FEP) patients and the factors associated with it. METHODS: Changes in diagnosis of 209 FEP patients were described during ten years of follow-up. Related factors with maintenance or change of schizophrenia diagnosis were evaluated in prospective and retrospective approaches through Binary Logistic Regressions, ROC and survival curves. RESULTS: Out of the 209 patients, 126 were diagnosed of schizophrenia six months after their inclusion in the clinical program. Prospective analyses showed that eight of those 126 schizophrenia patients had changed to a different diagnosis after ten years, and predictors of change were better childhood premorbid adjustment, less severity of clinical global impression at baseline, and diagnosis of comorbid personality disorder during follow-up. Retrospectively, out of the 154 patients with schizophrenia in the ten-year assessment, 36 had a different diagnosis at baseline, and those factors related with a different prior diagnosis than schizophrenia were better socioeconomic status and shorter duration of untreated psychosis (DUP). A survival analysis on the timing of schizophrenia diagnosis showed that male gender and longer DUP were predictors of earlier definite diagnosis. CONCLUSIONS: Diagnostic stability of schizophrenia in our FEP sample is high, especially prospective stability, and the group of patients with diagnostic change corresponded to a milder psychopathological profile before and at the onset of disease. Moreover, we observed a cautious attitude in the diagnosis of schizophrenia in patients with shorter DUP who had schizophrenia diagnosis after ten years
Biological and chemical factors controlling the patchy distribution of soil water repellency among plant species in a Mediterranean semiarid forest
Natural soil water repellency is a property that has already been observed in forest soils and is characterized by its patchy distribution. There are many factors involved in its development. In this work, we have studied a large number of chemical and biological factors under the influence of different plant species (. Pinus halepensis, Quercus rotundifolia, Cistus albidus and Rosmarinus officinalis) to learn which has the greatest responsibility for its presence and persistence in the top-soil layer. We observed strong and significant correlations between ergosterol, glomalin related soil protein (GRSP), extractable lipids, soil organic matter (SOM) content and water repellency (WR). Our results suggested lipid fraction as the principal factor. Moreover, apart from Pinus, fungal biomass seems to be also related to the SOM content. Soil WR found under Pinus appears to be the most influenced by fungi. Tracing the retina to analyze the integrity and phagocytic capacity of the retinal pigment epithelium
We have developed a new technique to study the integrity, morphology and functionality of the retinal neurons and the retinal pigment epithelium (RPE). Young and old control albino (Sprague-Dawley) and pigmented (Piebald Virol Glaxo) rats, and dystrophic albino (P23H-1) and pigmented (Royal College of Surgeons) rats received a single intravitreal injection of 3% Fluorogold (FG) and their retinas were analyzed from 5 minutes to 30 days later. Retinas were imaged in vivo with SD-OCT and ex vivo in flat-mounts and in cross-sections. Fifteen minutes and 24 hours after intravitreal administration of FG retinal neurons and the RPE, but no glial cells, were labeled with FG-filled vesicles. The tracer reached the RPE 15 minutes after FG administration, and this labeling remained up to 30 days. Tracing for 15 minutes or 24 hours did not cause oxidative stress. Intraretinal tracing delineated the pathological retinal remodelling occurring in the dystrophic strains. The RPE of the P23H-1 strain was highly altered in aged animals, while the RPE of the RCS strain, which is unable to phagocytose, did not accumulate the tracer even at young ages when the retinal neural circuit is still preserved. Design and baseline characteristics of SALT‐HF trial: hypertonic saline therapy in ambulatory heart failure
Aims Hypertonic saline solution (HSS) plus intravenous (IV) loop diuretic appears to enhance the diuretic response in patients hospitalized for heart failure (HF). The efficacy and safety of this therapy in the ambulatory setting have not been evaluated. We aimed to describe the design and baseline characteristics of the SALT-HF trial participants. Methods and results 'Efficacy of Saline Hypertonic Therapy in Ambulatory Patients with HF' (SALT-HF) trial was a multicenter, double-blinded, and randomized study involving ambulatory patients who experienced worsening heart failure (WHF) without criteria for hospitalization. Enrolled patients had to present at least two signs of volume overload, use >= 80 mg of oral furosemide daily, and have elevated natriuretic peptides. Patients were randomized 1:1 to treatment with a 1-h infusion of IV furosemide plus HSS (2.6-3.4% NaCl depending on plasmatic sodium levels) versus a 1-h infusion of IV furosemide at the same dose (125-250 mg, depending on basal loop diuretic dose). Clinical, laboratory, and imaging parameters were collected at baseline and after 7 days, and a telephone visit was planned after 30 days. The primary endpoint was 3-h diuresis after treatment started. Secondary endpoints included (a) 7-day changes in congestion data, (b) 7-day changes in kidney function and electrolytes, (c) 30-day clinical events (need of IV diuretic, HF hospitalization, cardiovascular mortality, all-cause mortality or HF-hospitalization). Results A total of 167 participants [median age, 81 years; interquartile range (IQR), 73-87, 30.5% females] were randomized across 13 sites between December 2020 and March 2023. Half of the participants (n = 82) had an ejection fraction >50%. Most patients showed a high burden of comorbidities, with a median Charlson index of 3 (IQR: 2-4). Common co-morbidities included diabetes mellitus (41%, n = 69), atrial fibrillation (80%, n = 134), and chronic kidney disease (64%, n = 107). Patients exhibited a poor functional NYHA class (69% presenting NYHA III) and several signs of congestion. The mean composite congestion score was 4.3 (standard deviation: 1.7). Ninety per cent of the patients (n = 151) presented oedema and jugular engorgement, and 71% (n = 118) showed lung B lines assessed by ultrasound. Median inferior vena cava diameter was 23 mm, (IQR: 21-25), and plasmatic levels of N-terminal-pro-B-type natriuretic peptide (NTproBNP) and antigen carbohydrate 125 (CA125) were increased (median NT-proBNP 4969 pg/mL, IQR: 2508-9328; median CA125 46 U/L, IQR: 20-114). Conclusions SALT-HF trial randomized 167 ambulatory patients with WHF and will determine whether an infusion of hypertonic saline therapy plus furosemide increases diuresis and improves decongestion compared to equivalent furosemide administration alone
Similar incidence of coronavirus disease 2019 (COVID-19) in patients with rheumatic diseases with and without hydroxychloroquine therapy
Background Hydroxychloroquine is not efficacious as post-exposure prophylaxis against coronavirus disease 2019 (COVID-19). It is not known whether as pre-exposure prophylaxis it may prevent COVID-19. Objective To compare the incidence of COVID-19 in Spanish patients with autoimmune rheumatic diseases treated with and without hydroxychloroquine. Patients and methods Retrospective electronic record review, from February 27th to June 21st, 2020, of patients with autoimmune inflammatory diseases followed at two academic tertiary care hospitals in Seville, Spain. The cumulative incidence of confirmed COVID-19, by PCR or serology, was compared between patients with and without hydroxychloroquine as part of their treatment of autoimmune inflammatory diseases. Results Among 722 included patients, 290 (40%) were receiving hydroxychloroquine. During the seventeen-week study period, 10 (3.4% [95% CI: 1.7%-6.7%] cases of COVID-19 were registered among patients with hydroxychloroquine and 13 (3.0% [1.6%-5.1%]) (p = 0.565) in those without hydroxychloroquine. COVID-19 was diagnosed by PCR in four (1.4%, 95% CI 0.38%-3.5%) subject with hydroxychloroquine and six (1.4%, 95% CI 0.5%-3.0%) without hydroxychloroquine (p = 0.697). Three patients on hydroxychloroquine and four patients without hydroxychloroquine were admitted to the hospital, none of them required to be transferred to the intensive care unit and no patient died during the episode. Conclusions The incidence and severity of COVID-19 among patients with autoimmune rheumatic diseases with and without hydroxychloroquine was not significantly different.Instituto de Salud Carlos III I3SNSMinisterio de Ciencia, Innovación y Universidades CP18/0014
Role of neurokinin B in the control of female puberty and Its modulation by metabolic status
[Abstract] Human genetic studies have revealed that neurokinin B (NKB) and its receptor, neurokinin-3 receptor (NK3R), are essential elements for normal reproduction; however, the precise role of NKB–NK3R signaling in the initiation of puberty remains unknown. We investigated here the regulation of Tac2 and Tacr3 mRNAs (encoding NKB and NK3R, respectively) in female rats and demonstrated that their hypothalamic expression is increased along postnatal maturation. At puberty, both genes were widely expressed throughout the brain, including the lateral hypothalamic area and the arcuate nucleus (ARC)/medial basal hypothalamus, where the expression of Tacr3 increased across pubertal transition. We showed that central administration of senktide (NK3R agonist) induced luteinizing hormone (LH) secretion in prepubertal and peripubertal females. Conversely, chronic infusion of an NK3R antagonist during puberty moderately delayed the timing of vaginal opening (VO) and tended to decrease LH levels. The expression of NKB and its receptor was sensitive to changes in metabolic status during puberty, as reflected by a reduction in Tacr3 (and, to a lesser extent, Tac2) expression in the ARC after a 48 h fast. Yet, acute LH responses to senktide in pubertal females were preserved, if not augmented, under fasting conditions, suggesting sensitization of the NKB–NK3R–gonadotropin-releasing hormone signaling pathway under metabolic distress. Moreover, repeated administration of senktide to female rats with pubertal arrest due to chronic undernutrition rescued VO (in ∼50% of animals) and potently elicited LH release. Altogether, our observations suggest that NKB–NK3R signaling plays a role in pubertal maturation and that its alterations may contribute to pubertal disorders linked to metabolic stress and negative energy balance.Ministerio de Ciencia e Innovación; BFU 2008-00984Ministerio de Ciencia e Innovación; BFU 2011-25021Andalucía. Precipitable water vapour content above the Roque de los Muchachos observatory from GPS estimations
ABSTRACT The requirements for current large and future extremely large telescopes as well as the quick development of IR instrumentation demand a proper characterization of precipitable water vapour (PWV) above astronomical sites. A comparison of PWV estimations from a photometer and a GPS (Global Positioning System) above the Observatorio del Roque de los Muchachos (ORM, La Palma, Canary Islands, Spain) was carried out and it was found a linear relation between both measurements. Such relationship will allow the calibration of the GPS measurements recorded at ORM for the period June 2001-December 2008. These large time series of PWV estimations from GPS were used to perform a statistical analysis of water vapour content above this astronomical site. Average annual value of night-time PWV of 4.86 mm was found. It was also found a clear seasonal behavior of the PWV above ORM, with smaller water-vapour columns during winter nights (average 3.36 mm). The largest values of PWV are reached in the summer nights (average 6.75 mm). The data indicate that a significant percentage of nights (~38%) are well suited for thermal infrared observations (with PWV < 3 mm), and 71% of nights present a "fair" or better IR observation opportunity at ORM
